Screening and assessment of childhood deafness: experience from a centralized multi-disciplinary service.

A paediatric hearing assessment unit was set up in Lothian in 1978 to provide a clear referral pathway for children with suspected sensorineural hearing impairment and particularly for those children detected by the health visitor screening test. Over a 6-year-period the incidence of bilateral sensorineural deafness was 1.3/1000 suggesting that all children in the region were being seen. The mean age of diagnosis fell significantly over this period. Children in high risk groups, through adverse perinatal events or a positive family history, were not detected any earlier than other children. Th

A study of some psycho-social characteristics of blind and deaf male students in Abha City, Asir region, Saudi Arabia.

Psychosocial characteristics including depression, attitude towards their handicap, hobbies and problems of living situations have been studied among 152 male students in the Institute for the Blind (n = 44) and in the Institute for the Deaf (n = 108) in Abha City. All students were subjected to a constructed Arabic version of the rating scale 'Children Depression Inventory (CDI)'. They were categorised according to their scores on this CDI, into depressed and non-depressed. Also, an interview questionnaire was administered to collect data related to the handicap, such as age of onset, and its

Mitochondrial ribosomal RNA gene mutation in a patient with sporadic aminoglycoside ototoxicity.

PURPOSE: Aminoglycoside-induced deafness has been described in a number of Chinese pedigrees. In nearly all of these families, affected individuals were related through the maternal side. Because mitochondrial DNA is transmitted exclusively through mothers, it had been speculated that a mutation in the mitochondrial DNA might predispose these maternally related family members to aminoglycoside ototoxicity. Recently, we analyzed three such families with multiple cases of ototoxic deafness and identified a pathogenic mutation in the mitochondrial 12S ribosomal RNA gene at nucleotide position 155

Study of asymptomatic microscopic hematuria in potential living related kidney donors.

Thirty potential living related kidney donors with asymptomatic microscopic hematuria of nonsurgical causes were entered in this study. They underwent thorough history taking, medical and ENT examination, laboratory and radiologic assessment and pure-tone audiometry. Family members were also subjected to urine analysis and audiometry. Moreover, the 30 donors were subjected to kidney biopsies which were examined by light microscopy, direct and indirect immunofluorescent microscopy, and electron microscopy. Hereditary nephritis (with or without sensorineural deafness) was found to be the most co

[Prospective study of the cardiovascular risk of patients with sudden deafness].

In this prospective case control study 135 patients with sudden hearing loss were compared with regard to their cardiovascular risk with 135 otologically healthy patients and 111 patients with coronary heart disease. The groups were matched according to age, sex and date of onset of disease. The risk factors investigated were: serum cholesterol, low-density lipoproteins, high-density lipoproteins, triglycerides, blood pressure, body weight, diabetes mellitus, positive family history of coronary heart disease and smoking habits. There was no significant difference between the patients with sudd

Genetic counseling for the deaf.

Genetic counseling is a process that emphasizes accurate diagnosis of hereditary conditions and communication of information to families. Genetic counseling involves systematic collection of family and medical history, a physical examination by a certified clinical geneticist, sharing of information with the family, and follow-up and support services. The issues that arise in genetic counseling can differ for every family and are often dependent on the degree of deafness present in the family, age of onset, and linguistic and cultural orientation. It is important for the genetic counselor to c

Innovative approach to genetic counseling services for the deaf population.

Genetic service providers have stressed the importance of genetic counseling that is nondirective and specific to the personal needs of consultants. Successful genetic counseling for deaf persons often requires special provisions for complex family histories, syndromic conditions, and diversity in communication methods and cultural orientation. The Gallaudet University Genetic Services Center (GSC) was established in 1984 to provide genetic education and counseling services to the deaf community. The GSC staff developed and implemented a standardized system of data collection (family and medic

[A clinical and ultrastructural study of Fechtner syndrome in two Japanese families].

This is a report of Fechtner syndrome in two Japanese families. Six members of family I and three members of family II were studied. All but one had macrothrombocytopenia and leukocyte inclusion bodies, four had deafness, four had persistent proteinuria and none had cataracts. Under a diagnosis of ITP, two of them had splenectomy which resulted in no response. History revealed, other family members with deafness and/or nephritis were confirmed in both families. Ultrastructural studies of leukocytes showed oval inclusion bodies with unclear borders containing many fine ribosome like granules an

Norrie's disease vs. PHPV: one family's dilemma.

This paper presents the numerous similarities between Norrie's disease and bilateral persistent hyperplastic primary vitreous. A description of each disease is given, as well as a case report which demonstrates the difficulty in distinguishing the two disease processes when there is a negative family history of blindness. A discussion section focuses on the need for a better differential protocol for Norrie's disease, which would aid the clinician when a family history of blindness is absent.

Genetic counseling of the deaf. Medical and cultural considerations.

Genetic counseling is a communication process where information is provided in a nondirective way. Genetic counseling emphasizes informed decision making and provision of medical, psychological, and social support; genetic counseling is not advice giving. Individuals who are members of the deaf community (culturally deaf) present unique challenges to the ability of genetic counselors to be nondirective. In contrast to the medical model which considers deafness to be a pathological condition, many deaf people do not consider themselves to be handicapped but define themselves as being part of a

Genetic aspects of antibiotic induced deafness: mitochondrial inheritance.

Analysis of 36 pedigrees with a positive family history of aminoglycoside antibiotic induced deafness, ascertained in a population of 483,611 in Zhabei District in Shanghai, showed that the susceptibility to antibiotic ototoxicity was transmitted by females exclusively, indicating mitochondrial inheritance. Reanalysis of 18 other published pedigrees confirmed this conclusion.

Müllerian tract abnormalities and associated auditory defects.

Fifteen women with müllerian defects and a control population of 15 with normal müllerian systems underwent dynamic audiometric testing. The rate of auditory defects in women with müllerian abnormalities (33%) was significantly higher (P less than .05) than in the control population (0%). Those patients manifested mild, moderate and severe sensorineural defects in the high-frequency range. The defects were not accounted for by age or occupation and were associated with renal agenesis in 60% of the cases. One patient with normal audiometric testing and a septate uterus had a family history sign

Recurrence risks for near relatives of children with sensori-neural deafness.

In a sample of children with sensori-neural deafness and no evidence of a syndrome, ototoxic exposure, or autosomal dominant or X-linked family history, seen in the Division of Medical Genetics of The Montreal Children's Hospital, the probability of a sibling being similarly affected was about 1 in 6, both in French-Canadian families and the remainder of the sample. The frequency of deafness of early onset was measured in the uncles and aunts of probands, and these figures were used to derive approximate recurrence risks for the offspring of probands (about 1 in 130, ignoring the possibility o

Gout and neurological abnormalities associated with cardiomyopathy in a young man.

A 21 year old man with a family history of gout and neurological deficits, developed severe idiopathic congestive cardiomyopathy after a long history of typical gouty attacks and neurological abnormalities. Clinical and laboratory evaluations showed borderline mental retardation, ataxia, sensorineural deafness, marked hyperuricaemia, and excessive uric acid excretion in the presence of impaired renal function. None of the known causes of cardiomyopathy was found. Even though red cell hypoxanthine guanine phosphoribosyltransferase enzyme activity was normal, this case probably represents an inb

[Macrothrombocytopenia with deafness, nephritis, cataract, short small intestine, and double ureter].

A 23 year old female, born in 1957, was diagnosed as having idiopathic thrombocytopenic purpura at the age of 3 and treated with prednisolone during her childhood with no response. On her regular check-up in 1978, facial edema and proteinuria suggested renal disease. The family history was negative for bleeding diathesis or renal disease. Close examination revealed the following: WBC 4,200/microliters without leukocyte inclusions, RBC 3.42 x 10(6)/microliters, Hb 11.7 g/dl. PT 10.6 sec, APTT 28.9 sec. Platelet count 4,500/microliters by HEMATRAK 360, and 40 x 10(3)/microliters measured by micr

Chronic nephritis, sensorineural deafness, growth and developmental retardation, hyperkinesis, and cleft soft palate in a 5-year-old boy. A new combination?

A 5-year-old Japanese boy showed nephritis similar to, but distinct from, that in Alport syndrome. Nephrotic syndrome without hematuria was noticed at age 2, although renal biopsy at age 4 revealed widespread irregular thickening of the glomerular basement membrane with splitting of the lamina densa on electron microscopy, characteristic of nephritis in Alport syndrome. Sensorineural deafness was noticed at age 4 weeks by no auditory brain stem response, unusually early for Alport syndrome. Goodpasture antigen and amyloid P component were found in the glomerular basement membrane. Thus, the an

Familial expansile osteolysis.

Familial expansile osteolysis (FEO) is a unique bone dysplasia, which has, over five generations, affected 42 members of a Northern Ireland family. The disease follows a classic autosomal dominant pattern of inheritance. The condition is distinct enough in its clinical features and natural history to be recognized as a new and unique disease. There are both general and focal skeletal changes, the latter having a predominantly peripheral distribution and an onset from the second decade. Progressive osteoclastic resorption accompanied by medullary expansion leads to severe and painful disabling

Waardenburg syndrome type I in a child with de novo inversion (2)(q35q37.3).

We report on a child with Waardenburg syndrome type I and a paracentric inversion of chromosome 2. This 20 month-old boy has dystopia canthorum, sensorineural deafness, heterochromia iridis, partially albinotic ocular fundi, and partial leukodermia. He does not have mental retardation or any skeletal abnormalities. Family history was unremarkable. Cytogenetic studies demonstrated that the patient has a paracentric inversion (2)(q35q37.3); his parents have normal chromosomes. These findings suggest that the locus of the gene for Waardenburg syndrome type I may be at 2q35 or 2q37.3.
