Neoadjuvant Glucocorticoids Facilitate Resection in Advanced B-Type Thymomas: A Propensity-Matched Analysis of Surgical and Survival Outcomes.

BACKGROUND: Locally advanced B-type thymomas frequently present technical challenges for R0 resection. Given the limitations of conventional neoadjuvant therapies, this study investigated glucocorticoids (GCs) as a potential downstaging strategy to improve surgical feasibility. METHODS: A propensity score-matched analysis was conducted in patients with Masaoka-Koga stage III-IV B-type thymomas (2017-2024). The intervention cohort (GC + Surgery, n = 36) received oral prednisone acetate (0.6 mg/kg/d, maximum dose 50 mg/d for 2-4 weeks) followed by surgery, while matched controls (Surgery, n = 3

Gu-Jin-Xiao-Ji Decoction potentiates anti-PD-1 immunotherapy in lung cancer by inhibiting the PI3K/AKT/NF-κB/PD-L1 axis.

This study aims to elucidate the molecular mechanisms by which the traditional Chinese medicine Gu-Jin-Xiao-Ji Decoction (GJXJD) enhances the anti-tumor efficacy of PD-1 inhibitors in lung cancer and to characterize its major chemical components. The major components of GJXJD were identified and quantitatively analyzed using ultra-performance liquid chromatography coupled with tandem mass spectrometry (UPLC-MS/MS). A mouse Lewis lung carcinoma (LLC) subcutaneous xenograft model was established, and high-throughput transcriptome sequencing was performed. Combined treatment with GJXJD and a PD-1

Effect of the Extent of Thymus Excision on the Risk of Subsequent Cancer Development.

OBJECTIVES: Thymus removal can alter immune function and potentially influence cancer development. However, the relationship between the extent of thymus removal and postoperative cancer development remains underexplored. This study aimed to identify risk factors for cancer development post-thymectomy, with particular focus on the extent of thymic resection. METHODS: We reviewed 254 patients who underwent thymectomy between 2005 and 2021. Patients with myasthenia gravis, combined resection of adjacent organs, thymic cancer, or tumour recurrence were excluded. Clinical characteristics and post

Serum HDL subfractions are impaired by COVID-19 vaccine in patients with thymic epithelial tumors.

Patients with thymic epithelial tumors (TET), who have a high risk of developing immunological disorders such as immunodeficiency and autoimmunity, were included among frail patients eligible for the COVID-19 vaccinal program. We previously found an increase of serum biomarkers of inflammation in 25 of 44 (56,8%) TET patients after the second dose (T2) of mRNA vaccine (BNT162b2 from Pfizer-BioNTech), although none developed immune-related complications at this time point. In this study we have investigated the metabolic process of high-density lipoproteins (HDL) that are among the main players

Advances in the Basic Sciences in Thoracic Oncology in the Last 20 Years and Their Translational Impact.

In this article, we summarize the progress made in lung cancer, mesothelioma, and thymic epithelial malignancy during the period 2005-2025. We enlisted multidisciplinary thoracic oncologic experts to tackle this task. The main focus of the article concerns how basic science with translational impact has improved the diagnosis, prognosis, and therapy of these cancers. During the past 20 years, we have come to the realization that "lung cancer" is a name that encompasses tumors with vast histologic, immune, and genomic differences that in turn influence prognosis and response to therapy. For exa

Iridium(III) Complexes for Lung Cancer Therapy: Recent Advances and Case Studies in DNA Targeting, Antioxidant Activity and Cytotoxicity.

Lung cancer remains a leading cause of cancer-related mortality, and resistance to platinum-based chemotherapy necessitates the development of alternative metal-based therapeutics. In this study, two iridium(III) complexes were synthesized and systematically investigated for their photophysical and biological properties. Spectroscopic characterisation confirmed effective ligand coordination and metal-to-ligand charge transfer behaviour. Both complexes exhibited significant interaction with calf thymus DNA, with intrinsic binding constants of 5.01 × 104 M-1 and 1.08 × 104 M-1, and showed strong

Fe metal-organic framework-derived heterojunction for metabolic diagnosis of thymic epithelial tumor.

Thymic epithelial tumors (TETs), rare yet clinically significant malignancies, face diagnostic challenges due to their occult presentation and lack of noninvasive risk-stratification tools, leading to systemic overtreatment and poor prognoses for high-risk subtypes. To address this unmet need, we developed a Fe3O4@Fe metal-organic framework heterojunction-enhanced laser desorption ionization mass spectrometry (LDI MS) platform for the efficient analysis of serum metabolic fingerprints (SMFs). Engineered through gradient pyrolysis, this nanomaterial synergizes ultraviolet absorption and phototh

Salvage Surgery Following Definitive Chemoradiotherapy and Immune Checkpoint Inhibitor Therapy for Locally Advanced Thymic Carcinoma.

Surgical resection improves prognosis for thymic carcinoma. Recent phase II trials have indicated the efficacy of immune checkpoint inhibitor therapy in unresectable disease. Here, we report a case of salvage surgery following immune checkpoint inhibitor therapy for an initially unresectable, locally advanced thymic carcinoma in a 67-year-old woman. Computed tomography revealed an anterior mediastinal mass and enlarged anterior mediastinal lymph nodes, diagnosed as an unresectable thymic epithelial tumour, due to suspected invasion into the left main pulmonary artery. Thoracoscopic biopsy conf

Integrative Multiomic Classification Reveals Distinct Origins and Evolutionary Trajectories of Thymic Epithelial Tumors.

UNLABELLED: Thymic epithelial tumors (TET), comprising various histologic types of thymomas and thymic carcinomas, originate from thymic epithelial cells (TEC). Each histologic type is typically associated with a distinct immune cell composition and clinical manifestation. A better understanding of the cellular origins and molecular pathways underlying this heterogeneity is needed to improve patient stratification and treatment. In this study, we conducted an integrated genomic and transcriptomic analysis of 124 thymomas and 13 thymic carcinomas, including 20 newly sequenced cases, combined wi

TCRγ constant usage tunes human γδ T cell antigen sensitivity, thymic programming, and peripheral function.

Activation in T cells through their antigen receptors has been closely tied to the strength of recognition of their cognate antigens, dictated by the variable complementarity-determining region loops. We show that, in the human gamma delta (γδ) T cell receptors (TCRs), the gamma constant domains modulate activation intensity independent of variable region antigen recognition. Using single-cell RNA sequencing data, we demonstrate that Cγ usage in vivo corresponds with distinct phenotypes, with cells using Cγ1 having a more differentiated cytotoxic effector phenotype in the thymus and higher gra

Emerging Strategies in the Management of Thymomas and Thymic Carcinomas.

Thymic tumors are exceedingly rare, and most available treatment data come from pooled retrospective analyses. The cornerstone of management is resection, with the use of systemic therapy and radiation therapy (RT) as tools to facilitate surgery and prevent recurrence, or as primary management in unresectable or metastatic disease. Despite the challenges of managing such rare entities, a number of advances have occurred in surgical techniques, systemic therapy, and RT indications and technologies. There is a growing shift toward minimally invasive surgical techniques and the use of surgery to

Exosome-Transferred STAT3 from Cancer-Associated Fibroblasts Expedites Thymic Carcinoma Malignant Progression and M2 Macrophage Polarization Through Transcriptionally Activating KLHL5.

Thymic carcinoma (THYM) is a mediastinal malignant tumor that seriously endangers human health. Cancer-associated fibroblasts (CAFs) are implicated in many cancers, but their role in THYM remains unclear. Western blot and reverse transcription-quantitative polymerase chain reaction were used to analyze expression levels. Exosomes were isolated and characterized by transmission electron microscopy and nanoparticle tracking analysis, and their uptake was assessed using PKH26 labeling. Cell functions were evaluated with cell counting kit-8, colony formation, and Transwell assays, while M2 macroph

Subtype-Dependent Expression Patterns of Core Hippo Pathway Components in Thymic Epithelial Tumors (TETs): An RT-qPCR Study.

Background/Objectives: Thymic epithelial tumors (TETs) are rare, histologically heterogeneous neoplasms lacking robust molecular biomarkers. Hippo pathway dysregulation-driving YAP/TEAD-dependent transcription-has been implicated across cancers, but transcript-level data in TETs are limited. Methods: We profiled 26 (23 TETs and three normal thymus) formalin-fixed and paraffin-embedded (FFPE) specimens by SYBR real-time quantitative polymerase chain reaction (RT-qPCR) across World Health Organization (WHO) subtypes, focusing on core Hippo components YAP1, TEAD4, MST1, SAV1, LATS1, and MOB1A. Antiproliferative and proapoptotic effects of Dalbergia odorifera flavonoids in cSCC.

Cutaneous squamous cell carcinoma (cSCC) lacks effective and well-tolerated pharmacological options for long-term management, highlighting the need for multi-target natural agents. In this study, we hypothesized that a flavonoid-rich extract from Dalbergia odorifera exerts (DOE) anti-cSCC effects by regulating apoptosis-related pathways. To test this hypothesis, we employed an integrated strategy combining UPLC-based chemical profiling, network pharmacology prediction, and in vitro and in vivo validation. UPLC analysis identified five major flavonoids in DOE-Luteolin, Naringenin, Butein, Liqui

Abscopal effect induced by conventional fractionated radiotherapy following anti-PD-1 immunotherapy in pulmonary metastatic thymic squamous cell carcinoma: a case report and literature review.

BACKGROUND: Radiotherapy (RT) can enhance immune control of distant metastases, known as the abscopal effect (AE), but it doesn't significantly alter the immunosuppressive tumor microenvironment (TME), resulting in low AE incidence. Combining RT with immunotherapy (especially anti-PD-1/PD-L1 agents) has increased AE occurrences, though questions remain about this approach, particularly in tumors with low immunogenicity such as thymic squamous cell carcinoma (TSCC). CASE DESCRIPTION: A 73-year-old woman with advanced TSCC and multiple metastases experienced disease progression after two therap
