Who Diagnoses Irreversible Dying? The Irreversible Dying Transition and Medical Responsibility Before Death.

Medicine assigns clear professional responsibility for determining and certifying death, but responsibility for determining when irreversible dying has begun is often less clearly assigned. This paper proposes the irreversible dying transition (IDT) as the point at which no medically reasonable intervention can restore a sustainable life-preserving clinical state. IDT is distinct from prognosis, terminal illness, actively dying, the futility of a particular intervention, and decisions to limit treatment. Its purpose is not to introduce another diagnostic label, but to make explicit a specific

Early disruptions in brain capillary blood flow control in a mouse model of Alzheimer's disease.

In Alzheimer's disease (AD), vascular dysfunction and impaired control of brain blood flow can disrupt cellular bioenergetics decades before cognitive decline. Identifying the earliest vascular changes is therefore critical for developing therapies that intervene before the cascading events leading to neuronal decline. Brain energy supply is dynamically controlled by the cerebral vasculature, where capillary thin-strand pericytes are emerging as key players. We recently identified a central role for ATP-sensitive potassium (K+, KATP) channels in enabling thin-strand pericytes to sense local ne

Contribution of artificial intelligence research to the development of inclusive applications for people with disabilities: A scoping review (2015-2025).

Artificial intelligence (AI) is rapidly transforming healthcare. Yet people with disabilities (PWD), representing 16% of the global population, remain disproportionately excluded from these advances. This exclusion is driven by algorithmic bias, non-representative datasets, and limited involvement of PWD in the design, development, and evaluation of AI applications. This scoping review examines the extent to which AI research in health contributes to the development of inclusive technologies for PWD, focusing on equity considerations. It identifies current trends, methodological gaps, and oppo

Enhancing Mental Wellness in Kenyan Adolescents and Young People: Protocol for a Platform Development and Usability Study.

BACKGROUND: Mental health disorders are a leading cause of disability among adolescents and young people in Kenya, yet access to appropriate and timely support remains severely limited due to shortages of trained professionals, stigma, financial barriers, and geographical inequities. Digital mental health interventions, particularly those leveraging AI, present a promising opportunity to expand access to culturally relevant, low-stigma mental wellness support in low- and middle-income country (LMIC) settings. However, evidence on the feasibility, usability, and acceptability of AI-powered ment

Automation in sleep state classification and maturity evaluation of EEG in preterm-born infants.

There is a growing need to automate EEG monitoring of preterm-born infants, particularly where expert evaluation is limited. However, existing models necessitate human intervention for comprehensive assessment, thereby limiting the clinical applicability. We collected 165 EEG records from 65 preterm-born infants. 8-channel EEG records were segmented every 30second and allocated into eight classes (three preterm-equivalent, four term-equivalent, and one noise class). We performed noise detection, sleep state classification, and maturity assessment through the classification. ImageNet pretrained

Exercise-induced epigenetic remodeling in COPD skeletal muscle dysfunction: From sedentary behavior to molecular rehabilitation.

Chronic obstructive pulmonary disease (COPD) is increasingly recognized as a multisystem disorder in which extrapulmonary manifestations substantially contribute to disease burden. Among these, skeletal muscle dysfunction (SMD) represents a major determinant of exercise intolerance, reduced physical activity, impaired quality of life, and mortality. Growing evidence suggests that physical inactivity is not merely a consequence of advanced COPD but may act as an early and potentially modifiable driver of skeletal muscle deterioration, being detectable even in patients with mild disease and pote

Disabling COMPASS Promoter Licensing Shifts HDAC-Inhibited Gastric Cancer Toward Sustained Transcriptional Suppression During Recovery.

Histone deacetylase inhibitors (HDACi) broadly increase chromatin acetylation but have limited and variable activity in solid tumors. To define the epigenetic buffering axis that sustains promoter output during HDAC inhibition in gastric cancer. We integrated pharmacologic profiling across a molecularly diverse gastric cancer panel with an epigenetic-focused pooled CRISPR-Cas9 screening library, domain-resolved genetic perturbation, structure-guided WIN-pocket targeting, CCK8 time-course proliferation assays, and pulse-washout clonogenic recovery assays. Factors Associated with Functional Disability and Walking Capacity in Adults with Chronic Low Back Pain: A Cross-Sectional Study.

Background/Objectives: Disability in chronic low back pain (CLBP) is multifactorial, yet the independent associations of pain, fear-avoidance beliefs, and comorbidity burden with self-reported disability and performance-based walking capacity remain unclear, particularly in Saudi Arabia. This study examined factors independently associated with functional disability and walking capacity in adults with CLBP and explored whether disability was independently associated with walking capacity after accounting for demographic and clinical factors. Methods: This cross-sectional study included 339 adu

Human iPSC-Derived Brain Cells, Organoids, and Organoids-on-Chips: Advancing Neurological Disease Modeling to Improve Clinical Outcome.

Neurological disorders are the leading cause of overall disease and disability worldwide, affecting more than 3.4 billion people globally and over 180 million Americans. Yet there are no curative treatments available for any of the cardinal neurological diseases, such as Alzheimer's disease (AD), Parkinson's disease (PD), amyotrophic lateral sclerosis-frontotemporal dementia (ALS-FTD) and Huntington's disease (HD). Many compounds that demonstrate excellent activity in preclinical models fail in clinical trials. The most compelling reason for this failure is attributed to inherent limitations o

Integrating Transcranial Ultrasound Stimulation with Exercise-Based Rehabilitation for Ischemic Stroke Recovery: A Preclinical Study.

Ischemic stroke remains a leading cause of long-term disability worldwide, and effective neurorehabilitative strategies to enhance functional recovery are limited. This preclinical study compared noninvasive low-intensity pulsed ultrasound (LIPUS), aerobic exercise (AE) rehabilitation, and their combination in a mouse model of ischemic stroke. Transient focal cerebral ischemia was induced in male C57BL/6 mice using middle cerebral artery occlusion (MCAO). Male mice assigned to LIPUS-containing groups received LIPUS preconditioning for three consecutive days before MCAO, followed by two days of

Gluten-Free and Gluten-Restricted Diets in Multiple Sclerosis: A Systematic Review of Disease Activity and Patient-Reported Outcomes.

Background: Dietary modification is widely used by people with multiple sclerosis (MS), and gluten avoidance is a common self-directed strategy. Biological plausibility has been proposed through gut-barrier and immune mechanisms, but whether gluten restriction improves clinical outcomes or objective disease activity remains uncertain. Objective: To systematically identify, appraise, and synthesise evidence on gluten-free and gluten-restricted diets in MS, with particular attention to whether observed effects can be attributed specifically to gluten. Methods: The protocol was registered with PR
