Evaluation of serial casting to correct equinovarus deformity of the ankle after acquired brain injury in adults.

OBJECTIVE: To determine the potential short-term benefit of serial plaster casting in the management of equinovarus deformity associated with acquired brain injury. DESIGN: Prospective uncontrolled interventional trial. SETTING: Inpatient rehabilitation facility in Australia. PARTICIPANTS: Sixteen patients (19 limbs) with equinovarus deformity or deteriorating ankle range of motion associated with severe plantarflexor and invertor muscle overactivity underwent serial plaster casting over an 18-month period. INTERVENTIONS: Below-knee plaster casts were reapplied weekly to increase joint ran

Sensory feedback in artificial control of human mobility.

Artificial motor control systems may reduce the handicap of motor impaired individuals. Sensors are essential components in feedback control of these systems and in the information exchange with the user. The objective of this paper is to give an overview of the applications of sensors in the artificial control of human mobility. These applications may either require an accurate estimate of the measured physical quantities or can be based on learning the relation between sensory information and control actions by example. Actual use of sensors in artificial motor control systems requires that

The use and effectiveness of assistive devices in an elderly urban population.

The aim of this study was to examine the type, frequency and usage rate of assistive devices in daily life activities in a community-based population aged 70 and over (N = 170), as well as the particular part of the intervention trial which focused on un-met needs, and the usage rate and effectiveness of assistive devices 6 and 12 months after prescription. Among persons under 80 years of age, 24% were users of assistive devices; in the 80 years or above group, this figure rose to 57%. Hygiene and mobility devices were the most common. The usage rate of assistive devices was 84%. The reasons f

Guidelines to determine the route of hysterectomy.

OBJECTIVE: To 1) test the validity of a method of assigning patients prospectively to a vaginal, abdominal, or laparoscopy-assisted vaginal approach to hysterectomy for benign disease; 2) compare the outcomes of these options from the day of surgery to the first day of returning to normal activities; and 3) estimate the proportion of hysterectomies by each route when patients were assigned according to this system, and the impact on hospital charges. METHODS: Six hundred seventeen women were assigned to a route of hysterectomy on the basis of uterine size (greater or less than 280 g), presump

Predictors of assistive technology abandonment.

Technology abandonment may have serious repercussions for individuals with disabilities and for society. The purpose of this study was to determine how technology users decide to accept or reject assistive devices. Two hundred twenty-seven adults with various disabilities responded to a survey on device selection, acquisition, performance, and use. Results showed that 29.3% of all devices were completely abandoned. Mobility aids were more frequently abandoned than other categories of devices, and abandonment rates were highest during the first year and after 5 years of use. Four factors were s

Self-care and mentors to reduce stress and enhance administrative ability.

This article has considered two options available for nurses seeking career advancement in LTC. Orem's self-care model offers an approach to assist the DON to cope with stressors in the LTC setting through self-care. Mentoring also offers a strategy to assist the novice leader with professional growth. Mentoring is an emerging concept in nursing. Mentoring is of special significance in LTC because there is a shortage of DONs. Experienced DONs in LTC who have learned to handle the stresses that overwhelm the novice need to mentor younger DONs and others interested in a DON position. The use of

The costs of infant carrying in captive cotton-top tamarins.

Marmosets and tamarins have a communal rearing system in which all group members help to care for the twin infants characteristic of this family of primates. Helpers are likely to incur time and energy costs by contributing to infant care. Predictions that cotton-top tamarin (Saguinus oedipus) helpers would change their behavior when carrying infants because of reduced mobility and/or a need for increased vigilance were tested in a captive colony. Tamarins carrying an infant spent significantly less time feeding, foraging, moving, or engaging in social activities such as grooming than they did

Wearable air supply for pneumatic artificial hearts and ventricular assist devices.

An experimental wearable air supply for pneumatic artificial hearts and ventricular assist devices has been built and tested. The unit eliminates the need for tethering to a large, stationery driver. The miniaturized air supply is designed for ambulatory patients with implanted pulsatile pneumatic total artificial hearts (TAH) or pneumatic left-ventricular assist devices (LVAD), to permit mobility in clinical and home settings. The device has major short-term utility as a supply for pneumatic TAH or VAD bridges in patients awaiting heart transplants. The system design for the wearable driver i

Participation and persistence in a nontraditional allied health education program.

The results of this study suggest the following three practical applications. First, adult educators and program planners in medical record administration can use the results of achievement motivation studies in recruitment, counseling, and screening efforts in both medical record technology and nontraditional ART progression programs to determine which ART candidates possess a strong need to achieve. Once identified, these individuals may be the most successful in reducing or eliminating situational, institutional, dispositional, and informational barriers to participation and persistence. Se

[Model for the regional allocation of the National Health Care Fund].

In 1978 a National Health Service (Servizio Sanitario Nazionale = SSN) was constituted in Italy which exercises jurisdiction in the sector of health care and is duty bound to assist all citizens. Basically speaking, the NHS is organized on three levels (national, regional and local) with the management of direct operations assigned to the (about 700) Local Health Boards (Unità Sanitaria Locale = USL) each of which covers a well determined territorial area. The Authors indicate that rarely discussed or evaluated are the procedures for the regional allocation of health care funding which is dete

Vulnerable groups*.

As a rule, refugees arrive in their host country with a bare minimum of possessions; in this respect every refugee can be classified as vulnerable. However, a certain category is especially at risk, namely those persons who would be considered vulnerable in a "normal society." These people are doubly in need of assistance, firstly, inasmuch as they are refugees and secondly, insofar as they are vulnerable. If these refugees can be helped to overcome (or at least reduce) their handicaps, they can begin to become self-reliant and form an active, productive parts of their community, rather than r
