Thymic Composition Predicts Radiation Pneumonitis in Locally Advanced NSCLC.

INTRODUCTION: Durvalumab consolidation after concurrent chemoradiotherapy (cCRT) is standard for unresectable stage III NSCLC; however, radiation pneumonitis (RP) remains a major treatment-limiting toxicity. Lung dosimetry incompletely explains the risk of toxicity. Recent studies have established radiographic thymic health as clinically relevant in cancer, but its relationship to treatment-related toxicity remains unclear. We evaluated whether percent thymic tissue (pTT), an interpretable CT-derived measure of residual thymic tissue, is associated with severe pneumonitis after cCRT in NSCLC.

PRRC2A-mediated m6A modification on NCOA4 promotes thymic epithelial tumor progression through PKM2-induced glycolysis.

Thymic epithelial tumors (TETs) are heterogeneous neoplasms. While most early-stage cases are curable after resection, advanced or aggressive subtypes have limited treatment options and poor outcomes. The molecular mechanisms underlying their metabolic reprogramming remain poorly understood. Proline-rich Coiled-coil 2a (PRRC2A), a N6-methyladenosine (m6A) reader protein, is significantly upregulated in TET tissues and is associated with poor clinical outcomes. Multi-omics analyses and molecular experiments revealed that PRRC2A binds to m6A-modified sites within the NCOA4 mRNA, thereby stabiliz

Distinct Molecular Pathways Driving Thymic and Pulmonary Neuroendocrine Neoplasms: Organ of Origin Defines Molecular Identity and Yields CD44 as a Marker of Pulmonary Neuroendocrine Tumors.

Thymic and pulmonary neuroendocrine neoplasms (NEN) are morphologically similar but arise from different embryonic tissues. Whether this similarity extends to shared molecular programs is unclear. We compared 45 thymic and 37 pulmonary NEN by immunohistochemistry, targeted and exome sequencing, NanoString RNA profiling and mass spectrometry-based proteomics. The RNA and proteomic profiles of pulmonary and thymic neuroendocrine tumors (LuNET and TNET) were clearly different, while the profiles of neuroendocrine carcinomas of both organs (LuNEC and TNEC) were largely overlapping. A subgroup of T

Immune remodeling after thymectomy in patients with anti-AChR antibodies.

BACKGROUND: Thymectomy is an established treatment for thymoma-associated myasthenia gravis (MG). However, a subset of patients develop new-onset MG after surgery. Although thymectomy affects immune homeostasis, its comprehensive impact on the peripheral immune system remains unclear, particularly at the single-cell transcriptomic level. Therefore, we aimed to characterize immune remodeling following thymectomy in anti-acetylcholine receptor (AChR) antibody-positive patients without overt MG. METHODS: Peripheral blood mononuclear cells were collected from three anti-AChR antibody-positive pat

Thoracic Surgery Oncology Quality Indicators (Part 2: Thymic Epithelial Tumors and Esophageal Cancer).

This article outlines key performance indicators for thymic epithelial tumors and esophageal cancer, emphasizing accurate staging, complete resection, and multidisciplinary care to optimize outcomes. For thymoma, priorities include computed tomography-based preoperative assessment, cautious use of biopsy, total thymectomy when feasible, selective use of neoadjuvant or adjuvant therapy, and 10-year surveillance. Evidence supports esophagectomy in high-volume centers with adequate lymph node harvest, synoptic pathology reporting and use of enhanced recovery after surgery protocols. Finally, moni

Immunopathological Profile of Patients with Thymic Epithelial Tumour and Good Syndrome in Advanced Stage.

PURPOSE: Thymic epithelial tumors (TETs) are associated with Good Syndrome (GS), a secondary immunodeficiency characterized by hypogammaglobulinemia, B-cell lymphopenia, and recurrent infections. This study investigated the immunological profile of TET patients to identify immune alterations associated with GS, independently of autoimmune diseases (AD) and disease stage. METHODS: Seventy patients with TETs were stratified according to GS status (GS+/GS-), AD status (AD+/AD-), and disease stage (advanced disease, IVA/B, or no evidence of disease, NED). Serum immunoglobulins, peripheral immune

Differential activation of proximal complement pathways and therapeutic potential of CD55 overexpression in intracerebral hemorrhage.

The complement cascade contributes to secondary brain injury after intracerebral hemorrhage (ICH). However, research remains scarce on the initiation characteristics of proximal complement pathways and the expression of complement inhibitors in the hemorrhagic brain. In this study, we used collagenase VII-S, thrombin, and hemoglobin injections into mouse brains to establish an ICH model and stimulate the coagulation cascade and hemolysis. Our findings highlighted notable discrepancies in the gene and protein expression profiles of proximal complement components and complement inhibitors within

Altered Thymopoiesis in Thymoma Is Associated with Defects in Negative Selection Machinery and Decreased Treg Abundance.

Thymomas are rare thymic epithelial tumors harboring a high but variable proportion of lymphocytes without obvious function. Autoimmunity is present in one third of patients at diagnosis. In this study, we performed a phenotypic, single-cell RNA sequencing and spatial analysis of both the T cells and tumoral cells. T cells at all stages of T-cell development-from immature to mature-were present in the tumor, suggesting active thymopoiesis in thymoma. However, data generated through multiple approaches suggested a maturation blockade at the double-negative to double-positive stage of T-cell dev

OPG-Producing B Cells and RANKL-Expressing T Cells Define Immune Signatures Predictive of Bone Metastases in Breast Cancer.

UNLABELLED: Bone metastases represent a common and devastating complication of breast cancer; however, the immune determinants of skeletal colonization, particularly the role of adaptive lymphocyte subsets, have only recently been elucidated. In this study, we identified a nonclassical regulatory role of CD19+ B cells in the modulation of bone remodeling and breast cancer metastatic progression. Using murine models of metastatic 4T1 and nonmetastatic 67NR mammary tumors, we show that 67NR-bearing mice exhibit increased trabecular bone mass, associated with osteoprotegerin (OPG) secretion by CD

Optimizing Diagnosis and Surgery for Pure Anterior Mediastinal Cysts: Insights from a Single-Centre Study.

OBJECTIVES: Pure anterior mediastinal cysts (pAMC) are uncommon and typically benign lesions. In contrast to solid anterior mediastinal tumours, there are no standardized diagnostic or management guidelines, despite a small but clinically significant risk of malignancy. The goal of this study was to identify preoperative factors associated with malignancy in order to develop a clinical algorithm to guide the diagnosis and management of pAMC. METHODS: This retrospective single-centre study was conducted at Toulouse University Hospital and included patients diagnosed with pAMC between January 2

Practical implications of the World Health Organization Reporting System for Lymph Node, Spleen, and Thymus Cytopathology: Categories and ancillary testing for subtyping of hematolymphoid tumors on FNA biopsy cytopathology using a pattern-based approach.

The objective of the recently published World Health Organization Reporting System for Lymph Node, Spleen, and Thymus Cytopathology (WHO system) is to standardize the diagnostic approach to fine-needle aspiration biopsies of hematolymphoid tissues. By categorizing specimens into five diagnostic groups-inadequate/insufficient/nondiagnostic, benign, atypical, suspicious for malignancy, and malignant-the system provides a structured framework that enhances diagnostic clarity and facilitates communication between cytopathologists and clinicians. Each category is associated with a defined risk of m

Neoadjuvant Glucocorticoids Facilitate Resection in Advanced B-Type Thymomas: A Propensity-Matched Analysis of Surgical and Survival Outcomes.

BACKGROUND: Locally advanced B-type thymomas frequently present technical challenges for R0 resection. Given the limitations of conventional neoadjuvant therapies, this study investigated glucocorticoids (GCs) as a potential downstaging strategy to improve surgical feasibility. METHODS: A propensity score-matched analysis was conducted in patients with Masaoka-Koga stage III-IV B-type thymomas (2017-2024). The intervention cohort (GC + Surgery, n = 36) received oral prednisone acetate (0.6 mg/kg/d, maximum dose 50 mg/d for 2-4 weeks) followed by surgery, while matched controls (Surgery, n = 3

Gu-Jin-Xiao-Ji Decoction potentiates anti-PD-1 immunotherapy in lung cancer by inhibiting the PI3K/AKT/NF-κB/PD-L1 axis.

This study aims to elucidate the molecular mechanisms by which the traditional Chinese medicine Gu-Jin-Xiao-Ji Decoction (GJXJD) enhances the anti-tumor efficacy of PD-1 inhibitors in lung cancer and to characterize its major chemical components. The major components of GJXJD were identified and quantitatively analyzed using ultra-performance liquid chromatography coupled with tandem mass spectrometry (UPLC-MS/MS). A mouse Lewis lung carcinoma (LLC) subcutaneous xenograft model was established, and high-throughput transcriptome sequencing was performed. Combined treatment with GJXJD and a PD-1

Effect of the Extent of Thymus Excision on the Risk of Subsequent Cancer Development.

OBJECTIVES: Thymus removal can alter immune function and potentially influence cancer development. However, the relationship between the extent of thymus removal and postoperative cancer development remains underexplored. This study aimed to identify risk factors for cancer development post-thymectomy, with particular focus on the extent of thymic resection. METHODS: We reviewed 254 patients who underwent thymectomy between 2005 and 2021. Patients with myasthenia gravis, combined resection of adjacent organs, thymic cancer, or tumour recurrence were excluded. Clinical characteristics and post

Serum HDL subfractions are impaired by COVID-19 vaccine in patients with thymic epithelial tumors.

Patients with thymic epithelial tumors (TET), who have a high risk of developing immunological disorders such as immunodeficiency and autoimmunity, were included among frail patients eligible for the COVID-19 vaccinal program. We previously found an increase of serum biomarkers of inflammation in 25 of 44 (56,8%) TET patients after the second dose (T2) of mRNA vaccine (BNT162b2 from Pfizer-BioNTech), although none developed immune-related complications at this time point. In this study we have investigated the metabolic process of high-density lipoproteins (HDL) that are among the main players
