Cyclosporiasis in Solid Organ and Hematopoietic Cell Transplant Recipients: Diagnosis, Treatment, and Prevention.

BACKGROUND: Cyclospora cayetanensis is a foodborne coccidian protozoan that causes prolonged, often relapsing watery diarrhea. Once considered largely a travel-associated infection, it has become a recurring cause of large, domestically acquired, produce-associated outbreaks in temperate countries. The 2026 US season was the largest on record and prompted a CDC Health Advisory, placing substantial numbers of solid organ transplant (SOT) and hematopoietic cell transplant (HCT) recipients at risk of exposure. AIM: To provide a practical, transplant-specific approach to the diagnosis, treatment,

Assessing the Effect of Malaria Exposure History on Tetanus Antibody Waning Rates Among Children in Jinja and Tororo Districts, Uganda.

BACKGROUND: Malaria causes immune perturbations and affects immunogenicity to vaccines. It is unknown, however, how repeated malaria infections affect the duration of vaccine protection. This study assessed variability in tetanus antitoxin immunoglobulin G (IgG) antibody kinetics including by malaria exposure history at district and individual levels. METHODS: We analyzed tetanus antitoxin IgG antibody concentrations in 3064 plasma samples from 2011 to 2017 in 597 children aged 6 months to 10 years residing in the Tororo (high malaria transmission) and Jinja (low transmission) districts in Ug

Clinical contribution of mNGS in the diagnosis and management of imported falciparum malaria in a malaria-free region: a five-case series from northern China.

BACKGROUND: Although malaria transmission has been eliminated in China, imported Plasmodium falciparum malaria remains a diagnostic challenge, particularly in non-endemic regions where clinical experience is limited. The nonspecific manifestations and potential for rapid progression to severe disease may result in delayed diagnosis and inappropriate management. Metagenomic next-generation sequencing (mNGS) has emerged as a complementary diagnostic approach for complex infectious diseases; however, its role in imported malaria management remains insufficiently characterized. METHODS: We retros

Mycobacterium leprae infection, Hansen's disease, and helminth infections: A cross-sectional study in southeastern Brazil.

BACKGROUND: Neglected tropical diseases are often co-endemic in affected areas but how they interact is less clear, especially in the case of Hansen's Disease (HD), commonly referred to as leprosy. Prior studies have demonstrated associations between helminths and HD and tuberculosis, but our understanding of the epidemiology of these infections is limited. Our goal was to assess the prevalence and potential associations of Mycobacterium leprae infection and parasitic infections in a sample of community members in a highly endemic area for HD in Minas Gerais, Brazil. METHODS: Adults and child

Microfilaremic loiasis is associated with T cell hyporesponsiveness against SARS-CoV-2.

BACKGROUND: Loiasis is a chronic filarial infection endemic to Central and West Africa. Although long considered benign, increasing evidence links loiasis to substantial morbidity and mortality. The infection is associated with immune modulation, including Th2-skewed responses and elevated regulatory cytokines. Clinically, loiasis is classified as microfilaremic (presence of circulating microfilariae) or amicrofilaremic ("occult") disease, the latter defined by a history of eyeworm migration without detectable microfilaremia. This study investigated how chronic L. loa infection influences anti

PCR-Confirmed Convexity Subarachnoid Neurocysticercosis with Negative Serology: A Case Report.

Extraparenchymal neurocysticercosis, caused by Taenia solium, is an uncommon form of neurocysticercosis involving the subarachnoid space and ventricular system. Clinical manifestations are heterogeneous, including seizures, meningitis, and intracranial hypertension, and are often associated with worse outcomes. Diagnosis remains challenging, as cysts may be difficult to detect on conventional imaging due to signal characteristics similar to cerebrospinal fluid, frequently leading to inconclusive results. We describe a patient presenting with a six-month history of focal seizures in whom the di

Illustrative case series of eosinophilia in UK Armed Forces personnel and recommended management for non-specialists.

Peripheral eosinophilia (>0.5×109/L) in the full blood count is caused by a variety of infectious and non-infectious aetiologies. Of particular importance to the UK military are parasitic infections, especially in individuals recruited from overseas and those who have deployed to areas that are highly endemic for schistosomiasis, strongyloidiasis and soil-transmitted helminths. These infections may persist for decades without causing symptoms. UK Armed Forces (UKAF) personnel have recently presented with more severe forms of strongyloidiasis, which can be fatal, especially following immunosupp

Spontaneous Clearance of Polymerase Chain Reaction-Positive Plasmodium vivax Parasitemia in Northwestern Thailand: An Epidemiological Perspective.

BACKGROUND: Asymptomatic, low-density Plasmodium vivax infections pose a significant challenge to malaria elimination because of their subclinical presentation and potential for ongoing transmission. However, the natural history of such untreated infections, including the extent to which they resolve spontaneously, remains poorly understood and has not been systematically studied for P. vivax. METHODS: We conducted a longitudinal cohort study from 2018 to 2023 in northwestern Thailand, incorporating 14 rounds of mass blood surveys (MBS) covering approximately 5000 individuals. Blood samples w

Prevalence of Strongyloides stercoralis in haemodialysis patients in Vienna.

BACKGROUND AND HYPOTHESIS: Strongyloides stercoralis is a neglected parasitic infection endemic to tropical regions, with rising prevalence in Europe. In kidney transplant (KTX) recipients, high-dose corticosteroids can induce a hyperinfection syndrome, which carries high mortality but is preventable with anthelminthic therapy. Screening for S. stercoralis is currently not included in pre-KTX protocols in Europe. This study assessed the prevalence of S. stercoralis among all haemodialysis patients in Vienna. METHODS: All consenting haemodialysis patients in Vienna (n = 704) underwent enzyme-l

Imported Malaria in Portugal: A Retrospective Analysis from a Tertiary Public Hospital.

Imported malaria remains a clinical and public health challenge in non-endemic countries. This retrospective study analyzed all adult malaria cases diagnosed at Hospitais da Universidade de Coimbra between 2020 and 2024, and the aim was to characterize the epidemiological profile, assess the impact of previous malaria history, and identify biomarkers associated with disease severity. A total of 88 patients were included, mostly male expatriates, with exposure primarily in Angola. Previous malaria history was reported in 52.3% of cases. Severe malaria was diagnosed in 25.0% of patients, being s

The Role of in Vivo Confocal Microscopy in Diagnosing and Managing Infectious Keratitis in Deployed French Military Personnel: A Case Series.

Fungal and amoebic keratitis are rare but serious infections that can lead to significant vision loss and are often difficult to diagnose promptly, particularly in resource-limited environments. Although conventional cultures and PCR have notable diagnostic limitations, in vivo confocal microscopy (IVCM) offers rapid and accurate pathogen detection. This retrospective case series examines four cases of infectious keratitis in deployed military personnel, highlighting the diagnostic and clinical utility of IVCM. All patients were young males, with a mean age of 29.75 years. Half had a history o

Serological screening for toxocariasis, strongyloidiasis and fascioliasis in individuals with eosinophilia in Khuzestan, Iran.

BACKGROUND: Eosinophilia is an important marker of helminthic infection. This study evaluated the seroprevalence of Toxocara spp., Strongyloides stercoralis, and Fasciola spp. among eosinophilic patients in Khuzestan, southwestern Iran. METHODS: From 2022 to 2024, 482 individuals (aged ≥5 y) with eosinophilia (>500 cells/µl) were recruited. Serum samples tested by ELISA for IgG antibodies against Toxocara spp., S. stercoralis, and Fasciola spp. RESULTS: Seroprevalence of S. stercoralis was 34.5% (166/482; 95% CI 30.338.9), whereas seropositivity for Toxocara spp. was low (0.8%; 4/482), and n

Quantifying the Role of Importation on Sustained Malaria Transmission in a Low-to-Moderate Burden Region of Southwest Uganda.

BACKGROUND: Parasite importation remains a challenge to malaria elimination. The extent to which human travel contributes to sustained transmission in low-to-moderate burden areas is poorly understood. METHODS: We conducted a 14-month longitudinal cohort study in Southwest Uganda. A total of 1918 individuals from 400 households were followed bimonthly. Travel histories and household characteristics were collected through surveys. Symptomatic and asymptomatic Plasmodium falciparum infections were captured through health facility surveillance and household reports (for symptomatic cases) and qP

Maggot infestation leading to Ignatzschineria larvae bacteremia and bladder outlet obstruction.

We report a case of a 61-year-old man with a history of alcoholic cirrhosis and impaired mobility who presented to the Emergency Department (ED) with hepatic encephalopathy, cellulitis, sepsis, and extensive maggot infestation (myiasis). Maggots were recovered from the patient's skin, urethra, and rectum, and the burden of maggots in the urethra had led to bladder outlet obstruction. Blood cultures revealed polymicrobial bacteremia, including Ignatzschineria larvae. Ignatzschineria larvae is a commensal bacterium that resides in the digestive tract of maggots. The organism is rarely pathogenic

Blastocystis hominis infection inducing gut microbiome dysbiosis and aggravating Parkinson's disease symptoms: a case report.

BACKGROUND: Blastocystis hominis infection in patients with Parkinson's disease may exacerbate gut microbiota dysbiosis, potentially worsening neurological symptoms; however, such associations remain speculative without microbiome data. Targeted interventions to restore gut microbial balance could mitigate disease progression and improve patient outcomes. CASE PRESENTATION: An 86-year-old Caucasian male presented to the emergency department with severe watery diarrhea, up to six episodes per day, of 1 month duration. The patient's diarrhea was nonbloody and nonmucoid. He denied fever, nausea,

Artesunate-pyronaridine-atovaquone-proguanil and artesunate-fosmidomycin-clindamycin compared with standard artesunate-pyronaridine for the treatment of uncomplicated malaria (MultiMal): a randomised, controlled, clinical, phase 2 trial in Gabon and Ghana.

BACKGROUND: The emergence of Plasmodium falciparum strains with reduced susceptibility to the artemisinin component of artemisinin combination therapies poses a serious threat to the treatment and control of malaria in sub-Saharan Africa. Regimens consisting of combinations of three or more conventional antimalarials have been proposed as a new treatment paradigm to overcome the impending problem of drug-resistant malaria. It was the aim of the MultiMal study to assess the safety, tolerability, and efficacy of the two novel multidrug antimalarial combination therapies, artesunate-pyronaridine-

Occurrence of autochthonous neurocysticercosis in Germany: a case report.

BACKGROUND: Neurocysticercosis [NCC] represents a severe manifestation of parasitic infection of the central nervous system [CNS] by larvae of Taenia (T.) solium, the pork tapeworm. Although it is one of the main causes of acquired epilepsy worldwide, endemic transmission in Europe has been largely eliminated, with only isolated autochthonous cases occurring. CASE PRESENTATION: A 43-year-old woman from a rural region in Saxony-Anhalt, Eastern Germany, with no history of travel abroad or contact with livestock or known tapeworm carriers presented with a first-time generalized epileptic seizure.
