Inflammatory myofibroblastic tumor (IMT) is an uncommon mesenchymal neoplasm of intermediate biological potential, characterized by proliferating myofibroblastic spindle cells with a prominent inflammatory infiltrate. Although IMT occurs more commonly in children and young adults, primary involvement of the anterior mediastinum is exceedingly rare, particularly in older male patients, and may mimic more common anterior mediastinal tumors such as thymoma or thymic carcinoma. The authors report on a 68-year-old male patient with an incidentally detected anterior mediastinal mass. Contrast-enhanc

Background: Thymic epithelial tumors (TETs) are the most common primary neoplasms of the anterior mediastinum and present a dual classification challenge, namely anatomical staging according to the Masaoka-Koga system and histological risk stratification according to the World Health Organization (WHO) classification. Both tasks rely on expert pathological assessment and may be affected by interobserver variability. This study applied supervised machine learning (ML) to quantitative immunohistochemical (IHC) H-score profiles to predict Masaoka-Koga stage and WHO risk group in TETs. Methods: Lo

The mediastinum contains critical components of the central venous axis, including the superior vena cava (SVC) and brachiocephalic veins (BCVs). Mediastinal tumors, although rare, may cause extrinsic compression or invasion of these vessels, significantly affecting the safety and feasibility of central vascular access device (CVAD)1 placement. Approximately 20%-40% of mediastinal masses cause clinically or radiologically significant compression of mediastinal vessels. This rate increases to 30%-50% in malignant tumors and exceeds 50% in mediastinal lymphomas. The anterior mediastinum represen

Thymolipoma is an uncommon, non-malignant mediastinal mass characterized by a mixture of mature fat and normal thymic tissue. It typically grows slowly and is often asymptomatic until it reaches a large size, causing compressive symptoms. We report on a 19-year-old woman who sought medical attention for chest pain, dyspnoea, and anterior chest swelling. Imaging revealed a well-defined anterior mediastinal mass, and surgical excision was performed. Histopathological examination showed a well-encapsulated lesion with mature adipose tissue admixed with normal thymic parenchyma containing Hassall'

A 79-year-old woman presented with progressive hemoptysis. Computed tomography revealed a systemic artery-to-pulmonary artery fistula (SA-PAF) extending from the left upper lobe to the anterior mediastinum. The SA-PAF was treated with transarterial embolization followed by surgical resection, resulting in resolution of the hemoptysis. Pathological examination of the resected specimen incidentally revealed ectopic pancreas (EP) adjacent to the SA-PAF, along with chronic inflammation of the surrounding lung parenchyma and pleura. These findings suggest that mediastinal EP may be associated with

Langerhans cell histiocytosis (LCH) is a rare clonal proliferative disorder originating from dendritic cells and primarily occurring in children. Although pulmonary LCH is relatively common among adult smokers, isolated mediastinal involvement is very rare. This study reports a 33-year-old female patient who presented with a 2-month history of right upper back pain. Positron emission tomography/computed tomography revealed an isolated 6.2 × 3.5 cm mixed-density mass in the right anterior mediastinum, raising suspicion for invasion of the pericardium, major vessels, and diaphragmatic pleura. BACKGROUND/OBJECTIVES: Children with congenital long QT syndrome (cLQTS) are at extremely high risk of torsades de pointes (TdP) and sudden cardiac death (SCD). Although the implantable cardioverter-defibrillator (ICD) is a cornerstone in SCD prevention, traditional transvenous ICDs are associated with venous access occupation, lead wear or fracture due to somatic growth, and difficulties with long-term lead revision or extraction. The extravascular ICD, which places the lead in the substernal extravascular space, has the potential to circumvent these lead-related complications. We report the

BACKGROUND: Thymomas are rare epithelial tumors of the anterior mediastinum associated with autoimmune disease, particularly myasthenia gravis (MG). The coexistence of thymoma, MG, and systemic lupus erythematosus (SLE) is uncommon and can complicate diagnosis due to overlapping autoimmune manifestations. Management of advanced thymoma is challenging given limited prospective data and an uncertain optimal role for neoadjuvant therapy.
CASE PRESENTATION: A 32-year-old woman with systemic lupus erythematosus (SLE) presented with progressive proximal muscle weakness, diplopia, and dysphagia. Exa

A 74-year-old man presented with leg edema and was found to have nephrotic-range proteinuria. Renal biopsy revealed AL amyloidosis. Subsequent bone marrow examination demonstrated an increase in monoclonal plasma cells accounting for 13% of nucleated cells. Right-sided pleural effusion developed and was confirmed to be chylothorax. Although treatment with daratumumab, lenalidomide, and dexamethasone (DLd regimen) was initiated, the pleural effusion continued to increase. Lymphangiography using ethiodized oil identified a leakage point in the anterior mediastinum. A chest drain was inserted on

INTRODUCTION: Management of recurrent high-grade serous ovarian cancer (HGSOC) remains a challenge. This case presents a successful multimodal treatment plan in a BRCA-mutated patient with repeated recurrences.
CASE PRESENTATION: A 60-year-old woman diagnosed with FIGO stage III HGSOC received initial debulking surgery followed by chemotherapy at another hospital in December 2017. The patient presented to our institution in March 2019 for further management because of a suboptimal response to 2nd-line chemotherapy. A disseminated disease extent was identified. She progressed (new bone metasta

We report a case of a 20-year-old male who presented with a two-month history of progressively worsening binocular diplopia, dizziness, chest tightness, and significant weight gain. A comprehensive evaluation, including serial imaging, biopsy, and cerebrospinal fluid analysis, ultimately led to the diagnosis of an extragonadal seminoma with simultaneous involvement of the anterior mediastinum and the brain. Notably, the patient had a longstanding history of nonspecific testicular microlithiasis. The patient exhibited a favorable clinical and radiological response to treatment with whole-brain

Thymomas most commonly arise in the anterior mediastinum, whereas ectopic thymomas occurring outside the normal thymic bed are rare. Lesions arising in the cardiophrenic angle can pose a diagnostic challenge, as they may mimic other mediastinal cystic lesions on imaging. We report the case of a 43-year-old male who was incidentally found to have a well-defined lesion in the right cardiophrenic angle on contrast-enhanced computed tomography. The mass appeared as a heterogeneously enhancing solid-cystic lesion closely abutting the pericardium, raising suspicion for a cystic mediastinal lesion. T

INTRODUCTION AND IMPORTANCE: We report a case of a thymoma that developed in the cyst wall during follow-up of a unilocular thymic cyst.
CASE PRESENTATION: A 70-year-old woman was found to have abnormal findings on a computed tomography (CT) scan of the chest. The chest CT revealed a well-circumscribed, hypodense mass in the left anterior mediastinum. During follow-up 5 years later, a small nodule emerged within the cystic lesion. Surgical resection was performed to establish a definitive diagnosis, which confirmed a type A thymoma, classified as pathologic stage I, arising within a thymic cy

BACKGROUND: Gitelman syndrome is an autosomal recessive renal tubular disorder caused by mutations in the SLC12A3 gene, with the pathogenic mechanism being impaired function of the thiazide-sensitive sodium-chloride cotransporter. The disease is mainly characterized clinically by hypokalemia, hypomagnesemia, metabolic alkalosis, and hypocalciuria. Retrosternal giant goiter is mostly caused by nodular goiter enlarging and extending downward into the anterior mediastinum, which may lead to tracheal compression and displacement. Patients with retrosternal giant goiter complicated with Gitelman sy

Patients with giant papillary thyroid carcinoma (PTC) and extensive metastases involving the anterior mediastinum and both lungs face a high risk of severe airway obstruction and ventilation difficulties during anesthesia induction. We report an 18-year-old female with a giant PTC causing critical tracheal compression and extensive metastases. Awake fiberoptic intubation was successfully performed while maintaining spontaneous ventilation. Total thyroidectomy combined with thoracotomy lasted for 10 h, and the blood loss during the operation was about 4,500 mL. After operation, the patient was

Neurofibromatosis type 1 (NF1) is an autosomal dominant disorder that affects multiple organ systems, most commonly the skin, peripheral nerves, and skeletal system. However, involvement of the anterior mediastinum is rare and has been sporadically reported in the literature. Patients with NF1 have a markedly increased risk of developing malignant peripheral nerve sheath tumors (MPNSTs), which are associated with poor 5-year survival outcomes; therefore, early diagnosis and timely intervention are essential to improving clinical outcomes. We report a case of a solitary neurofibroma located in

T-lymphoblastic leukemia/lymphoma (T-ALL/LBL) is an aggressive neoplasm of immature T-lymphoid precursors that is classified as T-cell acute lymphoblastic leukemia when the primary site of involvement is the bone marrow and peripheral blood and as T-cell lymphoblastic lymphoma when it presents as a solid mass, most commonly in the anterior mediastinum. The neoplastic cells are defined by expression of T-lineage antigens (cytoplasmic or surface CD3) together with one or more markers of immaturity (terminal deoxynucleotidyl transferase, CD1a, CD34, CD99, or CD117) and must not fulfill criteria f

INTRODUCTION AND IMPORTANCE: Primary hyperparathyroidism is most commonly caused by a cervical parathyroid adenoma; however, ectopic mediastinal lesions may delay diagnosis when neck imaging is negative.
CASE PRESENTATION: A 19-year-old woman presented with 4 years of progressive bone pain and osteoporosis with neuropsychiatric and gastrointestinal symptoms. Laboratory testing showed hypercalcemia (13 mg/dL) and markedly elevated PTH (614 pg/mL). Neck ultrasound and cervical CT were nondiagnostic. Technetium-99m sestamibi SPECT/CT localized a 38 × 40 mm hyperfunctioning lesion in the left ant

Lymphocyterich lesions account for the majority of relevant differential diagnoses in the anterior mediastinum and include non-neoplastic and neoplastic entities. With only 10-15% of cases, lymphomas account for a relatively small proportion of these. A systematic approach and consideration of epidemiological and clinical background information are crucial for a correct diagnosis, particularly in the case of small biopsies. A few entities dominate among mediastinal lymphomas: nodular sclerosing Hodgkin lymphoma (NSCHL) is the most common form, accounting for 50-70% of cases, followed by primar

BACKGROUND: Mucoepidermoid carcinoma (MEC) is a malignant tumor primarily composed of epidermoid, mucous, and intermediate cells. While it predominantly occurs in the salivary glands, MEC can also develop in the lungs, esophagus, breast, and thymus. Thymic MEC is rare, accounting for 2-3% of primary thymic malignancies. The primary clinical characteristic is incidental discovery in asymptomatic individuals. Due to the rarity of thymic MEC, much of the literature consists of case reports, and no standardized treatment protocol exists. Surgical resection is the primary treatment for MEC.
