While sympathetic overactivity is a well-established driver of disease progression in heart failure with reduced ejection fraction, its role in early-stage heart failure with preserved ejection fraction (HFpEF) remains unclear. In particular, renal sympathetic activity-an important contributor to neurohormonal dysregulation-has not been systematically evaluated in HFpEF. Early-stage HFpEF refers to patients who present with mild symptoms, only modest elevations in B-type natriuretic peptide (BNP), and invasively confirmed elevated left ventricular end-diastolic pressure (LVEDP) but without ove

Intrathoracic goitre (ITG) with posterior mediastinal extension is rare and may present with compressive respiratory symptoms. We report a 55-year-old woman with long-standing shortness of breath and a history of thyroid surgery. Examination revealed only minimal cervical swelling, but imaging demonstrated a large posterior mediastinal mass consistent with an ITG. The mass was successfully excised through a combined cervical and sternotomy approach, and the patient made an uneventful recovery. This case underscores the need to consider ITG in patients with unexplained respiratory symptoms even

Thymoma is a rare tumor originating from the thymic epithelium, typically located in the anterior mediastinum. Ectopic thymomas are extremely uncommon and may occur in the neck, middle or posterior mediastinum, lung, or pleura. We report the case of a 62-year-old woman presenting with cough, weight loss, and asthenia. Chest computed tomography (CT) revealed a right pleural mass. Complete surgical resection was performed, and the postoperative diagnosis confirmed a WHO Type AB thymoma, classified as Masaoka-Koga stage IIa. Following surgery, she underwent adjuvant radiotherapy, and no recurrenc

Mediastinal schwannomas primarily originate from the intercostal and sympathetic nerves located in the posterior mediastinum. Tumors originating from the vagus nerve in the middle mediastinum are extremely rare, accounting for 2% of all intrathoracic schwannomas. Although schwannomas are typically asymptomatic and discovered by accident, they can produce symptoms when they compress adjacent structures, which may lead to severe complications and are hard to manage. These cardiovascular risk factors include superior vena cava syndrome, cardiac tamponade, heart failure, and pulmonary hypertension

Schwannoma is a benign mesenchymal tumor that arises from the Schwann cells. It can occur in both males and females with an equal incidence (M: F-1:1), with the most common age being the third to fourth decade. The tumors show a predilection for the head and neck region, flexural surfaces of extremities, posterior mediastinum, and retroperitoneum. However, the external female genitals are an extremely rare anatomical site for their occurrence. Herein, we outline one such rare case of a benign vulvar schwannoma on the labia minora in a 34-year-old woman. Most patients of vulvar schwannoma prese

Pulmonary sclerosing pneumocytoma, a rare pulmonary adenoma, is a diagnostic dilemma because of its vague symptoms, ambiguous radiologic features, and varied histopathologic findings, leading to errors in diagnosis and management. We report a case manifesting with a picture of an aggressive malignant neoplasm that postoperatively proved to be benign. This patient presented with a large intrathoracic mass encompassing the pulmonary vessels in the hilum and extending to the posterior mediastinum. After extensive investigations, operability appeared to be a challenge. Complete surgical resection

Solid tumors of the cervicothoracic junction, the posterior mediastinum, or bilateral dorsal thoracic tumors represent a challenge in pediatric surgical oncology. The aim of this study was to evaluate trap-door thoracotomy and clamshell thoracotomy as surgical approaches. A single-center retrospective study of children with solid tumors in these specific localizations was performed. From 2015 to 2023, 26 children (17 girls; 9 boys) were treated at a median age of 54 months (range 8-229). Tumor resection was performed for neuroblastoma (n = 11); metastatic disease (n = 7); malignant rhabdoid tu

PURPOSE: This study aimed to assess the shifting patterns of the mediastinum, including the target volume and the isocenter point during the postoperative radiotherapy (PORT) process of non-small cell lung cancer (NSCLC), and to observe the occurrence of radiation injury. Additionally, we investigated the significance of mid-term assessment during the implementation of the PORT process.
MATERIAL AND METHODS: We established coordinate axes based on bone anatomy and measured the mediastinum's three-dimensional direction and the shift of the isocenter point's shift in the PORT process. Statistic

BACKGROUND: At present, anastomotic fistula cannot be avoided after adenocarcinoma of the esophagogastric junction (AEG). Once the anastomotic leakage occurs, the posterior mediastinum and the left thoracic cavity are often seriously infected, which further impairs respiratory and circulatory function, heightening the danger of the disease course. The aim of this study was to identify the characteristics of superior anastomotic leakage after surgery for AEG and recommend corresponding treatment strategies to improve the diagnosis and treatment of superior anastomotic leakage after surgery for

Esophageal bronchogenic cysts are very rare. A bronchogenic cyst is a congenital malformation resulting from abnormal sprouting of primitive bronchi because of a foregut bronchopulmonary malformation. An 18-year-old patient with a cystic tumor in the left posterior mediastinum was identified. The mediastinal tumor was removed by uniportal video-assisted thoracoscopic surgery. The operation was performed in the prone position through a single 4-cm incision on the lateral scapular line in the left ninth intercostal space. After tumor resection, the dissected esophageal muscle and mediastinal ple

BACKGROUND: Neurogenic tumors are the third most common tumors occurring within the mediastinum. Predominantly completely asymptomatic, they are most often found in the posterior mediastinum, although they may also be located, albeit rarely, in the middle mediastinum. Thus, in the cases of tumors localized in the middle mediastinum, schwannomas of the phrenic nerve must always be considered.
CASE REPORT: In this case, a male patient presented with a tumor of the middle mediastinum. PET/CT scan determined that it was a circumscribed tumor without signs of dissemination. However, due to the tum

Aortitis is the inflammation of the aortic wall. It can be caused by both infectious and non-infectious etiologies. Mycotic aneurysm is a rare, serious medical condition and typically requires prompt treatment with antibiotics, surgical intervention, or endovascular procedures to prevent rupture and complications. Here we reported, a 66-year-old male patient with a medical history of diabetes and hypertension, who presented to the emergency department (ED) with left-sided hemiplegia. Brain magnetic resonance imaging (MRI) revealed infarction in the right parietooccipital and left occipital lob

BACKGROUND: Posterior mediastinal leiomyosarcoma is an extremely rare malignant mesenchymal tumor with no special clinical symptoms, which is easily confused with some common tumors in the posterior mediastinum, affecting the accuracy of the first diagnosis by clinicians and delaying the treatment of patients.
CASE SUMMARY: We report a 59-year-old woman with a space-occupying lesion in the posterior mediastinum. The patient was mistakenly diagnosed with lumbar muscle or vertebral body lesions due to chest and back pain and underwent conservative treatment, but her symptoms did not improve sig

The occurrence of ectopic pancreas in the mediastinum is rare. Herein, we report a 22-year-old female who presented with right shoulder pain, dysphagia, fever and headaches. Chest computer tomography revealed a mass in the posterior mediastinum with accompanying signs of acute mediastinitis. Needle biopsy and fine-needle aspiration revealed ectopic gastral tissue and ectopic pancreas tissue, respectively. Surgical resection was attempted due to recurring acute pancreatitis episodes. However, due to chronic-inflammatory adhesions of the mass to the tracheal wall, en-bloc resection was not possi

Tumor-induced osteomalacia is caused by excessive fibroblast growth factor 23 production mainly from phosphaturic mesenchymal tumors. Surgical excision or tumor ablation are the preferred treatment. Information on bone microarchitecture parameters assessed by high-resolution peripheral quantitative computed tomography is limited. We report a woman with hypophosphatemic osteomalacia with generalized pain, weakness and recurrent fractures, and a large thoracic vertebral mass extending to the posterior mediastinum. Detailed radiologic and histopathologic evaluation revealed a phosphaturic mesench

Mediastinal haemangiomas pose diagnostic and therapeutic challenges owing to their rarity and complex anatomy. A 36-year-old man, with a history of smoking and drinking, presented with a posterior mediastinal mass with back pain. Initial investigations suggested a lymphangioma. However, owing to persistent symptoms and complex pathology, we performed surgical intervention involving open resection of the tumour, which was closely associated with the descending aorta and extended into the right posterior mediastinum. The surgical approach was influenced by the proximity of the tumour to vital st

BACKGROUND: The ectopic superior parathyroid in the tracheoesophageal groove and paraesophageal region is rare. Hyperparathyroidism results when these glands become hyperfunctioning. That may necessitate surgical intervention in the form of parathyroidectomy, which requires a transsternal or transthoracic approach due to a deeply seated mediastinal parathyroid gland. Minimally invasive strategies have emerged recently as an alternative approach with less morbidity.
CASE PRESENTATION: We present a case of the paraesophageal ectopic parathyroid gland in the superior posterior mediastinum, which

Melanotic schwannoma (MS) is a rare and infrequent subtype of schwannoma characterized by cytoplasmic deposits of melanosomes (melanin). Unlike the other schwannomas, it could have malignant transformation. Due to distinctive characteristics and atypical behavior from classic schwannomas subtypes, MS were renamed and reclassified as "melanocytic malignant neural sheath tumor" in the 5th ed. of the World Health Organization's classification of central nervous system tumors in 2021. We present two cases of MS that underwent complete surgical resection.
