BACKGROUND: Increased step time variability, particularly on an irregular surface, has been associated with impaired mobility function and a variety of diseases. However the biomechanical necessity, or advantage, of increasing step time variability has not been identified.
METHODS: We performed a secondary analysis of gait data previously obtained on 42 subjects age 50 or older with neuropathy who walked on smooth and irregular surfaces, the latter with and without three interventions (cane, ankle orthosis and wall touch) that provided frontal plane support.
FINDINGS: Step time variability o

OBJECTIVE: To investigate the links between knee pain characteristics and restricted mobility outside the home, and how these are influenced by mobility-specific activity limitation, age, sex, socioeconomic status, environmental factors, and comorbidity.
METHODS: We conducted a cross-sectional survey of community-dwelling adults age > or = 50 years. A total of 2,252 responders reporting pain in and around the knee in the last year were eligible. The primary outcome was self-reported restricted mobility outside the home in the previous 4 weeks (dichotomized as present or absent).
RESULTS: Kne

Triplane ankle fractures typically occur in the adolescent age group. Although many are minimally displaced and can be managed nonoperatively, some are displaced and difficult to reduce by closed methods and need open reduction and internal fixation. Traditionally satisfactory articular reduction is achieved through an open approach, which can be extensive. We describe our experience of treating displaced triplane fractures in four patients, assisted by ankle arthroscopy to ensure anatomical reduction and minimal soft tissue disruption. We achieved excellent reduction and stable fixation in al

The early identification of postmenopausal women at risk of fracture, followed by effective therapeutic intervention, can substantially improve patient outcomes. Bone mineral density measurements and comprehensive risk assessment are essential components in deciding when to intervene with osteoporosis pharmacologic treatment. Simple, easy-to-administer risk assessment tools are available to assist the physician in identifying patients in need of further evaluation or treatment and aid in patient counseling. Furthermore, effective pharmacologic treatments are available that have demonstrated re

Acquired brain injury frequently alters an individual's ability to make health care decisions based on a clear understanding of the situation and options. This exploratory study investigated the ways health care providers address issues of decisionmaking capacity (DMC) on a daily, functional basis. 33 clinicians providing rehabilitation services to persons with acquired brain injury participated in 1 of 5 semi-structured focus groups. All 33 participants, representing 8 different occupations, agreed that DMC determinations affected their practice every day. Participants underscored a multidime

The effects of intramuscular psoas lengthening on gait in cerebral palsy patients have been the subject of debates, and the indications for such procedure are still controversial. The purpose of this study was to evaluate the effects of intramuscular psoas lengthening on sagittal plane pelvic and hip motion in patients with spastic diparetic cerebral palsy and identify the factors linked to the best possible outcome. A retrospective study was performed in 26 independent ambulatory patients. All of them had undergone an intramuscular psoas lengthening over the pelvic brim. The mean age at the t

Proteins that interact with nucleic acids are central to numerous cellular processes, and their continuing characterization represents one of the foremost challenges in the postgenomic era. Here we describe a simple proteomics-based approach for the identification by mass spectrometry of proteins in crude extracts that interact with nucleic acids. It incorporates the electrophoretic mobility shift assay and is based on the finding that when a protein forms a complex with nucleic acid its electrophoretic mobility is affected as well as that of the nucleic acid. Our method should greatly reduce

OBJECTIVE: Imageless computer assisted total knee surgical systems have commonly relied on determination of the functional rotational center of the femoral head as a landmark for determining the lower extremity mechanical axis. This has been accomplished through range of motion and center of rotation calculations for the femur with respect to the pelvis as the lower extremity is taken through a range of motion. Our study evaluated the use of this algorithm with and without a pelvic tracker attached to the iliac crest.
MATERIALS AND METHODS: The functional center of the hip joint was also comp

Computer-assisted total knee arthroplasty (TKA) has traditionally been performed through an anterior midline incision approximately 16 cm long, using a capsular incision that separates the interval between the rectus femoris and vastus medialis muscles. The incision disrupts the suprapatellar pouch and frequently leads to adhesions and difficulty in fast and complete flexion recovery despite the wide exposure. The minimally invasive surgery subvastus approach with computer-assisted soft tissue balancing using OrthoPilot TKA (B. Braun Aesculap, Tuttlingen, Germany) not only provides optimal ali

In designing a posterior-stabilized total knee arthroplasty (TKA) it is preferable that when the cam engages the tibial spine the contact point of the cam move down the tibial spine. This provides greater stability in flexion by creating a greater jump distance and reduces the stress on the tibial spine. In order to eliminate edge loading of the femoral component on the posterior tibial articular surface, the posterior femoral condyles need to be extended. This provides an ideal femoral contact with the tibial articular surface during high flexion angles. To reduce extensor mechanism impingeme

Today's laparoscopic tools impose severe ergonomic limitations and are constrained to only four degrees of freedom. These constraints limit the surgeon's ability to orient the tool tips arbitrarily, and can contribute to a variety of complications. Robots external to the patient have been used to aid in the manipulation of the tools and improve dexterity. However, these robots are expensive, bulky, and are used for only select procedures. In vivo robotic assistants have the potential to enhance the capabilities of the surgeon, reduce costs, and reduce patient trauma. The motion of these in viv

We have developed a non-invasive measurement technique which can ultimately be used to quantify three-dimensional patellar kinematics of human subjects for a range of static positions of loaded flexion and assessed its accuracy. Knee models obtained by segmenting and reconstructing one high-resolution scan of the knee were registered to bone outlines obtained by segmenting fast, low-resolution scans of the knee in static loaded flexion. We compared patellar tracking measurements made using the new method to measurements made using Roentgen stereophotogrammetric analysis in three cadaver knee s

Seventy-four patients (120 knees, 360 injections) referred to an orthopedic surgery clinic by their primary care physician for osteoarthritis knee pain received 3 weekly intra-articular injections of hylan G-F 20 (Synvisc; Genzyme Biosurgery, Ridgefield, NJ). Osteoarthritic knee pain, the ability to perform activities, and the amount of pain medication, physiotherapy, and assistive device use were prospectively evaluated 1 month and 1 year after treatment. The majority of patients reported improved or much improved pain at 1 month (99%) and 1 year (91%) after hylan G-F 20 therapy. Patient acti

BACKGROUND: Capsulorrhaphy of the glenohumeral joint is a common surgical procedure for the treatment of instability caused by increased capsular laxity. The effect of capsulorrhaphy on the range of motion of the shoulder is poorly understood.
METHODS: We simulated localized capsular contractures by selective capsular plications in eight human cadaveric shoulders and studied the effect of such plications on the passive range of glenohumeral abduction, flexion, and external and internal rotation in different degrees of abduction. A 0.5 or 1-N-m torque was applied to the humerus, and the range

SK4/IK1 encodes an intermediate conductance, Ca2+ -activated K+ channel and fulfills a variety of physiological functions in excitable and nonexcitable cells. Although recent studies have provided evidence for the presence of SK4/IK1 channels in salivary acinar cells, the regulatory mechanisms and the physiological function of the channel remain unknown in these cells. Using molecular and electrophysiological techniques, we examined whether cytosolic ATP-dependent regulation of native SK4/IK1-like channel activity would involve endogenous cAMP-dependent protein kinase (PKA) in rat submandibula
