Radiation-Induced Epithelioid Angiosarcoma of the Urinary Bladder Mimicking Sarcomatoid Carcinoma: A Rare Entity and Literature Review.

Epithelioid angiosarcoma of the bladder is an exceptionally rare and aggressive vascular malignancy, characterized by a poor prognosis due to its high propensity for invasion and metastasis. Due to its rarity, the diagnosis is often challenging and may be misinterpreted as other high-grade malignancies. Patients commonly present with advanced-stage disease, including muscle invasion at the time of diagnosis, leading to unfavorable clinical outcomes. We report a 73-year-old male patient with a history of metastatic colon cancer (to the cervical lymph node) who presented with gross hematuria. Im

Eosinophilic Solid and Cystic Renal Cell Carcinoma (ESC RCC): Clinicopathologic Study of a Series of 4 Patients from Brazil With Report of Cell Cannibalism as a Novel Microscopic Finding.

We report the clinicopathological findings of the first series of 4 patients from Brazil with eosinophilic solid and cystic renal cell carcinoma (ESC RCC). In our study, there were 3 female patients and 1 male patient. The youngest female patient had bilateral tumors and a past history of tuberous sclerosis. The morphologic findings were in accordance with the literature. Macroscopically, the tumors were nonencapsulated disclosing solid, macrocystic and/or microcystic arrangement. In the largest tumor the arrangement was purely solid with frequent and extensive necrosis. The characteristic and

Intravascular large B-cell lymphoma presenting as encephalomyelitis diagnosed by renal biopsy: a case report.

BACKGROUND: The assessment of patients with multifocal lesions in the brain and spinal cord is complicated. A comprehensive neurologic workup requires extensive imaging evaluation, lumbar puncture, and sometimes a stereotactic biopsy to identify the final diagnosis. We report a case of intravascular large B-cell lymphoma (IVLBCL) manifesting as encephalomyelitis, which was diagnosed via percutaneous renal biopsy. IVLBCL should be considered as a differential diagnosis in cases with brain and spinal cord involvement. CASE PRESENTATION: A 43-year-old man with no significant medical history pres

Polydactyly and Risk of Cancer.

INTRODUCTION: Syndromic polydactyly is often associated with an increased cancer risk. However, limited data exist regarding nonsyndromic polydactyly and its associated risk of subsequent malignancy. Therefore, our purpose was to investigate differences in rates of both malignant and benign neoplasms among pediatric patients diagnosed with nonsyndromic polydactyly compared with matched controls. METHODS: The TriNetX US Collaborative Network database was queried using ICD-9 and ICD-10 codes to identify patients aged 10 and younger diagnosed with any form of polydactyly. Patients were then divi

Management of female para-urethral cyst with dyspareunia: a case report.

BACKGROUND: The diagnosis and management of female genital conditions (Rodriguez et al. in Clin Anat 34(1):103-107, 2020. https://doi.org/10.1002/ca.23654 ) are often challenging. The atypical presentations, combined with patient hesitancy to be subjected to an examination by a male urologist, are factors that limit a timely diagnosis. Para-urethral cysts (Pastor and Chmel in Int Urogynecol J 29(5):621-629, 2018. https://doi.org/10.1007/s00192-017-3527-9 ) are often incidentally detected by gynecologists during pelvic examination for other reasons. Radiotherapy-Related Bladder Fistula: A Hidden Cause of Recurrent Abdominal Symptoms.

BACKGROUND Non-traumatic bladder rupture, a rare yet potentially life-threatening condition, can stem from diverse factors such as malignancies, bladder inflammation, or bladder diverticulum rupture. Pelvic radiotherapy, in extremely rare instances, can lead to radiation cystitis and subsequent bladder fistula formation. Patients with such conditions often present with abdominal pain, hematuria, oliguria, and urinary ascites. The diagnosis of radiotherapy-related bladder fistula poses significant challenges, particularly in patients with protracted illnesses and atypical abdominal symptoms, as

Renal-Limited Thrombotic Microangiopathy Induced by Fruquintinib and Tislelizumab: A Case Report.

Thrombotic microangiopathy (TMA) is a rare but potentially severe condition induced by cancer treatments, including angiogenesis inhibitors and immune checkpoint inhibitors. This case report presents the first documented instance of renal-limited TMA potentially triggered by fruquintinib and tislelizumab in a patient with metastatic rectal cancer. A 60-year-old woman with stage IIIB rectal cancer developed nephrotic syndrome following treatment with fruquintinib and tislelizumab. She had no prior history of kidney disease but presented with lower limb oedema and proteinuria. Renal biopsy-confi

Pulsed TM-YAG laser (Thulio®): a new weapon in endourologists' hand in the conservative management of imperative cases of Upper Tract Urothelial Carcinoma (UTUC).

INTRODUCTION: Urothelial carcinomas (UC) represent the sixth most common tumor by incidence, involving the lower or upper urinary tracts (UTUC) (1). High-risk patients should be treated by nephroureterectomy with complete bladder cuff excision (2), conservative approach is reserved for low-risk UTUCs and/or imperative cases (3). MATERIALS AND METHODS: We present a 70-year-old male patient, smoker, with history of urothelial carcinoma. He underwent distal ureterectomy with ileal replacement in April 2019. Since then, he has developed several UTUC recurrences bilaterally and in the bladder, whi

Solitary recurrence of renal cell carcinoma in the ureter nearly two decades after radical nephrectomy.

We report a man in his late 60s who presents with gross haematuria. He had a history of radical nephrectomy for renal clear cell cancer almost two decades ago. The CT showed an enhancing lesion in the distal ureteric stump, possibly urothelial carcinoma (UC). Ureterectomy and bladder cuff excision were done. Surprisingly, the biopsy revealed intraluminal renal clear cell carcinoma (RCC). The postoperative course was uneventful. The patient is free from disease at a 2 year follow-up. It is crucial to consider recurrent RCC in such a clinical scenario. This case also highlights the importance of

Urologic Manifestations of Uterine Leiomyomata in Women with Spina Bifida.

OBJECTIVE: To examine our experience with presentation and management of uterine leiomyomata (fibroids) in women with spina bifida (SB). Advances in health care have dramatically improved the life expectancy for individuals living with SB, but they face unique health challenges surrounding reproductive health. Uterine leiomyomata have not previously been described in this population. We hypothesize that their course is likely complicated by SB-specific disabilities and prior surgeries. METHODS: We queried our institutional database for adults with SB and a diagnosis of uterine leiomyomata and

Do urinary bladder smooth muscle neoplasms show morphologic and immunophenotypic features of their uterine fumarate hydratase-deficient counterparts?

While the morphologic and immunophenotypic features of smooth muscle neoplasms of the uterus and skin have been well-described in relationship to fumarate hydratase (FH) deficiency (FHD), a potential association of urinary bladder smooth muscle tumors with FH tumor predisposition syndrome (FHTPS) has not been previously investigated. Given an index urinary bladder leiomyoma which showed some of the purported morphologic features seen in uterine FHD leiomyomas, we performed a multi-institutional search for bladder smooth muscle tumors to further evaluate a putative FHTPS association. Cases were

mRNA-Based Urine Test Performance in High and Very-High Risk Non-Muscle-Invasive Bladder Cancer Patients Undergoing Contextual Endoscopic Follow-up (VERNAL: Vesical Tumor Early Monitoring: mRNA-Based Follow-up).

INTRODUCTION AND OBJECTIVES: Non-muscle-invasive bladder cancer (NMIBC) patients need a strict follow-up with cystoscopy (UCS) and voided urinary cytology (vUC) due to high rate of recurrence and progression. To reduce invasiveness and costs, a new diagnostic biomarker, namely Xpert Monitor BCⓇ-detecting 5 mRNAs in voided urine-, has been proposed. We test Xpert Monitor BCⓇ ability to detect tumor recurrence at an early point during follow-up of high risk (HR) or very high risk (VHR) NMIBC patients, aiming at evaluating reliability of this test as a single procedure. MATERIALS AND METHODS: Be

Utility of restaging transurethral resection in patients with primary high-grade Ta bladder cancer.

PURPOSE: International guidelines present discrepancies concerning restaging transurethral resection (TURBT) for high-grade (HG) Ta bladder cancer. Recent studies with heterogeneous inclusion criteria have reported discordant results regarding its benefits. Our study aimed to assess the prognostic impact of restaging TURBT in patients with HG Ta that was fully resected during the initial TURBT. METHODS: This single-center retrospective study involved patients with HG Ta. Patients with a history of urothelial carcinoma ≥ HG Ta, macroscopically incomplete resection, or the absence of detrusor m

Does ureteral stent drainage prior to radical nephroureterectomy increase the risk of intravesical tumor recurrence?

PURPOSE: To investigate whether ureteral stent drainage (USD) increases the risk of intravesical recurrence in patients with upper tract urothelial carcinoma (UTUC) treated with radical nephroureterectomy (RNU). METHODS: This retrospective study was conducted involving 2,756 upper tract urothelial carcinoma (UTUC) patients treated with RNU between 2000 and 2022, of whom 77 had a history of USD. The primary endpoint was to assess the association between USD and post-RNU bladder recurrence-free survival (BRFS). A 1:3 propensity score-matching analysis was performed to compare patients with and

Increased 68 Ga-FAPI Uptake of Ureteric Fibroepithelial Polyp and Costal Cyst Mimicking Malignancy With Bone Metastasis.

A 70-year-old man presented with progressive dysuria. 18 F-FDG PET/CT showed no abnormal metabolic activity. 68 Ga-FAPI PET/CT revealed intense 68 Ga-FAPI in the lower left ureter and the left rib, mimicking malignancy with bone metastasis. However, postoperative pathological biopsy demonstrated the diagnosis of ureteric fibroepithelial polyp. In addition, the prior CT scan confirmed a bone cyst. Our case highlights the necessity of reviewing the patient's skeletal history when encountering a primary tumor with FAPI uptake and a concurrent FAPI-avid bone lesion.
