Cardiovascular Signs and Symptoms: Chest Pain: Evaluation in the Office Setting.

Chest pain is responsible for approximately 1% of primary care encounters. Although most etiologies are benign and self-limited, some reflect underlying pathology associated with significant morbidity and mortality. The initial office evaluation for patients presenting with chest pain should include a comprehensive clinical history, physical examination, and 12-lead electrocardiography to identify those with potential cardiac etiologies. Patients with clinical evidence of acute coronary syndrome or other life-threatening causes of acute chest pain should be transported to the emergency departm

Alignment Between Cardiologists and AI-Driven Diagnostic Systems: Mixed Methods Study.

BACKGROUND: The clinical value of artificial intelligence (AI)-based diagnostic systems depends not only on their accuracy but also on how well their outputs integrate with clinicians' judgments in practice. Critical knowledge gaps remain regarding diagnostic concordance between AI and clinicians in stress echocardiography interpretation, patient characteristics predicting discordance, and how cardiologists respond when AI recommendations conflict with their clinical diagnoses. OBJECTIVE: This study examined the diagnostic alignment between an AI-driven stress echocardiography system (EchoGo

Evaluation of Coronary Artery Disease Plaques Using Coronary CT Angiography in a High-Risk Population.

Background Coronary artery disease (CAD) remains a leading cause of cardiovascular morbidity and mortality. Coronary computed tomography angiography (CCTA) enables non-invasive assessment of coronary stenosis, plaque burden, and high-risk plaque (HRP) morphology, offering incremental risk information beyond traditional clinical scoring. Objective The objective of this study was to evaluate coronary plaque characteristics and the prevalence of HRP features on CCTA in high-risk patients presenting with stable chest pain at a tertiary care center in Assam, India, and to assess their association w

Simultaneous bright- and black-blood three-dimensional whole-heart magnetic resonance imaging for integrated coronary plaque detection and luminal stenosis assessment: A prospective comparison with coronary computed tomography angiography.

BACKGROUND: Coronary computed tomographic angiography (CCTA) is a first-line test for anatomical evaluation of the coronary arteries in stable chest pain. Despite technical advances, CCTA requires breath hold and exposes patients to ionizing radiation and contrast agents. Coronary cardiovascular magnetic resonance angiography (CCMRA) has been limited by long and unpredictable scan times, lower spatial resolution, and restricted plaque characterization. We developed a novel cardiovascular magnetic resonance sequence, Bright-blood and black-blood phase SensiTive (BOOST) sequence, which produces

Coronary plaque characteristics quantified by artificial intelligence-enabled plaque analysis: Insights from a multi-ethnic asymptomatic US population.

BACKGROUND: Ethnic differences in coronary atherosclerosis remain to be fully elucidated. We aimed to assess quantitative plaque characteristics from coronary CT Angiography (CCTA) in relation to ethnicity and cardiovascular risk factors in a multi-ethnic asymptomatic US population. METHODS: This cross-sectional study retrospectively evaluated 388 asymptomatic patients selected from a prospective CCTA registry. A total of 194 patients from ethnic minority groups (Asian, African American, and Hispanic) were matched by age, sex, and cardiovascular risk factors to 194 White patients. Quantitativ

The lethal fifth cardiac chamber: a rare autopsy case report of left atrial appendage aneurysm and review of literature.

Congenital left atrial appendage (LAA) aneurysm is an extremely rare condition characterized by abnormal dilatation of the LAA. We report a fatal case of a left atrial appendage aneurysm (LAAA). The presented case was a 19-year-old male who died suddenly during vigorous exercise with no relevant medical or family history. On gross examination, we observed a semi-rounded bulge at the LAA with a diameter of approximately 5 cm. Sectioning the bulge revealed an aneurysmal dilatation encompassing recent and huge lamellated mural thrombi filling its cavity. Though coronaries showed no stenosis, the

Effects of Combining Coronary Calcium Score With Treatment on Plaque Progression in Familial Coronary Artery Disease: A Randomized Clinical Trial.

IMPORTANCE: Coronary artery calcium (CAC) scoring provides prognostic information, especially in patients at intermediate risk for coronary artery disease (CAD). However, the benefit of combining CAC score with a primary prevention strategy has not been tested in a randomized trial. OBJECTIVE: To assess whether combining the CAC score with a prevention strategy can be used to limit plaque progression in intermediate-risk patients with a family history of premature CAD. Lipoprotein(a), Cholesterol, Triglyceride Levels, and Vulnerable Coronary Plaques: A PROSPECT II Substudy.

BACKGROUND: Although lipoprotein(a) (Lp[a]) has been associated with acute myocardial infarction (MI), the relationship between Lp(a) and the presence of high-risk "vulnerable" coronary plaques has not been studied. OBJECTIVES: The aim of this study was to investigate whether specific lipoproteins are associated with pancoronary plaque volume and lipid deposition vs the development of non-flow-limiting high-risk vulnerable plaques. METHODS: In PROSPECT (Providing Regional Observations to Study Predictors of Events in the Coronary Tree) II, 3-vessel coronary artery imaging was performed with

Exome Sequencing Enhances Screening for Familial Hypercholesterolemia Within a Multi-Site Healthcare System.

BACKGROUND: Familial hypercholesterolemia (FH) is an autosomal dominant genetic disorder that increases risk for premature coronary artery disease and has accessible and effective interventions. The Dutch lipid clinic network is currently the most used diagnostic criterion; however, genetic sequencing provides a definitive diagnosis of FH. The goals of this study were to determine whether germline genetic screening using exome sequencing could be used to efficiently identify individuals who were genotype positive for FH. METHODS: Participants were recruited from 3 geographically and racially

Anomalous left coronary artery from the pulmonary artery: A case report of ALCAPA syndrome with bicuspid aortic valve.

RATIONALE: Anomalous left coronary artery from the pulmonary artery (ALCAPA) syndrome, also known as Bland-White-Garland syndrome, is a rare congenital heart defect predominantly diagnosed in infants and associated with high early mortality rates without surgical intervention. While ALCAPA generally presents alone, it can sometimes occur alongside other congenital anomalies, such as a bicuspid aortic valve (BAV), a combination scarcely documented in the literature. This case report explores a rare presentation of ALCAPA syndrome in a 27-year-old adult with concurrent BAV. PATIENT CONCERNS: Th

Isolated non-ischemic left ventricular scar in athletes: scientific and clinical rationale behind the 2023 Recommendations of the Italian Sports Cardiology Guidelines (COCIS).

Isolated non-ischemic left ventricular scar (NLVS) refers to the presence of significant late gadolinium enhancement (LGE) with subepicardial/midmyocardial distribution detected through cardiac magnetic resonance (CMR) in the absence of other features of a specific disease and excluding the junctional spotty pattern that is not pathological. This nosographic entity is relatively recent, emerging with the widespread clinical use of CMR. The NLVS has recently been associated with cardiac arrest in sports. On the other hand, cohort where CMR was performed on apparently healthy volunteers revealed

Treated HIV infection is not associated with carotid vascular inflammation or plaque progression as assessed by dynamic contrast magnetic resonance imaging.

BACKGROUND: Inflammation and immune dysregulation are thought to drive residual cardiovascular disease risk among persons living with human immunodeficiency virus (HIV) (PLWH) despite effective viral suppression with antiretroviral therapy (ART). METHODS: We investigated differences in carotid inflammation and atherosclerosis in a longitudinal cohort of virally suppressed PLWH (N = 50; on stable ART with CD4 > 250 cells/mm3, viral load < 200 copies/mL for > 6 months) and HIV-uninfected controls (N = 51) matched for age, sex, hypertension, diabetes, smoking, hyperlipidemia, and family history

Factors Associated With Coronary Angiography Performed Within 6 Months of Randomization to the Conservative Strategy in the ISCHEMIA Trial.

BACKGROUND: ISCHEMIA (International Study of Comparative Health Effectiveness With Medical and Invasive Approaches) did not find an overall reduction in cardiovascular events with an initial invasive versus conservative management strategy in chronic coronary disease; however, there were conservative strategy participants who underwent invasive coronary angiography early postrandomization (within 6 months). Identifying factors associated with angiography in conservative strategy participants will inform clinical decision-making in patients with chronic coronary disease. METHODS: Factors indep

Cancer and left atrial enlargement in patients with ischemic stroke.

BACKGROUND: Cancer is associated with an increased risk of atrial fibrillation. Whether cancer is also associated with atrial cardiopathy, another atrial pathology associated with heightened ischemic stroke risk, is uncertain. METHODS: We conducted a retrospective cross-sectional study among consecutive patients hospitalized with acute ischemic stroke at a quaternary care center in New York, United States from 2011 through 2016. The study exposure was active cancer. The study outcome was atrial cardiopathy, defined as a left atrial volume index ≥35 mL/m2 on echocardiography. We used multivari

Clinical Profiles and Risk Factors of Acute Myocardial Infarction in Young Adults: A Cross-Sectional Study in Myanmar.

Introduction Coronary artery disease (CAD) remains a leading cause of morbidity and mortality globally, with an increasing prevalence of acute myocardial infarction (AMI) among younger populations. Despite this rising trend, there are limited data from Myanmar on the clinical profile and associated risk factors for premature CAD in young adults. This study aims to investigate the clinical characteristics and predisposing risk factors for AMI in individuals aged 40 years and below, contributing to a better understanding of disease patterns in this population.
