Breastfeeding after early-onset breast cancer: a scoping review.

OBJECTIVE: To synthesize the current evidence examining the breastfeeding experiences of women with a history of early-onset breast cancer. DATA SOURCES: A scoping review of the literature was conducted using the following databases: CINAHL, PubMed, Psych Info and Web of Science. Citations of relevant articles were also reviewed. STUDY SELECTION: We included articles published in English from 2000 to 2023 that were primary empirical full-text articles with objectives examining breastfeeding in women with a history of breast cancer under the age of 45. Thirty-seven full-text articles were ass

Breast cancer screening for persons experiencing homelessness.

BACKGROUND: Primary care providers (PCPs) care for their patients in part by helping them to follow cancer screening guidelines, including screening mammography. However, completing screening and follow-up due to abnormal results can be difficult for PCPs caring for patients with barriers to care, such as persons experiencing homelessness (PEH). The large population of PEH in the US have many unmet healthcare needs, high cancer prevalence and mortality, and therefore a need for cancer screening interventions. Hence we created and evaluated a specialized breast cancer screening program for PEH.

Primary Intermammary Breast Carcinoma Presenting With Direct Sternal Invasion: A Diagnostic Pitfall of Standard Screening Mammography.

BACKGROUND Standard screening mammogram views focus on the lateral and posterior breast tissues; the view that best visualizes the intermammary area, the 90-degree lateral or cleavage view, is not typically performed. The limited field of view of mammogram increases the importance of additional imaging with CT, ultrasound, and MRI as a tool in diagnosing neoplasm in the intermammary area. To date, intermammary primary breast carcinoma has been reported only a few times in the literature, and none of these described direct sternal invasion at presentation. CASE REPORT An 85-year-old woman with

Delays in breast cancer presentation, diagnosis, and treatment in young women.

BACKGROUND: Breast cancer (BC) in young patients is often identified at later stages, which can result in poor prognosis. It is important to investigate the factors influencing care delays and how delays affect outcomes. METHODS: This study included women aged ≤40 years at diagnosis of stage 0-IV BC in the Young Women's Breast Cancer Study enrolled between 2006 and 2016. Associations between patient and tumor factors and presentation delay (first symptom to provider visit, ≥90 days), diagnostic delay (presentation to diagnosis, ≥90 days), and treatment delay (diagnosis to treatment, ≥60 days)

Psychological consequences of mammography screening recommendations: a prospective cohort study in Iran.

BACKGROUND AND PURPOSE: Breast cancer is the most common malignancy among women. Despite advances in treatment, some women still present at advanced stages of the disease. Although mammography screening can improve survival through early detection, there is ongoing debate regarding its benefits and potential harms. This study aimed to investigate the psychological consequences of breast cancer screening and the factors associated with them. METHODS: This prospective cohort study was conducted in Iran and included 320 women aged 40-69 years who attended comprehensive health centers for gynecol

Mid- and Long-Term Risk of Venous Thromboembolism Among Female Breast Cancer Survivors: A Nationwide Cohort Study.

Venous thromboembolism (VTE) is a serious complication for breast cancer (BC) survivors. We identified records of female BC survivors from the Korean Health Insurance Service database between January 1, 2010, and December 31, 2016. After applying exclusion criteria (age < 18 years, prior cancers or VTE, absence of BC surgery, or missing data), 112,522 BC survivors were included; for the comparison group, BC survivors were matched (1:3) to women without cancer history. VTE risk was evaluated with a Fine and Gray model, accounting for death as a competing risk; multiple time periods were analyze

Priorities for breast cancer risk assessment in UK women under age 50: a survey and discrete choice experiment.

BACKGROUND: National guidelines recommend English women between 30 and 49 years with moderate/high risk of breast cancer (BC) be offered screening and prevention interventions. Identification  largely relies on family history-based criteria amongst women self-presenting in primary care. We aimed to understand the preference for different potential risk assessment strategies in this population. METHODS: A cross-sectional survey including a discrete choice experiment. Risk assessments were defined using six attributes, including initiation strategy, eligibility for a full/detailed risk assessme

Comparison of NICE criteria with the BOADICEA multifactorial risk model to guide breast cancer risk assessment and referral amongst women under age 50 within primary care.

BACKGROUND: The National Institute for Health and Care Excellence (NICE) recommends that women at above-population-level risk of breast cancer (BC) are offered enhanced screening and risk-reducing interventions. Identification of these women largely relies on referral through primary care based on family history criteria. METHODS: We compared the performance of the NICE criteria with versions of the BOADICEA multifactorial risk model in identifying women who developed BC within 10 years. Analyses included 1258 women aged under 50 years with family history, reproductive, lifestyle and polygeni

No genome-wide correlations and little shared genetic architecture between reproductive life-history traits and estrogen receptor-positive breast cancer risk.

Estrogen receptor-positive breast cancer (ER + BC) is one of the most prevalent cancers, but the evolutionary processes shaping genetic variation in ER + BC risk are poorly understood. Both evolutionary life-history theory and evidence from studies of individual ER + BC risk variants suggest that increased genetic ER + BC risk is associated with faster maturation, earlier reproduction, and/or increased reproductive success (i.e. there is a trade-off), but it is unclear how well this pattern is replicated when considering the polygenic architecture of these traits after controlling for potentia

Trastuzumab Deruxtecan Resistance via Loss of HER2 Expression and Binding.

UNLABELLED: Trastuzumab Deruxtecan (T-DXd) is clinically beneficial in HER2-positive and HER2-low metastatic breast cancer. However, therapeutic resistance emerges over time in most patients, with poorly defined resistance mechanisms. Through a molecular characterization of paired patient specimens before and after T-DXd treatment, we found that 49% cases had major decreases in HER2 expression at progression, and among them, 52% exhibited complete HER2 loss. Using isogenic model systems, we demonstrated that decreases in HER2 expression corresponded to reductions in T-DXd internalization and m

ARTHROSCOPIC REPAIR OF A LARGE ROTATOR CUFF TEAR USING THE SUTURE BRIDGE TECHNIQUE AUGMENTED WITH PLATELET-RICH FIBRIN: A CASE REPORT.

OBJECTIVE: To describe the arthroscopic double-row suture bridge technique for the treatment of large rotator cuff tears augmented with platelet-rich fibrin (PRF). BACKGROUND: Large rotator cuff tears are associated with a high retear rate because of poor tendon quality and extensive tissue defects. Platelet-rich fibrin (PRF) provides a stable fibrin scaffold enriched with multiple growth factors, including transforming growth factor-beta 1 (TGF-β1), platelet-derived growth factor (PDGF), vascular endothelial growth factor (VEGF), fibroblast growth factor (FGF), insulin-like growth factor (IG

Cross-phase modulation radiation between soliton and four-wave mixing states: publisher's note.

Genetic counseling and risk assessment, given to women with a family history of breast/ovarian cancer, are regularly based on pedigree analysis. In the Ashkenazi Jewish population, hereditary breast/ovarian cancer is mainly attributed to three founder mutations, namely, 185delAG, 5382insC, and 6174delT, in BRCA1/2 genes. The overall frequency of these mutations, in the Jewish Ashkenazi population, is as high as 2.5%. Based on clinical and family history data, the results of BRCA molecular testing, in Ashkenazi individuals at risk, are appropriately anticipated in most cases. Here we report on

GP perspectives on genomics in primary care: a qualitative study on using polygenic risk scores to evaluate cancer risk.

BACKGROUND: A polygenic risk score (PRS) enables personalisation of cancer risk and supporting risk stratification for melanoma, colorectal, breast, and prostate cancers. Including a PRS in a cancer risk assessment can facilitate risk-appropriate cancer screening by incorporating an individual's age, sex, family history, and genomic test results. GPs are the likely healthcare professionals to order PRS tests and deliver results to patients within existing preventative health models. AIM: To elucidate GPs' perspectives on the use of PRSs to tailor cancer screening in the Australian primary car

False-Negative Screening and Diagnostic Mammograms in the National Mammography Database From 2010 to 2022.

BACKGROUND. False-negative (FN) mammograms typically delay breast cancer diagnoses and may impact clinical outcomes. However, systematic evaluations of FN mammograms are challenging to conduct due to the low incidence of interval cancers. OBJECTIVE. The purpose of this study was to evaluate the rates of FN screening and diagnostic mammograms in the NMD and to assess associations of FN rates with patient- and facility-level characteristics. METHODS. This retrospective study included all screening and diagnostic mammograms in the National Mammography Database (NMD) obtained from January 1, 2010,

Mammographic Density as a Mediator for Breast Cancer Risk: Pooled Analysis of Four Population-Based Case-Control Studies on Women with Screening Mammograms before Age 50.

BACKGROUND: Multiple risk factors influence breast cancer risk, but the role of percent mammographic density (PMD) in mediating these associations remains unclear. METHODS: In this pooled analysis of women who had screening mammograms before age 50 years, we assessed whether PMD mediates associations between reproductive, structural, and lifestyle factors and breast cancer risk. Causal mediation analysis was used to estimate direct and indirect effects and the proportion mediated by PMD. RESULTS: PMD partially mediated 29% of the association between breast calcifications and breast cancer ri
