Diagnostic yield and safety of probe-based confocal laser endomicroscopy-guided pleural biopsy via semirigid thoracoscope in diagnosing patients with unknown pleural effusion: a protocol for a multicentre, randomised controlled trial (COLLABORATION-I).

INTRODUCTION: Semirigid thoracoscopy plays an important role in the diagnosis of pleural diseases. However, its diagnostic performance remains unsatisfactory particularly in terms of the negative likelihood ratio. Therefore, more effective supplementary diagnostic tools are required. Probe-based confocal laser endomicroscopy (pCLE), which allows live tissue imaging at the cellular level, can discriminate between malignant and benign pleura during medical thoracoscopy. However, the clinical relevance of pCLE in pleural disease remains unclear. This protocol describes a randomised controlled tri

Subacute normotensive cardiac tamponade as the initial presentation of metastatic lung adenocarcinoma: a case report.

BACKGROUND: Malignant pericardial effusion as the initial presentation of lung cancer is rare, and its diagnosis can be challenging, particularly when clinical features mimic common infectious or cardiogenic conditions. This case highlights the value of echocardiography and pericardial fluid cytology in identifying occult malignancy in patients with persistent cardiorespiratory symptoms and inconclusive initial workup. It also draws attention to the atypical occurrence of lung adenocarcinoma in a female patient, in whom breast cancer is more commonly associated with malignant pericardial effus

Case Report: Pembrolizumab unmasks a capillary leak syndrome with chylothorax in early TNBC.

Pembrolizumab has become standard of care in early-stage triple-negative breast cancer (TNBC), but its expanding use may induce rare adverse events involving vascular and lymphatic endothelium. Capillary leak syndrome (CLS) remains an exceptionally rare toxicity of PD-1 blockade, scarcely reported in breast cancer, especially in the adjuvant setting. We report the case of a 40-year-old female with early-stage TNBC who presented with progressive dyspnea, diffuse subcutaneous edema and bilateral pleural and peritoneal effusions after two cycles of adjuvant pembrolizumab. Imaging confirmed polyse

Docetaxel-Induced Serositis Mimicking Malignant Pleural Effusion.

Pleural effusions are common in patients with cancer. Although their development may signal progressive malignant disease, drug-induced effusions are a rare and often overlooked cause of exudative pleural effusions. We describe a 50-year-old woman with recurrent granulosa cell tumor of the ovary who developed bilateral pleural and pericardial effusions after 10 cycles of docetaxel and bevacizumab. Imaging showed stable pelvic disease without thoracic metastases. Pleural fluid analysis revealed exudates with negative cytology and cultures. Pleuroscopy demonstrated normal pleura, and biopsies co

Protein phosphatase 2 phosphatase activator (PTPA) promotes oncogene-induced senescence and carboplatin response in human malignant pleural mesothelioma cells.

PURPOSE: Malignant pleural mesothelioma (MPM) is a rare cancer of the pleura, frequently related to asbestos exposure. It is characterized by a grim prognosis, as few therapeutic options are available. Typically, MPM exhibits high molecular heterogeneity, providing opportunities for exploiting therapeutic vulnerabilities in well-defined subgroups of patients. Nevertheless, these options have been insufficiently explored. METHODS & RESULTS: We found that inactivation of Protein Phosphatase 2A (PP2A) is a recurrent event in MPM, affecting the cancer cell phenotype and chemotherapy response. Spe

Extrapleural Saline Hydrodissection Before CT-Guided Lung Biopsy to Reduce Pneumothorax: A Prospective Comparative Study in Pleura-Adjacent Lesions.

BACKGROUND: To evaluate whether extrapleural saline hydrodissection (EPSH) performed before CT-guided percutaneous transthoracic needle biopsy (PTNB) reduces pneumothorax in pleura-adjacent lung lesions. METHODS: In this prospective study, 90 patients with pulmonary lesions ≤1.5 cm from the pleura underwent CT-guided PTNB and were assigned to standard PTNB (Group 1, n = 45) or PTNB with EPSH (Group 2, n = 45). Demographic, lesion-related, procedural variables and complications were compared. Multivariable logistic regression evaluated the effect of EPSH on pneumothorax. In Group 2, lesions we

Cerebral Air Embolism after Small-bore Chest Tube Insertion in a Patient with Hydropneumothorax: A Case Report.

INTRODUCTION/BACKGROUND: Cerebral air embolism (CAE) is a rare but potentially fatal complication, characterized by the entry of air bubbles into the arterial circulation, subsequently obstructing cerebral blood flow and causing ischemic injury. While CAE is a recognized risk in cardiovascular surgery, barotrauma, trauma of the chest or head, it remains an extremely rare complication of pleural procedures, which is even more exceptional when occurring during chest tube placement. To our knowledge, we report the first case of CAE induced by insertion of a small-bore (12 Fr) chest tube via a saf

Doege-Potter syndrome caused by a solitary fibrous tumor arising from the greater omentum: a case report.

Doege-Potter syndrome is a rare paraneoplastic syndrome characterized by non-islet cell tumor hypoglycemia (NICTH) caused by the excessive production of insulin-like growth factor-II (IGF-II), which occurs in less than 5% of solitary fibrous tumors (SFT). SFTs most commonly arise from the pleura, whereas primary omental origins are exceedingly rare. We report a case of Doege-Potter syndrome in an 83-year-old woman who presented with recurrent hypoglycemic episodes caused by a large primary omental SFT. Low serum insulin levels during hypoglycemia and normal adrenocortical hormone levels exclud

Diagnostic performance and safety of image-guided pleural biopsy and medical thoracoscopy for undiagnosed exudative pleural effusion: a systematic review and network meta-analysis.

INTRODUCTION: Undiagnosed exudative pleural effusions often require pleural biopsy for a definitive diagnosis. The optimal sampling method, particularly when medical thoracoscopy is compared with image-guided pleural biopsy, remains uncertain. METHODS: This systematic review and network meta-analysis included studies that reported on the diagnostic performance and safety of medical thoracoscopy and image-guided pleural biopsy approaches. The primary outcome of this study was diagnostic yield, and the secondary outcomes included other diagnostic parameters (e.g. sensitivity, negative likelihoo

Thoracic surgery for spontaneous pneumothorax in a long-term survivor of infantile Krabbe disease: a case report.

BACKGROUND: Krabbe disease is a rare autosomal recessive lysosomal disorder caused by a deficiency of galactocerebrosidase, leading to progressive demyelination and severe neurological impairment. The infantile-onset form has a particularly poor prognosis. Early hematopoietic stem cell transplantation can improve survival, but long-term survival into adolescence is exceedingly rare. Here, we report a case of an adolescent with infantile Krabbe disease who survived long-term after unrelated cord blood transplantation and underwent thoracic surgery for spontaneous pneumothorax. Analysis of pleural reaction following CT-guided hookwire localization for pulmonary nodules.

BACKGROUND: Computed tomography (CT)-guided hookwire localization for pulmonary nodules is widely applied in clinical practice; however, it may be complicated by pleural reaction (an acute syndrome of hemodynamic and respiratory instability), seriously affecting patient safety and prognosis. This study aimed to investigate the risk factors for pleural reaction following CT-guided hookwire localization, thereby providing a reference for the early identification of high-risk patients. METHODS: A retrospective case-control study was conducted of patients who underwent CT-guided hookwire localiza

Transudative Pleural Effusion as a Rare Manifestation of Disseminated Tuberculosis With Tubercular Myocarditis: A Case Report.

Tuberculosis (TB) remains a major public health burden, particularly in India, and is known for its wide spectrum of clinical presentations ranging from pulmonary to extrapulmonary disease. Pleural effusion is a well-recognised complication of TB and is characteristically exudative in nature. We report a rare case of a 27-year-old immunocompetent female patient who presented with a right-sided pleural effusion that was transudative by Light's criteria. Rather than excluding TB, clinical suspicion was maintained, leading to a thorough diagnostic workup. The patient was ultimately diagnosed with
