Intrathoracic Omental Hernia Repaired by Laparoscopy: A Case Report.

Intrathoracic omental hernia (ITOH) is a rare disease with only 17 previously reported cases. ITOH is a hiatal hernia in which the hernia content is only greater omentum, but the cause of the onset is unknown. There have been reports of ITOH requiring emergency surgery due to intussusception during follow-up, and surgery is recommended even if the patient is asymptomatic. The patient is a 79-year-old man. He visited his local doctor with a chief complaint of chest tightness during swallowing. Chest X-ray revealed a mediastinal tumor, and he was referred to our hospital. He was diagnosed with I

Rare Presentation of Cervical Ganglioneuroma: A Case Report.

Ganglioneuromas are rare, benign neoplasms of autonomic origin that represent the most differentiated end of the spectrum of peripheral neuroblastic tumors. Most commonly found in the posterior mediastinum and retroperitoneum, cervical involvement is distinctly uncommon. These tumors usually remain asymptomatic until they exert pressure on surrounding structures. We report the case of a 19-year-old boy who presented with a progressive right-sided cervical mass and contralateral vocal cord paralysis. Imaging revealed a well-circumscribed, encapsulated lesion medial to the carotid space. Surgica

Surgical Management of a Giant Mediastinal Schwannoma Associated with Neurofibromatosis Type 1: A Case Report.

INTRODUCTION: Neurogenic tumors commonly develop in the posterior mediastinum in both pediatric and adult patients. In patients with neurofibromatosis type 1, distinguishing benign schwannomas from malignant peripheral nerve sheath tumors is challenging. In this study, we aimed to present the surgical management of a giant schwannoma that required differentiation from a malignant peripheral nerve sheath tumor. CASE PRESENTATION: A 15-year-old boy presented with a large right mediastinal mass discovered on chest radiography at high school admission. Subsequent contrast-enhanced CT of the chest

Spinal Schwannoma With Intrathoracic Extension: Successful En Bloc Resection via Uniportal Video-Assisted Thoracoscopic Surgery (VATS).

Schwannomas are benign, well-encapsulated tumors arising from Schwann cells, which are responsible for the myelination of peripheral and central nerves. This report highlights a rare incidence of a T4 thoracic schwannoma with right paraspinal extension into the thoracic cavity. A 77-year-old woman presented with bilateral lower limb weakness; MRI revealed a T3-T5 intradural extramedullary lesion extending from the spinal canal into the right posterior mediastinum. A multidisciplinary surgical approach was employed: the neurosurgical team first performed a posterior laminectomy to resect the in

Complete resection of an incidentally discovered posterior mediastinal ganglioneuroma in an asymptomatic child: a case report.

Ganglioneuromas (GNs) are rare, benign tumors originating from sympathetic ganglia, typically discovered in symptomatic children due to compressive effects. We report an unusual case of an 8-year-old girl with a large posterior mediastinal GN, incidentally discovered on chest X-ray during a routine evaluation. Computed tomography imaging revealed a well-defined, 11 × 7 × 5 cm homogeneous mass extending from the aortic arch to the T10 vertebral level, adherent to the descending aorta and vertebrae. A left posterolateral thoracotomy was performed for complete excision without complications. Hist

Case Report: An azygos vein aneurysm with dysphagia assisted diagnosis by esophageal endoscopic ultrasonography.

BACKGROUND: Azygos vein aneurysm (AVA) is a rare thoracic pathology that is frequently misdiagnosed. While contrast-enhanced chest computed tomography (CT) or magnetic resonance imaging (MRI) can delineate AVA location and size, these techniques lack the capability for dynamic real-time assessment of internal architecture. CASE PRESENTATION: We present a highly unusual case of a 67-year-old woman who had an isolated azygos vein aneurysm presenting with dysphagia. The mass was first found by a chest X-ray. Subsequent contrast-enhanced chest CT revealed a 3.5 × 3.74 × 1.4 cm soft-tissue mass in

Left robotic assisted thoracic surgery (RATS) stellate ganglion ganglioneuroma resection with post-operative Harlequin syndrome.

Ganglioneuromas are rare, benign neurogenic tumours, most often located in the posterior mediastinum. We present an exceptional case of a stellate ganglion ganglioneuroma at the left cervicothoracic junction, abutting the subclavian artery, oesophagus, sympathetic chain and vertebral bodies. The patient presented with significant shortness of breath and paraesthesia in the left arm. Cross-sectional imaging confirmed a well-encapsulated paravertebral mass in the left posterior mediastinum. Given the intricate anatomy of the thoracic inlet and proximity to multiple critical structures, a robotic

Right colon interposition in benign esophageal stricture and gastric atrophy: technical nuances and functional outcomes from a complex case.

We present the case of a 43-year-old woman with complete esophageal and gastric stenosis after caustic ingestion. Following initial emergency surgery for gastric perforation, she remained dependent on a jejunostomy for over a year, developing severe malnutrition, dysphagia, and psychological decline. Definitive reconstruction was achieved with a one-stage subtotal esophagectomy and total gastrectomy. A right colon interposition graft, routed through the posterior mediastinum, was used after confirming adequate perfusion despite the absence of the right colic artery. The procedure included a ce

CT Findings of a Müllerian Cyst Originating from the Posterior Mediastinum: A Case Report.

Müllerian cysts in the posterior mediastinum are extremely rare benign cysts of Müllerian origin. On radiographs, Müllerian cysts appear as well-defined round masses with homogeneous water density. As these radiologic features resemble those of other posterior mediastinal cysts, the preoperative diagnosis of Müllerian cysts can be challenging. Herein, we report the imaging and pathological findings of a 54-year-old female patient with a posterior mediastinal Müllerian cyst.

Multidisciplinary management of a rare case of a large functional paraganglioma in the posterior mediastinum in an adolescent: a case report and literature review.

A pheochromocytoma (PPGL) in the mediastinum can originate from the paravertebral sympathetic nerve chain in the posterior mediastinum, or from the intervertebral, subclavian, coronary, pulmonary, or paravertebral paraganglia in the anterior or middle mediastinum. This case report presents the diagnosis of a large, functional paraganglioma in the posterior mediastinum, accompanied by excessive catecholamine secretion, in a 14-year-old male patient. Through multidisciplinary collaboration, radical tumor resection was performed with successful outcomes. This study systematically summarizes the s

Clinical characteristics and surgical management of rare thoracic myelolipoma: experience from a single center with literature review.

BACKGROUND: Myelolipoma is a rare benign tumor composed of mature adipose and hematopoietic tissue, most commonly occurring in the adrenal glands. Thoracic myelolipoma is extremely uncommon, particularly when originating in the lung or chest wall, and is frequently misdiagnosed due to its nonspecific clinical and radiological features. This study aimed to retrospectively characterize the clinical, imaging, pathological, and surgical features of thoracic myelolipoma and to conduct a comprehensive literature review to improve understanding of this rare entity. METHODS: Thirteen cases of thoracic

Case Report of a Giant Mediastinal Neurofibroma Misdiagnosed as Massive Pleural Effusion.

Mediastinal neurogenic tumors are benign neoplasms commonly found along nerves in the posterior mediastinum, with rare malignant transformation. We report a case of a 36-year-old woman admitted to the respiratory department with complaints of chest tightness, cough, and sputum production. Chest CT revealed a large hypodense lesion in the left thoracic cavity with scattered streaky hyperdense shadows, initially suspected as massive pleural effusion. Thoracentesis failed to aspirate fluid. Subsequent ultrasound and contrast-enhanced CT identified a giant mediastinal mass, prompting referral to t

Mediastinal epithelioid sarcoma: a case report.

Epithelioid sarcoma (ES) is a rare soft tissue malignancy of uncertain differentiation that can arise in various anatomical locations; however, its occurrence in the mediastinum is exceptionally uncommon. Herein, we present a 57-year-old woman diagnosed with mediastinal ES. A soft tissue mass in the mediastinum was initially detected incidentally during a routine physical examination; however, the patient did not perceive it as concerning and therefore did not seek further evaluation or treatment. Approximately 5 months later, the patient presented to our hospital with complaints of cough and

Case Report: Extra skeletal Ewing's sarcoma of mediastinum: clinical experience and literature review.

Extraskeletal Ewing sarcoma (EES) is a malignant tumor that arises in soft tissues outside the skeleton. It commonly involves the paravertebral regions, the lower extremities, and the chest wall, with mediastinal involvement being less frequent. Here, we report an 11-year-old male with pathologically confirmed EES occurring in the mediastinum. Chest computed tomography (CT) showed a large soft tissue density mass on his left posterior mediastinum, containing internal low-density cystic necrotic areas. The solid component was isointense to muscle on T1-weighted imaging (T1WI) and mildly hyperin
