Rechallenge With Immune Checkpoint Inhibitors in Advanced Gastric or Gastroesophageal Junction Adenocarcinoma.

AIMS: While immune checkpoint inhibitors (ICIs) are widely used for advanced gastric/gastroesophageal junction (G/GEJ) adenocarcinoma, most patients still experience disease progression, and subsequent treatment options are limited. ICI rechallenge strategies have been thoroughly studied in other cancers, but evidence in gastric cancer remains scarce. This study evaluated the efficacy, prognostic factors, and safety of ICI rechallenge in advanced G/GEJ adenocarcinoma. MATERIALS AND METHODS: Patients who received ICI rechallenge between November 2019 and April 2025 after progression on first-l

Intestinal cystic lymphangioma.

A 7-year-old girl, coming from a rural area in Cameroon, presented to the emergency department with a 3-months history of abdominal pain. Her family also reported vomiting and minimal food intake for two weeks. Physical examination showed a palpable and mobile abdominal mass. An ultrasound showed a large intrabdominal multicystic lesion of about 10cm, close to the intestine and with no solid lesions in other organs. A laparotomy was scheduled and a mobile mass dependent on the jejunum was found. The mass caused an intestinal obstruction and was composed of several large cysts with whitish flui

Rectal Colonization with Multidrug-Resistant Organisms and Subsequent Bloodstream Infections in Hematological Patients Undergoing Cellular Therapies.

Bacterial infections, especially bloodstream infections (BSIs), compromise the overall survival of patients with hematological malignancies undergoing cellular therapies. The interplay between rectal colonization by different types of multidrug-resistant organisms (MDRO) and subsequent BSIs is not fully determined. We aimed to describe the natural history and risk factors for colonization by MDRO, as well as its impact on subsequent BSIs in patients treated with cellular therapies in an environment of antimicrobial resistance. Retrospective, single-center cohort study of consecutive adult hema

[An autopsy case report of combined hepatocellular-cholangiocarcinoma in a young woman without chronic liver disease].

This case concerns a woman in her 20s with abdominal pain. Abdominal computed tomography revealed a large hepatic tumor with extensive infiltration of both liver lobes. She developed acute liver failure and was immediately admitted to our hospital. Her condition rapidly deteriorated, progressing to multiple organ failure, leaving insufficient time for comprehensive evaluation. She died 4 days after admission. Autopsy showed marked hepatomegaly due to tumor, with the liver weighing 3400g. Pathological diagnosis was combined hepatocellular-cholangiocarcinoma (cHCC-CCA), a rare primary liver canc

Stoma-free precision sphincter-preserving surgery with J-pouch for metastatic low rectal neuroendocrine tumor.

R0 resection and maximal preservation of anal function without a permanent stoma remains challenging in patients with a rectal neuroendocrine neoplasm (R-NEN) within 5 cm of the anal verge and lymph node metastasis in the iliac vascular region. A 22-year-old man presented with a 1-year history of paroxysmal palpitations and cold sweats. Colonoscopy revealed a 2-cm mass with well-defined margins located 5 cm from the anal verge. Biopsy revealed a neuroendocrine tumor. Endoscopic submucosal dissection and pathology revealed a G2 R-NEN with negative margins but vascular invasion. Enhanced MRI and

Testosterone Treatment in Prostate Cancer Survivors With Hypogonadism: A Randomized Clinical Trial.

IMPORTANCE: A majority of men with prostate cancer have low-grade cancer and an excellent prognosis after radical prostatectomy. Hypogonadism and associated symptoms impair quality of life in prostate cancer survivors. Many guidelines, citing a lack of randomized clinical trials showing safety and efficacy of testosterone replacement therapy (TRT), consider a history of prostate cancer a contraindication for TRT. OBJECTIVE: To evaluate the short-term safety and efficacy of TRT in prostate cancer survivors with symptomatic hypogonadism. DESIGN, SETTING, AND PARTICIPANTS: This randomized, plac

Histopathological evidence of direct hepatic invasion by Anisakis from the liver surface: A case report.

Anisakis occasionally parasitizes extra-gastrointestinal organs, including the liver, pancreas, and ovary. We herein report a case of hepatic anisakiasis in which the route of hepatic invasion by Anisakis was histologically confirmed. A 50-year-old Japanese man with a history of left testicular seminoma underwent follow-up contrast-enhanced dynamic computed tomography, which revealed a liver mass suspicious for metastasis. The scan showed a low-attenuation area near the liver surface in segment 7. Laparoscopic partial hepatectomy was performed for diagnosis and treatment. Histologically, the m

The role of multi-organ cancer predisposition genes in the risk of inherited and histologically diverse gastric cancer.

BACKGROUND: Approximately 10% of cases with gastric cancer (GC) exhibit familial clustering, however, only 1-3% of cases can be explained by two known hereditary syndromes: Hereditary Diffuse Gastric Cancer (HDGC) caused by CDH1 and CTNNA1 pathogenic germline variants; and Gastric Adenocarcinoma and Proximal Polyposis of the Stomach (GAPPS), caused by germline variants in APC 1B promoter. Familial intestinal gastric cancer (FIGC) has been defined clinically, but it remains mostly genetically unexplained. Likewise, the heritability of mixed histology GC remains to be known. We aimed to estimate

High-Frequency Micro-Ultrasound Transrectal Imaging Enhances Staging and Preoperative Planning of Prostate Radiation Induced Rectal Cancer: A Case Report.

High-frequency ultrasound, known as micro-ultrasound (microUS), is a sonographic imaging modality capable of acquiring high-resolution, microscopically detailed images. This technology has been increasingly utilized for prostate cancer biopsy and diagnosis. Its utility outside of prostate cancer is under-described. A 76-year-old male with a past medical history of brachytherapy 25 years prior developed rectal cancer. Before undergoing resection, transrectal microUS was utilized to determine the relation of the rectal mass to the urogenital organs, which better established the planned surgical

Pancreatic Neuroendocrine Tumors in French VHL Mutation Carriers.

CONTEXT: Von Hippel-Lindau disease (VHL) is a rare, autosomal-dominant hereditary cancer-predisposition syndrome caused by germline pathogenic variants (PVs) in the VHL gene. It is associated with a high penetrance of benign and malignant vascular tumors in multiple organs, including pancreatic neuroendocrine tumors (PanNETs), whose long-term natural history is ill-known. OBJECTIVE: The aim of this study was to identify prognostic factors associated with VHL-related PanNETs, notably the role of genotype-phenotype correlations. METHODS: Patients with both documented germline PV in the VHL gen

Predicting Liver Cancer Risk Using Comprehensive Medical History.

BACKGROUND AND OBJECTIVES: Liver cancer mortality is rising faster than any other cancer, significantly impacting life expectancy due to its relatively young median age at diagnosis and high mortality rate. There are currently no consistently recommended screening tests for liver cancer in individuals with a high-risk profile or abnormalities in body systems other than liver disease with cirrhosis. This study aims to screen various body system diseases that might be associated with liver cancer risk. METHODS: The study utilized the All of Us database, including 410 361 US-based adults aged 18

Adenocarcinoma of the gallbladder: an incidental histopathological finding post-operative laparoscopic cholecystectomy in Saudi Arabia.

Gallbladder adenocarcinoma (GBC) is a rare and challenging diagnosis, often associated with chronic gallstones and discovered incidentally during histopathological examination. It may present in early or late stages without overt clinical or radiological signs. Laparoscopic cholecystectomy is commonly performed for gallstones, and in such cases, gallbladder cancer may be detected even in the absence of a visible mass or atypical clinical presentation. A 68-year-old female with no significant medical history presented with a gallstone, confirmed via abdominal ultrasound, showing multiple stones
