Giant Cell Tumor of the Uterus: A Report and Review of the Literature.

Giant cell tumors are neoplasms that usually occur in the long bones of young adults. They can rarely present in the soft tissue and may display malignant behavior. Giant cell malignancies have previously been reported as tumors primary of the uterus but are exceptionally rare. Although uncommon, it is important to consider when a neoplasm with osteoclast-like giant cells is encountered. In this report, we describe a 70-year-old female patient with an unremarkable past history who presented with an enlarged uterus. Transvaginal ultrasound demonstrated multiple fibroids and a thickened endometr

Ovarian Mesonephric-Like Adenocarcinoma Arising from an Endometriotic Cyst with an Identical KRAS Mutation in Both Components: A Case Report.

Mesonephric-like adenocarcinoma (MLA) is a rare tumor that occurs in the uterine corpus and ovary. It shares the morphological and immunohistochemical features of cervical mesonephric adenocarcinoma. Recent reports have suggested that a majority of ovarian MLAs are associated with endometriosis. Herein, we report the novel tumor of a 54-year-old Japanese woman with a history of bilateral ovarian endometriotic cysts who presented with abdominal distension. Pelvic magnetic resonance imaging revealed a 14-cm left ovarian cystic tumor with an intra-cystic solid mass. Histological examination revea

Surgical management of ovarian torsion in children: Detorsion, cyst/mass excision, and ovarian fixation.

This study evaluated surgical interventions based on the presence or absence of ovarian cysts or masses in pediatric and adolescent patients with ovarian torsion (OT) and analyzed their impact on retorsion development. A retrospective review was conducted on 25 ovaries from 23 patients under 18 years of age who underwent surgery for OT between July 2019 and January 2025. Patients were divided into two groups: Group 1 (n = 14), with ovarian or paraovarian cysts or masses that were excised; and Group 2 (n = 11), without detectable lesions. Detorsion was performed in all cases, and adnexal fixati

Histopathological profile of endometrium among peri and post-menopausal women with abnormal uterine bleeding and its correlation with endometrial thickness by transvaginal sonography: a retrospective study.

BACKGROUND: Abnormal uterine bleeding (AUB) is a prevalent clinical concern, particularly in women approaching or beyond menopause. With a myriad of possible etiologies ranging from benign hyperplasia to malignant transformations, accurate diagnosis becomes crucial. This study aims to examine the histopathological patterns of the endometrium in peri- and postmenopausal women presenting with abnormal uterine bleeding and correlate these findings with endometrial thickness (ET) measured by transvaginal sonography (TVS). METHODS: A retrospective cohort of 307 women aged 40 and above presenting w

Adult bilateral primary B-lymphoblastic lymphoma of the testis.

A man in his late 20s presented with a 7 day history of painless right scrotal swelling. Scrotal ultrasound revealed complete tumour infiltration of the right testis and two-thirds infiltration of the left testis. A right radical orchidectomy and left testicular biopsy confirmed B-lymphoblastic leukaemia/lymphoma. Subsequent bone marrow and peripheral blood tests showed no systemic involvement. Persistent left testicular disease was found following induction chemotherapy, and orchidectomy of the contralateral testis was subsequently conducted. This case highlights the unique nature of bilatera

Pseudocyst Formation in Residual Ovary Syndrome: A Unique Consequence of Ovarian Preservation.

Residual ovary syndrome (ROS) is a consequence of ovarian preservation after pelvic surgery. One unique and understudied manifestation of ROS includes patients who present with pseudocyst formation. These patients often have a history of multiple pelvic surgeries. Although the etiology is poorly understood, ovarian pseudocysts presumably develop because of ovulation into an enclosed space secondary to the formation of inflammatory adhesions. Herein, we describe a small cohort of patients with ROS from pseudocyst formation to highlight a rational approach to the diagnosis and management of this

Outcomes of transvaginal natural orifice transluminal endoscopic hysterectomy: a multi-centre retrospective study from Turkey (TR-MIGS).

BACKGROUND: Vaginal natural orifice transluminal endoscopic surgery (vNOTES) hysterectomy is gaining increasing popularity among gynaecological surgeons worldwide due to its minimally invasive nature and associated benefits. However, despite its growing adoption, it remains a relatively novel surgical technique that continues to be evaluated and refined in clinical practice. The primary objective of this study is to share and analyse our initial experiences with the implementation of vNOTES hysterectomy across six specialised gynaecological centres in Turkey. METHODS: This retrospective analy

Development and validation of biopsy free nomograms for predicting clinically significant prostate cancer in men with PI-RADS 4 and 5 lesions.

To develop and validate biopsy-free nomograms to more accurately predict clinically significant prostate cancer (csPCa) in biopsy-naïve men with prostate imaging reporting and data system (PI-RADS) ≥ 4 lesions. A cohort of 931 patients with PI-RADS ≥ 4 lesions, undergoing prostate biopsies or radical prostatectomy from January 2020 to August 2023, was analyzed. Various clinical variables, including age, prostate-specific antigen (PSA) levels, prostate volume (PV), PSA density (PSAD), prostate health index (PHI), and maximum standardized uptake values (SUVmax) from PSMA PET-CT imaging, were ass

Association of prostate-specific antigen density with prostate cancer mortality after a benign systematic prostate biopsy result.

OBJECTIVE: To assess the association between prostate-specific antigen (PSA) density (PSAD) and prostate cancer mortality after a benign result on systematic transrectal ultrasonography (TRUS)-guided prostate biopsy. PATIENTS AND METHODS: This retrospective study used data from the Finnish Randomised Study of Screening for Prostate Cancer (FinRSPC) collected between 1996 and 2020. We identified men aged 55-71 years randomised to the screening arm with PSA ≥4.0 ng/mL and a benign systematic TRUS-guided biopsy result. The cumulative prostate cancer mortality of men stratified by a PSAD cutoff o

An Artificial Intelligence Model Using Diffusion Basis Spectrum Imaging Metrics Accurately Predicts Clinically Significant Prostate Cancer.

PURPOSE: Conventional prostate magnetic resonance imaging has limited accuracy for clinically significant prostate cancer (csPCa). We performed diffusion basis spectrum imaging (DBSI) before biopsy and applied artificial intelligence models to these DBSI metrics to predict csPCa. MATERIALS AND METHODS: Between February 2020 and March 2024, 241 patients underwent prostate MRI that included conventional and DBSI-specific sequences before prostate biopsy. We used artificial intelligence models with DBSI metrics as input classifiers and the biopsy pathology as the ground truth. The DBSI-based mod

Optimizing Prostate Cancer Diagnostic Work-Up Through Micro-Ultrasound: Minimizing Unnecessary Procedures and Reducing Overdiagnoses.

INTRODUCTION: We aim to critically assess Microultrasound (mUS) clinical performance in an outpatient setting, focusing on its ability to reduce unnecessary diagnostic procedures, potentially reshape prostate cancer (PCa) diagnostic protocols, and increase the ability to rule out clinically significant (Gleason Score ≥ 3 + 4) PCa (csPCa). MATERIALS AND METHODS: Between November 2018 and April 2022, we conducted a prospective study involving men who underwent mUS examination due to clinical symptoms, PSA elevation, or opportunistic early detection of PCa. Experienced urologists performed mUS a

Integrating radiological and clinical data for clinically significant prostate cancer detection with machine learning techniques.

In prostate cancer (PCa), risk calculators have been proposed, relying on clinical parameters and magnetic resonance imaging (MRI) enable early prediction of clinically significant cancer (CsPCa). The prostate imaging-reporting and data system (PI-RADS) is combined with clinical variables predominantly based on logistic regression models. This study explores modeling using regularization techniques such as ridge regression, LASSO, elastic net, classification tree, tree ensemble models like random forest or XGBoost, and neural networks to predict CsPCa in a dataset of 4799 patients in Catalonia

Evaluation of the Performance of Systematic Sampling Combined With MRI-TRUS Fusion Biopsy and Concordance After Radical Prostatectomy.

BACKGROUND: MRI-guided prostate biopsy allows the targeted sampling of suspicious lesions. However, limitations of MRI and MRI-TRUS fusion biopsy techniques may lead to an underdiagnosis, and thus, some patients may benefit from combining systematic sampling with targeted sampling during prostate biopsy. We aim to assess the diagnostic yield of systematic sampling on MRI-TRUS fusion biopsy pathological result and the histopathological concordance after radical prostatectomy. METHODS: We retrospectively compared the prostate cancer (PCa) and clinically significant prostate cancer (csPCa) detec

Incidence and prevention strategies for postprostate biopsy infections in the United States from 2012 to 2021.

PURPOSE: Infectious complications following prostate biopsy are costly and potentially deadly. Multiple strategies have been devised to avoid infections, including augmented prophylaxis and transperineal biopsy (TPBx). Their uptake and success in reducing population-level infectious outcomes following biopsy are largely unknown. Here, we evaluate contemporary postbiopsy infections and hospital admissions in a large insurance claims dataset. METHODS AND MATERIALS: The Merative MarketScan Database was queried for prostate biopsies from 2012 to 2021. MarketScan contains inpatient and outpatient
