Adverse impact of bladder biopsy on the rate of intravesical recurrences following radical nephroureterectomy for upper tract urothelial carcinoma.

INTRODUCTION: Seeding and implantation are major mechanisms of intravesical recurrence (IVR) following the radical nephroureterectomy (RNU) of upper tract urothelial carcinoma (UTUC). We herein hypothesized that bladder biopsies (BB) may increase the IVR risk by promoting implantation in injured bladder mucosa. PATIENTS AND METHODS: This retrospective study included 172 patients with UTUC with no history of bladder UC who underwent RNU between January 2010 and December 2024 at two institutions. The primary endpoint was the association between BB and IVR-free survival (IVRFS) after RNU. A dire

Nephroureterectomy for upper tract urothelial carcinoma recurrence in bladder cancer patients treated with radical cystectomy: a multicentric propensity score matched analysis on predictors, practice patterns and survival outcomes.

AIM: (I) To determine clinicopathological determinants of metachronous Upper Tract Urothelial Carcinoma (UTUC) requiring Radical Nephroureterectomy (RNU) after Radical Cystectomy (RC). (II) To evaluate long-term survival of patients who underwent RC + RNU compared with matched RC only controls. METHODS: Patients undergoing RNU for metachronous UTUC were extracted from a multi-institutional RC database. A 1:2 Propensity Score Match (PSM) was performed based on age, gender, BMI, CCI, Smoking Status, and cT stage between RC only and RC + RNU patients. Simon-Makuch plots, landmark analyses and Mu

Development and multicohort validation of an interpretable postoperative model for intravesical recurrence after radical nephroureterectomy in upper tract urothelial carcinoma.

PURPOSE: Intravesical recurrence (IVR) after radical nephroureterectomy (RNU) for upper tract urothelial carcinoma (UTUC) is frequent, but postoperative recurrence risk is heterogeneous. We developed and multicohort-validated an interpretable postoperative model for IVR risk stratification after RNU. METHODS: We included 813 patients with pathologically confirmed UTUC treated with RNU across four predefined cohorts: retrospective development (n = 400), retrospective external validation (n = 173), internal prospective validation (n = 166), and external prospective validation (n = 74). Villous Adenoma of the Renal Pelvis: Clinicopathologic Spectrum and Diagnostic Considerations.

Villous adenoma (VA) of the renal pelvis is a rare intestinal epithelial neoplasm. It generally arises in elderly men and is frequently associated with chronic irritation, infection, or long-standing obstruction, most often due to nephrolithiasis or hydronephrosis. We report four previously unpublished patients diagnosed with VA of the renal pelvis. Each patient was evaluated clinicopathologically, radiologically, immunohistochemically to clarify morphologic diversity and biological behavior. We also review previously reported patients in the literature. All four patients were men, aged 45-70

Urethral syringocele: a rare cause of recurrent urinary tract infections.

A man in his early 30s presented with a 3 week history of dysuria, urinary frequency, urethral discharge and cloudy urine despite two courses of antibiotics. He subsequently developed a tender perineal swelling. His urine flow rate was normal and on flexible cystoscopy, a cystic bulging area distal to the external urethral sphincter was visualised. A pelvic MRI scan revealed a cystic abnormality at the bulb in the midline measuring 3.5×2.1×1.9 cm, consistent with a syringocele of the bulb. The patient underwent endoscopic laser deroofing of the syringocele. Syringocele is a rare abnormality in

Retroperitoneal Robot-Assisted Nephroureterectomy: Technical Description and Single Center Experience.

INTRODUCTION: Robot-assisted radical nephroureterectomy (RANU) is increasingly used for upper tract urothelial carcinoma (UTUC). The retroperitoneal approach avoids bowel mobilization, but its adoption in robotic surgery remains limited because of the confined working space and technical complexity. We described our step by step retroperitoneal technique and evaluated perioperative outcomes in an initial single center series. METHODS: We retrospectively reviewed consecutive patients who underwent intended fully robot assisted retroperitoneal RANU at our institution between July 2022 and Septe

Long-term patient-reported outcomes of open urorectal fistula repair after prostate cancer treatment.

OBJECTIVES: To evaluate long-term outcomes of open urorectal fistula (URF) repair, including URF recurrence, need for re-intervention, and patient-reported outcomes. PATIENTS AND METHODS: This retrospective study included men undergoing open URF repair between 2014 and 2024. Data collected encompassed comorbidities, prostate cancer treatment history, prior URF interventions, and intraoperative details. Endpoints were: (i) URF recurrence-free survival, (ii) re-intervention-free survival (no further disease-related procedures), and (iii) validated patient-reported outcome measures (PROMs). Kapl

Unilateral Cortical Ribboning and Corticomedullary Lesions in a Rare Case of Coexisting Anti-N-methyl-D-aspartate Receptor Encephalitis and Neuronal Intranuclear Inclusion Disease.

Anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis is an autoimmune disorder characterized by psychiatric manifestations, seizures, and dyskinesias. Neuronal intranuclear inclusion disease (NIID) presents heterogeneously with dementia, neuropathy, and episodic encephalitis-like events, typically showing bilateral corticomedullary hyperintensities on diffusion-weighted imaging (DWI). The simultaneous occurrence of these conditions is extremely uncommon, complicating diagnosis due to overlapping clinical and radiological features. We report the case of a 68-year-old female with a histo

Urachal adenocarcinoma: Case report, literature review, and therapeutic algorithm proposal.

Urachal adenocarcinoma is an extremely rare malignancy, accounting for less than 1% of all bladder tumors. Diagnosis is often delayed due to nonspecific symptoms and low clinical suspicion. We report the case of a 52-year-old male with no relevant medical history who presented with lower urinary tract symptoms. Cystoscopy revealed a cystic lesion at the bladder dome. Transurethral resection confirmed mucinous urachal adenocarcinoma. CT imaging showed no locoregional or distant spread, and tumor markers were within normal limits. The patient underwent laparoscopic partial cystectomy with en blo

Pancreatic serous cystadenoma masquerading as malignancy.

Pancreatic cysts include benign, premalignant and malignant lesions. Distinguishing them preoperatively remains challenging due to overlapping features. Serous cystadenomas are benign but can mimic other neoplasms, including pancreatic neuroendocrine tumours. We describe a case of a female in her 80s with an incidentally detected pancreatic tail mass and a history of resected melanoma. 18F-fluorodeoxyglucose positron emission tomography (PET) showed mild uptake, while chromogranin-A and urinary 5-hydroxyindoleacetic acid were slightly elevated. Magnetic resonance imaging (MRI) demonstrated a c

Cholangiocarcinoma with metachronous urothelial malignancy as a rare manifestation of Lynch syndrome with no gastrointestinal tumour.

Lynch syndrome, an autosomal dominant disorder caused by defective DNA mismatch repair genes, is linked to early-onset colorectal, ovarian, gastric, urinary tract and, less commonly, small bowel, biliary and pancreatic cancers. We present a case of a man in his late 50s who presented with obstructive jaundice due to a biliary hilar mass with a history of urothelial malignancy requiring surgical intervention. Family history revealed colon cancer in mother and brother both below the age of 50 years. He underwent curative resection and reconstruction. Histopathology confirmed hilar cholangiocarci

Metachronous renal cell carcinoma with delayed testicular metastasis coexisting with synchronous prostatic adenocarcinoma.

Renal cell carcinoma (RCC) is a notorious tumor, known for its unpredictable metastatic behavior; however, metastasis to the testis remains exceptionally rare. We report an unusual case of incidentally detected testicular metastasis from renal cell carcinoma in a patient who was undergoing treatment for coexistent prostatic adenocarcinoma. A 70-year-old man presented with lower urinary tract symptoms and hematuria and was diagnosed with high-grade prostatic adenocarcinoma, for which transurethral resection of the prostate and bilateral orchidectomy were performed. Histopathological examination

Predictive Value of Serum NLRP3 for Delayed Radiographic Progression After PFNA Fixation in Patients with Intertrochanteric Femoral Fractures.

OBJECTIVE: Multiple endocrine neoplasia type 2A (MEN 2A) and familial medullary thyroid carcinoma (FMTC) are genetic diseases due to activating mutations of the RET proto-oncogene. Affected patients develop medullary thyroid carcinoma (100%), in an isolated form (FMTC) or in association with phaeochromocytoma (30-50%), and primary hyperparathyroidism (10-20%) (MEN 2A). The presence of cutaneous lichen amyloidosis (CLA) has been anecdotally described in few families harbouring RET proto-oncogene mutation in codon 634. The aim of the study was to evaluate the incidence of CLA in MEN 2A/FMTC fami

Kidney Involvement in Sarcoidosis-Lymphoma Syndrome: A Report of Two Cases.

Sarcoidosis-lymphoma syndrome, first described by Henrik Brincker in 1986, is defined by the association of sarcoidosis with lymphoid neoplasms. This condition may present with sarcoidosis preceding, occurring concurrently with, or developing after the neoplasm. Its pathogenesis remains poorly understood. Kidney involvement by granulomatous interstitial nephritis is well-documented, including in cases of chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL), where granulomas typically occur in proximity to neoplastic infiltrates. However, no prior reports in the literature describe

Primary Enteric-type adenocarcinoma of the bladder managed with colon-type chemotherapy: A rare case and literature review.

Primary adenocarcinoma of the urinary bladder is the rarest form of urinary tract malignancy, accounting for less than 2% of bladder cancers in the United States. Adenocarcinomas of the bladder are extremely rare, with distinct histological features, and, due to their morphological similarity to colorectal adenocarcinoma, they pose a diagnostic challenge. We report the case of a 62-year-old male with a significant history of tobacco use who presented with gross hematuria and irritative voiding symptoms. Imaging revealed a mass involving the posterior and inferior bladder wall, and biopsy confi

The "perfect storm" in bladder oncology: a case of diverticular carcinoma with plasmacytoid features and its clinical ramifications.

BACKGROUND: Carcinoma arising within a bladder diverticulum is a rare and diagnostically challenging entity. Its management is further complicated by aggressive histological variants such as plasmacytoid urothelial carcinoma (PUC). The anatomical lack of muscularis propria in diverticula predisposes to early extra‑diverticular spread and impedes accurate clinical staging. The coexistence of a diverticular location and plasmacytoid histology creates a high‑risk clinical scenario with limited evidence to guide therapy. CASE PRESENTATION: A 74‑year‑old male with a history of high‑grade urothelia
