Cystic Fluorodeoxyglucose-Avid Thymic Hyperplasia with Lymphoepithelial Sialadenitis-like Features.

Thymic hyperplasia with lymphoepithelial sialadenitis-like features (LESA-like TH) is a rare benign thymic lesion sometimes associated with autoimmune diseases or mucosa-associated lymphoid tissue lymphoma. A 47-year-old man was incidentally found to have 2 anterior mediastinal masses. Contrast-enhanced computed tomography (CT) showed irregularly walled cysts, and positron emission tomography-CT demonstrated partial fluorodeoxyglucose (FDG) uptake. A total thymectomy was performed, and histopathology confirmed LESA-like TH. This is the first report to document both imaging-visible multiplicity

Four cases of unique thoracic duct anatomical characteristics identified by fluorescence thoracoscopy.

We identified one unique kind of anatomical characteristics of thoracic duct (TD) in four cases using fluorescence thoracoscopy during right lung cancer surgery. In these patients, a collateral branch was gave off from TD at the level of the azygos vein arch, which ran along the superior edge of the azygos vein arch, moving from posterior to anterior, and then turned upward in the anterior mediastinum with complex branching patterns. The branches can be easily injured during surgery, which could lead to postoperative chylothorax.

Clinicopathologic Characteristics, Treatment Outcomes, and Survival in Thymic Neuroendocrine Neoplasms (t-NEN): A 25-Year Single-Center Experience.

Background/Objectives: Thymic neuroendocrine neoplasms (t-NENs) are rare, biologically aggressive malignancies of the anterior mediastinum. Due to their low incidence, clinical evidence remains limited, and treatment recommendations are primarily based on expert opinion and extrapolation from other neuroendocrine tumors. This study aimed to analyze the clinicopathological features, treatment patterns, and survival outcomes of patients with t-NENs treated at a single comprehensive cancer center over 25 years. Methods: A retrospective review was performed on 19 adult patients diagnosed with t-NE

Robotic subxiphoid surgical approach for mediastinal lesions: One-year experience.

BACKGROUND: Median sternotomy has been considered the gold standard approach for anterior mediastinal tumor resection. However, recent advances in video-assisted thoracoscopic surgery and robotic-assisted thoracoscopic surgery with carbon dioxide insufflation have allowed minimally invasive approaches even for large and locally invasive tumors of the upper-anterior mediastinum. The subxiphoid robotic optical approach is a recently developed technique for accessing the mediastinum. The trans-subxiphoid technique offers excellent exposure of the surgical field, reduces postoperative pain, facili

Spleen metastasis as rare systemic manifestation of thymoma: a case report.

BACKGROUND: Thymomas are thymic epithelial tumors (TETs) and the most common tumors of the anterior mediastinum. In the natural course of this malignancy, there is no tendency towards extrathoracic metastasis. This report describes an extremely rare case of splenic metastatic disease from a thymoma. Additionally, the patient developed Good's syndrome, a thymoma-associated immunological disorder which makes the patient vulnerable for opportunistic infections. This syndrome is generally treated with intravenous immunoglobulins. CASE DESCRIPTION: The patient is an asymptomatic 73-year-old man of

A Rare Case of Recurrent Sarcoma Presenting as an Anterior Mediastinal Mass: Case Report.

INTRODUCTION: Soft tissue sarcomas (STS) represent a heterogenous group of mesenchymal neoplasms that frequently arise in the extremities. In cases of disease recurrence, the lungs are the most commonly affected site. We report a rare case of a previously treated lower extremity STS with subsequent delayed recurrence in the anterior mediastinum, an uncommon site for distant metastasis. CASE PRESENTATION: Our patient was a 64-year-old woman with a history of a left medial thigh pleomorphic/spindle cell sarcoma previously treated with wide local excision and adjuvant radiation therapy in 2013.

Primary poorly differentiated enteric-type adenocarcinoma of the thymus with characteristic intracytoplasmic vacuoles on cytology.

Primary thymic enteric-type adenocarcinomas are exceedingly rare. This rare case highlights the significance of intracytoplasmic vacuoles (ICVs) as a cytologic marker for primary enteric-type adenocarcinoma of the thymus. We present a case of a 62-year-old Japanese man who experienced progressive dyspnea for two months. Chest computed tomography (CT) revealed a 66 × 65 × 50-mm irregular mass in the right anterior mediastinum. Both CT-guided biopsy and needle-washing cytology were performed. Papanicolaou-stained specimens showed atypical cells with a high nuclear-cytoplasmic (N/C) ratio, bare n

Recurrent Thymic Carcinoma Manifesting as a Diaphragmatic Mass: A Rare Presentation.

Thymic carcinoma is a rare and aggressive tumor of the anterior mediastinum, known for its varied presentations and unpredictable behavior. We report a case of a gentleman in his fifties, with a prior history of thymic carcinoma, who presented with a large thoracic mass measuring nearly 20 cm, suggestive of a recurrence, yet remarkably with no evidence of distant spread. He was managed through a multidisciplinary approach and remains disease-free at the end of one year. This case highlights the locally invasive nature of thymic tumors and underscores the importance of comprehensive, team-based

A retrospective study of prognostic factors in 29 dogs with surgically treated thymic epithelial tumors.

Client-owned dogs (N = 127) were divided into 3 categories: those with postliminary meniscal tears, those with no meniscal tears for a minimum of 2 y post-tibial plateau leveling osteotomy (TPLO), and those with meniscal tears at the time of TPLO. The latter 2 groups were randomly selected using a random-number generator. Logistic regression analysis was used to identify associations between variables. Post-hoc power was calculated. The objective was to identify risk factors leading to meniscal tears at time of TPLO and postliminary meniscal tears. Forty-four out of the 2158 cases 44 (2%) were

Defining "Giant" Mediastinal Tumors: Proposal of a New Clinical-Radiological Classification and Case Report.

Background/Objectives: Mediastinal tumors, regardless of their location, can grow to significant sizes, causing compression-related symptoms. The term "giant" mediastinal tumor is inconsistently defined in the literature. This study presents a new clinical-radiological classification (CRC) for mediastinal tumors and evaluates its applicability through a systematic review and a detailed case analysis of a giant thymolipoma. Methods: A systematic review of the literature from the past decade was conducted using PubMed to identify relevant studies on "giant" mediastinal tumors. The inclusion crit

Rare case of a massive anterior mediastinal schwannoma causing cardiac compression with concurrent cervical spine lesion causing spinal cord compression.

Schwannoma is the most common neurogenic tumor in the posterior mediastinum, however it is rarely found in the anterior mediastinum (Mediastinum 4, 2020; Radiat Med 13(4):175-7). We report a large anterior mediastinal schwannoma causing cardiac compression along with a concurrent cervical spine lesion causing right-sided weakness. Current literature having only a few case reports describing an anterior location for this neoplasm and even rarer to have a concurrent symptomatic cervical lesion. We present this case to highlight this rare finding and increase awareness of this potential diagnosis

Ectopic thymoma arising in the pleura: Diagnostic challenges and clinical implications: A case report.

Thymoma is a rare tumor originating from the thymic epithelium, typically located in the anterior mediastinum. Ectopic thymomas are extremely uncommon and may occur in the neck, middle or posterior mediastinum, lung, or pleura. We report the case of a 62-year-old woman presenting with cough, weight loss, and asthenia. Chest computed tomography (CT) revealed a right pleural mass. Complete surgical resection was performed, and the postoperative diagnosis confirmed a WHO Type AB thymoma, classified as Masaoka-Koga stage IIa. Following surgery, she underwent adjuvant radiotherapy, and no recurrenc

Incidental mediastinal masses detected on chest computed tomography scans during the COVID-19 pandemic.

OBJECTIVES: The prevalence of mediastinal masses in large-scale populations in China has been rarely reported. During COVID19 pandemic, many incidentalomas were reported due to the large amount of chest computed tomography scan performed in emergency setting. METHODS: Retrospective analysis of emergency chest computed tomography scans (February 2020-February 2021) for COVID-19 screening, including mediastinal abnormalities (excluding lymph nodes, dysplasia, pneumomediastinum and other non-mass alterations), with computed tomography features, diagnostic workup and 1 year follow-up data were re

Therapeutic management of patients with advanced thymic malignancies: A review for clinicians.

Thymic epithelial tumors (TETs) are a heterogeneous group of rare tumors that arise from thymic epithelial cells in the anterior mediastinum. They can be divided into three different histological subtypes: thymomas, thymic carcinomas (TC), and neuroendocrine carcinomas (TNET). TCs and TNETs are rarer but more aggressive entities with frequent distant metastasis. Thymomas occur in 90 % of cases in a localized/locally advanced stage, on the other hand about 70 % of TCs are locally advanced at the time of diagnosis. Surgery plays a primary role in the management of patients in whom complete resec

A Rare Case of Thymic Mucosa-Associated Lymphoid Tissue (MALT) Lymphoma.

Thymic mucosa-associated lymphoid tissue (MALT) lymphoma is a type of diffuse B-cell lymphoma located in the thymus that is extremely rare. This case report illustrates the treatment of a 39-year-old Asian female patient with thymic MALT lymphoma. She presented with constitutional symptoms and chest pain. A CT scan of the chest revealed a soft tissue density in the right anterior mediastinum measuring 4×4.2 cm, likely originating from the thymus. After counseling between the options of surveillance imaging, CT-guided biopsy, or upfront surgery, the patient opted for surgical resection of the m

Synovial Sarcoma Arising From the Falciform Ligament.

Synovial sarcoma is a rare soft tissue malignancy that is most commonly found in the extremities of adolescents and young adults. Despite accounting for a small percentage of all soft tissue sarcomas, its aggressive behaviour makes early recognition and management crucial. We present the rare case of a 64-year-old man with a large epigastric mass extending into the mediastinum and involving the pericardium, which was found to be a synovial sarcoma arising from the falciform ligament. The tumour was locally invading the liver and anterior mediastinum and was associated with pulmonary metastases
