Multicore disease in sibs with severe mental retardation, short stature, facial anomalies, hypoplasia of the pituitary fossa, and hypogonadotrophic hypogonadism.

We describe a family in which two adult sibs presented with a history of congenital nonprogressive myopathy, severe mental retardation and evidence of mild generalized weakness, short stature, musculoskeletal deformities, facial anomalies, sexual infantilism, and radiologic evidence of pituitary hypoplasia. The parents were first cousins. An excess of other, apparently unrelated, genetic conditions were present in other family members. Results of histochemical and electron microscopy studies of muscle biopsies from both affected individuals were compatible with multicore disease. This newly de

A clinical study of Gilles de la Tourette syndrome in the United Kingdom.

The clinical features of 53 British-born patients with Gilles de la Tourette syndrome are described. The mean age at onset of body tics was seven years and for vocalisations 11 years. Coprolalia was present in 39%, copropraxia in 21%, echolalia in 46% and echopraxia in 21%. Complicated antics and mannerisms were also common, often involving the compulsive touching of objects or self-injurious behaviour. Forty-six per cent of cases had a family history of tics in a single close relative and in two individuals a further member of the family had Gilles de la Tourette syndrome. Focal dystonia was

[Family anamnestic study of 89 suicides following release from inpatient psychiatric treatment].

This study was prompted by other large, family-history oriented studies which showed that there is no correlation between frequencies of suicide and psychosis. A multidimensional, neuro-psychiatric diagnosis program was performed on 89 cases of suicide after hospital discharge . (The time-span of the study was between June 1, 1969 and August 31, 1980.) Primarily, however, this study was oriented on family history, particularly concerning suicides and endogenous psychosis. In 69.7% of the cases a suicide-positive family history was established. In 56.2% of the suicide cases (N = 89) the simulta

Dementia in elderly outpatients: a prospective study.

We prospectively studied the evaluation of dementia in 107 unselected outpatients; 83 had so-called "irreversible" dementias, including 74 who had an Alzheimer-type dementia. Fifteen patients had potentially reversible dementias, of which hypothyroidism and drug toxicity were the commonest causes. Distinguishing features of reversible dementia were shorter duration, use of more prescription drugs, and less severe dementia. Almost half of the patients had other previously unrecognized treatable medical diseases. Most diagnoses were made from patient history and physical and mental status examin

Curious neurologic sequelae in galactosemia.

Two siblings with classic transferase deficiency galactosemia that was detected at birth have been treated with lactose restriction since the neonatal period. Both patients developed a unique and progressive neurologic syndrome of mental retardation, tremor, and ataxia. Careful review of the family history and medical records, the absence of metabolic disturbances other than those related to galactosemia, and the aggregate physical findings and neurodiagnostic studies ruled out other neurologic disorders in these siblings. It is therefore proposed that these patients represent a subgroup of tr

Parents who physically abuse their children.

Increased knowledge of child abuse has mad what was once an infrequently diagnosed condition become recognised as a major pediatric and social problem. While there is no typical profile of an abusive parent, characteristics that have been found more frequently include a history of having been deprived and possible abused in the parent's own childhood, low self-esteem in the mother and increased ill-health in the immediate family members. Personality testing shows that the parents relate poorly to their children and to other people and that they have immature and impetuous personalities. Predic

The clinical and neuroendocrine features of psychotic depression.

The authors compared 65 patients with major depression and psychotic features to 192 patients with major depression and no psychotic features in terms of clinical features, family history, and hypothalamic-pituitary-adrenocortical axis function. In accord with other studies, patients with psychotic depression were more likely to have bipolar depression, psychomotor disturbance, a family history of schizophrenia, and a more severely disordered hypothalamic-pituitary-adrenocortical axis. Whether psychotic depression is best considered apart from nonpsychotic depression or as simply a more severe

Behavior and bladder disturbance of enuretic children: a rational classification of a common disorder.

A group of 126 enuretic children were evaluated as to their psychiatric and neurological states, functional bladder volume and environmental circumstances. Few differences were found when the children were grouped according to presence or absence of social disadvantage, the presence of a family history of enuresis, or the nature of onset of enuresis. No classification differentially predicted response to treatment. However, children with an associated psychiatric disorder had significantly lower bladder volumes and had more developmental delays. It is concluded that the classifications of prim

Early alcoholism treatment: the Emmanuel Movement and Richard Peabody.

The history of alcoholism treatment in the early twentieth century is outlined. The methods of the Emmanuel Movement and of Richard Peabody are described, biographical details of their main practitioners are given, the populations treated are described, and the predecessors and successors of the two methods are discussed. In addition, the two methods are compared with each other and with the methods of Alcoholics Anonymous and Freudian psychoanalysis. The founder of the E. Movement was a clergyman, Dr. Elwood Worcester, whose method was designed to treat a variety of neurotic disorders. He fel

[2 autopsy cases of congenital muscular dystrophy of Fukuyama type--a typical and an atypical cases].

Two autopsy cases of congenital muscular dystrophy of Fukuyama type (F-CMD) were described. The first case was diagnosed clinically and pathologically as its typical case. Neither his family history nor the history of his prenatal period were contributory. He had suffered from muscle weakness and atrophy since his birth. Serum CPK was markedly elevated. EMG and muscle biopsy proved dystrophic changes of the skeletal muscles. In addition, he manifested mental retardation and attacks of convulsion. EEG failed to elicit remarkable changes, but PEG represented ventricular dilatation. He died of re

Etiologic factors and long-term prognosis of convulsive disorders in the first year of life.

Etiological factors and long-term prognosis were studied in 562 cases with convulsive disorders in the first year of life; 114 (20.3%) were prenatal, 114 (20.3%) perinatal, 24 (4.3%) postnatal, and 257 cases (45.7%) were cryptogenic. The remaining 53 (9.4%) patients were doubtful cases. The mortality before six years of age of the pre- and perinatal cases at 17.6% and 15.7%, respectively was significantly higher than in the other etiologic groups. The incidence of a family history of epilepsy or other convulsive disorders was highest in the cryptogenic cases (23.5%), and lowest in the prenatal

Prediction of response to stabilizing lithium treatment.

As lithium has a wide range of biological effects, it is not surprising that the benefit from lithium treatment has been observed in several types of psychiatric disorders. Mood stabilization has been seen in episodic disorders; antiaggressive effect has been reported in mental retardation and other illnesses, and some endocrine and hematological effects have been utilized in internal medicine and neurology. To date, however, only the stabilizing effect on recurrent mood disorders appears to be reliably predictable. The prediction is based primarily on the diagnosis, quality of free interval a

The utility of a multivariate normal model for studying familial patterns in medical and psychiatric data.

Quantitative traits (such as blood pressure, height or anxiety level) are usually more similar in related than in unrelated individuals. A positive correlation between two family members indicates that there are common factors (genetic and/or environmental) which predispose to the trait values in this pair of individuals. A negative correlation can indicate that there is (environmental) competition or some other cause for negative interaction between the pair of relatives. Using a flexible method for the analysis of quantitative data measured over pedigrees, it is possible to estimate the magn

The method of social-geriatric examination is described. This type of examination by an ambulatory team takes place at the patient's home. The examination is firstly directed to the interactions in the human-environmental system. By means of a scheme as an aid the interactions can be analyzed. This analysis, how people are dealing with each other and with need for care and with care, precedes the analysis of the chain of interacting unfavourable conditions of social, mental and physical nature, which are responsible for the disturbance of the balance of the system. This disturbance is signaled

Technical considerations for platelet aggregation and related problems.

Platelet aggregation generally is ordered by the physician to evaluate platelet function in hemorrhagic or thrombotic disorders. Malfunction of the platelet may be the result of an intrinsic congenital defect or an acquired problem induced by drugs or certain circulating plasma factors. It is necessary to obtain information from the patient with respect to family history, drug ingestion, physical or mental stress. In addition, other laboratory studies should be obtained to rule out general coagulation disorders affecting the plasma factors. A bleeding time will be helpful in establishing the s

Differential diagnosis and pathogenesis of anorexia nervosa.

Anorexia nervosa is a disease of increasing frequency with serious medical and psychological consequences. The presentation is one of significant weight loss. The initial assessment of such a patient must differentiate between an underlying systemic medical illness and an eating disorder. This paper will review the more common medical conditions causing weight loss and their distinguishing characteristics, including malignancy, inflammatory bowel disease, infections and metabolic disorders. Once an organic disease is ruled out, anorexia nervosa must then be differentiated from other eating dis

Analysis of background variables in 350 patients with TMJ disorders as reported in self administered questionnaire.

All 350 patients referred to the Department of Stomatognathic Physiology during a 4-month period were given a self-administered questionnaire intended to give a review of the patients' history. The questions concerning general somatic and psychic health, education and social conditions are dealt with in this article. Twenty-five percent of the patients, more women than men, considered their general state of health to be impaired or poor. Seventy-two percent of the patients denied having had nervous or psychic disorders while 9% were currently being treated for such symptoms. Patients who had a

High alcoholism rate in patients with essential tremor.

The authors reviewed the charts of 36 patients discharged from a VA medical center with a diagnosis of essential tremor over a 10-year period for evidence of alcohol dependency or abuse and for family history of alcoholism and other psychiatric disorders. A sex- and age-matched sample of patients admitted during the same period for herniorrhaphy was used as a control group. Patients with essential tremor had a significantly higher frequency of alcohol dependence and abuse than control subjects and also had a much higher rate of positive first-degree family history of alcoholism. Family history of suicidal behavior among suicide attempters.

Efforts to predict suicide and attempted suicide are hampered by their relative rarity on the one hand and the inadequate specificity of clinical characteristics and relevant antecedent events on the other; that is, these features are found widely among the nonsuicidal as well. In an effort to further understand these phenomena, the authors studied family history of suicidal behaviors (suicide, attempted suicide, and suicide threats) in 127 patients hospitalized following a suicide attempt. Patients with personality disorders (antisocial personality disorder, alcoholism, somatization disorder,
