Identification of abdominal aortic aneurysm patients with different clinical features and clinical outcomes.

To determine the clinical characteristics and factors influencing outcome in patients with atherosclerotic abdominal aortic aneurysms (AAA), 526 patients who underwent aneurysmal resection were retrospectively reviewed: Group I had clinical evidence of atherosclerotic occlusive disease; Group II had no evidence of atherosclerotic occlusive disease. The incidence of ruptured AAA, multiple aneurysms, and a family history of AAA was higher in Group II patients. We concluded that patients with AAA and without atherosclerotic occlusive disease in other areas represent a subgroup with peculiar clini

Chest pain in a young woman.

Chest pain in a young person is often caused by chest wall tenderness, associated with mitral valve prolapse, or attributed to psychologic factors. Ischemic cardiac pain may be overlooked because of its rare occurrence in this age group. A 35-year-old woman complained of substernal chest pressure precipitated by exertion and relieved by rest. The symptom had been noted for 15 years. Worsening of the symptom during dancing prompted her to seek medical advice. She had no other illnesses, was taking no medications, was a nonsmoker, and had no family history of coronary disease. Physical examinati

[Arrhythmias in hypertrophic cardiomyopathy. Significance and therapeutic consequences].

The natural history of the patient with hypertrophic cardiomyopathy is characterized by slow progression of symptoms and left ventricular hypertrophy; severe functional limitation is unusual and encountered only in about 20%. Most of the deaths are sudden and occur irrespective of the functional status of the patient and there are no routine electrocardiographic or hemodynamic measurements indicative of poor prognosis. Sudden death may be associated with syncope and young age at diagnosis as well as with a family history of sudden death. The mechanisms underlying the cause of sudden death have

Exercise testing in suspected coronary artery disease.

The interpretation and selection of exercise tests depends on the pretest probability of CAD. Imperfect tests (like exercise tests) provide probability estimates, not definite statements (such as "the patient has CAD" or "the patient does not have CAD"). In patients with a low pretest probability of CAD (asymptomatic persons or men and women with nonanginal chest pain), abnormal exercise test results provide probability estimates that are much too low to conclude that the patient has CAD. In patients with anginal pain and normal exercise tests, the probability of CAD is too high to conclude th

Family-directed preventive cardiology.

Cardiovascular events continue as the leading cause of death in the United States, despite a decline in recent years. Epidemiological evidence has confirmed that in adults smoking, high blood pressure, hypercholesterolemia, a family history of atherosclerotic disease at an early age, diabetes mellitus, and certain behavioral patterns are associated with a higher risk for myocardial infarction and other manifestations of coronary artery occlusion. In addition, a lack of regular physical activity, being overweight, and excess ethanol consumption are associated with increased cardiac disease rate

Family history as an independent risk factor for coronary artery disease.

The risk of family history of ischemic heart disease independent of other well described risk factors has remained difficult to quantitate. Significant coronary artery disease was determined by coronary arteriography to be present in 223 patients and absent in 57 control subjects. Age, sex, blood pressure, serum cholesterol, cigarette smoking and the presence of diabetes and left ventricular hypertrophy on the electrocardiogram were tabulated for each patient and the data used to assign a risk score based on the American Heart Association multivariate model. Subjects were stratified and matche

Coronary revascularization under age 40 years. Risk factors and results of surgery.

Groups of patients such as the elderly, the diabetic and women have been studied to evaluate the effectiveness of coronary revascularization. In this report 77 patients under age 40 years undergoing coronary revascularization were studied. There was a high prevalence rate of predisposing factors. Sixty-eight percent reported a family history of heart disease and 27 percent a history of diabetes; 57 percent were hypertensive, 43 percent were overweight, 91 percent smoked, 5 percent were diabetic and 16 percent had abnormal glucose tolerance curves. Sixty-four percent had hypercholesterolemia (c

Concomitants of atherosclerotic carotid artery stenosis.

To identify metabolic and other concomitants of a single important type of atherosclerotic cerebrovascular disease, 75 patients with angiographically and surgically proven internal carotid stenosis were compared with age and sex matched control subjects with respect to plasma cholesterol, triglycerides and glucose. They were also compared for blood pressure, cigarette smoking, evidence of ischemic heart disease, peripheral vascular disease and for a family history of these diseases. Patients with carotid stenosis had higher systolic and diastolic blood pressures and higher plasma cholesterol a
