Giant teratomas with bilateral mediastinal extension are rare. The present case report highlights the challenging resection of a giant anterior mediastinal cystic teratoma extending bilaterally using video-assisted thoracoscopic surgery (VATS), rendering it one of the largest mediastinal teratomas managed with VATS in the current literature. A 47-year-old woman presented with a 1-year history of central chest pain. The physical examination did not reveal any notable findings. A computed tomography coronary angiogram revealed a thin-walled cyst in the anterior mediastinum, measuring ~12x10x9 cm

BACKGROUND: Thymomas are rare neoplasms of the mediastinum with an incidence of 0.15 per 100,000 person-years but represent the most common tumor of the anterior mediastinum in adults. Complete surgical removal is the mainstay treatment for thymomas. While minimally invasive surgeries (MIS) for thymoma are increasing, it remains unknown if MIS is oncologically equivalent to traditional open surgical techniques (OTs). The aim of the present study was to characterize outcomes and identify risk factors associated with recurrent disease in patients undergoing surgery for early-stage thymoma, with

Mediastinal teratomas demonstrating unidirectional pancreatic differentiation are exceedingly rare. We describe a 36-year-old woman with progressively worsening anterior chest pain. Computed tomography demonstrated a 5.9 cm multilocular cystic mass in the anterior mediastinum, accompanied by pleural and pericardial effusions. Given the lesion's size and presumed inflammatory reaction, we performed radical excision via a hemiclamshell incision. Histopathology revealed predominantly pancreatic acinar tissue with scattered non-pancreatic epithelial elements, prompting reclassification as a medias

INTRODUCTION: Venous aneurysms in the anterior mediastinum are rare. While there are scattered reports on thoracic venous aneurysms, such as those involving the azygos or brachiocephalic veins, those involving the internal thoracic vein are exceedingly rare. We herein report the case of an internal thoracic venous aneurysm that was initially suspected of being a tumor and diagnosed intraoperatively.
CASE PRESENTATION: A 50-year-old woman underwent CT during hospitalization for cerebral infarction, which revealed a well-defined 14-mm nodular lesion in the anterior mediastinum. Contrast-enhance

Anterior mediastinal soft tissue chondroma is an exceedingly rare benign tumor that poses a diagnostic challenge due to its overlapping imaging features with more common mediastinal masses. Accurate diagnosis is crucial to ensure appropriate surgical management and avoid overtreatment of benign lesions. In the present case, a 36-year-old asymptomatic woman presented with a calcified anterior mediastinal mass incidentally detected on a routine chest computed tomography. Imaging showed a well-circumscribed, lobulated mass with punctate calcifications, suggestive of a benign etiology. The mass wa

Extracardiac images are occasionally revealed on Tc-99m tetrofosmin single photon emission computed tomography-computed tomography (SPECT-CT) imaging due to the known oncotropic properties of the tracer. A large area of inhomogeneous tracer uptake was revealed on SPECT-myocardial perfusion imaging in a 59-year-old male patient referred to our department for suspected coronary artery disease. On subsequent SPECT-CT, the mass was located in the anterior mediastinum, characterized by regular margins not infiltrating the adjacent anatomical structures, and had diameters of 6 cm × 5.2 cm × 10.3 cm.

BACKGROUND: Thymic epithelial tumors (TETs) are rare, but they are the most common tumors in the anterior mediastinum. The use of artificial intelligence (AI) in the medical field is rapidly advancing. Especially in a rare disease such as TET, AI can help to improve existing care and foster new innovations. However, there are many different AI models with various implications. This systematic review aims to give an overview of recent studies on AI applications for the diagnosis and prognosis of TETs.
METHODS: In this systematic review, six electronic databases were searched for research paper

INTRODUCTION: Ectopic mediastinal parathyroid adenomas (MPAs) present unique diagnostic and surgical challenges due to their proximity to vital thoracic structures. Primary hyperparathyroidism (PHPT) is often caused by parathyroid adenomas, with approximately 15–20% of cases involving ectopic parathyroid glands. CASE PRESENTATIONS: We report three cases of ectopic MPAs with varied clinical presentations and surgical approaches. Case I involved a 64-year-old woman who had previously undergone an unsuccessful sternotomy. Imaging identified a retrosternal adenoma, which was successfully excised v

Osilodrostat is an oral steroidogenesis inhibitor used in the treatment of hypercortisolism. It works by inhibiting 11-beta-hydroxylase, a key enzyme in cortisol synthesis. As a consequence of drug action, adrenal insufficiency can be observed in about 40% of patients. Although this effect has been accepted as a consequence of therapy, recent reports suggest adrenal insufficiency can persist even after discontinuation of osilodrostat. We present the case of a 74-year-old female patient who developed prolonged adrenal insufficiency after withdrawal of osilodrostat. The patient was diagnosed wit

This case report details the successful use of awake fiberoptic intubation in a 28-year-old pregnant patient at 21 weeks gestation, who presented for diagnostic mediastinoscopy with biopsy secondary to a large anterior mediastinal mass suspected to be lymphoma. MRI displayed encasement of the superior mediastinal vasculature and 13.0 cm × 7.6 cm coalescence in the anterior mediastinum. Given the size of the mass and potential for airway and vascular complications, awake fiberoptic intubation was chosen to ensure safety for both mother and fetus. The case underscores the efficacy of awake fiber

A 67-year-old man presented with lower back pain, numbness in the lower limbs, and general malaise. Over time, he experienced insomnia, abnormal behavior, muscle weakness, weight loss, orthostatic hypotension, and vesicorectal dysfunction. He was given a diagnosis of Morvan syndrome. Chest computed tomography revealed a 4.5-cm mass in the anterior mediastinum. He underwent steroid pulse therapy and therapeutic plasmapheresis, followed by robotic subxiphoid-optical extended thymectomy. His neurologic symptoms improved, and he was discharged 3 months after surgery. This case highlights the poten

Robot-assisted thoracoscopic surgery (RATS) has emerged as a new option for treating mediastinal tumors, offering advantages of high precision and minimal invasiveness. This study uses a subxyphoid approach to compare the short-term outcomes of RATS versus video-assisted thoracoscopic surgery (VATS) for anterior mediastinal lesions; additionally, it assesses the short-term outcomes ofsubxyphoid bilateral port placement for these lesions and provides a preliminary evaluation of dual-port robotic assistance in terms of safety and feasibility. A retrospective analysis of patients treated at the T

BACKGROUND: Thymic cysts are the most common benign lesions of the anterior mediastinum and are often misdiagnosed as solid tumors, such as thymomas, on chest computed tomography (CT), leading to unnecessary surgeries. Chest magnetic resonance imaging (MRI) has emerged as a superior modality for distinguishing thymic cysts from solid lesions. This study aimed to investigate the long-term natural course of thymic cysts diagnosed using MRI and assess the need for surgical intervention based on follow-up CT imaging.
METHODS: We conducted a retrospective single-center study of 194 patients diagno

Malignant transformation of mature teratomas is rare, occurring in 1%-3% of cases, and no standard treatment has been established. According to the latest World Health Organization (WHO) histological classification, germ cell tumors with a somatic malignant component (sarcoma, carcinoma, or both) are defined as germ cell tumors with somatic malignancy (GCTSM) and have a poor prognosis. Herein, we report a rare case of a mediastinal mature cystic teratoma that showed malignant transformation. The patient achieved complete remission with multimodal treatment. A 74-year-old female non-smoker pres

Pneumomediastinum (PM) is a rare but significant complication of asthma exacerbation. We present the case of a 57-year-old woman with a history of asthma and alcoholic liver cirrhosis who presented to the ED with severe shortness of breath and an oxygen saturation of 83% on room air, as reported by EMS. On arrival, she was tachycardic and hypoxic, requiring 5 L/min of oxygen via nebulizer mask, with diffuse wheezing noted on auscultation. Despite aggressive medical management, including high-flow oxygen, bronchodilators, and steroids, her symptoms persisted. A chest radiograph was unremarkable

OBJECTIVES: This study evaluates the initial surgical results of our proposed minimally invasive extended thymectomy procedure using the "lateral view preceding method," which precedes dissection of the lower poles of the thymus and mediastinal fat around the cardiophrenic angles with lateral thoracoscopic views, followed by a robot-assisted subxiphoid approach for the remaining extended thymectomy.
METHODS: The lateral view preceding method was introduced in October 2020. We retrospectively reviewed patients with suspected thymomas in the anterior mediastinum having anti-acetylcholine recept

INTRODUCTION: Thymomas are typically located in the anterior mediastinum. Primary ectopic intrathyroidal thymoma are exceedingly rare, with only nine cases reported in the English literature. Papillary thyroid carcinoma represents the most common type of thyroid carcinomas, accounting for almost 80% of cases. Herein, we present the first case reported in the literature of primary ectopic intrathyroidal thymomas occurring synchronously with a papillary thyroid carcinoma. We aim to discuss its clinical, morphological, and immunohistochemical features, and highlight the diagnostic challenges pose
