Spinal Cord Stimulation With Integrated Pain Psychology for Pediatric Refractory Neuropathic Pain: A Case Report.

We present a 14-year-old girl with a history of acute lymphoblastic leukemia who developed chronic pain from several overlapping conditions, including chemotherapy-related neuropathy, sacroiliitis, lumbar degenerative changes, and post-surgical complex regional pain syndrome (CRPS). Her pain was largely nonmalignant and remained difficult to control despite prior treatment measures. She underwent spinal cord stimulation (SCS) implantation with concurrent pain psychology support. After implantation, her reported pain scores improved from 10/10 to 5/10, opioid use remained minimal, and she demon

A new c.681dup RUNX1 variant in familial leukemia.

Constitutional RUNX1 gene variants are associated with Familial Platelet Disorder (FPD) and predispose to a variety of hematological malignancies, included Acute Myeloid Leukemia (AML) and, albeit less frequently, Acute Lymphoblastic Leukemia (ALL). In this study, we report on a proband with primary diagnosis of AML, followed by T-ALL after transplant, and a positive familial history for leukemia over three generations. A new heterozygous germline pathogenic RUNX1 (c.681dup, p.(Leu228ThrfsTer33)) variant was found in the proband and his affected mother.

Lymphomatous-type adult T-cell leukemia/lymphoma associated with HTLV-1 presenting as chronic respiratory illness in a young adult: a case report.

Adult T-cell leukemia/lymphoma (ATLL) is a rare, human T-lymphotropic virus type 1 (HTLV-1)-driven neoplasm that is often underrecognized in low-endemic regions. We describe a 23-year-old man with an eight-year history of chronic dry cough who later developed weight loss, night sweats, odynophagia, and dyspnea. CT revealed cervical/supraclavicular lymphadenopathy with innumerable pulmonary micronodules, interlobular septal thickening, and ground-glass opacities; abdominal imaging showed hepatosplenomegaly, ascites, and extensive retroperitoneal/mesenteric adenopathy. HTLV-1 ELISA and confirmat

Co-occurrence of acute leukemia and type 1 diabetes among children: a caseseries.

BACKGROUND: Leukemia is the most common malignancy in children, while type 1 diabetes mellitus (T1DM) is one of the most prevalent autoimmune diseases among children. The etiology of both conditions is still largely unexplained, yet emerging evidence suggests certain similarities in environmental exposures and genetic predispositions. In this case-series, we sought a new potential factor behind the co-occurrence of leukemia and T1DM by thoroughly reviewing the medical records of patients diagnosed with both diseases. METHODS: We conducted a retrospective case series at Tampere University Hosp

Anaphylactic transfusion reaction to group B platelets related to alpha-gal syndrome: A case report.

BACKGROUND: Alpha-gal syndrome (AGS), a distinct form of IgE-mediated hypersensitivity to the carbohydrate galactose-α-1,3-galactose (α-Gal), typically occurs after repeated tick bites and leads to allergic reactions after ingestion of mammalian meat. Patients with AGS may experience a broad spectrum of allergic reactions, from pruritus and urticaria to angioedema and anaphylaxis. Although classically associated with food-triggered reactions, emerging reports describe AGS-related anaphylaxis following exposure to group B blood products, including cases without accompanying gastrointestinal sym

T-cell large granular lymphocytic leukemia associated with palindromic rheumatism: A case report.

Large granular lymphocytic leukemia (LGLL) is often associated with autoimmune diseases, with rheumatoid arthritis diagnosed in approximately one-third of the patients. Nevertheless, other rheumatologic conditions should also be considered. We describe a male patient in his 50 s with a long-standing clinical history suggestive of palindromic rheumatism, who was diagnosed with LGLL leukemia during an evaluation for fever and neutropenia.

Adult Case of Stüve-Wiedemann Syndrome.

Stüve-Wiedemann Syndrome (SWS) is a rare autosomal recessive skeletal dysplasia caused by variations in the leukemia inhibitory factor receptor (LIFR) gene. Due to its high rate of mortality in early life, current literature is largely limited to children with SWS. We present a case report of a 35-year-old female with SWS, one of the few documented adults with SWS. She was diagnosed at age 34 after clinical exome sequencing identified homozygous pathogenic variants in LIFR at c.756dup (p.Lys253*), a known pathogenic variant associated with SWS. Her past medical history consists of feeding and

Risk of Cancer in Patients With Primary Sjögren's Disease and Their Relatives.

OBJECTIVES: Familial clustering and HLA haplotype association studies suggest that genetic factors disrupting the regulation of the immune system may predispose to risk of both neoplasms and autoimmune diseases, potentially leading to an increased frequency of cancer development in patients with primary Sjögren's disease (SjD), as well as their relatives. In this study, we aimed to assess the risk of cancer in patients with primary SjD and their close relatives. METHODS: Primary SjD patients who were actively followed-up in the rheumatology outpatient clinic at Gazi University Hospital and wh

Catheter-associated bacteremia caused by Asaia lannensis in an immunosuppressed patient: a case report.

BACKGROUND: Asaia spp. are an environmental Gram-negative bacterial species rarely implicated in human infections. Only a handful of cases have been reported, primarily involving catheter-associated bloodstream infections or infections related to intravenous drug abuse. We present here a case of bacteremia caused by Asaia lannensis in an immunocompromised patient with hairy cell leukemia. CASE PRESENTATION: An 84-year-old man with a history of hairy cell leukemia presented with relapse and was treated with cladribine via a subclavian central venous catheter. The patient developed a febrile epi

Epidemiological Profile of Pediatric Patients with Acute Lymphoblastic Leukemia Admitted to Four Hospitals in Curitiba, Southern Brazil.

BACKGROUND/OBJECTIVES: Acute lymphoblastic leukemia (ALL) is the most common childhood malignancy. Understanding its epidemiological characteristics is essential for guiding public health strategies. In this study, we characterized the epidemiological profiles that may contribute to the risk of ALL in children in southern Brazil. METHODS: Clinical and epidemiological data from 71 children (1-15 years old) admitted and newly diagnosed with ALL at four hospitals in Curitiba, Paraná, Brazil, were retrieved and analyzed. RESULTS: Among the 71 children with ALL, the majority were male (n = 43, 60

Relapse of adult B-cell acute lymphoblastic leukemia in the bilateral lacrimal glands: A case report and literature review.

RATIONALE: Extramedullary relapse of acute lymphoblastic leukemia (ALL) involving the orbit is uncommon, and lacrimal gland infiltration is particularly rare in adults. Bilateral lacrimal gland enlargement can be clinically nonspecific and may be misinterpreted as inflammatory or autoimmune disease, potentially delaying diagnosis. This case report describes a rare adult case of extramedullary relapse of B-cell ALL presenting as bilateral lacrimal gland enlargement following allogeneic hematopoietic stem cell transplantation (HSCT). PATIENT CONCERNS: A 55-year-old woman presented with a 4-day

Hodgkin lymphoma mimicking the IgG4-related disease: A diagnostic pitfall.

This is a diagnostically challenging case that concerns a 29-year-old man with an eight-year history of right-sided neck swelling. The magnetic resonance imaging (MRI) of the neck revealed multiple well-encapsulated, lobulated, matted right cervical lymphadenopathy. He reported significant weight loss, but denied shortness of breath, altered bowel habits, or bone pain. His initial investigations revealed a normocytic hypochromic anaemia, mild leukocytosis, and thrombocytosis. The initial histological findings showed many IgG4-positive plasma cells, but lacked stromal fibrosis, granuloma, oblit

Genomic profiling of Mexican patients with B-cell precursor acute lymphoblastic leukemia reveals clinically significant somatic and potential germline variants.

B-cell precursor acute lymphoblastic leukemia (preB-ALL) is characterized by pathogenic variants currently used in precision oncology. However, the mutational landscape of Mexican children with preB-ALL has not yet been thoroughly explored and defined in terms of the clinical significance. We used a custom-designed next-generation sequencing exome panel, along with high-resolution chromosome microarrays and gene fusion-targeted assays, to characterize the mutational landscape of 73 Mexican children with preB-ALL, exploring the profile of clinically significant variants. We classified the varia

Case Report: Acute monocytic leukemia mimicking large-vessel vasculitis on 18F-FDG PET/CT.

BACKGROUND: Systemic inflammatory manifestations caused by hematologic malignancies may be very similar to autoimmune diseases, making diagnosis difficult. Acute monocytic leukemia (AML-M5) is characterized by significant cytokine production and, in rare cases, can mimic large vessel vasculitis (LVV). CASE PRESENTATION: A 56-year-old female with a two-week history of recurrent fever accompanied by significantly elevated inflammatory markers. Laboratory tests showed elevated levels of erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), and ferritin. The infection and autoimmune ass

Fusariosis in Patients With Hematologic Malignancies in the Era of Antifungal Prophylaxis.

BACKGROUND: Invasive fusariosis (IF) is an uncommon but frequently fatal infection among patients with hematologic malignancies (HM). Despite widespread mould-active prophylaxis, breakthrough infections and treatment failures may occur. We aim to describe the clinical epidemiology, diagnostic approaches, antifungal management strategies, and outcomes of IF in the era of routine antifungal prophylaxis. METHODS: We conducted a retrospective study of all cancer patients diagnosed with IF at a tertiary cancer center that met probable/proven criteria for IF from 2015 to 2024. We collected data on

Hematologic malignancies in pediatric patients with RUNX1-familial platelet disorder with associated myeloid malignancy.

Deleterious germ line RUNX1 variants cause familial platelet disorder with associated myeloid malignancy (FPDMM), characterized by thrombocytopenia, platelet dysfunction, and predisposition to hematologic malignancies (HM). We examined clinical and laboratory findings of pediatric (<19 years) patients with RUNX1-FPDMM enrolled in the National Institutes of Health RUNX1 Natural History Study. Pediatric patients have thrombocytopenia (131 ± 49 ×103/μL vs 314 ± 55 ×103/μL) and mild eosinophilia (372 ± 244/μL vs 165 ± 100/μL) similar to adults. CAA or not? When cerebral microbleeds are not enough.

Lobar cerebral microbleeds (CMBs) are significant indicators of cerebral amyloid angiopathy (CAA) and are increasingly recognized in both clinical and population studies. Although CMBs are common incidental findings, their progression in CAA patients underscores their diagnostic relevance. Recent criteria highlight additional markers of small vessel disease (SVD), such as white matter hyperintensities (WMHs) and enlarged perivascular spaces (EPVSs), yet the current diagnostic framework does not fully address the implications of multiple SVD markers. This case report discusses a 55-year-old man

Therapy-Associated Polyposis in Pediatric Cancer Survivors.

Childhood cancer survivors face lifelong risks requiring ongoing monitoring. We report three cases of survivors who subsequently developed colon polyps in their teens or twenties within 10 years of completing cancer treatment. They presented with gastrointestinal symptoms, leading to early colonoscopies revealing tubular adenomas and other polyps. None had a family history of colon cancer or positive germline testing. Two had a history of astrocytoma, and one a history of relapsed T-cell acute lymphoblastic leukemia; all had high cyclophosphamide equivalent doses (CED) of chemotherapy, and som
