Salivary gland-like low-grade clear cell carcinomas of the thoracic cavity: A clinical, immunohistochemical, and molecular analysis of three cases.

Three cases of an unusual neoplasm with striking clear cell features resembling salivary gland origin of the thoracic cavity are presented. The patients were three men between the ages of 52 and 69 years (average: 60.5 years), who presented with non-specific symptoms, such as chest pain, cough, and dyspnea. Diagnostic imaging showed that two tumors were intrapulmonary neoplasms, one in right lower lobe and one in the left upper lobe, while the third tumor was located in the anterior mediastinum. Surgical resection was accomplished in all cases. Grossly, the tumors were described as light tan,

Management of Advanced Thymoma Presenting with Myasthenia Gravis in a Resource-limited Setting: A Case Report.

Thymomas are rare tumours which generally account for only 0.2 - 1.5% of mediastinal tumours in adults. Around 40% of patients present with systemic symptoms such as motor weakness due to myasthenia gravis (MG), pure red cell aplasia, and hypogammaglobulinemia. Based on recent guidelines, management of advanced thymoma uses a multimodal approach, which is thymectomy followed by radiotherapy, but not all health care centers have radiotherapy facilities. A 52-year-old woman presented with nasal voice and had difficulty swallowing food. Patient was diagnosed with myasthenia gravis (MG). CT scan w

Inter-reader agreement for diagnosing thymic cysts on chest MRI in two tertiary referral centers.

Despite the substantial role of chest MRI for the diagnosis and follow-up of thymic cysts, information about inter-reader agreement and optimal MR sequences is still limited. We aimed to investigate the inter-reader agreement for diagnosing thymic cysts using various combinations of MR sequences and to assess the effect of the addition of CT on inter-reader agreement. A total of 76 anterior mediastinal lesions (≤ 30 mm) from two tertiary referral hospitals (55 from Institution A and 21 from Institution B) who underwent chest CT and contrast-enhanced chest MR were included. Internal and externa

Anterior mediastinal masses and thymic cysts.

Cystic lesions of the anterior mediastinum in children suggest a well-known group of benign lesions that are comparatively frequent. Thymic cysts (TCs) are mostly positioned in the anterior mediastinum and some patients in the neck. Benign TCs classified as congenital intra-thoracic mesothelial cysts are commonly asymptomatic and have slight clinical significance. Multilocular TC, which can mimic another anterior mediastinal cystic tumor and is seen in adults, is more clinically important. It is a sporadic mediastinal lesion thought to arise in the course of acquired inflammation. Congenital m

Distal muscle weakness as the main onset symptom in thymoma-associated myasthenia gravis: a case report and literature review.

Myasthenia gravis (MG) is an autoimmune disorder within the spectrum of neuromuscular rare diseases, characterized by fluctuating muscle weakness. This report presents a case of a middle-aged woman with a chronic onset of asymmetric upper limb weakness accompanied by difficulty in finger extension, without ptosis or fluctuation for 4 years. The patient was finally diagnosed with MG by a significant decrement of Compound Muscle Action Potential in repetitive nerve stimuli, positive anti-acetylcholine receptor antibodies as well as the presence of a mass located in the anterior mediastinum. Advanced thymic epithelial tumour resection: vascular resection and reconstruction strategy.

BACKGROUND: Thymic epithelial tumors (TETs) represent the predominant primary malignancy of the anterior mediastinum, often necessitating complex surgical interventions due to their invasive nature. The prognosis of advanced TET relies significantly on achieving complete resection with microscopically clear margins (R0). This frequently entails resection and reconstruction of major vascular structures like the superior vena cava (SVC) and brachiocephalic veins, and in some cases, extra pleural pneumonectomy (EPP). The aim of the study is to interrogate our series of advanced TET resections. M

Anterior Mediastinal Lymph Node Metastasis of Intrahepatic Cholangiocarcinoma: A Case Report and Literature Review.

INTRODUCTION: Intrahepatic cholangiocarcinoma (ICC) is the second most common liver malignant tumor with a poor prognosis. Lymph node (LN) metastasis is found in 15% of ICC at the time of initial diagnosis. However, the LN metastasis to the anterior mediastinum is extremely rare. Herein, we report a case of anterior mediastinal LN metastasis of ICC. CASE PRESENTATION: The patient is a 74-year-old man who had surgery for cervical esophageal cancer. During follow-up, a low-density hepatic tumor and swollen LNs in the anterior mediastinum were detected. The tumor of the liver was diagnosed as IC

Unusual tumour of the anterior mediastinum: Intrathymic bronchogenic cyst mimicking a thymoma.

Bronchogenic cysts are rare congenital cystic lesions. Their occurrence in the anterior mediastinum as intrathymic bronchogenic cyst is extremely uncommon. These cysts present as mediastinal masses, and are generally asymptomatic and often diagnosed incidentally. Despite being benign, their intrathymic location and variable radiological appearance can mimic more common conditions such as thymomas. Surgical resection is the treatment of choice to confirm the diagnosis. We report a case involving a partial thymectomy performed for a suspected thymoma in the anterior mediastinum, which was subseq

Costal chondrosarcoma in a woman with hereditary multiple exostoses - a case report.

In this report, we present a case of a 32-year-old female previously diagnosed with hereditary multiple exostoses(HME) who was incidentally found to have an asymptomatic anterior mediastinal mass during a routine examination. Computed tomography imaging revealed a well-defined mass measuring approximately 2.3 cm x 4.0 cm x 4.7 cm in the anterior mediastinum with multiple nodular areas of high density within. The mass caused compression and narrowing of the right ventricle. The patient subsequently underwent intralesional resection of the tumor, and histopathological examination confirmed a dia

Evaluation of a second-generation intercostal extravascular implantable cardioverter defibrillator lead with a pectoral pulse generator for sensing, defibrillation, and anti-tachycardia pacing.

AIMS: Intercostal extravascular implantable cardioverter defibrillator (EV-ICD) leads may work better in contact with the pericardium thereby directing pacing and defibrillation energy towards excitable myocytes. We report 3-month safety and performance outcomes with a second-generation intercostal EV-ICD lead paired with standard, commercially available ICD pulse generators (PGs). METHODS AND RESULTS: Subjects undergoing a transvenous ICD (TV-ICD) procedure received a concomitant intercostal EV-ICD lead system. The intercostal EV-ICD lead was connected sequentially to a PG in a left pectoral

Case Report: Type A invasive thymoma with renal metastasis.

Thymomas are rare malignancies originating from the epithelial cells of the thymus, typically presenting in the anterior mediastinum. Despite their indolent nature and slow growth, thymomas have the potential to metastasize to various organs, including the lungs, bones, and liver. However, renal metastasis is exceedingly rare. This case reported the details a 65-year-old male diagnosed with invasive thymoma (Type A, Masaoka stage IV) with renal metastasis seven years after the initial diagnosis. The patient underwent laparoscopic-assisted radical nephrectomy, followed by chemotherapy. The case

One-Stage Bilateral Pulmonary Nodule Resection via Unilateral Thoracic Cavity Access: A Single-Center Experience of 12 Cases.

BACKGROUND: Surgical intervention remains the primary therapeutic modality for managing multiple pulmonary nodules. However, in cases with bilateral pulmonary nodules, one-stage bilateral resection is discouraged due to tumor aggressiveness and surgical invasiveness. In light of this, we investigated an innovative approach, termed one-stage bilateral pulmonary nodule resection via unilateral thoracic cavity access. METHODS: From July 2022 to September 2024, a cohort of 12 patients with bilateral pulmonary nodules were enrolled in this study. This technique involves initial unilateral transcos

A Case of Chronic Expanding Pericardial Hematoma Treated Surgically.

Chronic expanding hematoma (CEH) is a rare condition characterized by a slowly enlarging hematoma. While chronic expanding hematomas can occur following trauma or surgery, their idiopathic presentation is exceedingly rare. An expanding hematoma can silently lead to diastolic cardiac dysfunction. A 69-year-old woman without a history of thoracic surgery or chest trauma presented with an incidentally discovered pericardial mass measuring 90 × 60 × 45 mm. Imaging revealed a mass with a calcified capsule that compressed the right heart in the anterior mediastinum. Despite initial attempts at off-p
