Human iPSC-Derived Brain Cells, Organoids, and Organoids-on-Chips: Advancing Neurological Disease Modeling to Improve Clinical Outcome.

Neurological disorders are the leading cause of overall disease and disability worldwide, affecting more than 3.4 billion people globally and over 180 million Americans. Yet there are no curative treatments available for any of the cardinal neurological diseases, such as Alzheimer's disease (AD), Parkinson's disease (PD), amyotrophic lateral sclerosis-frontotemporal dementia (ALS-FTD) and Huntington's disease (HD). Many compounds that demonstrate excellent activity in preclinical models fail in clinical trials. The most compelling reason for this failure is attributed to inherent limitations o

Integrating Transcranial Ultrasound Stimulation with Exercise-Based Rehabilitation for Ischemic Stroke Recovery: A Preclinical Study.

Ischemic stroke remains a leading cause of long-term disability worldwide, and effective neurorehabilitative strategies to enhance functional recovery are limited. This preclinical study compared noninvasive low-intensity pulsed ultrasound (LIPUS), aerobic exercise (AE) rehabilitation, and their combination in a mouse model of ischemic stroke. Transient focal cerebral ischemia was induced in male C57BL/6 mice using middle cerebral artery occlusion (MCAO). Male mice assigned to LIPUS-containing groups received LIPUS preconditioning for three consecutive days before MCAO, followed by two days of

Gluten-Free and Gluten-Restricted Diets in Multiple Sclerosis: A Systematic Review of Disease Activity and Patient-Reported Outcomes.

Background: Dietary modification is widely used by people with multiple sclerosis (MS), and gluten avoidance is a common self-directed strategy. Biological plausibility has been proposed through gut-barrier and immune mechanisms, but whether gluten restriction improves clinical outcomes or objective disease activity remains uncertain. Objective: To systematically identify, appraise, and synthesise evidence on gluten-free and gluten-restricted diets in MS, with particular attention to whether observed effects can be attributed specifically to gluten. Methods: The protocol was registered with PR

HPV vaccination coverage and barriers for people with disability: A scoping review.

This scoping review explored human papillomavirus (HPV) vaccination coverage and barriers for people with disability. Our global search identified 33 studies, all from high-income countries, published between 2006 and 2025. We found evidence of inequalities in HPV vaccination coverage for people with disability across a range of countries, sexes/genders, settings, and disability groups. Common vaccination barriers included misassumptions around sexual activity, lack of vaccination recommendations from healthcare professionals in primarily healthcare-facility-based HPV vaccination programme set

Peripheral Nerve Stimulation for Essential Tremor: a Systematic Review and Meta-Analysis of Effectiveness and Safety.

BACKGROUND: Essential tremor (ET) is the most prevalent movement disorder worldwide. There are limited current pharmacological therapies, and their side effects often preclude adequate doses. Surgical therapies, such as Deep Brain Stimulation (DBS) and Focused Ultrasound (FUS), are reserved for patients with tremor that debilitates activities of daily living (ADLs). Peripheral nerve stimulation (PNS), including transcutaneous afferent patterned stimulation (TAPS) and transcutaneous peripheral nerve stimulation (TPNS), has emerged as a promising non-invasive neuromodulatory approach for many pa

Immediate effects of spinal manipulation, exercise therapy, and open-label placebo on lifting performance and pain in individuals with chronic non-specific back pain: a motion laboratory-based crossover randomized controlled trial.

BACKGROUND: Chronic non-specific back pain (CBP) is a leading cause of disability worldwide and is commonly associated with limitations in movements such as lifting. Although spinal manipulation (SMT) and exercise therapy (ET) are recommended treatments for CBP, their effects on objectively measured movement performance remain insufficiently explored. Open-label placebo (OLP) has gained attention for engaging placebo-related mechanisms ethically. This study aimed to investigate the immediate effects of SMT, ET, and OLP on lifting performance and pain in individuals with CBP, and to evaluate th

Upper extremity motor outcomes during paired vagus nerve stimulation in chronic ischemic stroke: a retrospective single-health-system cohort study.

BACKGROUND: Persistent upper extremity (UE) impairment after stroke is a major cause of long-term disability, and treatment options in the chronic phase are limited. In randomized trials, vagus nerve stimulation paired with task-specific rehabilitation (Paired VNS) produced greater improvement in UE function and activity than the same rehabilitation with sham stimulation, leading to FDA approval in 2021. METHODS: We conducted a retrospective, observational cohort study of adults with chronic stroke and persistent UE deficits who underwent Vivistim implantation and participated in a Paired VNS

A systematic review of mortality and long-term functional outcomes after decompressive craniectomy for acquired brain injury: The missing rehabilitation reporting.

BACKGROUND: Decompressive craniectomy (DC) is a surgical procedure for refractory intracranial hypertension following severe acquired brain injury (sABI) and is an effective, life-saving treatment. However, there is significant controversy regarding its impact on long-term functional outcome. Furthermore, the role of postoperative rehabilitation in shaping recovery after DC has been largely overlooked in the literature. OBJECTIVES: To systematically review randomized controlled trials (RCTs) evaluating the long-term functional outcomes of DC in patients with sABI, stratified by underlying eti

From molecular advances to limited clinical impact: The persistent gap in chromoblastomycosis.

BACKGROUND: Chromoblastomycosis (CBM) is a chronic implantation mycosis of considerable morbidity, first described more than a century ago, predominantly affecting rural populations in tropical and subtropical regions. Although historically associated with impoverished settings, an increasing number of cases have been reported in both endemic and non-endemic areas, including high-income countries, reflecting an underrecognized epidemiological landscape. Despite its burden, key metrics such as disability-adjusted life years (DALYs) remain undefined, as highlighted by recent global health initia

Anti-CD20 Drugs and Natalizumab Confer Equal Protection Against Progression Independent of Relapse Activity (PIRA) in People With Multiple Sclerosis.

BACKGROUND: Highly effective (HE) disease-modifying therapies (DMTs) reduce relapse-associated worsening (RAW) and progression independent of relapse activity (PIRA). Natalizumab (NTZ) and anti-CD20 agents show similar efficacy on relapse suppression, but long-term comparative data on PIRA are limited. We aimed to compare PIRA accrual in relapsing-remitting MS (RRMS) patients treated with NTZ and anti-CD20 drugs over a long follow-up. METHODS: We included RRMS patients followed at our centre and recorded in the Italian MS and Related Disorders Register with at least 5 years of continuous trea

Turning Back the Ageing Clock? Brain Age in Neuroprotective and Clinical Interventions.

Neuroimaging-derived brain age has emerged as an intuitive marker of brain health and cognitive longevity. The difference between brain age and chronological age, termed brain-predicted age difference (brain-PAD), quantifies deviation from normative brain ageing and may help evaluate whether interventions could attenuate brain ageing trajectories. This review synthesizes the methodological foundations of brain age estimation, factors linked to brain-PAD across healthy ageing and disease, evidence from clinical interventions, and emerging translational applications in clinical trials. Across no

Knee osteoarthritis management across the joint-preservation continuum: current evidence and future directions.

Knee osteoarthritis is a leading cause of chronic pain, disability, and mobility limitation, with increasing prevalence driven by population aging, obesity, and joint injury. Although historically viewed as passive cartilage "wear and tear," knee osteoarthritis is now understood as a heterogeneous whole-joint disease involving cartilage matrix degradation, chondrocyte dysfunction, synovial inflammation, subchondral bone remodeling, genetic and epigenetic susceptibility, and altered biomechanics. This narrative review examines current and emerging strategies for knee osteoarthritis management a
