Technical considerations in endovascular thrombectomy for distal vessel occlusions.

The role of endovascular thrombectomy (EVT) in acute ischemic stroke due to distal vessel occlusions (DVOs) remains to be clarified. Anatomical complexities in the distal vasculature, such as tortuosity and vessel fragility, render technique selection particularly paramount during EVT. Herein, we present two illustrative cases of anatomically similar DVOs treated with two different techniques at a single institution to frame a discussion of important procedural considerations during EVT for DVO.

WSSV355 enhances IDH1 enzymatic activity and promotes efficient WSSV replication.

White spot syndrome virus (WSSV) drives extensive metabolic reprogramming to support viral replication, including activation of reductive carboxylation, a pathway in which host isocitrate dehydrogenase 1 (IDH1) has a central role. However, the mechanisms by which viral factors regulate IDH1 and associated metabolic networks remain unclear. Here, we identified WSSV355 as a key viral regulator that contributed to IDH1 binding and activation during infection. Using yeast two-hybrid screening, WSSV355 was identified as an LvIDH1-interacting protein and further validated by co-immunoprecipitation a

Novel Digital Tools for Clinical Research: Vestibular Simulation for Benign Paroxysmal Positional Vertigo.

Benign paroxysmal positional vertigo is the most prevalent peripheral vestibular disorder, yet important aspects of its diagnostic variability and treatment response remain incompletely understood. In recent years, advances in computational modeling have provided new opportunities to explore the biomechanical basis of otoconial displacement within the semicircular canals. This review examines the role of vestibular simulation technologies in enhancing both mechanistic understanding and clinical practice. Three-dimensional models that integrate labyrinthine anatomy, head kinematics, gravitation

Brain energy allocation is consistent with adaptive thermogenesis.

Caloric restriction and weight loss reduce resting metabolic rate (RMR) beyond what is explained by the loss of fat-free mass, a phenomenon known as adaptive thermogenesis (ATRMR). Here, we frame ATRMR conceptually as the result of shifts in energy budgets within the body. Given the brain's central role as both a key regulator of energy intake and a major energy consumer within the body, we hypothesize that ATRMR represents one component of the weight-loss-associated reduction in energy expenditure and may reflect a redistribution of energy from the body to the brain, consistent with the hypot

Untargeted salivary metabolomics for early prediction of chemotherapy-induced oral toxicities in patients with oral squamous cell carcinoma.

Chemotherapy-induced oral toxicities, including oral mucositis, xerostomia, and dysgeusia, are common complications in patients with oral squamous cell carcinoma (OSCC), often leading to impaired quality of life, treatment interruptions, and poor therapeutic outcomes. Reliable non-invasive biomarkers capable of predicting these toxicities before severe clinical manifestations remain limited. This study investigated salivary metabolomic alterations associated with chemotherapy-induced oral toxicities using untargeted liquid chromatography-quadrupole time-of-flight mass spectrometry (LC-QTOF-MS)

Ensemble averaging of symbolic numbers: Criterion mean judgments beat heuristics.

Can observers compute an ensemble mean from symbolic numbers, or is averaging-like performance driven by heuristic shortcuts? We addressed this question using a criterion-based design that yields clear predictions for mean-based integration. In two experiments, participants viewed a 2 × 2 array of four Arabic digits and decided, while timed, whether the arithmetic mean of the set was smaller or larger than 5. In Experiment 1 ( N = 40 ), accuracy and response time showed a robust ensemble-distance effect: decisions were slowest and least accurate when the set mean was closest to 5 and improve

Investigation of Endoplasmic Reticulum Stress in Pregnant Women with Hyperemesis Gravidarum.

The role of the endoplasmic reticulum (ER) stress response in hyperemesis gravidarum (HG) remains incompletely understood. This study evaluated circulating PERK and ATF-4 levels and their associations with clinical and laboratory characteristics in HG. This prospective case-control study, conducted from August to November 2024, included 85 pregnant women with HG within the first 16 weeks of gestation and 85 healthy pregnant controls. Serum PERK and ATF-4 levels were measured using ELISA. Group comparisons, Spearman correlation analyses, and receiver operating characteristic (ROC) analysis were

Perioperative Outcomes of Acute Normovolemic Hemodilution in Cardiac Surgery: Evidence From Randomized Controlled Trials.

Acute normovolemic hemodilution (ANH) is a blood conservation strategy used in cardiac surgery, but its efficacy in reducing allogeneic blood transfusion remains debated. This systematic review and meta-analysis updates the existing evidence by incorporating recent randomized controlled trials (RCTs) to provide a contemporary assessment of ANH's impact on perioperative transfusion outcomes. We searched PubMed/MEDLINE, Scopus, and Cochrane Central up to August 2025 for RCTs comparing ANH to standard care in adult cardiac surgery. Primary outcomes were the number of allogeneic red blood cell uni

Understanding the involvement of HtrA4 during syncytiotrophoblast differentiation.

The syncytiotrophoblast (STB), the outermost layer of the human placenta, is vital to pregnancy health as it functions as the direct maternal-fetal interface. Being terminally differentiated and multinucleated, STB cannot divide but is formed/sustained by fusion of cytotrophoblasts throughout pregnancy. HtrA4, produced only in placentas of humans and high-order primates, is obligatory for syncytialization, including differentiation of trophoblast stem cells (TSCs) into STB as HtrA4 knockdown (KD) blocks the process; however, how HtrA4 mediates the process remains unknown. The current study aim

Who Diagnoses Irreversible Dying? The Irreversible Dying Transition and Medical Responsibility Before Death.

Medicine assigns clear professional responsibility for determining and certifying death, but responsibility for determining when irreversible dying has begun is often less clearly assigned. This paper proposes the irreversible dying transition (IDT) as the point at which no medically reasonable intervention can restore a sustainable life-preserving clinical state. IDT is distinct from prognosis, terminal illness, actively dying, the futility of a particular intervention, and decisions to limit treatment. Its purpose is not to introduce another diagnostic label, but to make explicit a specific

Early disruptions in brain capillary blood flow control in a mouse model of Alzheimer's disease.

In Alzheimer's disease (AD), vascular dysfunction and impaired control of brain blood flow can disrupt cellular bioenergetics decades before cognitive decline. Identifying the earliest vascular changes is therefore critical for developing therapies that intervene before the cascading events leading to neuronal decline. Brain energy supply is dynamically controlled by the cerebral vasculature, where capillary thin-strand pericytes are emerging as key players. We recently identified a central role for ATP-sensitive potassium (K+, KATP) channels in enabling thin-strand pericytes to sense local ne

Contribution of artificial intelligence research to the development of inclusive applications for people with disabilities: A scoping review (2015-2025).

Artificial intelligence (AI) is rapidly transforming healthcare. Yet people with disabilities (PWD), representing 16% of the global population, remain disproportionately excluded from these advances. This exclusion is driven by algorithmic bias, non-representative datasets, and limited involvement of PWD in the design, development, and evaluation of AI applications. This scoping review examines the extent to which AI research in health contributes to the development of inclusive technologies for PWD, focusing on equity considerations. It identifies current trends, methodological gaps, and oppo

Enhancing Mental Wellness in Kenyan Adolescents and Young People: Protocol for a Platform Development and Usability Study.

BACKGROUND: Mental health disorders are a leading cause of disability among adolescents and young people in Kenya, yet access to appropriate and timely support remains severely limited due to shortages of trained professionals, stigma, financial barriers, and geographical inequities. Digital mental health interventions, particularly those leveraging AI, present a promising opportunity to expand access to culturally relevant, low-stigma mental wellness support in low- and middle-income country (LMIC) settings. However, evidence on the feasibility, usability, and acceptability of AI-powered ment

Automation in sleep state classification and maturity evaluation of EEG in preterm-born infants.

There is a growing need to automate EEG monitoring of preterm-born infants, particularly where expert evaluation is limited. However, existing models necessitate human intervention for comprehensive assessment, thereby limiting the clinical applicability. We collected 165 EEG records from 65 preterm-born infants. 8-channel EEG records were segmented every 30second and allocated into eight classes (three preterm-equivalent, four term-equivalent, and one noise class). We performed noise detection, sleep state classification, and maturity assessment through the classification. ImageNet pretrained

Exercise-induced epigenetic remodeling in COPD skeletal muscle dysfunction: From sedentary behavior to molecular rehabilitation.

Chronic obstructive pulmonary disease (COPD) is increasingly recognized as a multisystem disorder in which extrapulmonary manifestations substantially contribute to disease burden. Among these, skeletal muscle dysfunction (SMD) represents a major determinant of exercise intolerance, reduced physical activity, impaired quality of life, and mortality. Growing evidence suggests that physical inactivity is not merely a consequence of advanced COPD but may act as an early and potentially modifiable driver of skeletal muscle deterioration, being detectable even in patients with mild disease and pote
