Familial expansile osteolysis. A new dysplasia.

We report 40 cases in one family of an autosomal dominant bone dysplasia, which, though similar in some aspects to Paget's disease, seems unique in some features and in its natural history. The disease shows both general and focal skeletal changes, the latter being mainly in the limbs with an onset from the second decade. Progressive osteoclastic resorption is accompanied by medullary expansion which leads to pain, severe deformity and a tendency to pathological fracture. The serum alkaline phosphatase and urinary hydroxyproline are variably elevated, while other biochemical indices are normal

Profile analysis of pupils at a school for black deaf children. A pilot study.

The pupils at a school for black hearing-disabled children were clinically investigated and the findings were analysed with regard to age, sex, and nature and possible cause of deafness. A total of 169 children were examined. The majority were boys, and 95% were hearing-disabled before the age of 3 years. One-tenth of the children had a positive family history. An apparent cause could be established with a degree of certainty in only 30% of cases.

Progressive erythrokeratodermia and cochlear hearing impairment. A case report and review of the literature.

A female child with congenital progressive erythrokeratodermia combined with sensory hearing loss observed through a period of 5 years is reported. She demonstrates symmetrical hyperkeratotic skin changes, verrucous plaques on her nose cheeks, ears, chin, knees, elbows, and heels. Electron microscopic studies of her skin did not reveal qualitative changes, her moderate to severe hearing impairment is of cochlear origin, moderately progressive, and particularly affecting the high frequencies. Up to now a vascularizing keratitis could not be detected. Her family history is not contributory. Twen

Hearing loss and ischemic heart disease.

One hundred three patients with ischemic heart disease (IHD) were compared with 29 patients with organic heart disease and normal coronary arteries (OHD) and with a control group of 101 patients free of heart disease and matched for age and sex. Twelve patients in the control group, 4 in the OHD group, and 34 patients in the IHD group were found to have hearing loss (HL) of different degrees, cause, and duration (P = 0.0003). Of the HL-IHD group, 19 of the 34 patients had no underlying etiologic factor compared with 4 of the 12 patients in the control group (P = 0.0005); age was not an importa

[Atypical erythrokeratodermia with deafness, keratitis and double mycotic infection].

We report on a male patient suffering from a congenital ichthyosiform dermatosis, whose family history offered no clue to the diagnosis. The skin lesions were mainly localized on the trunk, hands, elbows, and knees. During the last few years, the cutaneous lesions had slowly changed for the worse and the following additional symptoms had developed: keratitis, deafness, diffuse alopecia, dysarthria, and ataxic abasia. According to the clinical features, we established the diagnosis of KID syndrome (keratitis, ichthyosis, deafness). Although the skin lesions of our patient clinically and histolo

Aetiology of bilateral sensori-neural deafness in children.

The study was carried out in the Department in children born during the four year period 1981-1984 in a well defined geographical areas known as the Greater Manchester County. The children were investigated for possible congenital infections. Perinatal assessment was carried out in conjunction with the Paediatricians for aetiological adverse factors. Family histories were obtained and parents and siblings were examined for any hearing impairment. A total of 155 cases were investigated. Cases with a positive family history with deafness in siblings or parents constituted 18.7% of cases. An even

Communication between health care professionals and deaf patients.

A review of the literature about communication between health care professionals and deaf people demonstrates that a continuum of accuracy in perceptions of deaf people and their communication problems exists in health care settings. The author provides information on deaf culture, history, population size, sign language, and communication needs that is used to evaluate professional journal articles. Examples of research on this topic are presented and the need for further studies and the role of social workers are discussed.

Congenital cytomegalovirus infection: a cause of sensorineural hearing loss.

Prospective studies have suggested that about 108 children with congenital cytomegalovirus (CMV) infection and bilateral sensorineural hearing loss are born each year in England and Wales; this represents about 12% of all children with congenital sensorineural hearing loss. Over a nine year period 1644 children aged between 6 months and 4 years who were attending the Nuffield Hearing and Speech Centre were screened for CMV infection. The prevalence of CMV in the urine of children with sensorineural hearing loss but no immediate family history of deafness was nearly twice that (13%) found in ot

Observation concerning the age of onset and the nature of optic atrophy in Wolfram's syndrome (DIDMOADS).

A 31/2-year-old boy, who was diagnosed to have diabetes 6 months earlier, was referred for routine fundus check-up. No family history of diabetes was known. Ophthalmologic findings were: slight hyperopic astigmatism, visual acuity right = left = 1/5, examination of the visual field not yet possible, optic atrophy, no diabetic retinopathy, very narrow vessels. ERG: distinctly reduced potentials. Audiogram: loss of high tones in the inner ear. Pediatric examination ruled out other endocrine disorders, no symptoms of diabetes insipidus were found. These results perhaps reveal the nature of the op

Renal prognosis in women with hereditary nephritis.

Renal prognosis cannot be easily predicted in females with hereditary nephritis (HN). Thirty-six women with persistent urinary abnormalities, belonging to 24 families with progressive HN, were studied. Renal biopsy specimens were available in 23 patients and were studied by light and electron microscopy (EM). Nine women (group I) progressed to early renal failure, at 35 yrs of age or less. Five women (group II) progressed to late renal failure at 45 yrs of age or more. In contrast, 22 patients have so far normal renal function, and 14 of these (group IV) range from 31 to 62 yrs of age. Hearing and balance disorders. How to recognize, when to refer.

Nearly one in ten Americans has some degree of hearing loss. The primary care physician is frequently the first to examine patients with hearing and balance disorders. Examination begins with a thorough history and examination and may also include specific tests performed in the office. In many cases, the primary care physician can diagnose and treat disorders successfully. In other cases, referral to a specialist is necessary.

Ear anomalies, deafness and facial nerve palsy in infants of diabetic mothers.

Two children with an unusual combination of ear anomalies, deafness and facial nerve palsy are described. They are both children of insulin dependent diabetic mothers. There is no other family history of genetic deafness or congenital anomaly. The defects in both children may be accounted for by abnormal embryological development of the petrous temporal bone. It is postulated that poor metabolic control of the diabetes has led to these malformations.

Waardenburg's syndrome: variations in expressivity.

Twenty-seven subjects with a family history of Waardenburg's syndrome were examined with respect to 18 specific characteristics of the syndrome, with particular emphasis in identifying the spectrum of the phenotypic expression of affected persons. Our results indicate that patients with the syndrome may have a variety of clinical signs, and an accurate diagnosis will therefore depend on a thorough and pertinent family history and physical examination.

At the Nephrology Department of the Hospital Infantil de Mexico, four cases of Alport's syndrome. with no relation among themselves, were studied between, June 1978 and November 1979. The diagnosis was based on the presence in the patients, of proteinuria and/or hematuria and in some of the relatives, a history of renal disease, deafness and a variable degree of renal failure; at the renal biopsy examined with electronic microscopy, there were findings compatible with Alport's disease in all four cases. Field studied were carried in the largest possible number of relatives for each family grou

Syndrome of ichthyosis congenita, neurosensory deafness, oligophrenia, dental aplasia, brachydactyly, clinodactyly, accessory cervical ribs and carcinoma of the thyroid.

We report the case of a 15-year-old girl with a uneventful family history. Her skin condition was clinically, histologically and ultrastructurally compatible with the diagnosis of ichthyosis congenita. She suffered from neurosensory deafness and oligophrenia. Further findings included dental aplasia, brachydactyly, clinodactyly and accessory cervical ribs. At the age of 14, a thyroid carcinoma was diagnosed. Therapy with a retinoid derivative (Ro 10-9359) resulted in a marked improvement of the ichthyosis. We assume a genetic syndrome with autosomal-recessive inheritance.

Frequency of the branchio-oto-renal (BOR) syndrome in children with profound hearing loss.

Nineteen of 421 white children in Montreal schools for the deaf had preauricular pits. The branchio-oto-renal (BOR syndrome was identified in four of the nine families who agreed to family investigation, including audiograms and intravenous pyelograms (IVPs) and may have been present in several others. The penetrance of this autosomal dominant syndrome appears to be high. It is estimated that severe renal dysplasia occurs in about 6% of heterozygotes. The presence of a preauricular pit at birth suggests that the child has at least one chance in 200 of severe hearing loss, and this warrants a c
