Prevalence and Factors Associated With Hepatitis C in Danish Prisons, 2022-2024: A Multicentre Cross-Sectional Study.

Prisons offer a critical opportunity for hepatitis C virus (HCV) elimination, yet current data from Danish correctional facilities are sparse. We conducted a cross-sectional study in 16 prisons across Eastern Denmark between October 2022 and August 2024, enrolling 651 incarcerated individuals. All participants underwent HCV antibody and RNA testing using dried blood spots and completed a bio-behavioural risk survey. The prevalence of HCV antibody and HCV RNA was 4.2% (n = 26) and 2.0% (n = 13) respectively. HCV exposure was most prevalent among individuals with a history of injecting drug use

Risk factors of postoperative non-compliance behavior in elderly patients with gastrointestinal malignant tumors.

BACKGROUND: Elderly patients with gastrointestinal malignancies frequently exhibit postoperative non-compliance behaviors, which includes failure to persist with adjuvant therapy, attend regular follow-up appointments, and take medications strictly as prescribed. These non-compliant behaviors seriously affect the prognosis of patients. METHODS: We conducted a large-scale retrospective study in China, which enrolled 284 elderly patients who underwent surgery for gastrointestinal malignancies at Jingzhou Hospital Affiliated to Yangtze University between February 2018 and February 2023. The pati

Familial adenomatous polyposis-associated desmoid disease: comprehensive review.

BACKGROUND: Familial adenomatous polyposis (FAP)-associated desmoid disease is a rare but important manifestation of FAP and remains a major cause of morbidity and mortality. In contrast to sporadic desmoids, which most commonly occur in the abdominal wall or extra-abdominal sites, approximately 70% of FAP-associated desmoids arise intra-abdominally, usually within the small bowel mesentery. These lesions can cause bowel and ureteric obstruction, sepsis, intestinal failure, and death. This review summarizes current evidence regarding the pathogenesis, risk factors, diagnosis, surveillance, and

Development of an Interpretable Triage Tool for Colorectal Polyp Risk Stratification Within a Population-Based Screening Program: Machine Learning Approach.

BACKGROUND: Colorectal polyps are a major source of precancerous lesions in colorectal cancer (CRC). In many population-based screening programs, a major challenge is the efficient triage of high-risk individuals for diagnostic colonoscopy amid limited endoscopic resources. OBJECTIVE: To enrich current screening frameworks, we aimed to develop an accessible, noninvasive risk stratification tool to serve as a digital triage mechanism for colorectal polyps using machine learning (ML) and routinely collected data in China. METHODS: We conducted a cross-sectional study in Wenzhou, China. A total

Hereditary Endometrial Cancer: Lynch Syndrome, Mismatch Repair Deficiency, and Emerging Genetic Predispositions-A Comprehensive Review with Clinical and Laboratory Guidelines.

Endometrial cancer is the most common gynaecologic malignancy in high-income countries, with a rising incidence largely driven by reproductive factors, obesity, and prolonged exposure to unopposed oestrogens. Although most cases are sporadic, approximately 2-5% are associated with hereditary cancer syndromes, of which Lynch syndrome represents the most important contributor. Lynch syndrome results from germline mutations in DNA mismatch repair (MMR) genes and is associated with a substantially increased lifetime risk of endometrial cancer, reaching up to 71% in carriers of MutS homologue 6 (MS

Implementing Timely Germline Genetic Testing for Patients With Pancreatic Cancer Using a Genetics Copilot for Point-of-Care Education and Health Assessment.

PurposePancreatic ductal adenocarcinoma (PDAC) ranks third in cancer-related deaths. Although national guidelines recommend germline multigene panel testing (MGPT) for all patients with PDAC, only 6%-19% undergo testing, mostly due to under-referral and genetic counselor shortages. We evaluated a coordinator-led and digitally supported workflow designed to increase access to pretest education, improve MGPT uptake, and reduce turnaround time (TAT) for patients newly diagnosed with PDAC at our center.MethodsThrough the Genetic Information for Treatment Decisions (GIFTD) Initiative, a Genetic Ris

PSC-DM: A Validated Clinical Diagnostic Model for Distinguishing Primary From Secondary Sclerosing Cholangitis.

BACKGROUND AND AIMS: Primary sclerosing cholangitis (PSC) is a rare, progressive cholangiopathy for which diagnosis remains challenging because of the absence of disease-specific markers and the presence of secondary causes of sclerosing cholangitis (SSC) that closely mimic its clinical presentation. Diagnostic uncertainty can delay appropriate management and add to the psychological burden experienced by patients. This study aimed to identify clinical parameters that support the early and accurate differentiation of PSC from SSC. METHODS: We conducted a multicentre retrospective study of adu

A Bayesian Prevalence-Incidence Mixture Model for Screening Outcomes With Misclassification.

Screening and surveillance programs for cancer, such as colorectal cancer (CRC), often yield electronic health records (EHR) of screening time, test results, and covariates. We consider EHR from CRC surveillance of individuals who have a high cancer risk due to their family history. These individuals, therefore, receive regular colonoscopies with the goal of finding and removing adenomas, precursor lesions to CRC. Our objective is to estimate time to adenoma incidence and explore associations with covariates. However, in doing so, several challenges of the CRC surveillance EHR have to be addre

Haplotype GWAS in Colorectal Cancer Patients with a Family History of Gastric or Prostate Cancer.

Previous haplotype Genome Wide Association Studies (GWASs) have suggested several rare loci with a shared increased risk of colorectal, gastric, and prostate cancer. This study aimed to find out more about markers specifically addressing the shared risk of colorectal and gastric cancer, as well as the shared risk of colorectal and prostate cancer. One analysis used 426 colorectal cancer cases with gastric cancer, with no prostate cancer cases in their families, and another analysis used 324 colorectal cancer cases with prostate cancer but no gastric cancer among relatives. The computational pr

Screening strategies for colorectal cancer: adjusting for individual risk factors.

INTRODUCTION: Colorectal cancer (CRC) is a common cause of cancer-related mortality. While overall incidence has declined, certain populations - particularly younger patients - have experienced a rise. This suggests a more complex risk profile than the dichotomous average vs high risk delineated in guidelines. Risk is currently largely characterized by polyp and family history; however, many additional variables contribute to risk. AREAS COVERED: We synthesize the available data (Pubmed database, focus on article updates in the past 10 years) on non-modifiable and modifiable risk factors for

Development and validation of an epidemiological risk score for early detection of pancreatic cancer in high-risk individuals within the IMAGene project.

BACKGROUND: Early detection is critical in pancreatic ductal adenocarcinoma (PDAC)-one of the most lethal malignancies due to its typically late diagnosis. METHODS: In this study, we aimed to validate an epidemiological risk score (ERS) designed to identify individuals at increased risk of developing PDAC prior to the use of imaging or other diagnostic procedures. The ERS was constructed through a meta-analysis of 24 well-established epidemiological risk factors. We applied this score to a prospective cohort of 178 high-risk individuals with a family history of PDAC within the IMAGene project

Comparable remission and health care use in real-world inflammatory bowel disease patients initiating originator biologics vs biosimilars.

BACKGROUND: Biologic therapies, including anti-tumor necrosis factor agents, have significantly improved the management of inflammatory bowel disease (IBD). However, their high cost can limit patient access. Biosimilars are a more affordable option than originator biologics and offer potential for improved accessibility, though real-world evidence comparing their effectiveness remains limited. AIM: To describe real-world, comparative outcomes of IBD patients taking biologics and biosimilars, focusing on remission and healthcare use. METHODS: We used data from a multicenter registry-based coh

Systemic Glucocorticoid Use and Risk of Colorectal Cancer, Especially Early-Onset Type: A Nationwide Cohort Study.

BACKGROUND: Evidence on the association between systemic glucocorticoid use and colorectal cancer (CRC) risk is limited. This study aimed to determine whether systemic glucocorticoid use is associated with an increased risk of overall CRC (diagnosed at any age) and early-onset CRC (EOCRC; diagnosed before 50 years of age). METHODS: A nationwide cohort study (follow-up: 2005-2018) was conducted using data from multiple nationwide registers in Sweden. The study included 10,921,991 individuals, of whom 2,174,744 used glucocorticoids; among these, 54,443 were newly diagnosed with CRC. The risk of
