Catastrophic Esophageal Tumor Perforation within One Week after First-Line Chemo-Immunotherapy for Advanced Esophageal Squamous Cell Carcinoma: A Case Report.

INTRODUCTION: Esophageal perforation secondary to advanced esophageal squamous cell carcinoma (ESCC) is a life-threatening oncologic emergency that can rapidly progress to mediastinitis, empyema, and septic shock. With the increasing use of first-line chemo-immunotherapy, early tumor necrosis-particularly in ulcerated or deeply invasive lesions-may precipitate catastrophic perforation. CASE PRESENTATION: A man in his 70s was diagnosed with unresectable middle thoracic ESCC with distant metastases to the iliopsoas muscle and left supraclavicular lymph node (cT3brN2M1b, cStage IVB). First-line

Safe Resection of a Giant Mediastinal Liposarcoma with Severe Cardiac Compression.

INTRODUCTION: Dedifferentiated liposarcomas of the mediastinum are exceedingly rare, and surgical resection is the primary treatment of choice. Mediastinal mass syndrome (MMS) is an oncological emergency characterized by compression or invasion of the heart, great vessels, or trachea by a large mediastinal tumor, particularly during the induction of anesthesia. We report a case of a giant dedifferentiated liposarcoma in the anterior mediastinum, surgically resected due to severe cardiac compression and presenting as an oncological emergency. CASE PRESENTATION: A 70-year-old male presented wit

Genomic landscape and subgroup stratification of thymic epithelial tumors: a systematic meta-analysis of next-generation sequencing data.

INTRODUCTION: Thymic epithelial tumors are rare cancers of the anterior mediastinum with heterogeneous clinical behaviors. Despite numerous attempts to characterize their mutational landscape, a comprehensive understanding of their genetic alterations remains limited due to small sample sizes. METHODS: To address this gap, we conducted a systematic meta-analysis of somatic mutations reported in 729 patients across twenty studies, integrating single-variant data into a unified dataset. RESULTS: This approach identified three molecular subgroups with distinct biological and clinical features.

Giant Anterior Mediastinal Hamartoma in a Middle-Aged Woman: A Rare Encounter.

Hamartomas are benign neoplasms most commonly arising in the lung, typically identified as incidental solitary pulmonary nodules. Occurrence in the mediastinum is exceedingly rare. Because anterior mediastinal lesions are most often malignant, hamartomas in this location pose a diagnostic challenge. We present the case of a 53-year-old female with a background of endometrial carcinoma who was evaluated for chest pain and palpitations. A chest radiograph revealed a large opacity in the left upper hemithorax, corresponding to a well-circumscribed anterior mediastinal mass on computed tomography.

Intracranial Metastasis of a Primary Mediastinal Seminoma Mimicking a Convexity Meningioma: A Case Report.

Convexity germ cell tumors are exceedingly rare and may closely mimic meningiomas, particularly in young men. This case report presents a rare convexity germ cell tumor characterized by dural invasion and skull destruction. An 18-year-old man presented with headache and nausea. Neuroimaging demonstrated a right frontoparietal extra-axial mass accompanied by adjacent skull hyperostosis and significant cerebral edema. Based on these radiological findings, a convexity meningioma was strongly suspected, and a craniotomy was performed for tumor resection. Unexpectedly, histopathological examination

The surgical approach strategies in the treatment of anterior mediastinal tumors.

BACKGROUND: Anterior mediastinal tumors, involving structures such as the thymus and lymph nodes, pose significant clinical challenges due to their asymptomatic nature and potential to cause severe symptoms as they grow or invade surrounding organs. Treatment varies by tumor type, with surgery being crucial, especially for benign tumors and thymomas. The complex anatomy of the anterior mediastinum makes surgical approach selection critical for treatment outcomes. METHOD: A comprehensive literature search was conducted to identify studies related to surgical approaches for anterior mediastinal

Cardiac papillary fibroelastoma in an unusual left atrial location in combination with mediastinal thymic cyst: a case report.

BACKGROUND: Cardiac papillary fibroelastomas are rare, benign endocardial tumors most commonly found on cardiac valves. Non-valvular locations, particularly the posterior left atrial wall, are exceptionally uncommon. Thymic cysts are also rare lesions. The presence of these two non-homologous tumors in a single patient has not been previously documented. CASE PRESENTATION: This report presents a case of cardiac papillary fibroelastoma located on the posterior left atrial wall coexisting with a thymic cyst in the anterior mediastinum, both of which were incidentally detected and surgically trea

Rapidly growing anterior mediastinal teratoma in a young adult female: successful resection via robotic-assisted thoracoscopic surgery.

INTRODUCTION: Mediastinal teratomas are rare, typically benign germ cell tumors that are often discovered incidentally due to their slow growth and asymptomatic nature. PRESENTATION OF THE CASE: We present this unusual case of a 22-year-old woman who presented with retrosternal chest discomfort, and was incidentally found to have a large anterior mediastinal mass measuring 8 × 8 × 5 cm on computed tomography (CT) scan. Imaging characteristics - including fat and calcification - were consistent with a mature cystic teratoma. Notably, a CT scan performed 4 years earlier was completely normal, s

Confronting Upside-Down Video-Assisted Thoracic Surgery for Anterior Mediastinal Leiomyoma.

INTRODUCTION: Leiomyoma is a benign smooth muscle tumor typically arising in the uterus or gastrointestinal tract; its occurrence in the anterior mediastinum is extremely rare. The differential diagnosis of anterior mediastinal tumors usually includes thymoma, germ cell tumors, and lymphoma, whereas leiomyoma is rarely considered. CASE PRESENTATION: A 50-year-old woman with no notable medical history presented with chest pain and cough. Chest radiography showed a mass in the left lower lung field. Tumor markers (HCG, CEA, AFP, CYFRA 21-1, ProGRP, anti-AchR antibody, sIL-2R) were normal. CT re

Retrosternal gastric reconstruction after esophagectomy using the "waterfall" method for posterior mediastinal dead space filling.

BACKGROUND: Advanced esophageal squamous cell carcinoma (ESCC) may require combined resection or result in non-curative resection, increasing the risk of severe complications related to the posterior mediastinal dead space created after esophagectomy, such as airway instability and mediastinitis. Although filling this space with the greater omentum via the posterior mediastinal route is commonly used, this approach may impair gastric conduit alignment and function due to intrathoracic negative pressure. We developed a novel technique to overcome these limitations. METHODS: We retrospectively

Comparison of subxiphoid and lateral intercostal approach in anterior mediastinal pathologies.

BACKGROUND: The anterior mediastinum is the compartment where mediastinal lesions are most frequently seen. Both subxiphoid approaches (SVATS) and lateral intercostal approaches (LVATS) are used in anterior mediastinal lesions. In this study, we aimed to compare the outcomes and advantages of SVATS and LVATS. METHODS: Between January 2013 and December 2024, the patients who underwent minimally invasive surgery for anterior mediastinal lesions were retrospectively analyzed. The study's main objectives were the characteristics of the patients, operation time, length of hospital stay, drain type

Robotic-assisted biventricular durable assist device implantation: The first in human experience.

We report a novel approach of robotic-assisted, minimally invasive biventricular assist device (BiVAD) implantation using HeartMate 3 in a 61-year-old male with severe biventricular dysfunction due to dilated cardiomyopathy. Robotic assistance enabled sternum-sparing, implantation of both left and right durable ventricular assist devices via left and right thoracotomies. Postoperative recovery was uneventful, and the patient remains clinically stable and discharged home with no device-related complications. This approach allows for a precise dissection and implantation technique in a limited m

A case of giant thymolipoma: case report.

BACKGROUND: Thymolipoma is a rare benign anterior mediastinal tumor. Traditionally, resection of giant thymolipoma (often ≥15 cm) required open surgery. This report describes a case of successful resection of a 19-cm thymolipoma using dual-port video-assisted thoracoscopic surgery (VATS). CASE DESCRIPTION: A 67-year-old male patient presented with progressive cough and dyspnea. Computed tomography (CT) revealed a 19 cm × 13 cm × 4 cm fat-density mass in the anterior mediastinum, consistent with the typical features of a thymolipoma. Given the high diagnostic certainty on imaging, the patient

Unusual Association of a Thymic Cyst and Thymolipoma: A Case Report.

Benign thymic lesions are uncommon and mainly include thymic cysts, thymic hyperplasia, and thymolipoma. Their coexistence is exceptionally rare and may represent a significant diagnostic challenge, particularly because of the potential for confusion with malignant tumors of the anterior mediastinum. We report the case of a 68-year-old woman who presented with a large anterosuperior mediastinal mass that was radiologically suggestive of a thymoma, in the absence of clinical or biological features of myasthenia gravis. Complete surgical excision was performed, and histopathological examination

[Mediastinal Metastasis of Mixed Hepatocellular Carcinoma Requiring Differential Diagnosis from Thymic Carcinoma].

A 71-year-old man underwent S8 segmentectomy for mixed hepatocellular carcinoma. The disease recurred three months postoperatively, prompting a right lobectomy of the remaining liver. Prior to the liver recurrence, three nodules were identified in the anterior mediastinum. At the patient's request, dendritic cell vaccine therapy was administered following the recurrence. Two of the nodules decreased in size, while one nodule increased. Consequently, the patient was referred to our department for further evaluation. Differentiating between thymic carcinoma and mediastinal metastasis of hepatoce
