B-phosphatidylethanol testing to identify hazardous alcohol use in primary health care-a game changer and a challenge for general practitioners: a qualitative study.

BACKGROUND AND AIMS: Alcohol use disorder (AUD) and hazardous alcohol use are common but underdiagnosed in primary health care (PHC). This study aimed to explore general practitioners' (GPs') experiences and perceptions of using B-Phosphatidylethanol (PEth), a specific quantitative biomarker for alcohol use, in their clinical work with patient consultations and treatment follow-up in Swedish PHC. DESIGN, PARTICIPANTS, AND SETTING: Individual interviews were conducted with GPs and resident GPs (n 20) in Swedish PHC and analysed using qualitative content analysis. FINDINGS: The overarching the

Effect of smartphone breathalyzer feedback on willingness to drive in moderately intoxicated individuals: A randomized trial.

OBJECTIVE: The present study examined the impact of smartphone breathalyzer feedback on individuals' decisions to drive when they are moderately intoxicated. Although the legal driving limit for blood alcohol concentration (BAC) in the United States is < 0.08%, crash risk begins to increase at moderate BACs ≥ 0.04%. METHOD: We conducted a laboratory-based, randomized controlled trial in 20 adult drivers aged 21-39 with a history of binge drinking. Participants were given sex- and weight-based doses of alcohol over 90 min with a target peak BAC of 0.10%. Smartphone breathalyzer measurements we

The Relationship Between Methadone and Buprenorphine Enrollment and Overdose Prevention and Treatment Behaviors Among a Community Sample of People Who Use Opioids in Baltimore, Maryland.

BACKGROUND: Methadone and buprenorphine can reduce overdose-related mortality. Behavioral approaches can also reduce fatal overdoses. The current study examined the relationship between methadone and buprenorphine and overdose history and overdose prevention and treatment behaviors. METHODS: Between December 2022 and August 2024, 647 individuals who used opioids in the prior month enrolled in a community recruited study on overdose. Participants were administered a face-to-face survey. Key behaviors assessed included overdose recency, testing drugs for potency, ingesting drugs slowly, using f

"I don't want it in any form:" A qualitative study on black perceptions of buprenorphine in the fentanyl era.

BACKGROUND: The current phase of the US opioid overdose epidemic is marked by growing racial inequities, driven in part by unequal access to medications for opioid use disorder (MOUD) including buprenorphine. This report provides a much needed update on the racialized patterns of treatment uptake in an evolving drug use and treatment landscape. METHODS: We conducted semi-structured qualitative interviews with 28 African American people with a self-reported history of OUD who had lived in the Twin Cities (Minneapolis and Saint Paul), Minnesota for at least a year. Interviews explored participa

Synthetic Cannabinoid (Mojo)-Induced ST-Segment Elevation Myocardial Infarction: A Case Report.

Synthetic cannabinoids, such as Mojo, are increasingly used among adolescents and young adults due to their perceived legality and accessibility. These compounds exhibit significantly greater potency than natural cannabis, with full agonist activity at cannabinoid receptors, and have been associated with a range of adverse effects, including cardiovascular toxicity. The underlying mechanisms may involve sympathetic stimulation, platelet aggregation, and endothelial injury, which can precipitate acute coronary events even in otherwise healthy individuals. We report the case of a 32-year-old man

Prevalence of aggression and associated factors among inpatients with mental illness at tertiary hospitals in Southwestern Uganda.

BACKGROUND: In mental health treatment facilities around the world, aggression aimed towards medical personnel and other patients is a serious issue. Types of aggression include; verbal aggression, aggression towards property, self-harm/auto-aggression, and physical aggression. Studies show 1 in 5 patients admitted to acute mental health care wards in high-income countries commit an act of physical violence during admission. In Uganda, there is limited literature on aggression among patients with mental illness admitted in psychiatric wards in our setting. This study aimed to investigate preva

Exploring Pathways from Childhood Adversity to Substance Use in Young Adults.

Adverse childhood experiences (ACEs) are recognized risk factors for later substance use. Yet, data remain scarce-particularly regarding the differentiated effects of specific types of ACEs and their distinct associations with various psychoactive substances. The current study is one of the first in Lithuania to explore the associations between specific ACEs and psychoactive substance use in young adulthood (ages 18-29). This cross-sectional study included a total of 709 participants who completed an online survey. ACEs were measured using a combination of adapted ACEs items and the MACE quest

Exploration of novel harm reduction approaches to increase client engagement (ENHANCE): protocol for a prospective cohort study.

BACKGROUND: Syringe services programs (SSPs) reduce the risk of overdose by distributing supplies like naloxone. SSPs also support clients in meeting their basic needs via referrals to organizations providing food, housing, and healthcare. This paper describes the Exploration of Novel Harm Reduction Approaches to Increase Client Engagement (ENHANCE) Project, a prospective cohort study developed in partnership with people who use drugs that aims to characterize the influence of longitudinal engagement in SSPs on overdose risk behaviors. METHODS: The ENHANCE Project protocol was developed with

Decreased Opioid Prescriptions and Evolving Trends in Multimodal Pain Management Following Anterior Cruciate Ligament Reconstruction.

BACKGROUND: Anterior cruciate ligament (ACL) reconstruction is a common surgery, following which pain control medications are often prescribed. In recent years, efforts have been made to minimize opioids and other nonnarcotic medications as multimodal regimens evolve following such surgeries. METHODS: Opioid-naïve ACL reconstruction patients were identified from the PearlDiver M165Ortho data set. Those with a history of substance abuse were excluded. Prescriptions of pain management medications were evaluated in the 90 days following surgery per 1000 ACL reconstructions and grouped into the f

Behavioral Therapy as an Adjunct to Buprenorphine Treatment for Opioid Use Disorder: A Secondary Analysis of 4 Randomized Clinical Trials.

IMPORTANCE: Several large, randomized clinical trials have tested the efficacy of adding behavioral therapy to medical management (high-quality, low-intensity medical counseling) and buprenorphine treatment of opioid use disorder. These studies have consistently reported strong rates of treatment response overall, without a significant additive benefit of additional behavioral therapy. OBJECTIVE: To address gaps in knowledge about additional behavioral therapy for patients receiving buprenorphine, including the association of additional behavioral therapy with retention and functional outcome

Steroid-Induced Hyperglycaemia in a Patient With Possible Body Dissatisfaction: A Case Report.

Steroid use for body enhancement was historically associated with bodybuilding and skin bleaching, but now may be gaining traction as a means of non-surgical cosmetic enhancement for young women unsatisfied with parts of their body, such as the buttocks and hips. Here, the side effects of steroids, such as fluid retention and weight gain, are marketed as positive outcomes. This case report describes a young African woman who presents with persistent hyperglycaemia, frequency, dysuria, and malaise after four months of taking dexamethasone tablets to gain a curvier frame. This case offers an opp

Children's exposure to cocaine detected by hair analysis: a systematic review and meta-analysis.

BACKGROUND: Cocaine is a widespread drug of abuse to which children can also be exposed. The modes of exposure may vary depending on the age of the child. In addition to blood and urine analysis, hair analysis is currently used in some clinical contexts to investigate children's exposure to drugs of abuse, including cocaine. This systematic review aims to collect data on children's exposure to cocaine proven by hair analysis, and to understand whether the concentration of cocaine in the hair of children varies with age, potentially providing insights into the nature of their exposure. METHODS

Perinatal healthcare experiences of pregnant and parenting people with a history of substance use disorder: a qualitative study.

BACKGROUND: Clinical guidelines in the United States (U.S.) recommend a patient-centered approach to healthcare for pregnant people with substance use disorders (SUD); however, pregnant people with SUD often describe experiencing stigmatization and shame when seeking prenatal care. We explored the perspectives of pregnant and parenting people engaged with SUD treatment regarding their experiences with healthcare providers during the perinatal period to improve guidance for patient-centered care. MATERIALS AND METHODS: Using an adapted phenomenological approach, we conducted in-depth interview

Childhood sexual abuse and post-cesarean pain.

BACKGROUND: Sexual abuse before the age of 10 is reported by 2.7% of US women. Chronic pain has been linked to sexual abuse, but little is known about acute postoperative pain in those with a sexual abuse history and no history of chronic pain. We hypothesized that those who report prepubertal sexual abuse experience more pain at 7 days after hospital discharge from a cesarean delivery. OBJECTIVE: To evaluate whether patients who report prepubertal sexual abuse experience more pain 7 days after hospital discharge from a cesarean delivery. Sudden cardiac death due to isolated right ventricle thromboembolism in a young male.

Sudden cardiac death due to an isolated right ventricular thromboembolism is an uncommon cause of death. In the reported case, a healthy, asymptomatic young male with no past medical history outside of obesity and his age was found unresponsive by a family member who suspected him to be sleeping. Examination of the heart at autopsy revealed a large serpiginous, organized and obstructive blood clot compromising most of the right ventricle with no other cardiac abnormalities and without any evidence of a pulmonary embolism. The patient was found to not have any history of substance abuse, geneti

Grey matter morphometry in young adult e-cigarette users, tobacco cigarette users & non-using controls.

Despite the rise in electronic cigarette use in recent years, the neurobiological effects of daily e-cigarette use versus smoking cigarettes in young adults remains unknown. This study aimed to investigate the impact of regular, exclusive e-cigarette use on grey matter morphometry in young adults, age 18-25. Structural MRI data were collected from 3 distinct groups of participants (n = 78): daily, exclusive e-cigarette users; tobacco cigarette users; and non-using controls, to assess grey matter volume (GMV) differences. Voxel-based morphometry revealed significant GMV reductions in tobacco ci

Vice versa: sex work and drug use during the HIV epidemic in Thatcher's Britain.

This article traces the origins of the present-day association of prostitution and drug addiction in Britain. Through an examination of a variety of political discussions in the 1980s that centred on sex work, drug use, and HIV/AIDS, I locate the emergence of the cultural legitimacy of narratives of addiction within discourses of prostitution. I argue that it was in the 1980s, within the context of the rise of heroin use and the concomitant emergence of AIDS, that the interwoven discourses of drug addiction and prostitution were forged and cemented. Within the context of these two 1980s epidem
