Endoscopic Ultrasound With Bronchoscope-Guided Fine-Needle Aspiration (EUS-B-FNA) in a six-Month-Old Infant: A Case Based Review of Literature.

Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is an established, minimally invasive modality for evaluating mediastinal pathologies in adults. However, its application in infants remains extremely limited because of airway size constraints and procedural safety concerns. We report the case of a 6-month-old infant presenting with recurrent lower respiratory tract infections and a posterior mediastinal lesion causing bronchial compression. Contrast-enhanced computed tomography demonstrated a well-defined cystic lesion in the right posterior mediastinum, with differ

Case Report: A case of thoracic schwannoma: diagnostic work-up of an incidentally detected mediastinal lesion.

Schwannomas represent the most common neurogenic tumors of the posterior mediastinum. Although typically slow-growing and asymptomatic, degenerative changes such as cystic transformation may alter their imaging appearance and mimic necrotic lymphadenopathy or malignant mediastinal masses. A 61-year-old man underwent chest CT for respiratory symptoms. Careful image review revealed an incidental subcarinal mediastinal lesion initially suspected to represent necrotic lymphadenopathy. Further evaluation with contrast-enhanced CT, 18F-FDG PET/CT and endobronchial ultrasound-guided transbronchial ne

Cavernous ganglioneuroma of the thoracic spine: surgical nuances and outcomes following limited anterior resection.

BACKGROUND: Spinal tumors constitute approximately 15% of all primary central nervous system neoplasms, with extradural tumors accounting for the majority. Ganglioneuromas are rare, benign tumors of neural crest origin, most often located in the posterior mediastinum or retroperitoneum. Involvement of the spinal column from a benign tumor is uncommon and poses significant diagnostic and therapeutic challenges. CASE PRESENTATION: We present the case of 25-year-old woman who was admitted with progressive dyspnea, stridor dysphagia, and chest pain. A thoracic CT and MRI scans revealed a 12 × 8 ×

Extracranial schwannomas of peripheral nervous system: An aesthetic challenge - A comprehensive analysis of 38 cases.

BACKGROUND: Schwannomas are benign, slow-growing tumours that arise from Schwann cells. Extracranial schwannomas are less common than those occurring at the intracranial vestibulocochlear and cerebellopontine angles. Because extracranial schwannomas are uncommon, it is difficult to make an accurate clinical diagnosis. Therefore, histopathological examination is the gold standard. METHODS: In this study, we aim to describe the incidence, presenting clinical features, anatomical location, size, and initial (presumption) diagnosis of thirty-eight cases that were histopathologically diagnosed as

Successful resection of a large duplication cyst via retropleural thoracotomy approach in a 3-month-old infant from Kabul, Afghanistan: a case report.

INTRODUCTION: Duplication cysts are rare congenital malformations arising from developmental anomalies of the primitive foregut, with an incidence of approximately 1 in 8200 live births. These cysts may present at any point along the esophagus but most commonly occur in the upper thoracic region. CASE PRESENTATION: This report presents a rare case of a large thoracic duplication cyst in a 3-month-old female Asian infant who presented with respiratory distress and cyanosis. Imaging studies, particularly computed tomography scans, revealed a large cystic mass in the posterior mediastinum. Surgi

Surgical management of atypically located thoracic schwannomas: a single-center case series.

Schwannomas are benign nerve sheath tumors most commonly located in the posterior mediastinum. However, atypically located thoracic schwannomas are rare and may pose challenges in diagnosis and surgical management. In this retrospective case series, 14 patients who underwent surgical treatment for atypically located thoracic schwannomas between 2011 and 2025 were evaluated. Demographic, clinical, radiological, surgical, and histopathological data were analyzed. The associations between tumor location and clinical or surgical variables were assessed using Fisher’s exact test and the Chi-square

Preventing Recurrent Laryngeal Nerve Injury During Transhiatal Esophagectomy Through Targeted Cervical Dissection: A Case Report.

Injury to the recurrent laryngeal nerve (RLN) is a well-recognized complication of esophagectomy and is associated with significant postoperative morbidity, including dysphonia, aspiration, and pulmonary complications. The risk is particularly relevant during transhiatal esophagectomy, where limited mediastinal visualization increases the likelihood of traction-related nerve injury. We report a case of a 68-year-old male with distal esophageal carcinoma who underwent transhiatal esophagectomy following neoadjuvant therapy, in whom a targeted cervical dissection strategy was employed to achieve

Improved surgical outcomes and predictors of prolonged hospitalization in descending necrotizing mediastinitis: A 20-Year retrospective study.

OBJECTIVE: Descending necrotizing mediastinitis (DNM) is a life-threatening infection that causes by cervical infections. An appropriate diagnosis and surgery are critical for improving outcomes. We retrospectively analyzed 37 surgically treated patients over a 20-year period to identify factors associated with improved survival and prolonged hospitalization. METHODS: Patients who underwent combined cervical and thoracic drainage for DNM at our institution in 2004–2024 were reviewed. Clinical data, surgical approaches, outcomes, and risk factors for long-term hospitalization were analyzed. RES

An Unexpected Diagnosis After a Viral Infection: A Case of a Giant Mediastinal Ganglioneuroma.

Ganglioneuromas (GN) are rare, benign tumors that develop from sympathetic ganglia. They usually appear in children and young adults and are mostly found in the posterior mediastinum or retroperitoneum. When they occur in older adults, diagnosis can be complicated due to their slow growth, vague symptoms, and similarities in imaging to other malignant masses in the mediastinum. We describe the case of a 61-year-old woman who had a persistent dry cough, wheezing, and shortness of breath after an influenza infection. Initial medical treatment was ineffective, prompting imaging that showed a hete

The Impact of Aging on the Anatomical Course of the Azygos Vein: A Retrospective, Computed Tomography Study.

The azygos vein (AV) is typically described as ascending vertically to the right of the vertebral column before arching anteriorly to drain into the superior vena cava. However, a small number of studies suggest that it is found leftward in older adults. A retrospective analysis of the contrast-enhanced thoracic computed tomography studies from 291 individuals was conducted with ethical approval (158 females; ages 0-100). The vertebral level (VL) of the AV arch was designated "V0" with subsequent caudal VLs designated V1-V5. At each VL, the position of the AV and thoracic aorta (TA) relative t

Cervical esophagocolic anastomosis leakage caused by a tension pneumothorax after an esophagocoloplasty for caustic esophageal stricture in a child: a case report.

BACKGROUND: Esophageal stricture is the most frequent caustic ingestion sequela, leading to dysphagia and malnutrition. The colon is the most commonly used graft for esophageal reconstruction. The most dreadful early complication of colonic interposition is leakage of cervical esophagocolic anastomosis, which can be caused by several factors. Tension pneumothorax has never been reported in the medical literature. CASE PRESENTATION: A 5 year-old Black African male patient with retrosternal pain and dysphagia was admitted to the department of surgery. The patient had a history of accidental cau

A surgical case of posterior mediastinum chordoma requiring differentiation from neurogenic tumor.

INTRODUCTION: Chordoma is a malignant bone and soft tissue tumor derived from remnants of the embryonic notochord. It accounts for 1–4% of all malignant bone tumors and most commonly arises in the sacrum and skull base. Mediastinal chordomas are extremely rare, comprising only 1–3% of all chordomas. CASE PRESENTATION: A 46-year-old male patient presented to the clinic for a routine check-up and was referred to our hospital after an abnormal chest shadow was detected during a routine physical examination. A contrast-enhanced computed tomography scan revealed a spindle-shaped lesion with an extr

Comparison of 18 F-FDG and 18 F-FAPI PET/CT Findings of Primary Extraskeletal Ewing Sarcoma.

Primary extraskeletal Ewing sarcoma is rare. This report presents a 47‑year‑old man with primary extraskeletal Ewing sarcoma located in the posterior mediastinum, who underwent both 18 F-FDG and 18 F-FAPI-74 PET/CT imaging. The scans demonstrated a notable discordance in tracer uptake: the mass exhibited intense FDG avidity, while FAPI uptake was relatively low and confined to the peripheral rim. The biopsy pathology confirmed the diagnosis of Ewing sarcoma.

A rare case of unicentric Castleman disease in the posterior mediastinum: case report and integrated literature review.

BACKGROUND: Castleman disease (CD) is a complex lymphoproliferative disorder characterized by nonspecific clinical manifestations and imaging features. When occurring within the mediastinum, it is easily confused with other diseases, posing diagnostic challenges. We report a case of unicentric hyaline vascular Castleman disease (UCD) located in the posterior mediastinum. This case report is supplemented by a retrospective analysis of the literature, aiming to enhance understanding and improve diagnostic accuracy for this condition. CASE SUMMARY: This paper provides a detailed account of the c

Rare Posterior Mediastinal Müllerian Cyst Resected by VATS: Case Report and Literature Review.

Background: Müllerian cysts of the posterior mediastinum are exceedingly rare benign lesions that closely resemble other mediastinal cysts on imaging, making preoperative diagnosis difficult. Methods: Here, we report the case of a 36-year-old woman in whom a paravertebral cystic lesion at the T8 level was incidentally detected during evaluation of nonspecific pain in the right upper limb. Laboratory tests and chest computed tomography were unremarkable apart from the well-circumscribed homogeneous mediastinal cyst. The lesion was completely excised via video-assisted thoracoscopic resection. R

Mediastinal parathyroid adenoma: diagnostic and therapeutic challenges.

Mediastinal parathyroid adenomas (MPAs) represent an uncommon but clinically important cause of persistent or recurrent primary hyperparathyroidism. Though most cases of sporadic primary hyperparathyroidism are due to a parathyroid adenoma in the cervical area; ectopic glands account for a fifth of cases, with only 1-2% situated in the mediastinum. Aberrant embryological descent of the inferior parathyroid glands along the thymic descent pathway gives rise to MPAs, with variable locations within the anterior or posterior mediastinum. The anatomical variability, coupled with limited accessibili

The Wandering Pancreas: A Rare Case of Complex Hiatal Hernia Presenting With Thoracic Symptoms.

Hiatal hernia (HH) is defined as the protrusion of abdominal contents through the esophageal hiatus of the diaphragm into the mediastinum. While type I sliding hernias are common, type IV hernias, which are characterized by the herniation of organs other than the stomach, such as the colon or spleen, are uncommon. Even more exceptional is the herniation of the pancreas, given its retroperitoneal fixation. We report the case of a 60-year-old female with a history of obesity (BMI 32 kg/m²) who presented with a six-month history of chronic dry cough, dyspnea, and retrosternal heaviness. Initial c

Incidentally Detected Mediastinal Mass by Transesophageal Echocardiography in a Patient With Severe Mitral Stenosis.

A mediastinal mass can be incidentally detected during transesophageal echocardiography, so clinicians performing this procedure should be aware of this possibility and able to interpret related echocardiographic findings. In evaluating a 69-year-old man with dyspnea, mitral stenosis was identified on transthoracic echocardiography. Subsequently, during transesophageal echocardiography, a mediastinal mass in the posterior mediastinum was observed. The computed tomography scan confirmed the transesophageal echocardiography findings and suggested that lymphoma and metastatic malignancy should be

Retroperitoneal ganglioneuroma with ectopic inferior vena cava invasion: a case report.

Ganglioneuroma (GN) is a rare benign tumor from neural crest tissue, composed of mature Schwann cells, ganglion cells, and nerve fibers. These tumors most often arise in the retroperitoneal region (52%) and posterior mediastinum (39%), while extra-adrenal lesions account for 51% of abdominal GN cases. This paper presents a 66-year-old female with paroxysmal atrial fibrillation found to have a left retroperitoneal mass on imaging. A 9.5-cm tumor was fully resected using robot-assisted laparoscopy. Pathology confirmed GN with ectopic inferior vena cava (IVC) invasion. This case highlights the ne
