Closing the quality gap: promoting evidence-based breastfeeding care in the hospital.

Evidence shows that hospital-based practices affect breastfeeding duration and exclusivity throughout the first year of life. However, a 2007 CDC survey of US maternity facilities documented poor adherence with evidence-based practice. Of a possible score of 100 points, the average hospital scored only 63 with great regional disparities. Inappropriate provision and promotion of infant formula were common, despite evidence that such practices reduce breastfeeding success. Twenty-four percent of facilities reported regularly giving non-breast milk supplements to more than half of all healthy, fu

Influenza vaccination in the emergency department: are our patients at risk?

BACKGROUND: Influenza is responsible for substantial morbidity and mortality annually. Vaccination strategies target high-risk patients. The percentage of Emergency Department (ED) patients at high risk is largely unknown. OBJECTIVES: The percentage of patients at high risk for influenza was determined and compared to the United States (US) population. METHODS: Medical records of a convenience sample of all patients presenting to the ED of a university-affiliated hospital in Tucson, Arizona from February 1-7, 2006 were reviewed (n = 1359). Patients were stratified as 1) at high risk for infl

The cost of community-managed viral respiratory illnesses in a cohort of healthy preschool-aged children.

BACKGROUND: Acute respiratory illnesses (ARIs) during childhood are often caused by respiratory viruses, result in significant morbidity, and have associated costs for families and society. Despite their ubiquity, there is a lack of interdisciplinary epidemiologic and economic research that has collected primary impact data, particularly associated with indirect costs, from families during ARIs in children. METHODS: We conducted a 12-month cohort study in 234 preschool children with impact diary recording and PCR testing of nose-throat swabs for viruses during an ARI. We used applied values t

Growth monitoring and promotion: review of evidence of impact.

UNLABELLED: [Table: see text] SUMMARY: The rationale for growth monitoring and promotion is persuasive but even in the 1980s the appropriateness of growth monitoring programmes was being questioned. The concerns centred largely around low participation rates, poor health worker performance and inadequacies in health system infrastructure that constrained effective growth‐promoting action. More recently there has been a call for a general review of the impact of large‐scale growth monitoring and promotion programmes to determine if the investments are justified. The launch of the new World Hea

Supraventricular tachycardia in the pediatric primary care setting: Age-related presentation, diagnosis, and management.

As many as 1 in 250 children experience supraventricular tachycardia (SVT), but its presentation is often vague and its symptoms mistakenly attributed to other common pediatric conditions. If SVT is correctly identified in a timely manner, most children will go on to live normal healthy lives. SVT is not covered in depth in most pediatric advanced practice nursing programs, but because of its prevalence, it should be familiar to all pediatric primary care providers. This article reviews common mechanisms of SVT and their age-related presentation, diagnosis, and management. A case study of an 8

Optimal positions for the release of primitive neonatal reflexes stimulating breastfeeding.

BACKGROUND: Despite widespread skills-teaching, 37% of UK mothers initiating breastfeeding stop by six weeks suggesting a need to reappraise current support strategies. Rooting, sucking and swallowing have been studied extensively but little is known about the role other primitive neonatal reflexes (PNRs) might play to support breastfeeding. AIMS: To describe and compare PNRs observed during feeding, investigating whether certain feeding behaviours and positions, collectively termed Biological Nurturing, (BN) are associated with the release of those reflexes pivotal in establishing successful

Trends in the health of young children in California.

When it comes to school readiness, children's health matters. Healthy children are better prepared to learn, concentrate and develop the skills needed to succeed in school--and throughout life. Although good health supports learning among children of all ages, it is particularly important during early childhood when a crucial period of rapid physical and emotional growth occurs between birth and age five. Therefore, providing children early on with an environment conducive to healthy development and learning lays the foundation for a successful future. Not all children, however, have this oppo

Known-groups analysis of the harris infant neuromotor test.

BACKGROUND AND PURPOSE: The Harris Infant Neuromotor Test (HINT) is a screening tool designed to identify neuromotor or cognitive/behavioral concerns in infants who are healthy or at high risk between the ages of 3 and 12 months. The purpose of this study was to determine whether the HINT could distinguish between infants at high risk and infants at low risk for neuromotor delays. SUBJECTS AND METHODS: Following HINT administration by trained health care professionals, scores were compared for 54 high-risk infants and 412 low-risk infants with a t test. RESULTS: Mean HINT scores for infants

[Activities of medical practitioners in nursery schools for children under 6 years-old: the expectations of the facilities' directors].

This work aims to describe the activities of nurseries' medical practitioners and the expectations of the nurseries' supervisors. It is based on the analysis of 139 questionnaires received from nursery schools' managers from the Pays de Loire region in 2005. Ninety percent of the nurseries that accommodate children less than six years-old, do employ a medical practitioner. She or he is often a salaried paediatrician with a full-time job or clinical practice, who consults twice a month for two hours and forty minutes in each nursery which employs him/her. Her/his main activity consists of condu

Healthy Families America effectiveness: a comprehensive review of outcomes.

This paper reviews 33 evaluations of Healthy Families America sites, with emphasis on 15 studies that include a control or comparison group. Outcome domains include child health and development, maternal life course, parenting, and child maltreatment. Parenting outcomes (e.g., parenting attitudes) show the most consistent positive impacts. Mixed results in other domains indicate the need for in-depth research to identify factors associated with better outcomes. Several factors that may contribute to differences in outcomes are discussed, including site implementation and quality, differences i

Lactation resources for clinicians.

Breastfeeding is widely acknowledged as the optimal infant feeding choice. However, many clinicians working in maternal and child health do not receive adequate university preparation to support breastfeeding. Knowledge and skill are most often gained through on-the-job and personal experience. Myriad resources exist to support clinicians in delivering the best quality care to breastfeeding clients. Among the available resources are policies and protocols of professional organizations, governmental, and health advocacy groups. Breastfeeding-focused academic and continuing education programs ar

REACH-Meharry community-campus partnership: developing culturally competent health care providers.

An important national health care effort is elimination of racial and ethnic disparities in six specific conditions: infant mortality, cancer screening and management, cardiovascular disease, diabetes, human immunodeficiency virus infection, and child and adult immunizations. To address this concern, several health entities in Nashville, Tennessee responded to a grant initiative from the Centers for Disease Control and Prevention to develop a Racial and Ethnic Approaches to Community Health (REACH) demonstration project. The resulting award is the Nashville REACH 2010 Project, charged to devel

[The primary prophylaxis of neural tube defects in the Podkarpacian region].

The paper presents results of investigations carried out in years 2002/ 2003 on prevalence of primary prophylaxis of neural tube defects among children 0 to 3 years old. Subjects were 222 healthy mothers residing in the Podkarpacian region. The results indicate that primary prophylaxis of neural tube defects is propagated quite poorly in this region. A dominant role in its spreading play physicians and at the same time, other sources of knowledge are marginal. There is a strong need for intensification of activities of this kind.

Physician risk attitudes and hospitalization of infants with bronchiolitis.

OBJECTIVES: To determine whether the risk attitudes of pediatric emergency physicians are related to the likelihood that otherwise healthy infants with bronchiolitis will be admitted for inpatient care. METHODS: Risk aversion and discomfort with diagnostic uncertainty were assessed among 46 pediatric emergency physicians from three hospitals participating in the Child Health Accountability Initiative. Study physicians managed 397 otherwise healthy infants ages 0 to 12 months presenting to their hospital emergency departments with bronchiolitis. Mean risk aversion and discomfort with diagnosti

Who's calling the shots? Pediatricians' adherence to the 2001-2003 pneumococcal conjugate vaccine-shortage recommendations.

BACKGROUND: A national shortage of heptavalent pneumococcal conjugate vaccine (PCV7) occurred from September 2001 through May 2003. In December 2001 and January 2002, the Advisory Committee on Immunization Practices and the American Academy of Pediatrics (AAP) issued PCV7-shortage recommendations, emphasizing that all health care providers decrease the number of doses for healthy children so that more children could receive some PCV7. OBJECTIVES: We assessed (1) how the PCV7 shortage affected pediatricians, (2) whether children in the public and private sectors were vaccinated differently dur

Overprescribing of antibiotics to children in rural Vietnam.

100 children (aged 1-6 y), who received an antibiotic prescription after health examination in the Bavi health care system (79 children at a district hospital, 21 at a community health centre) were analysed regarding antibiotics prescribed in relation to serum C-reactive protein (CRP). A control group consisted of 35 healthy children. Children who had been treated with antibiotics within 1 week prior to the study were excluded in the community health centre and control groups. Capillary blood samples were collected and the serum CRP concentration was analysed. A questionnaire interview with th

Public health policies as guides for local public policies: the experience of Sobral-Ceará, Brazil.

The accelerated urbanisation process that Brazil has gone through in the last 50 years has given rise to daunting challenges for public managers, especially in terms of local public policy management for the building of "healthy cities". In Sobral, a municipality of 173,000 inhabitants in Ceará in the North-eastern region of Brazil, a number of municipal policies were initiated beginning in 1997, many in partnership with the federal and state governments. They were inspired by the vision of a healthy and equitable city and were marked by strategic planning and the implementation of intersector

Scaling up integrated management of childhood illness to the national level: achievements and challenges in Peru.

This paper presents the first published report of a national-level effort to implement the Integrated Management of Childhood Illness (IMCI) strategy at scale. IMCI was introduced in Peru in late 1996, the early implementation phase started in 1997, with the expansion phase starting in 1998. Here we report on a retrospective evaluation designed to describe and analyze the process of taking IMCI to scale in Peru, conducted as one of five studies within the Multi-Country Evaluation of IMCI Effectiveness, Cost and Impact (MCE) coordinated by the World Health Organization.
