Cumulative incidence of advanced breast cancer in women aged 40-49 years in the Japan Strategic Anti-cancer Randomised Trial (J-START) of adjunctive ultrasonography: a prespecified secondary analysis.

BACKGROUND: The J-START randomised controlled trial found that adjunctive ultrasonography was associated with significantly higher rates of breast cancer detection than mammography alone. This report aims to evaluate the long-term effect of adjunctive ultrasonography screening on the cumulative incidence of advanced breast cancer as a prespecified secondary outcome of J-START. METHODS: We enrolled asymptomatic women in 42 study sites in 23 of 47 prefectures in Japan. Eligible women were aged 40-49 years without a history of breast cancer, including in-situ cancer, or other cancers in the prev

Impact of prior solid tumor on outcomes of hematopoietic stem cell transplantation for hematologic malignancies: a propensity score-matched study.

BACKGROUND: The Hematopoietic Cell Transplantation-Specific Comorbidity Index (HCT-CI) assigns a high-risk score to patients who develop secondary hematologic malignancies following solid tumors, indicating an increased risk of non-relapse mortality (NRM). This study aimed to evaluate the impact of prior solid tumors on outcomes after hematopoietic stem cell transplantation (HSCT). METHODS: From a cohort of 2,382 patients who underwent HSCT for acute lymphoblastic leukemia (ALL), acute myeloid leukemia (AML), or myelodysplastic syndrome (MDS) between January 2014 and July 2024, we included 43

Concurrent high-intensity interval plus resistance training improves vascular health in breast cancer survivors with high chemotherapy exposure.

BACKGROUND: The aim was to determine the effects of 8 weeks of concurrent training of abbreviated high-intensity interval-training and low–load resistance training (CTHIIT+RT) on functional and structural vascular outcomes of breast cancer survivors with a history of high or low exposure to chemotherapy sessions. METHODS: Breast cancer survivor women (n = 21, 58.7 ± 8.7 years) were divided into high– (HVchemo, n = 11) or low–volume chemotherapy groups (LVchemo, n = 10). Pulse wave velocity (PWV) and carotid intima–media maximum (cIMTmax) were the primary outcomes, while flow-mediated dilation

Magnetic resonance imaging findings of non-mass enhancements in the breast and their association with malignancy.

Our aim is to investigate the relationship between the MRI (Magnetic Resonance Imaging) features of non-mass enhancement (NME) lesions and malignancy. MRI findings of 200 and 2 NME lesions were retrospectively evaluated. Age, family history of breast cancer, lesion pathology results, and follow-up durations of the patients were obtained from medical records. Based on follow-up findings and pathology results, the lesions were classified into 3 groups as benign, high-risk, and malignant. The association between these groups and MRI features was analyzed statistically. Segmental (48.4%) and diffu

Characterizing pseudoangiomatous stromal hyperplasia (PASH) of the breast.

AIMS: To investigate the associated histopathology and immunohistochemical (IHC) profile of pseudoangiomatous stromal hyperplasia (PASH) of the breast in a large, contemporary cohort. METHODS AND RESULTS: We reviewed 193 benign breast biopsies with PASH from 185 patients without history of breast atypia or carcinoma. A tissue microarray was created and subjected to a panel of IHC. Ischemic fat necrosis was the most common finding in biopsies with PASH (181/193; 93.8%), while fibrocystic changes (170/193; 88.1%) were frequently present. PASH showed consistent, diffuse, strong staining for CD34

[The concept of futility in medicine].

A 79-year-old woman with a history of breast cancer was incidentally found to have a suspicious lung nodule. Though reluctant to accept treatments with disabling side effects, she was persuaded to undergo biopsy after oncologists stressed the low toxicity and potential benefit of EGFR inhibitors. The biopsy confirmed lung adenocarcinoma but caused complications, including pneumothorax and progressive dyspnea, without improving outcome. Her primary wish was to return home and limit hospital exposure, an expectation ultimately unmet. This case underlines the need to respect patient preferences,

Breast cancer screening and cancer prognosis in patients with Alzheimer disease and related dementias.

BACKGROUND: Breast cancer screening is crucial for early detection and improved survival in Alzheimer disease and related dementias (ADRD) patients, but real-world evidence of its effects on survival and prognosis remains insufficient. METHODS: We conducted a retrospective cohort study using Surveillance, Epidemiology, and End Results-Medicare data (1999-2019) to analyze the impact of breast cancer screening on prognosis (ie, cancer stage) and survival in ADRD women with breast cancer diagnosed at ages 67 years and older. Logistic and Cox regression models were employed to assess the relation

The association between global and prime diet quality scores and the risk of breast cancer: a case control study.

BACKGROUNDS: Despite established links between diet and breast cancer, the association of comprehensive diet quality indices specifically Global Diet Quality Score (GDQS) and Prime Diet Quality Score (PDQS) with BC risk remains underexplored. This case-control study evaluated the association between global and prime diet quality scores and the risk of breast cancer. METHODS: Dietary intake of 125 newly diagnosed, histopathologically confirmed breast cancer cases and 250 age-frequency-matched controls (women ≥30 years) at Shaqra General Hospital was assessed using a validated semi-quantitative

AI-based BRAIx risk score for the intermediate-term prediction of breast cancer: a population cohort study.

BACKGROUND: Artificial intelligence (AI)-based algorithms are being implemented in breast screening to detect breast cancers on mammographic images. We aimed to apply an epidemiological approach to demonstrate how a cancer detection algorithm can be leveraged as an intermediate-term predictor of breast cancer (current and 4-year risk) to deliver greater risk-based personalisation in screening mammography. METHODS: In this population cohort study, we used detection scores from an AI cancer detection algorithm (BRAIx AI Reader), which was calibrated using a training dataset of 397 648 women age

Tamoxifen Use and Risk of Uterine Diseases in Young Women With Breast Cancer.

OBJECTIVE: Tamoxifen use has been associated with an increased risk of endometrial cancer in women with breast cancer, but data on premenopausal women in Asia remain limited. We investigated the association between tamoxifen treatment and risk of developing uterine diseases in women of premenopausal age with breast cancer. METHODS: We conducted a retrospective cohort study using a target trial emulation framework. Women aged 20-50 years who were diagnosed with estrogen receptor-positive breast cancer and had undergone mastectomy or lumpectomy from 2010 to 2019 were identified with Taiwan's Na

Underserved Latinas' Perceptions and Implications Around Breast Cancer Risk Assessment.

INTRODUCTION: Breast cancer remains a leading cause of death among Latinas. Although breast cancer risk assessment models exist and show promise in reducing mortality, few studies have elicited women's interest in and perceptions around breast cancer risk assessment, particularly among underserved populations, like Latinas, who are susceptible to disparities in breast cancer outcomes. These disparities stem from factors such as limited access to healthcare services, lower rates of screening utilization, language barriers, and socioeconomic challenges, all of which compound their risk and hinde

Fatty Breasts and Palpable Lumps: Diagnostic Mammogram Alone May Be Sufficient to Exclude Cancer.

OBJECTIVE: To assess whether diagnostic mammogram alone is sufficient to identify breast cancer in women with a palpable lump and almost entirely fatty breast density. METHODS: All 2D full-field diagnostic mammograms performed in a large health system were retrospectively identified for patients assigned "almost entirely fatty" breast density from January 2014 through December 2018 who presented with a palpable lump and had at least 2 years of follow-up. Women were excluded if they had a history of breast cancer, ipsilateral surgery, or clinical suspicion of abscess. Other exclusions included

Familial papillary thyroid cancer; A male case with breast and thyroid cancer and his three sisters with thyroid cancer: A case series and review of the literature.

Thyroid cancer is the most widespread endocrine malignancy. Of all the cases of thyroid cancer, approximately 95% arise from follicular cells. Familial non-medullary thyroid cancers constitute 3-9% of all the reported cases of thyroid cancer and are diagnosed by the presence of two or more first-degree relatives with cancer. Most familial cases of non-medullary thyroid cancer are caused by papillary thyroid cancer and its histological variants. Although some genes predisposing to familial papillary thyroid cancer have been identified, their functionality is unclear, and routine examination is

Germline BRCA testing in routine clinical practice: a single-center experience.

The identification of gBRCA1/2 mutations in breast cancer patients is crucial. Successful identification of the mutations has the potential to alter disease treatment and healthcare management of patients whose relatives harbor pathogenic/likely pathogenic (P/LP) variants. In this retrospective analysis, patient- and disease-specific medical data were analyzed in a cohort of breast cancer patients with a known gBRCA1/2 status who were treated between 2019-2021. The prevalence and type of gBRCA1/2 P/LP variants, and their relation to the histopathological data of the cancers, were studied. The

General practitioners' responses to dense breast notifications resulting from a screening mammography program.

Breast density reporting is recommended; however, guidance for general practitioners (GPs) is limited. This study investigates GPs' beliefs, attitudes, and practices within the only state-run Breast Screen program in Australia that has been notifying women if they have dense breasts for over a decade. A mixed-methods study was undertaken from four multi-GP practices. Qualitative interviews from GPs and de-identified data from a randomized sample of 692 women who attended screening mammography (2019-2022) were collated. Descriptive statistics, binomial regression for quantitative data, and both

Breast Cancer Screening Knowledge and Sentiments in Singaporean Women: Mixed Methods Study Using Topic Modeling, Sentiment Analysis, and Structured Questionnaire Data.

BACKGROUND: Mammography screening uptake in Singapore remains below 40% despite campaigns and subsidies. Natural language processing (NLP) can extract nuanced attitudes from free text that fixed response options miss, revealing latent factors influencing breast cancer (BC) screening behavior. OBJECTIVE: This study characterized women's attitudes toward mammography using mixed methods data, examined associations between BC awareness and screening willingness, and identified barriers and facilitators through NLP of free-text responses. METHODS: We conducted a cross-sectional study within the B

Impact of Ethnicity on the Uptake of Risk Reducing Mastectomy in Unaffected BRCA1/2 and PALB2 Carriers in Canada.

BACKGROUND: While several studies have reported on factors influencing the uptake of risk-reducing mastectomy (RRM) in patients with germline pathogenic variants (gPV) in BRCA1/2, there remains a paucity of data regarding variations in uptake by ethnicity. METHODS: We conducted a retrospective cohort study of unaffected female patients with a confirmed gPV in BRCA1/2 or PALB2 between March 2000 to March 2023 at two academic institutions in Montreal, Canada. Ethnicity was self-reported as: White/European, Black/African/Caribbean, Asian/Southeast Asian, Hispanic/South/Central American, Middle E

Radiation-Induced Angiosarcoma of the Breast With Diffuse GATA-3 Positivity: A Possible Diagnostic Pitfall.

Radiation-induced angiosarcomas of the breast are rare malignancies that carry a poor prognosis. Expected immunohistochemical profiles reflect vascular differentiation: CD31 and ERG positivity, with variable CD34 positivity. GATA-3 is a transcription factor important for mammary gland development, playing a role in ductal epithelial cell differentiation. It serves as an important immunohistochemical marker of breast-origin carcinomas and is not associated with angiosarcomas. We report a case of an 83-year-old woman with breast radiation-induced angiosarcoma, which displayed diffuse GATA-3 posi

Breaking barriers to completing genetic testing for inherited breast cancer among at-risk Black women using a community-based participatory research approach.

National guidelines from the US Preventive Services Task Force and the National Comprehensive Cancer Network recommend the use of family-health-history (FHH)-based risk assessment tools to guide genetic testing (GT) among women with an increased risk of inherited cancer and inform personalized cancer risk management. Prior research has focused on attitudes toward and decisions about initial uptake of GT in Black patients but little is known about the factors that impact the subsequent completion of GT after they have already provided consent. Using a community-based participatory research (CBP
