High relapse rates after budesonide in microscopic colitis: a large retrospective analysis revealing the need for advanced therapies.

BACKGROUND: Microscopic colitis (MC) is a chronic inflammatory bowel disorder causing watery diarrhea despite normal endoscopic findings. Budesonide is the only approved therapy, yet long-term durability and the need for alternatives in steroid-dependent or -refractory cases remain unclear. METHODS: We performed a retrospective single-center cohort study of all patients with histologically confirmed MC between 2013 and 2022, with ≥3 months of follow-up. Demographics, clinical features, treatments, and outcomes were collected. Clinical remission followed Hjortswang criteria: fewer than three s

Global, regional, and national burden of HIV/AIDS, 1990-2023, with the estimated burden attributable to intimate partner violence and forecasted impacts of funding cuts through 2030: results from the Global Burden of Disease Study 2023.

BACKGROUND: Despite a general improvement in the HIV burden over the past two decades, gendered social determinants including intimate partner violence (IPV) continue to affect women's vulnerability to HIV, which could be further exacerbated as resources dwindle over the coming years. Our study aimed to quantify recent trends in the HIV burden, the magnitude of the HIV burden associated with IPV, and the potential impact of declining financial support globally. METHODS: Using the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2023 framework, we estimated annual HIV incidenc

Global, regional, and national burden of meningitis, its risk factors, and aetiologies, 1990-2023: a systematic analysis for the Global Burden of Disease Study 2023.

BACKGROUND: Meningitis remains the leading infectious cause of neurological disabilities globally, disproportionately affecting children younger than 5 years and populations in the African meningitis belt. Whereas previous global estimates focused on ten pathogen categories, this study presents the most comprehensive analysis to date, assessing the meningitis burden attributable to 17 causative pathogens based on the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2023 framework. METHODS: GBD is a systematic, scientific effort aimed at quantifying the comparative magnitude o

Asynchronous bilateral pulmonary pathology: left-sided congenital pulmonary airway malformation with contralateral malignant pleuropulmonary blastoma in an adolescent.

A boy in early adolescence was diagnosed with left-sided congenital pulmonary airway malformation (CPAM) at birth and remained asymptomatic until early adolescence, when he presented with dry cough, right-sided chest pain and breathlessness on exertion. Imaging showed right-sided pleural effusion with pulmonary nodules and skeletal metastases. Thoracoscopic biopsy confirmed pleuropulmonary blastoma (PPB) of type III. Despite receiving aggressive chemotherapy, the disease progressed rapidly and the outcome was fatal. This case highlights the diagnostic challenges of advanced PPB in adolescents a

Understanding Infection, Viral Exacerbation and Respiratory Symptoms at Admission-Longitudinal (UNIVERSAL) study: a prospective observational cohort study protocol.

BACKGROUND: Respiratory viral infections (RVIs) are a significant cause of morbidity and hospital admission worldwide. However, the management of most viral infection-associated diseases remains primarily supportive. The recent COVID-19 pandemic has underscored the urgent need for a deeper understanding of RVIs to improve patient outcomes and develop effective treatment strategies. The Understanding Infection, Viral Exacerbation and Respiratory Symptoms at Admission-Longitudinal Study is an observational study which addresses this need by investigating the heterogeneity of RVIs in hospitalised

Highly Variable Expressivity of a CNV Deletion Involving TBX4 in Three Deceased Siblings With Lung Developmental Disorder and Their Mildly Affected Mother and Grandfather.

Single nucleotide variants (SNVs) and copy-number variant (CNV) deletions involving TBX4 have been associated with pulmonary arterial hypertension, ischiocoxopodopatellar syndrome, and lethal lung developmental disorders (LLDDs). Thus far, all large CNV deletions encompassing entire TBX4 have been found to have arisen de novo. Here, we present a three-generation family with three neonate siblings who died within 35-66 days due to histopathologically diagnosed LLDD. Whole-genome sequencing identified an ~108-kb CNV deletion encompassing TBX4 in all three infants. The deletion was also found in

Emphysema at Baseline Low-Dose CT Lung Cancer Screening Predicts Death from Chronic Obstructive Pulmonary Disease and Cardiovascular Disease Up to 25 Years Later.

Background The prognostic value of baseline visual emphysema scoring at low-dose CT (LDCT) in lung cancer screening cohorts is unknown. Purpose To determine whether a single visual emphysema score at LDCT is predictive of 25-year mortality from all causes, chronic obstructive pulmonary disease (COPD), and cardiovascular disease (CVD). Materials and Methods In this prospective cohort study, asymptomatic adults aged 40-85 years with a history of smoking underwent baseline LDCT screening for lung cancer between June 2000 and December 2008. Follow-up continued until death, loss to follow-up, or De

A Case of APC Gene Mutation-Associated Familial Adenomatous Polyposis With Multiple System Malignancies.

BACKGROUND: Familial adenomatous polyposis (FAP) is an autosomal dominant inherited disorder, with a nearly 100% risk of developing colorectal cancer by the age of 40. The primary gene mutated in FAP is APC, and mutations in certain regions of the APC gene may be associated with thyroid disorders, including malignant neoplasms, benign nodules, and endocrine diseases of the thyroid. FAP-associated colorectal cancer (FAP-CRC) demonstrates poorer treatment outcomes compared to sporadic colorectal cancer, which may be attributed to several factors such as distinct molecular pathogenesis, chromosom

Emerging Vector-Borne Nipah Virus Infection: Unexplored Hazards, Diagnostic Challenges, and the Potential of Phytomedicine-Based Therapeutics.

INTRODUCTION: Millions of people have died from zoonotic illnesses, like COVID-19 and Nipah virus infection (NiV), throughout history. Fruit bats (Pteropus sp.) are the main reservoir host for NiV, an RNA virus belonging to the Henipavirus group, which causes extremely infectious illnesses, such as COVID- 19. NiV outbreaks have posed significant public health concerns, especially in South and Southeast Asia. The Nipah virus (NiV) infection is caused by a virus that belongs to the Paramyxoviridae family's Henipavirus genus. It is the source of zoonosis, which causes respiratory and neurological

[A case of diffuse cystic lung disease with severe pulmonary hypertension in a reproductive-aged female].

Pulmonary Langerhans cell histiocytosis (PLCH) and lymphangioleiomyomatosis (LAM) are both rare diseases with low incidence. They not only present characteristic diffuse cystic lesions on chest CT, but also share highly similar clinical manifestations, and most often occuring in young people, which makes differential diagnosis challenging. Here, we reported a case of a reproductive-aged female patient who was initially diagnosed with LAM based on the presence of multiple cystic lesions in both lungs shown by imaging. However, insufficient attention was given to her smoking history and to the s

Examining trends in lung cancer screening over 10 years: Eligibility, participation, cancer detection, and quality implications.

BACKGROUND: More than a decade ago, the National Lung Screening Trial led to the recommendation of low-dose computed tomography (CT) for lung cancer screening (LCS). However, few studies have explored how program changes and external factors affect patient outcomes. METHODS: This retrospective study included LCS-eligible individuals from Kaiser Permanente Colorado between May 1, 2014, and December 31, 2024. Rates and proportions of LCS orders, completions, adherence to recommended follow-up after baseline screening, and lung cancer yield were calculated. Multivariable log-binomial regression

Sports Preparticipation Evaluation for Healthy Adults: A Consensus-Based German Guideline.

The benefits of physical activity are undisputed. However, adverse events can occur in rare cases, particularly during high-intensity or prolonged exercise. During physical activity, at-risk patients can experience major cardiac events, whereas adverse events affecting the musculoskeletal system are more common but less severe. A sports preparticipation evaluation (PPE) for apparently healthy adults is designed to detect at-risk individuals and prevent potentially fatal events. This guideline for conducting PPEs was developed by consensus among 16 medical societies and sports associations and
