Emergency Department Visits for Minor Illnesses Among Recent Refugee and Immigrant Children.

IMPORTANCE: Health care resource constraints across North America are leading to decreased access to primary care, particularly for newcomers. In Canada, several pathways to permanent residency shape early navigation to the publicly funded health care system. OBJECTIVE: To explore access to primary care by analyzing the percentage of all minor illness visits seen in an emergency department (ED) in recently arrived refugee and immigrant children compared with their Ontario-born peers. DESIGN, SETTING, AND PARTICIPANTS: This population-based cohort study took place in Ontario, Canada, and incl

Implications of Federal Nutrition Policy Changes for Maternal and Child Health.

Signed into law on July 4, 2025, the One Big Beautiful Bill Act (OBBBA) fundamentally changes the Supplemental Nutrition Assistance Program (SNAP), the largest food assistance program in the United States and a key support for maternal and child food and nutrition security. The OBBBA reduces SNAP eligibility, expands work requirements, eliminates nutrition education, prevents future benefit increases, and shifts significant costs to states. Combined, these changes may have a direct negative effect on women and children who rely on SNAP for adequate nutrition and may also reduce their access to

Ferritin Reference Curves and Optimal Curves in Preadolescent Children.

IMPORTANCE: Ferritin levels are commonly used to assess iron status. For interpretation, clinicians traditionally rely on age-partitioned reference intervals (RIs), which report 2.5th (lower) and 97.5th (upper) percentiles. OBJECTIVES: To estimate sex-specific continuous ferritin reference curves (RCs) and optimal curves (OCs) for preadolescents; to create age- and sex-specific RIs and optimal intervals (OIs); to interpret estimates alongside ferritin thresholds for iron deficiency; and to develop an interactive web-based computational tool. DESIGN, SETTING, AND PARTICIPANTS: This cross-sect

Health system stakeholders' perceptions of religious and socio-cultural influences on measles vaccine hesitancy in Madagascar: a qualitative study.

BACKGROUND: In Madagascar, measles remains a leading cause of infant mortality, and recurrent epidemics highlight the urgent need to strengthen vaccination strategies. Despite regular mass immunisation efforts, such as the national campaign in October 2024, the effectiveness of these programmes is often undermined by complex non-medical factors. This study aims to qualitatively explore the specific religious and socio-cultural beliefs and perceptions that act as barriers to the acceptance of the measles vaccine across the country. METHODS: This qualitative exploratory study was integrated int

SPIRIT-Children and Adolescents (SPIRIT-C) 2026 extension statement: enhancing the reporting and usefulness of paediatric randomised trial protocols.

Randomised controlled trial (RCT) protocols contain essential details needed to understand and evaluate the trial's planned aims, design, data collection methods, monitoring, data analysis, and participants' safety. However, key information is often omitted from paediatric RCT protocols, including details on dose adjustments of interventions based on age, body surface area, or weight; developmental appropriateness of trial outcome measures and processes; or strategies to minimise participants' anxiety and pain. These deficiencies impair the planning and implementation of potentially impactful

CONSORT-Children and Adolescents (CONSORT-C) 2026 extension statement: enhancing the reporting and impact of paediatric randomised trials.

Appropriately designed, conducted, and reported randomised controlled trials (RCTs) in children and adolescents inform treatment and health-care decisions made by young people, families, researchers, clinicians, regulators, funders, policy makers, and other interest holders. To critically evaluate, interpret, and apply trial results, readers require access to a complete and transparent report of what was planned, done, and found, taking unique considerations specific to children and adolescents into account. Harmonised guidance based on evidence and consensus is needed to optimise standardised

"Actionable" risk for preterm birth: patterns and prediction in California singleton births 2016-2020.

BACKGROUND: Preterm birth (PTB, < 37 weeks of gestation) is the leading cause of child mortality in the United States (U.S.) and worldwide, and has substantial short- and long-term health consequences for mothers and infants. Each year, > 350,000 infants in the U.S. are born preterm, and rates continue to rise in parallel with maternal risk factors such as hypertension, diabetes, anemia, asthma, and mental health conditions. Evidence-based interventions exist for many of these conditions and are associated with improved pregnancy outcomes, including low-dose aspirin for preeclampsia prevention

Co-designing a study on a health interventions with Myanmar migrant workers to address work-related musculoskeletal disorders in Thailand's seafood industry.

BACKGROUND: Myanmar migrant workers in Thailand's seafood processing industry develop work-related musculoskeletal disorders from tasks such as repetitive shrimp peeling and standing for extended periods. These workers remain largely unreached by standard health interventions due to language barriers and cultural isolation. Developing effective study for this vulnerable population requires meaningful involvement of workers in the study design process, recognizing their experiential expertise while acknowledging the distinct roles of researchers and community members. This paper documents how i

Study protocol - the Australian National Child hearing Health Outcomes Registry (ANCHOR): collecting and connecting national data into a child deafness Learning Health System.

BACKGROUND: Despite early identification of deafness through universal newborn hearing screening, deaf and hard of hearing children can still face inequitable challenges in their development and life opportunities. Large scale registries, collecting standardised information on all individuals in a population with the relevant condition, can reveal variations in practices, processes, and outcomes, and identify targets for improvement. For childhood deafness, where care delivery spans multiple service sectors, data linkage is the only feasible way to bring data together. Data linkage also minimi

Quantifying the fatal and non-fatal burden of disease associated with child growth failure, 2000-2023: a systematic analysis from the Global Burden of Disease Study 2023.

BACKGROUND: Child growth failure (CGF), which includes underweight, wasting, and stunting, is among the factors most strongly associated with mortality and morbidity in children younger than 5 years worldwide. Poor height and bodyweight gain arise from a variety of biological and sociodemographic factors and are associated with increased vulnerability to infectious diseases. We used data from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2023 to estimate CGF prevalence, the risk of infectious diseases associated with CGF, and the disease mortality, morbidity, and overal

Prediction of Bacteremia and Bacterial Meningitis Among Febrile Infants Aged 28 Days or Younger.

IMPORTANCE: Fever in the first month of life is often the only sign of life-threatening invasive bacterial infection, specifically bacteremia or bacterial meningitis. Most international guidelines recommend routine lumbar punctures for all febrile infants 28 days or younger to rule out bacterial meningitis. Clinical prediction rules may allow for select testing, but limited information exists on their performance to identify infants at low risk for invasive bacterial infections. OBJECTIVE: To evaluate the diagnostic accuracy of the updated Pediatric Emergency Care Applied Research Network (PE

Values and preferences of female sex workers in Zimbabwe for long-acting injectable pre-exposure prophylaxis and the dapivirine vaginal ring: results of a mixed-methods research study.

INTRODUCTION: Female sex workers (FSWs) in sub-Saharan Africa are at high risk of HIV acquisition. Here we explore the values and preferences of Zimbabwean FSW for long-acting pre-exposure prophylaxis (PrEP). METHODS: We employed mixed methods; focus group discussions (FGD) (n=15), a respondent-driven sampling (RDS) survey (n=4444) from 22 sites across Zimbabwe and a nested discrete choice experiment (DCE) (n=435) conducted in 4/22 sites in 2021. Purposively selected FSWs aged 18 or over who reported being HIV negative were eligible for inclusion in FGDs. Analysis of self-reported HIV negativ

Understanding factors influencing sustainability and sustainment of evidence-based bronchiolitis management of infants in Australian and New Zealand hospital settings: a qualitative process evaluation.

BACKGROUND: The 2017 Paediatric Research in Emergency Departments International Collaborative (PREDICT) Bronchiolitis Knowledge Translation (KT) Study, a cluster randomised trial in 26 Australasian hospitals, found targeted interventions provided over one bronchiolitis season effectively de-implemented five low-value practices (salbutamol, glucocorticoids, chest radiography, antibiotics and epinephrine) by 14.1% (adjusted risk difference, 95% CI 6.5% to 21.7%; p<0.001). A 2-year follow-up study found de-implementation was sustained. This process evaluation aimed to identify factors that influe

Global, regional, and national burden of tuberculosis and multidrug-resistant tuberculosis by HIV status, 1990-2023: a systematic analysis for the Global Burden of Disease Study 2023.

BACKGROUND: Tuberculosis (TB) is the leading global cause of death from a single infectious agent. Recent reductions in global health funding have threatened TB control, making comprehensive assessment of TB, HIV-related TB, and drug-resistant TB burdens before these disruptions essential for shaping effective responses. The WHO End TB Strategy sets targets of a 95% reduction in TB deaths and a 90% reduction in TB incidence between 2015 and 2035. Using results from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2023, this study aims to assess the burden of TB and multidr
