The Detection of BRCA1 and BRCA2 Mutations in Saudi Females with Breast Cancer in Al-Madinah Province by Next Generation Sequencing.

BACKGROUND: Breast cancer has the highest incidence among Saudi women and is often diagnosed at a younger age with distinct genetic features. This study aimed to determine the prevalence and mutation of germline BRCA1 and BRCA2 mutations in Saudi women in Al-Madinah Province with breast cancer. METHODS: This cross-sectional study included 54 Saudi females with a median age of 46.5 years (IQR 39.0 - 53.7 years). Genomic DNA from peripheral blood was screened using high-depth next-generation sequencing (NGS), which encompassed all coding exons and splice regions of BRCA1 and BRCA2. Variants wer

Latent profiles of adherence to lymphedema prevention behaviors and associated factors among breast cancer patients after surgery: a cross-sectional study.

BACKGROUND: Breast cancer-related lymphedema (BCRL) is a common complication after breast cancer surgery and can have lasting effects on physical function and quality of life. Prevention behaviors are an important component of BCRL risk reduction, yet patients do not necessarily engage in these behaviors in the same way. Previous studies have largely assessed adherence at the overall level, providing limited insight into differences in behavioral patterns among postoperative patients. A person-centered approach may therefore help identify distinct adherence profiles and provide a basis for tar

Montgomery Tubercles in the Male Areola: Histological Observations and a Brief About Past Investigators.

There are conflicting statements in the literature about the existence of Montgomery tubercles in the male areola. We wished to determine whether the male breast contains Montgomery tubercles and to determine additional pathology, if any, in the male areola. Serial sections from areola of paraffin blocks from six retrospective and five prospective cases of male breast carcinoma were evaluated. Only hematoxylin-stained and eosin-stained sections were studied. There were definite Montgomery tubercles in two cases and putative Montgomery tubercles in two other cases, among the 11 cases (age range

Mid- and Long-Term Risk of Venous Thromboembolism Among Female Breast Cancer Survivors: A Nationwide Cohort Study.

Venous thromboembolism (VTE) is a serious complication for breast cancer (BC) survivors. We identified records of female BC survivors from the Korean Health Insurance Service database between January 1, 2010, and December 31, 2016. After applying exclusion criteria (age < 18 years, prior cancers or VTE, absence of BC surgery, or missing data), 112,522 BC survivors were included; for the comparison group, BC survivors were matched (1:3) to women without cancer history. VTE risk was evaluated with a Fine and Gray model, accounting for death as a competing risk; multiple time periods were analyze

Priorities for breast cancer risk assessment in UK women under age 50: a survey and discrete choice experiment.

BACKGROUND: National guidelines recommend English women between 30 and 49 years with moderate/high risk of breast cancer (BC) be offered screening and prevention interventions. Identification  largely relies on family history-based criteria amongst women self-presenting in primary care. We aimed to understand the preference for different potential risk assessment strategies in this population. METHODS: A cross-sectional survey including a discrete choice experiment. Risk assessments were defined using six attributes, including initiation strategy, eligibility for a full/detailed risk assessme

Comparison of NICE criteria with the BOADICEA multifactorial risk model to guide breast cancer risk assessment and referral amongst women under age 50 within primary care.

BACKGROUND: The National Institute for Health and Care Excellence (NICE) recommends that women at above-population-level risk of breast cancer (BC) are offered enhanced screening and risk-reducing interventions. Identification of these women largely relies on referral through primary care based on family history criteria. METHODS: We compared the performance of the NICE criteria with versions of the BOADICEA multifactorial risk model in identifying women who developed BC within 10 years. Analyses included 1258 women aged under 50 years with family history, reproductive, lifestyle and polygeni

Trastuzumab Deruxtecan Resistance via Loss of HER2 Expression and Binding.

UNLABELLED: Trastuzumab Deruxtecan (T-DXd) is clinically beneficial in HER2-positive and HER2-low metastatic breast cancer. However, therapeutic resistance emerges over time in most patients, with poorly defined resistance mechanisms. Through a molecular characterization of paired patient specimens before and after T-DXd treatment, we found that 49% cases had major decreases in HER2 expression at progression, and among them, 52% exhibited complete HER2 loss. Using isogenic model systems, we demonstrated that decreases in HER2 expression corresponded to reductions in T-DXd internalization and m

Preemptive hematopoietic stem cell transplantation in RUNX1 familial platelet disorder: a shared decision-making framework.

RUNX1 familial platelet disorder (RUNX1-FPD) is associated with a 35-50% lifetime risk of hematologic malignancy (HM). Like all germline HM predisposition syndromes, RUNX1-FPD can only be cured with allogeneic hematopoietic stem cell transplantation (HSCT). Current genetic screening techniques allow for early detection of germline predisposition and, consequently, the opportunity for HSCT before overt development of HM (i.e., preemptive HSCT). However, there is as yet no consensus on the use of preemptive HSCT for RUNX1-FPD. Described here is the case of an individual with RUNX1-FPD and a fami

Aryl Hydrocarbon Receptor Ligands Drive Pancreatic Cancer Initiation and Progression through Protumorigenic T-cell Polarization.

UNLABELLED: Although smoking is a risk factor for pancreatic adenocarcinoma (PDAC), the underlying mechanisms promoting tumorigenesis and progression are unknown. In this study, we show that aryl hydrocarbon receptor (AHR) ligands found in cigarette smoke, like the carcinogen 2,3,7,8-tetrachlorodibenzo-p-dioxin, promote pancreatic dysplasia and PDAC progression in a mouse model of this disease. This effect is mediated by AHR activation in CD4+ T cells, leading to their polarization to IL22-producing TH22 cells and regulatory T cell accumulation, ultimately driving a blunted CD8+ T-cell effecto

The fully human anti-GPRC5D CAR T-cell therapy RD118 induces durable remissions in relapsed/refractory multiple myeloma.

GPRC5D has emerged as a promising therapeutic target in relapsed/refractory multiple myeloma (R/R MM), particularly following progression after B-cell maturation antigen (BCMA)-directed chimeric antigen receptor (CAR) T-cell (CAR-T) therapies. RD118 is a novel CAR-T therapy incorporating a fully human single-domain antibody fragment targeting GPRC5D. In this phase 1 study, 18 R/R patients (17 with MM and 1 with a history of primary plasma cell leukemia) received a single infusion of RD118 at 1.0 × 106, 2.0 × 106, or 3.0 × 106 CAR+ T cells per kg. At a median follow-up of 17.0 months, the overa

Lymphoid malignancy and clonality in the POT1-mediated long telomere syndrome.

Long telomere length (TL) extends replicative capacity in vitro and predisposes to clonal hematopoiesis. We characterized the cancer phenotype in 51 individuals from 24 families with mutant POT1, a negative regulator of telomerase elongation (median age, 51 years [range, 5-94]). Hematologic malignancies were second in prevalence after melanoma (27%), and lymphoid subsets were more common. They clustered with history of sarcoma, thyroid cancer, and chronic myeloproliferative neoplasms. UK Biobank participants with pathogenic POT1 variants had long TL and higher lymphoid malignancy rates (45% by
