Delivery of Behavioral Insomnia Care in VA Primary Care Mental Health Integration Clinics: Sustainment Outcomes of a Hybrid Type 3 Trial.

BACKGROUND: Insomnia is a common disorder associated with significant negative health and functional consequences. Behavioral interventions, such as cognitive behavioral therapy for insomnia (CBT-I), are recommended and supported by training efforts, but access to care remains challenging for many. OBJECTIVE: Evaluate change in the delivery of a brief behavioral treatment for insomnia (BBTI) following the withdrawal of implementation strategy support in Department of Veterans Affairs (VA) Primary Care Mental Health Integration (PCMHI) clinics. DESIGN: A pre-post analysis of the withdrawal of

Discriminatory Practice in Opioid Use Disorder: In a Post-X-Waiver Era, Barriers to Buprenorphine Persist.

Multiple court settlements established that it is a violation of the Americans with Disabilities Act (ADA) for skilled-nursing facilities (SNF) to refuse to care for patients with opioid use disorder (OUD) who are on medication for opioid use disorder (MOUD). Yet, this discriminatory practice persists. This perspective discusses the case of a patient with cirrhosis and OUD, who was not allowed to return to his SNF due to starting buprenorphine during a hospitalization, and highlights why stigma and knowledge deficits continue to be barriers to this mortality-reducing treatment resulting in inc

General Population Screening For Celiac Disease: A Scoping Review of Evidence, Outcomes, and Implementation.

BACKGROUND AND AIMS: Early diagnosis of celiac disease (CeD) through general population screening remains controversial. This review summarizes the remaining evidence gaps limiting implementation of population-based screening for CeD. METHODS: A scoping review was conducted in accordance with the PRISMA-ScR. PubMed was searched for literature related to CeD-screening published up to April 18, 2025. Twelve paired reviewers independently screened selected abstracts and assessed full texts. Included articles were then categorized by primary outcome and summarized descriptively. RESULTS: Of 2,01

Monitored, unmonitored, and solitary opioid use among overdose survivors in San Francisco and Boston.

BACKGROUND: Fatal overdoses occur when people use drugs alone. Decedents may be solitary where no one was likely to find them or near others, yet unmonitored. Characterizing monitored, unmonitored, and solitary opioid use and overdose circumstances can inform overdose prevention strategies. METHODS: We conducted a secondary analysis of REBOOT 2.0 trial data (San Francisco and Boston, 2019-2023). Participants had opioid use disorder, prior overdose, and unregulated opioid use. We categorized opioid use circumstances over 120-days before study entry and up to the three most recent overdoses rep

Impact of prehospital scene-to-door time on functional outcomes after endovascular thrombectomy: insights from a regional stroke network.

BACKGROUND: Minimizing time to endovascular thrombectomy (EVT) is crucial for the treatment of acute ischemic stroke (AIS). While prehospital transport interval is widely used to describe the overall ambulance transfer process, the prognostic relevance of scene-to-door time (SDT)-the interval from departure at the emergency scene to hospital arrival-remains unclear. This study aimed to examine the association between SDT and 90-day functional outcomes after EVT, with focus on the modifying role of pre-stroke disability. METHODS: We retrospectively analyzed patients who underwent EVT at a sing

Contextual conditions shaping the upward referral of obstetric emergencies from community health centres in the Eastern Cape: a Straussian Grounded Theory study.

INTRODUCTION: Maternal mortality remains a significant public health concern in South Africa, particularly in resource limited settings where timely access to higher-level care is vital. Community Health Centres are the first point of contact for women faced with obstetric emergencies, yet systemic, contextual, and operational challenges impede effective upward referral. Guided by Straussian Grounded Theory, this qualitative study explored the contextual conditions shaping the upward referral of obstetric emergencies in the OR Tambo district, Eastern Cape. METHODS: Data was collected through

Effects of Iran Light Crude Oil and Corexit EC9500A on the UV-Induced Formation, Colloidal Stability, and Bacterial Toxicity of Gold Nanoparticles in Simulated Seawater.

Oil-derived organic matter and dispersant components can generate reactive oxygen species under irradiation, but their role in mediating Au(III) reduction and gold nanoparticle formation remains unclear. Here, laboratory-prepared WAO, WAD, and DWAO systems in simulated seawater were used as model matrices for Au(III) photoreduction. EPR analysis revealed the generation of O2•-, •OH, and 1O2 during nanoparticle formation. Scavenger experiments suggested that O2•- was a major contributor to Au(III) reduction, while •OH and 1O2 appeared to influence the process indirectly. Stability assessments s

Incidental detection of maternal malignancy in non-reportable results from non-invasive prenatal testing: experience in Hong Kong and literature review.

INTRODUCTION: Non-invasive prenatal testing (NIPT) for fetal aneuploidy occasionally yields non-reportable results due to persistent 'noisy' signals, which may indicate occult maternal malignancy. This study investigated the clinical and genetic implications of non-reportable NIPT results, focusing on multiple chromosomal aberrations associated with maternal malignancy, with the goal of developing standardised management protocols for pregnant women with such suspicious NIPT results. METHODS: From 2020 to 2023, women with persistent non-reportable NIPT results were identified and followed up

Hypoglycemia and Clinical Outcomes in Non-Diabetic Adults Hospitalized with Acute Infection.

BACKGROUND: Hypoglycemia in hospitalized adults without diabetes is often considered incidental, but acute infection may disrupt glucose homeostasis. AIMS: To determine the incidence of hypoglycemia in adults without diabetes hospitalized for acute infection and evaluate its association with clinical outcomes. METHODS: We conducted a retrospective cohort study of 64,871 adults without diabetes admitted for acute infection between 2010 and 2025 at a tertiary academic center. Patients with diabetes or prior glucose-lowering therapy were excluded. Mild hypoglycemia was defined as 55-69 mg/dL an

Historically Black Colleges and Universities as anchor institutions for the rural pharmacy workforce.

BACKGROUND: Rural U.S. communities face persistent pharmacist shortages and pharmacy closures that restrict access to medications, chronic disease management, and other essential care. Pharmacy workforce scholarship has paid little attention to Historically Black Colleges and Universities (HBCUs), although they educate a disproportionate share of Black pharmacists, several are located in or near the rural South, and their missions emphasize service to underserved communities. Rural workforce research rarely examines race, and HBCU health professions research rarely examines rural practice, lea

De-diagnosis and the problem of revisability in adult ADHD and autism.

Recorded diagnoses of attention-deficit/hyperactivity disorder (ADHD) and autism in adults have risen sharply across high-income health systems, and demand for assessment has risen faster still, while the best available population estimates of prevalence appear to have moved comparatively little. This paper treats that divergence as an analytic problem. We argue that adult neurodevelopmental diagnosis has undergone a threefold transformation: it has become demand-led and co-produced rather than professionally imposed, identity-conferring rather than merely descriptive, and self-reinforcing rat

Perceived HIV-related stigma, mental health conditions and sexualized drug use among men who have sex with men living with HIV in Chongqing, China: a cross-sectional study.

BACKGROUND: Sexualized drug use (SDU) has become a significant public health concern among men who have sex with men (MSM) worldwide. While much of the existing research has focused on the association of SDU with high-risk sexual behaviors and the risk of human immunodeficiency virus (HIV) infection, relatively little attention has been paid to the broader range of sociodemographic, behavioral, and psychosocial factors that may be associated with SDU. This study aimed to investigate these correlates of SDU among MSM living with HIV. METHODS: Between June 2022 and June 2023, MSM living with HI

Evaluating disparities in counseling and resources for chemotherapy-induced hair loss: qualitative analysis of a survey-based approach.

PURPOSE: Chemotherapy-induced alopecia (CIA) is a distressing side effect of cancer treatment that significantly impacts patients' quality of life. Despite the availability of interventions such as scalp cooling and topical treatments, many patients report inadequate counseling and limited access to these resources. This study aimed to evaluate disparities in CIA-related counseling and resource access, with particular attention to how socioeconomic and demographic factors shape the patient experience. METHODS: We conducted a mixed-methods, cross-sectional survey study of 101 adults who had re

Ethics of Telerehabilitation in Supportive Cancer Care Through a Responsible Innovation Lens.

In recent years, remote exercise services ("telerehabilitation") have been increasingly explored and implemented, including in supportive cancer care, on the promise of greater convenience, access independent of geographic location, reduced infection risk, and cost savings. Ethical commentary on telehealth in general is substantial, but sustained discussion of questions specific to telerehabilitation remains comparatively sparse. As contributors to PREFERBALE-II, a European Union-funded project evaluating live-remote exercise for people recovering from cancer treatment, we sought practical gui

Mortality and healthcare services in Pennsylvania state prisons following extreme ambient temperatures.

Extreme temperatures pose severe risks for population health and healthcare systems. However, little research has examined implications for incarcerated populations, who may be particularly vulnerable given their limited control over their thermal environments. Here, we show that among people imprisoned in Pennsylvania (2000-2023), both warmer and cooler temperatures were significantly associated with higher rates of mortality, transfers to outside medical facilities and prison infirmary admissions, relative to outcome-specific reference minimum threshold temperatures. As temperature extremes
