Post-transplant diabetes mellitus in renal allograft recipients: a matched-pair control study.

The incidence of post-transplant diabetes mellitus was evaluated retrospectively in 901 consecutive renal transplant recipients. Thirty-two (3.6%) patients developed diabetes mellitus requiring drug therapy. 18 of 32 became hyperglycaemic within 3 months of transplantation. Post-transplant diabetes mellitus occurred in 24 of 628 (3.8%) patients treated with conventional therapy consisting in azathioprine and prednisone, and in 8 of 273 (2.9%) patients receiving cyclosporin A (CsA) in addition (triple therapy). To identify predisposing factors 32 non-diabetic patients matched for age, sex, numb

Obesity and diabetes.

The factors which are involved in the development of diabetes and obesity have been reviewed. These include the age of the individual, the duration and magnitude of the obesity, whether there is a family history of diabetes mellitus, the regional distribution of body fat, and the function of the pancreatic islet. These factors help in understanding the reasons why obesity poses such a high risk for developing diabetes mellitus.

[Insulin receptor function in subjects with above-normal body weight].

The insulin receptor binding ability was studied in 26 persons with above normal body mass (15 women and 11 men), mean age 44.15 +/- 10.1 years without family history of diabetes mellitus. According to the degree of obesity they were classified into 3 groups. In the persons with I-II degree of obesity parallel with the strongly reduced number of insulin receptors (total and the high affinity) an increase of the receptors affinity appears as a compensatory mechanism which ensures appropriate insulin receptor binding. In the persons with III-IV degree of obesity the number of insulin receptors i

Relationship between bleeding/plaque ratio, family history of diabetes mellitus and impaired glucose tolerance.

In the present study, the relationship between bleeding/plaque ratio, family history of diabetes mellitus and oral glucose tolerance pattern were investigated in 85 non-diabetic individuals without periodontal breakdown. In this group, no relationship between bleeding/plaque ratio, family history of diabetes mellitus and impaired glucose tolerance was found. The results of the present study suggest that the observed variation in bleeding/plaque ratio cannot be explained by impaired glucose tolerance or family history of diabetes mellitus. It is hypothesized that in subjects with impaired gluco

[Clinical significance, diagnosis and treatment of diabetes in pregnancy (gestational diabetes)].

Early diagnosis of glucose intolerance by glucosuria testing is unreliable and should be replaced by blood sugar screening in the 24th to 28th week of gestation. If screening results are pathological (blood sugar in capillary whole blood over 140 mg% one hour after oral ingestion of 50 g glucose), a standardized glucose tolerance test should be performed. This screening test will reveal a glucose intolerance in approx. 3% of pregnant women. High-risk patients for gestational diabetes (previous pregnancies, complicated by gestational diabetes, family history of diabetes mellitus (first degree r

Metabolic abnormalities in first-degree relatives of type 2 diabetics.

Diabetic relatives and obese subjects are at increased risk for development of diabetes mellitus, and therefore are classed as potential abnormality of glucose tolerance (POT-AGT). Disturbances of lipid and purine metabolisms have been reported in diabetic and obese non-diabetic subjects. In obese subjects above alterations are probably due to hyperinsulinemia. This study aimed at verifying whether similar metabolic abnormalities could be found in relatives of non-insulin dependent diabetic patients and whether they could be related to possible glucose intolerance. We have studied 10706 outpat

Clinical aspects of the relationship between oral contraceptives, abnormalities in carbohydrate metabolism, and the development of cardiovascular disease.

Although large epidemiologic studies indicated no difference in the frequency of diabetes mellitus in nonusers and everusers of high-dose combination oral contraceptives, other studies had shown an increased risk of impaired glucose tolerance in current users, which is estimated to be roughly twice as frequent as that in nonusers. Women at risk of developing impaired glucose tolerance while receiving high-dose oral contraceptives either had previous gestational diabetes mellitus or were older, obese, or had a positive family history of diabetes mellitus. The tendency to decreased glucose toler

Accuracy of reported family history of diabetes mellitus. Results from San Luis Valley Diabetes Study.

There are two possible sources of bias in the assessment of family history of diabetes: 1) a person with diabetes may be more likely to report a diabetic relative than a nondiabetic person would be, and 2) relatives of individuals with diabetes may be more likely to be tested for diabetes than relatives of nondiabetic individuals. We conducted a study on a subsample of families of subjects in the San Luis Valley Diabetes Study to examine these issues. A sample of 5 White and 5 Hispanic subjects (probands) with diabetic glucose tolerance tests and the same number with normal glucose tolerance w

Glycosylated haemoglobin (HbA1c) levels in normal pregnancy: sequential changes and relation to birth weight.

Twenty-five healthy pregnant females with the absence of family history of diabetes mellitus were investigated serially for glycosylated haemoglobin (HbA1c) as per cent haemoglobin A1 at the interval of 4 weeks. HbA1c values were also determined in 10 healthy non-pregnant females who were studied as control cases. As compared to non-pregnant females HbA1c values were found to be low at 14-16 weeks of pregnancy (4.9 +/- 0.25%). After 16 weeks the HbA1c levels showed an upward trend to reach maximum at 32-34 weeks (5.54 +/- 0.19%). After 34 weeks the levels fell again to reach the lowest by 40 w

[Risk factors for changes in glucose metabolism in pregnancy].

Eighty women were studied having between 20-25 weeks of gestation at the National Institute of Perinatology. Weight and height were taken, and they were questioned about the risk factors (positive family history of diabetes mellitus, age, overweight and number of gestations). Two hours postprandial glucose tests were carried out using 100 g glucose load. Plasma glucose values were determinated according to the glucose-oxidase technique. Results showed that 36% of the population had at least glucose metabolism alteration (GMA) (glucose value 120 mg/dL). No significant difference was found for t

Connective tissue disease in southeastern Georgia: a case-control study of etiologic factors.

The authors conducted a population-based case-control study of connective tissue disease in a four-county area in southeastern Georgia. Interviews with 44 cases and 88 controls were conducted in September and October of 1986. The association with a family history of these disorders was reconfirmed, and a positive association with the use of hair dyes was found. A negative association was found with a family history of diabetes mellitus.

Metabolic effects of three new low-dose pills: a six-month experience.

We evaluated the effects on glucose and lipid metabolism in 57 healthy volunteers randomly assigned to one of three low-dose oral contraceptives: two monophasic (desogestrel + ethinylestradiol, EE, and cyproterone acetate + EE) and one triphasic (gestodene + EE) contraceptives. Glucose and insulin responses during OGTT were slightly affected by the cyproterone pill. The insulin area/glucose area ratio and HbA1c level were unchanged in all women. No preparation affected total and LDL-cholesterol levels. Triglycerides rose in all groups, while HDL-CH did only in women taking the two monophasic p

Circadian rhythm of albumin excretion rate in healthy subjects.

Many studies have reported the predictive role of unphysiological levels of microalbuminuria for the development of diabetic nephropathy. It is known that physiological levels of 24-h microalbuminuria do not exceed 20 micrograms/min (28.8 mg/day), but a few data are available for shorter periods. Since an accurate 24-h urine collection is difficult to obtain in screening programs, it seems important to study whether a circadian oscillation exists and, if so, to find more precise reference values. Eleven healthy subjects (mean age 30.6 +/- 2.46 years) with normal glucose tolerance and no family

Spasmodic torticollis: clinical and biologic features and their implications for focal dystonia.

A previous investigation of IST in Rochester, Minnesota, from 1960 through 1979 yielded an incidence of 1.1/100,000 person-years. The rate in women was almost three times higher than that in men. Thyroid disorders, other endocrine disorders, right-hand preference, symptomatic ovarian cysts, other disorders of the female genito-reproductive system, emotional depression, cerebral aneurysm, and family history of diabetes mellitus were common. In 347 cases of IST personally evaluated (151 seen between 1969-1971, a sample of 49 seen in 1978, and 147 other cases evaluated in 1985) the female-to-male

Fractional hepatic extraction of insulin in man: is it constant?

The present study was designed to compare insulin extraction by the liver following oral glucose administrations of different size, in order to evaluate insulin removal by the liver in relation to the insulin exposure, and to the amount of ingested glucose. Insulin secretion by the pancreas was estimated by the measurement of peripheral C-peptide levels, and insulin extraction by the liver by the analysis of peripheral C-peptide to insulin ratios and relations. Ten healthy subjects (5 males and 5 females), aged 16 to 66 yr, with normal bw, and without family history of diabetes mellitus were i

Family history of diabetes mellitus and oral glucose tolerance testing criteria.

A review of 185 obstetrical patients, having a family history of diabetes mellitus without medical history of glucose intolerance in the non-pregnant state was conducted. A 3-hour 100-g oral glucose tolerance test was performed on all patients between 20 and 34 weeks of gestation. According to O'Sullivan's criteria for glucose tolerance testing, normal glucose tolerance occurred in 89.7%, while Class A diabetes was identified in 10.3% of patients tested. 3.8% of the study population fulfilled the O'Sullivan criteria for abnormal glucose intolerance and required insulin treatment during pregnan

Islet cell and other organ-specific autoantibodies in healthy first-degree relatives to insulin-dependent.

The presence of organ-specific autoantibodies including islet cell surface, cytoplasmic and cytotoxic as well as thyroid-gastric antibodies were determined in healthy, non-diabetic, first-degree relatives to 30 insulin-dependent diabetic (IDDM) children. Thirty healthy families without family-history of diabetes mellitus served as controls. The prevalence of organ-specific autoantibodies among the healthy members in the diabetic families was increased compared to the control families (p less than 0.005). Islet cell cytoplasmic antibodies were only detected in diabetes families, since 23% (7/30
