[Surgical access for thoracic and intrathoracic goiter].

OBJECTIVE: To evaluate the effectiveness of surgical approach for resection of retrosternal and intrathoracic goiter. MATERIAL AND METHODS: There were 33 patients who underwent surgery for retrosternal (31) and intrathoracic goiter (2) through various surgical approaches. For retrosternal goiter, a Farabeuf hook was used to remove a large retrosternal component of tumor. RESULTS: In 28 cases (84.8%), substernal goiter was resected through cervical collar incision. In 4 cases, a Farabeuf hook was used to extract retrosternal part of the goiter to the neck and avoid sternotomy. In one case, a

An Unusual Case of Extracavitary/Solid Variant Primary Effusion Lymphoma With Associated Hemophagocytic Lymphohistiocytosis.

Primary effusion lymphoma (PEL) is a rare, aggressive large B-cell lymphoma variant that is invariably associated with human herpesvirus 8 (HHV8), predominantly in human immunodeficiency virus (HIV)-infected patients, and its oncogenicity is often augmented by coinfection with Epstein-Barr virus. It typically presents as a serous effusion in body cavities without detectable solid tumors. The extracavitary variant of PEL may represent a diagnostic challenge. A 37-year-old man with HIV/acquired immunodeficiency syndrome (AIDS) was transferred to our hospital for evaluation of a mediastinal mass

Intrathoracic Schwannoma With Horner Syndrome: A Rare Association.

An incidental large mediastinal mass on the left hemithorax was noted in the chest radiograph of a 21-year-old male with a history of active smoking (five pack years) after a manual reduction for left shoulder dislocation. Horner syndrome (HS) was elicited from history and physical examination. A contrast computed tomography (CT) of the thorax showed a large (8.1 x 7.3 x 7.3 cm), well-defined, heterogeneous enhancing mass over the left apical-posterior mediastinum. The CT-guided biopsy showed spindle cells, suggestive of lung neoplasm. Initial concerns were thus present for lung malignancy and

A rare case of posterior mediastinal myelolipoma resected using robot-assisted thoracic surgery.

INTRODUCTION: Myelolipomas are rare tumors containing adipose tissue and normal hematopoietic cells that mainly occur in the adrenal cortex. Mediastinal myelolipomas are extremely rare. The three-dimensional, high-definition view with up to 10 times image magnification, 7 degrees of freedom of surgical instruments, and filtration of physiologic hand tremors with robotic systems are ideal for removing mediastinal tumors in a small space. Herein, we report a case of posterior mediastinal myelolipoma treated using robotic resection. CASE PRESENTATION: A 72-year-old man with an abnormal shadow on

Successful Management of a Penetrating Cardiac Injury by Date Palm Thorn: A Case Report.

Penetrating cardiac injury in children is a life-threatening emergency. The initial reaction of the witness and medical team management plays a crucial role in survival. We are reporting a five-year-old girl who had a penetrating cardiac injury after an accidental fall on a sharp date tree thorn, witnessed by her parents. They took their child immediately to the hospital without manipulating the thorn. The initial workup included an X-ray, echocardiography, and computed tomography (CT) of the chest, revealing that the thorn transfixed the heart from the anterior chest wall to the posterior med

Beyond the Neck: When Is Thoracic Operation Needed for Thyroidectomy?

BACKGROUND: Although most patients with intrathoracic thyroid glands can undergo thyroidectomy via a cervical incision, some cases necessitate intraoperative thoracic surgery assistance. This study aimed to identify CT findings that predict the need for thoracic surgery assistance during thyroidectomy. STUDY DESIGN: We reviewed administrative data at our institution (2012 to 2023), encompassing 7,370 thyroidectomies, to identify patients who underwent thyroidectomy for intrathoracic thyroid glands, defined as thyroid glands that extended below the sternal notch. Detailed CT analysis was condu

A Case of Intramural Esophageal Bronchogenic Cyst.

A bronchogenic cyst is a congenital malformation with a rare esophageal origin. Occasionally, it is discovered incidentally, and excision is the only treatment. A 43-year-old patient was referred to our hospital for treatment of a cystic tumor in the left posterior mediastinum. We suspected the tumor was a bronchogenic cyst by imaging findings; therefore, we performed video-assisted thoracoscopic resection. The patient was discharged 9 days postoperatively. The tumor origin was found to be the esophageal wall. This report presents a rare case in which the esophageal wall was the origin of the

Primary mediastinal tumors in children and adults: a clinicopathological spectrum.

BACKGROUND: Mediastinal tumors are a heterogenous group of unrelated neoplasms that share the mediastinal anatomical location. There are no local publications regarding mediastinal masses, as such the clinical and pathological characteristics of this group of tumors in our population has been overlooked. The study aims to describe the mediastinal masses clinicopathological spectrum in children and adults. METHODS: With the objective to compare the clinicopathological spectrum of mediastinal tumors among children and adults we retrospectively review the medical charts and pathology reports of

Rare Repeat: A Case Report and Literature Review of Recurrent Intrathoracic Schwannomas.

A schwannoma is a benign peripheral nerve tumor often located in the head and neck, posterior mediastinum, and, less frequently, in the chest wall. The recurrence of schwannoma at the same anatomical site after complete surgical resection is rare. This study discusses the case of a 60-year-old man with a recurring intercostal nerve schwannoma, which appeared eleven years after initial surgical resection in the same area of the chest wall. A second intrathoracic surgery was performed to remove a 6.0 x 3.0 x 2.5 cm partially encapsulated mass. The tumor was completely excised through a thoracoto

Mature Teratoma of Posterior Mediastinum in a Newborn: A Case Report.

Teratomas are germ cell tumors that can appear in different parts of the body, most often in the ovaries or testes. Treatment depends on whether the tumor is mature or immature: mature teratomas are usually removed surgically, while immature ones often require chemotherapy as well. Teratomas located in the mediastinum, particularly in the back part (posterior mediastinum), are rare. These tumors are typically discovered by chance during imaging exams, and only a few cases cause noticeable symptoms. In this report, we describe a rare case involving a 9-day-old female newborn with no prior medic

Mullerian cyst of the posterior mediastinum: utilizing indocyanine green for thoracic duct visualization during robotic-assisted resection.

We describe a case of a 45-year-old female who underwent robot-assisted resection for a posterior mediastinal cyst abutting the thoracic duct. During this operation, near-infrared fluorescence lymphography with indocyanine green was successfully utilized for real-time intraoperative visualization of the thoracic duct. This technique should be considered when operating on structures near the thoracic duct to avoid and promptly detect iatrogenic injury.

Rare case of a massive anterior mediastinal schwannoma causing cardiac compression with concurrent cervical spine lesion causing spinal cord compression.

Schwannoma is the most common neurogenic tumor in the posterior mediastinum, however it is rarely found in the anterior mediastinum (Mediastinum 4, 2020; Radiat Med 13(4):175-7). We report a large anterior mediastinal schwannoma causing cardiac compression along with a concurrent cervical spine lesion causing right-sided weakness. Current literature having only a few case reports describing an anterior location for this neoplasm and even rarer to have a concurrent symptomatic cervical lesion. We present this case to highlight this rare finding and increase awareness of this potential diagnosis

Uniportal thoracoscopic excision of a bronchogenic cyst impersonating neurogenic tumor: a case report.

INTRODUCTION: Mediastinal bronchogenic cysts (BC) comprise 50%-60% of all mediastinal cysts but are rare in the posterior mediastinum. The infrequent occurrence, unusual features, and atypical locations of posterior BC can result in misdiagnosis before surgery. CASE PRESENTATION: A 37-year-old male presented with an incidental finding of paravertebral mass during his workup for renal cell carcinoma of the left kidney. Initially, the mass was diagnosed as a neurogenic tumor which after uniportal video-assisted thoracic surgery (uVATS) turned out to be a BC in an ectopic location. DISCUSSION:

Echinococcus granulosus in atypical localizations: Five case reports.

BACKGROUND: Hydatid cyst disease, caused by Echinococcus granulosus, primarily affects the liver and lungs, but it can also develop in rare locations such as the kidneys, thyroid, subcutaneous tissues, bones, and the mediastinum. These atypical presentations often pose diagnostic challenges, as they can mimic benign and malignant pathologies, leading to potential misdiagnoses and inappropriate treatments. Early and accurate detection of hydatid cysts in uncommon sites is crucial for optimal patient management. CASE SUMMARY: This case report series presents five patients with hydatid cysts loc

Posterior Mediastinal Mass With Intracardiac Protrusion: A Diagnostic Conundrum Resolved With Multimodality Imaging.

BACKGROUND: Posterior mediastinal masses are rare and remain asymptomatic until they enlarge enough to compress vital vascular structures, the tracheobronchial tree, the spine, or cardiac chambers. Computed tomography (CT) and cardiac magnetic resonance (CMR) provide precise locations and characterization of lesions. CASE SUMMARY: A young woman was found to have a cystic mass in the posterior mediastinum extending into the interatrial septum and left atrium (LA) on echocardiography. Cardiac CT and CMR showed it protruding into the interatrial septum and LA. The diagnosis was confirmed as hyda

[Angiolipoma Arising in the Posterior Mediastinum:Report of a Case].

The patient was an 82-year-old man. A posterior mediastinal tumor was noted on chest computed tomography (CT). Magnetic resonance imaging (MRI) scan showed a high signal on both T1- and T2-weighted imaging. Lipoma was suspected, but since the border with the vertebral body was partially unclear, video-assisted tumor resection was performed for diagnosis and treatment purposes. Finally, we diagnosed it as non-infiltrating angiolipoma. Angiolipoma is a benign tumor that accounts for 5-17% of all lipomas. Pathologically, they can be divided into two types:infiltrating and non-infiltrating. Most a
