Prevalence and determinants of cardiometabolic comorbidities among people living with HIV receiving antiretroviral therapy at Zewditu Memorial Hospital: evidence from Ethiopia's longest serving AIDS clinic.

BACKGROUND: The advent of highly active antiretroviral therapy (HAART) has significantly improved life expectancy among People Living with HIV (PLHIV). However, this success has been accompanied by an unprecedented rise in non-communicable diseases, particularly cardiometabolic comorbidities. While evidence from Sub-Saharan Africa demonstrates the significance of these comorbidities, comprehensive data from Ethiopia remains limited. OBJECTIVE: To determine the prevalence and identify the clinical, behavioral, and antiretroviral therapy-related factors associated with cardiometabolic comorbidit

Premature "asymptomatic" peripheral artery disease and correlates among HIV-positive adults in Ghana: findings from the SCRIPT study.

BACKGROUND: Peripheral artery disease represents a significant but yet under-recognized cardiovascular manifestation in people living with HIV. Despite the known increased global risk for PAD in people living with HIV, the burden of PAD remains unexplored in Ghana. The study assessed the prevalence and associated risk factors for PAD among HIV-positive persons in Ghana. METHODS: The study utilized a cross-sectional comparative design, recruiting consecutive HIV-positive adults from the HIV clinic of the Komfo Anokye Teaching Hospital and HIV-negative adults from surrounding communities in Ghan

Tell or Not to Tell: Changes in Ukrainian Older Adults' HIV Status Disclosure Practices During Intersecting Covid-19 and War Crises.

Humanitarian crises exacerbate challenges to accessing HIV care. Older people with HIV (OPWH, aged ≥ 50 years) may seek additional support with HIV care, which often requires disclosing their HIV status to healthcare providers, family, or friends. We sought to understand how crises, including COVID-19 and the war with the Russian Federation, have changed OPWH disclosure behaviours over time. We surveyed OPWH in Kyiv, Ukraine, by phone at four-time points: May-June 2020 (Wave 1), January-February 2021 (Wave 2), January-February 2022 (Wave 3) and May-June 2022 (Wave 4). Recommendations from the 2nd Consensus Workshop on Analytical Treatment Interruption in HIV Research Trials.

Analytical treatment interruption (ATI) is an important tool to evaluate interventions designed to achieve HIV eradication or long-term control without antiretroviral therapy. We convened a stakeholder workshop to update the published recommendations for the conduct of ATI studies. Important changes to the recommendations include more lenient entry criteria on CD4 count, CD4 nadir, and past medical history, and allowance for greater variation based on regional standards. As the mechanism of potential HIV remission might rely on immune control, the criteria for antiretroviral therapy resumption

Validation of a Risk Prediction Nomogram for Virologic Failure Among Patients on First-Line ART After ONE-J Program Implementation in Northwest Ethiopia.

IntroductionVirological failure is defined as a plasma viral load >1000 copies/mL on 2 consecutive tests after 3 months of adherence support and 6 months of ART. In Ethiopia, limited studies have developed risk prediction models, though such models are vital for guiding patient-specific interventions and improving HIV treatment outcomes.ObjectiveThis study aims to determine the incidence and prognostic factors of virologic failure among HIV patients on first-line HAART following the one-J program at Felege Hiwot Hospital, Northwest Ethiopia, in 2025.MethodsA retrospective cohort study was cond

New onset seizure etiologies and outcomes among adults with HIV in the era of broadly available antiretroviral therapies.

OBJECTIVES: In this era of widely available antiretroviral therapy (ART), information regarding new onset seizure, seizure recurrence and mortality is needed to guide clinical decision-making. METHODS: A rural, prospective, cohort study was conducted April 2016 to June 2019. Inclusion criteria were HIV positivity, age ≥ 18 years, and new onset seizure. Two-year seizure recurrence was the outcome. Risk factors for seizure recurrence were evaluated using a Fine-Gray model to account for the competing outcome of death. RESULTS: 258 people were screened, 95 were eligible and 89 were enrolled. 47

Prevalence, associated factors and geospatial patterns of HIV drug resistance in South Africa among the virally unsuppressed adults: updated results from the 2017 national HIV household survey.

BACKGROUND: South Africa bears the highest HIV burden globally, with substantial antiretroviral therapy (ART) scale-up. HIV drug resistance (HIVDR) threatens progress toward UNAIDS 95–95-95 targets. We assessed national HIVDR prevalence and associated factors among virally unsuppressed adults in 2017, before dolutegravir (DTG)-rollout. METHODS: Secondary analysis of the 2017 national HIV household survey, included 1 253 virally unsuppressed (defined as ≥ 1 000 copies/mL) adults (≥ 15 years) with HIVDR results. Survey-weighted analysis generated nationally representative estimates. Weighted mul

Deaths of Female Sex Workers from HIV/AIDS in Kenya, Nigeria, and the Democratic Republic of the Congo 2019-2023 and Barriers to Antiretroviral Therapy Adherence.

INTRODUCTION: Female sex workers (FSWs) face heightened health risks, particularly from HIV, which accounts for 8% of FSW deaths in low- and middle-income countries. Adherence to antiretroviral therapy (ART) remains suboptimal and few studies have explored drivers of ART non-adherence among FSWs who died from HIV in sub-Saharan Africa. METHODS: Using a community knowledge approach methodology, data on HIV-related deaths among FSWs were collected from a convenience sample across Kenya, Nigeria, and the Democratic Republic of the Congo between 1 February 2022, and 28 February 2023. Church-based problem-solving therapy for adolescent girls and young women with a history of gender-based violence in Zambia: study protocol for a hybrid type 1 randomized controlled trial.

BACKGROUND: Adolescent girls and young women (AGYW) in sub-Saharan Africa face a disproportionate burden of gender-based violence (GBV), HIV risk, and mental health challenges. Despite the demonstrated effectiveness of psychological interventions such as problem-solving therapy (PST), there is limited evidence on culturally adapted models for GBV-exposed AGYW that concurrently address HIV and mental health, particularly in settings with constrained mental health infrastructure. OBJECTIVE: This study outlines the protocol for a randomized controlled trial evaluating Mpata Yathu, a church-based

Impact of HIV diagnosis on job separation in Japanese employed men: a retrospective matched cohort study using an administrative claims database.

BACKGROUND: People living with HIV (PLWH) have been reported to have higher unemployment rates than the general population. However, most previous studies were cross-sectional and lacked appropriate controls. To assess the impact of HIV diagnosis on job separation, we conducted a retrospective matched cohort study using a Japanese claims database of employed men. METHODS: We conducted a retrospective matched cohort study of newly diagnosed male PLWH and non-PLWH controls using a Japanese administrative claims database mainly covering employees of larger enterprises. PLWH were matched 1:5 with

Needs Assessment to Inform a Life Course-Based Support Programme for Adolescents with Perinatally Acquired HIV in Rural Limpopo, South Africa.

This study explored the challenges and support needs of adolescents identified as living with perinatally acquired HIV based on clinical history and early childhood treatment records to generate evidence that can inform the development of an adolescent-centred support programme. A mixed-methods, sequential exploratory design was employed. In the qualitative phase, semi-structured interviews were conducted with 21 APHIV (mean age 15.5 years, range 12-19; 61.9% female) recruited from health facilities in rural Limpopo, South Africa. In the quantitative phase, a survey informed by qualitative fin

Cardiovascular diseases among people living with HIV/AIDS in Ethiopia: A scoping review.

BACKGROUND: Human Immunodeficiency Virus (HIV)/ Acquired Immunodeficiency Syndrome (AIDS) remains a significant burden in Ethiopia. People living with HIV/AIDS (PLWHA) are at increased risk of cardiovascular diseases due to both the virus and the side effects of antiretroviral therapy. Despite a growing body of research on cardiovascular diseases in PLWHA in Ethiopia, no review has synthesised the available evidence. This scoping review aims to summarise the existing literature on cardiovascular diseases among PLWHA in Ethiopia. METHODS: This scoping review followed the Arksey and O'Malley (2

Combined injectable regimens in treatment-experienced HIV/AIDS patients at a southern academic medical center: a case series.

BACKGROUND: Many people living with HIV (human immunodeficiency virus) or their caregivers are challenged with consistent adherence to oral antiretroviral therapy (ART) and have a strong preference for long-acting injectable options. However, there are limited data describing the safety and efficacy of fully injectable regimens beyond long-acting cabotegravir-rilpivirine (CAB/RPV-LA) alone. METHODS: From January 1, 2023 to August 1, 2025, we identified fifteen patients who required completely injectable ART regimens but were ineligible for CAB/RPV-LA alone due to resistance. Data were collect

Determinants of cardiovascular and metabolic noncommunicable diseases among people living with HIV in Rwanda: protocol for the NCOHIRWA prospective cohort study.

BACKGROUND: Noncommunicable diseases (NCDs), particularly cardiovascular diseases (CVDs), hypertension and diabetes, are major contributors to morbidity and mortality globally and in Rwanda, where the dual burden of HIV and NCDs is increasing. Evidence remains limited on how biological, behavioural, social, environmental and health-system determinants influence cardiovascular and metabolic outcomes among people living with HIV (PLHIV) in Rwanda. The NCOHIRWA cohort was established to prospectively investigate determinants associated with CVDs, hypertension, diabetes and related NCD outcomes am

Transgender and gender diverse persons' sexually transmitted infection testing: a mixed methods study to understand primary care providers' training gaps and desires to promote sexual health equity.

BACKGROUND: Transgender (trans) and gender diverse (TGD) people experience sexual health inequities, including high rates of HIV and other sexually transmitted infections (STIs), driven by anti-trans stigma. Yet, TGD people also face barriers to STI testing related to anti-trans stigma, including a lack of access to knowledgeable and affirming primary care providers (PCPs), including physicians, nurse practitioners, and physician assistants. We sought to understand the current practices, comfort, and knowledge gaps of PCPs in performing gender-affirming STI testing, particularly non-HIV STI te
