Rib-Origin Ewing Sarcoma With Cranial Nerve Involvement From Orbital Metastasis: A Rare Case Report.

Ewing sarcoma (ES) is an aggressive tumor of the bone in young adults, commonly originating from the pelvis and long bones. It is characterized by chromosomal translocation t(11;22) (q24;q12), resulting in the EWSR1-FLI1 fusion gene. It presents with metastasis commonly to the lungs and other soft tissues. While the metastasis to lungs is common, orbital involvement with cranial involvement is exceptionally rare. The case highlights the need for the early detection and aggressive treatment in order to reduce mortality in patients ES. A 23-year-old South-Asian male presented with a 2 month hist

[Successful Surgical Treatment for Traumatic Extrapleural Hematoma in an Elderly Patient].

An 82-year-old man receiving anticoagulants was referred to our hospital with dyspnea and back pain. One day earlier, he bruised his back in his house. On arrival, the hemoglobin value was 5.6 g/dl, and computed tomography( CT) showed bilateral pleural effusion along with left lower rib fractures. In the left hemithorax, irregular high-density lesion which was surrounded by a low-density stripe was observed. He was initially diagnosed with left traumatic hemothorax, and managed through inpatient care with bed rest and blood transfusion. Although anemia was improved, bilateral compression atele

[Adult pulmonary blastoma:a case report].

We reported a case of pulmonary blastoma treated with surgical resection, in which we analyzed the clinical features, pathological characteristics, diagnosis, differential diagnosis, treatment, and prognosis. The clinical features, histopathology, and immunohistochemical staining results were observed, along with perioperative adjuvant therapy. The patient, a 21-year-old female, was admitted with the chief complaint of "lung mass discovered during physical examination for over one year." PET-CT revealed a metabolically active soft tissue mass measuring 7.8 cm×8.4 cm in the right lower lobe, wi

[Management of primary spontaneous pneumothorax in France: An overview of practices in 2024].

INTRODUCTION: French guidelines on the management of primary spontaneous pneumothorax (PSP) were first communicated in 2022 and subsequently published in 2023. They emphatically promoted outpatient management. We aimed to assess current therapeutic management of PSP following the release of these guidelines. METHODS: A nationwide online survey was conducted to evaluate therapeutic management strategies for large PSP without signs of severity. A standardized questionnaire was sent via the French Society of Respiratory Diseases (SPLF) and the French Federation of Pulmonology (FFP). One response

The negative impact of pleural adhesion on pulmonary function: a retrospective study.

BACKGROUND: Pleural adhesion refers to fibrotic or proliferative changes resulting in lesions between the visceral and parietal pleura, which can be caused by various factors. It is common and often asymptomatic, but may lead to impaired pulmonary function, especially in cases of severe or extensive adhesion. Existing studies mainly focus on pleural thickening caused by conditions such as tuberculous pleuritis. However, research on mild or localized pleural adhesions and their impact on preoperative pulmonary function remains limited. Therefore, this study aims to evaluate the specific effects

Clinical utility and prognostic value of GATA3 in epithelioid malignant mesothelioma: a practical and cost-effective approach for resource-limited settings.

AIMS: Distinguishing epithelioid malignant mesothelioma (EMM) from poorly differentiated lung adenocarcinoma (PD-LUAD) remains challenging, particularly when 21.7% of PD-LUADs lack lineage-specific markers (thyroid transcription factor-1 (TTF-1)/Napsin A), creating a diagnostic blind spot. While GATA-binding protein 3 (GATA3) is established in sarcomatoid mesothelioma, its complementary diagnostic value and prognostic relevance in EMM are not well defined. METHODS: This retrospective study analysed 115 tissue specimens (55 EMMs; 60 PD-LUADs). Immunohistochemistry for GATA3, calretinin, Wilms'

Case Report: Evolution of a BRAF V600E-mutant papillary thyroid carcinoma from latissimus dorsi metastasis and squamous transformation to PD-L1 upregulation and effective immunotherapy.

This case report describes an unusual clinical course of a patient with papillary thyroid carcinoma (PTC). The patient sequentially developed metastases to the cervical lymph nodes, lungs, pleura, and brain. Following tumor invasion into the latissimus dorsi muscle, a biopsy of this metastatic lesion revealed a BRAF V600E-mutant squamous cell carcinoma (SCC) phenotype indicating a high-grade dedifferentiation from the primary PTC. This metastatic focus also demonstrated upregulation of programmed cell death ligand 1 (PD-L1) expression. Subsequently, the patient exhibited a robust sensitivity t

Home-Based Management of a Child With Persistent Pulmonary Air Leak Complicated by Pneumonia.

BACKGROUND: Pneumothorax is a condition where air leaks into the space between the visceral and parietal pleura. Community acquired pneumonia is a rare cause of pneumothorax in children. Pneumothorax can sometimes develop as a complication of pneumonia when infection erodes the distal airway into the pleural cavity causing air leak. If the air leak persists for more than 7 days, then it is termed a persistent air leak. CASE SUMMARY: A 6-year-old South Asian girl presented with right upper and middle lobe pneumonia with empyema, for which a chest tube drain was inserted, which progressed to a

Differential diagnosis of benign and malignant pulmonary nodules attached to the interlobar pleura.

BACKGROUND: Pulmonary nodules attached to the interlobar pleura (PNs-AIP) represent a distinct subset of pulmonary nodules (PNs), for which accurate differentiation between benign and malignant status is critical for guiding clinical decisions. Chest computed tomography (CT) has become an effective method for evaluating PNs. However, the correlation between specific CT characteristics of PNs-AIP-particularly their spatial relationship with the interlobar pleura (IP)-and their pathological nature remains inadequately understood. Therefore, this study aimed to investigate the clinical and CT fea

Stromal-only endometriosis catamenial pneumothorax: A diagnostic algorithm with potential pitfalls.

Catamenial pneumothorax (CP) is a rare form of recurrent spontaneous pneumothorax occurring synchronously with menstruation and represents the most common manifestation of thoracic endometriosis. CP predominantly involves the right hemithorax as a result of the transdiaphragmatic migration of endometrial tissue through congenital or acquired diaphragmatic defects, a phenomenon thought to be facilitated by the clockwise circulation of peritoneal fluid and by the absence of an effective anatomical barrier on the right side to limit transdiaphragmatic spread. Diagnosis may be particularly challen

First report of pleural mesothelioma treated with cytoreductive surgery and hyperthermic intrathoracic chemotherapy in Kazakhstan: a case report.

BACKGROUND: Malignant pleural mesothelioma (MPM) is a rare but aggressive neoplasm arising from the mesothelial lining of the pleura.Despite advancements in systemic chemotherapy and targeted therapies, the prognosis for MPM remains poor, with a median survival of 8-14 months. Standard treatment modalities include surgery in combination with radiotherapy and/or chemotherapy. Hyperthermic Intrathoracic Chemotherapy (HITOC) has emerged as a promising adjunct to surgery. CASE PRESENTATION: A 35-year-old Asian male was diagnosed with Stage II malignant pleural mesothelioma (T2N0M0). The patient u

Primary thoracic synovial sarcoma in a 45-year-old male: A case report and literature review.

Synovial sarcoma (SS), a rare soft tissue cancer, develops at various body sites, with a predominant occurrence in the limbs. This report presents a rare case of primary thoracic SS (PTSS) in a 45-year-old male patient, characterized by chronic cough and occasional shortness of breath. Imaging examination revealed a substantial tumor mass in the left thoracic cavity. Although preoperative biopsy misdiagnosed the tumor as a solitary fibrous tumor, postoperative molecular analysis revealed that the tumor was SS based on the characteristic t(X;18)(p11.2;q11.2) translocation. The precise origin of

Outcomes of radiologically diagnosed solitary fibrous tumours of the pleura.

INTRODUCTION: Solitary fibrous tumours of the pleura (SFTP) are rare neoplasms for which surgical resection is considered the gold standard treatment. Anecdotally, SFTPs are sometimes conservatively managed, but there are little data on this in published literature. In this study, we aimed to collect data on radiological features in SFTP which might be associated with a benign clinical course to assist in risk stratification, and therefore selection for a more conservative management pathway. METHODS: A service evaluation of outcomes of radiologically diagnosed SFTP was completed in 12 UK cen

Pure isolation, culture, and postinjury lineage tracing of mouse visceral pleural mesothelial cells.

Pleural mesothelial cells (PMCs) are a major component of the pleura. The involvement of visceral PMCs has been suggested in several pleural or subpleural lung diseases, thus highlighting the importance of animal experiments using these cells. However, in wild-type rodent lungs, no direct and pure isolation method for visceral PMCs has been established so far. Using reanalyzed single-cell RNA sequencing data, we identified that mesothelin (Msln), a specifically expressed gene in visceral PMCs, can be useful for live cell sorting. After collecting cells by scraping the visceral pleura, MSLN+EpC

A case of delayed recurrence of pleural mesothelioma with intradural invasion.

BACKGROUND: Pleural mesothelioma (PM) is a tumor of the pleural epithelium with a poor prognosis. Asbestos exposure is closely related to its occurrence. To date, there have been 13 reports of PM with intradural invasion. CASE DESCRIPTION: A 73-year-old male was admitted with rapid progression of back pain and paraparesis. At age 67, he was diagnosed with PM (epithelial type) and underwent chemotherapy and a left thoracotomy for tumor resection. At present, the magnetic resonance newly showed T7-T9 spinal cord compressed due to an intradural tumor. Originating from the left pleura and invadin

Unraveling the multidimensional pleural ecosystem of lung cancer-associated malignant pleural effusion.

Lung cancer–associated malignant pleural effusion (LC-MPE) is a defining manifestation of advanced disease and a clinically consequential pleural compartment associated with dyspnea, repeated pleural interventions, and poor outcomes. This review synthesizes current evidence across mechanisms, diagnostics, and therapeutics to reframe LC-MPE as an active pleural ecosystem rather than a passive fluid collection. We summarize how partially reversible CD8⁺ T-cell dysfunction, regulatory T-cell and macrophage programs, mesothelial/fibro-inflammatory remodeling, vascular leak with impaired drainage,

Upregulated CCN1 from pleural mesothelial cells alters collagen I conformation and drives fibrosis via ITGB1/MAPK signaling.

BACKGROUND: Pleural mesothelial cells (PMCs) have been identified as key contributors to pleural fibrosis. Cellular communication network factor 1 (CCN1), a matricellular protein, regulates cell–matrix interactions and fibrotic signaling in multiple contexts. However, the specific role and mechanism of CCN1 in PMCs during pleural fibrosis remained unclear. METHODS: Expression of CCN1 was evaluated in human pleural fibrosis samples and bleomycin-induced mouse models. In vitro, PMCs were treated with recombinant CCN1 or fibrotic matrix. Mechanistic studies included gene knockdown, immunoprecipit
