Radiological features and diagnostic pitfalls of idiopathic azygos vein aneurysm: A case report.

INTRODUCTION AND IMPORTANCE: We present a case of idiopathic azygos vein aneurysm (AVA) and describe its radiological features. Clinicians should be aware of the imaging findings to avoid the risk of bleeding caused by biopsy. CASE PRESENTATION: An asymptomatic 46-year-old woman was found to have an abnormal shadow on a chest computed tomography (CT) scan during a medical checkup. Plain CT revealed a well-defined, homogeneous mass on the posterior side of the right main bronchus. Contrast-enhanced CT revealed a mass with marked enhancement pointing to a vascular structure. Three-dimensional r

Extramedullary plasmacytoma with associated multiple myeloma as a presentation of the posterior mediastinal mass: a rare clinical encounter.

A plasmacytoma is a single, isolated tumour of abnormal plasma cells. It can develop within the bone, known as solitary plasmacytoma of bone, or outside the bone, referred to as extraosseous (extramedullary) plasmacytoma, without spreading to other parts of the body. Plasmacytoma, an uncommon presentation in the posterior mediastinum, usually arises as solitary or multiple lesions in bone or soft tissues. The standard treatment involves definitive radiotherapy, potentially curative for extramedullary cases. The prognosis varies, being more favourable without concurrent multiple myeloma and wor

Adrenal bronchial cyst: A rare case report and literature review.

Bronchial cyst (BC), also known as anterior intestinal cyst, is often located in the posterior mediastinum and rarely in the retroperitoneum, especially in the adrenal area. The patient was a 65-year-old male with a left adrenal lesion (adrenal adenoma was considered) indicated by imaging examination at initial diagnosis. After completing the preoperative preparation, the patient underwent laparoscopic partial adrenalectomy under general anesthesia. The postoperative examination revealed no recurrence in the postoperative follow-up for 9 months. This article introduces the diagnosis and treatm

Optimizing outcomes in paraesophageal hernia repair: a novel critical view.

BACKGROUND: The recurrence rate of paraesophageal hernia repair (PEHR) is high with reported rates of recurrence varying between 25 and 42%. We present a novel approach to PEHR that involves the visualization of a critical view to decrease recurrence rate. Our study aims to investigate the outcomes of PEHR following the implementation of a critical view. METHODS: This is a single-center retrospective study that examines operative outcomes in patients who underwent PEHR with a critical view in comparison to patients who underwent standard repair. The critical view is defined as full dissection

An Uncommon Cause of Recurrent Dysphagia and Chest Pain in an Adolescent Boy.

An 11-year-old boy was brought to the emergency department with a week-long history of widespread pain in his upper abdomen that worsened with deep breathing and eating, sialorrhea, food impaction sensation, and a recent fever. Ten months prior, he had similar symptoms and was diagnosed with a pharyngeal phlegmon. He was treated with antibiotics and dexamethasone. In the current episode, he presented with mild elevation of inflammatory markers, a slight deviation of the trachea on chest X-ray, and a tubular esophageal duplication was identified on a thoracic CT, with its opening observed durin

PET/CT Manifestations of a Case of Cavernous Hemangioma Misdiagnosed as Reproductive Tumor.

Cavernous hemangioma of mediastinum is a rare, benign tumor originating from vascular endothelial cells. It typically arises in the anterior mediastinum but has also been reported in the posterior mediastinum or the middle mediastinum, and occasionally involves both the anterior and middle compartments. We present the case of a 20-year-old man with a cavernous hemangioma of mediastinum initially misdiagnosed as a reproductive tumor by 18 F-FDG PET/CT.

Diagnostic value of combined CT lymphangiography and 99Tcm-DX lymphoscintigraphy in primary chylopericardium.

OBJECTIVE: To investigate the diagnostic value of combined 99Tcm-DX lymphoscintigraphy and CT lymphangiography (CTL) in primary chylopericardium. METHODS: Fifty-five patients diagnosed with primary chylopericardium clinically were retrospectively analyzed. 99Tcm-DX lymphoscintigraphy and CTL were performed in all patients. Primary chylopericardium was classified into three types, according to the 99Tcm-DX lymphoscintigraphy results. The evaluation indexes of CTL include: (1) abnormal contrast distribution in the neck, (2) abnormal contrast distribution in the chest, (3) dilated thoracic duct

Robot-assisted resection of ganglion cell neuroma with a diameter of 78 mm: A case report.

RATIONALE: Intrathoracic paragangliomas are typically found within the intricate posterior mediastinal region adjacent to the vertebrae, often presenting with substantial volume. Surgical excision of such tumors presents formidable challenges and is conventionally performed via open surgical procedures. PATIENT CONCERNS: In this report, we present the case of a 53-year-old female patient who presented with the discovery of a left intrathoracic mass during a routine physical examination approximately 1 month prior. She complained of chest tightness and chest pain. DIAGNOSES: She complained of

Mediastinal mayhem: the challenges of incidental thoracic arteriovenous malformations-a case report.

BACKGROUND: Arteriovenous malformations (AVMs) within the mediastinum are rare vascular anomalies. With the increasing number of coronary angiographies being performed, the number of incidentally found cases is rising. This presents challenges in terms of determining the appropriate treatment strategy. CASE SUMMARY: We present the case of a 79-year-old man with aortic stenosis, experiencing functional dyspnoea and fatigue. Echocardiography showed a bicuspid aortic valve, and while left heart catheterization confirmed no significant coronary stenosis, it revealed a tortuous vessel originating

Predictive factors for refractory anastomotic stricture after cervical triangular anastomosis with gastric conduit reconstruction through the posterior mediastinum in minimally invasive esophagectomy.

BACKGROUND: After esophagectomy, anastomotic strictures disturb food passage and increase the incidence of aspiration pneumonia. Multiple endoscopic balloon dilatations are required for stricture treatment. Therefore, long-term quality of life and nutritional status may be adversely affected. This study aimed to identify risk factors for strictures after cervical triangular anastomosis using a gastric conduit among patients who underwent minimally invasive esophagectomy (MIE). METHODS: A total of 188 patients who underwent MIE for esophageal cancer between 2010 and 2020 at Kobe University Hos

Metastatic Medullary Thyroid Carcinoma Presenting as an Incidental Posterior Mediastinal Mass.

Medullary thyroid carcinoma (MTC) is an uncommon neuroendocrine tumour that is usually asymptomatic at its onset and therefore may not present clinically until the patient has developed advanced or metastatic disease. Common metastatic sites include cervical lymph nodes, liver, bone and lung. This is the case of a patient who presented with an incidental posterior mediastinal mass. Because the posterior mediastinum is an unusual location for MTC, MTC was not a consideration and preliminary histopathological testing did not include calcitonin, which would have been diagnostic. This case highlig

Mediastinal extension of pancreatic pseudocyst through the esophagus hiatus: A case report.

Pancreatic pseudocysts have a high amylase concentration and are surrounded by a fibrous capsule without a true epithelial lining. They are most frequently located in the peripancreatic region, and rarely extend into the mediastinum. We report a case of a 46-year-old male patient with a history of pancreatitis due to eat and drink too much presented with nausea and vomiting, MRI of the abdominal demonstrated a cystic mass connecting the abdominal cavity to posterior mediastinum and compressing the heart and stomach, ultrasound-guided aspiration of the cystic mass revealed high levels of amylas

Combined Intrathoracic and Intraspinal Approach to a Neurogenic Dumbbell Tumour.

A neurogenic dumbbell tumour is a rare occurrence in which the tumour has an extension through an intervertebral foramen, acquiring an hourglass shape. Surgical strategies to resect these tumours are rapidly evolving, and there is no definite consensus on the approach, stages, and plan of surgery. Here, we present a case report on a dumbbell tumour that was successfully resected via laminectomy and video-assisted thoracoscopic surgery (VATS) approach. A 53-year-old lady had an incidental finding of a mediastinal mass from chest radiography. Computed tomography (CT) and magnetic resonance imagi

Adrenal ganglioneuroma with retroperitoneal lymph node metastasis: A rare scenario.

Ganglioneuroma (GN) is the most differentiated and benign variant of neuroblastic tumors, most commonly located in the posterior mediastinum, followed by the retroperitoneum and adrenal gland. Children of <10 years of age are more commonly affected than adults. Though benign, GNs can very rarely metastasize to regional lymph nodes or distant sites like liver, bone, spleen, and soft tissues. Metastatic lesions are assumed to represent neuroblastomas in which the metastasis and the primary tumor, both have matured. This differentiation can occur spontaneously or after treatment. We present a pri

Giant Parathyroid Adenoma of the Posterior Mediastinum.

Ectopic parathyroid adenoma is a rare pathology. We present a clinical case of a giant ectopic parathyroid adenoma (PA) in an unusual location, which brought significant diagnostical and therapeutical challenges. The tumour in the mediastinum was found incidentally on chest computed tomography (CT). A biopsy was conducted, and histological examination revealed a well-differentiated neuroendocrine tumour. The tumour was excised via right thoracotomy. The final histological examination revealed a parathyroid adenoma, which was unexpected for our team. After three years of observation, there is n

FDG PET/CT Image of Extragastrointestinal Stromal Tumor in the Posterior Mediastinum.

Extragastrointestinal stromal tumor (EGIST) in the posterior mediastinum is very rare. Herein, we report FDG PET/CT findings of posterior mediastinum EGIST in a 77-year-old man. On FDG PET/CT, it manifested as a solitary posterior mediastinum mass with heterogeneous necrosis and intense FDG uptake. The final pathology supported a diagnosis of EGIST. This case hints us when we encounter a soft tissue mass with intense FDG uptake in the posterior mediastinum; EGIST should be regarded as a differential diagnosis.

Rare posterior mediastinal hemangiomas invading the spinal cord through the intervertebral foramen and causing acute symptoms: illustrative case.

BACKGROUND: Hemangioma is a rare vascular tumor characterized by endothelial cell proliferation that primarily occurs in soft tissues. While most cases in the paraspinal posterior mediastinum are clinically indolent, they can also exhibit aggressive growth, leading to severe clinical outcomes and necessitating urgent treatment. OBSERVATIONS: A 61-year-old right-handed female presented with reduced lower-extremity mobility that had begun 4 days earlier. An examination revealed that both lower limbs had grade 4 muscle strength. Computed tomography and magnetic resonance imaging revealed a right
