Limiting dilution assay for human B cells based on their activation by mutant EL4 thymoma cells: total and antimalaria responder B cell frequencies.

The analysis of human B cell responses at the clonal level (limiting dilution assay) is still technically difficult. In the present study we report on a culture system that leads to activation, proliferation and differentiation into antibody-secreting cells (ASC) of about 90% of B cells from peripheral blood or spleen. In this system, B cells are cultured in the presence of a mutant subclone of the mouse thymoma EL4 for B cell activation and human T cell plus macrophage supernatant as source of proliferation and differentiation factors. ASC precursors generating clonal responses of IgM only, I

Clinical spectrum in 107 cases of toxoplasmic lymphadenopathy.

Lymphadenopathy is the most frequent clinical manifestation of acute acquired infection with Toxoplasma in the immunocompetent individual. One hundred seven cases of histologically verified toxoplasmic lymphadenitis were reviewed in an effort to determine the usual modes of clinical presentation and the incidence of extranodal disease. Toxoplasmic lymphadenitis most frequently involved a solitary lymph node in the head and neck regions, without systemic symptoms or extranodal disease and with a benign clinical course. However, serious extranodal disease did occur in some patients and included

Human toxocariasis. Review with report of a probable case.

A 23-year-old woman with fever, myalgias, stridor, pelvic cramping, lower abdominal pain, and profound eosinophilia was presumed to have toxocariasis. An enzyme-linked immunosorbent assay titer of 1:64 and an elevated IgE level supported the diagnosis. The patient became asymptomatic without treatment, and the eosinophil count returned to normal over several months. A stool sample from her dog was found to contain Toxocara canis ova, but the patient had no history of geophagia. Presumably, she acquired infection by hand-to-mouth transmission of infectious ova after handling the dog. Toxocarias

Malaria in pregnancy.

Malaria is a potentially lethal disease. In pregnancy its deleterious effects are aggravated by the depressed immune state. Every pregnant patient presenting with pyrexia of unknown origin should be screened for malarial parasites, especially if she has travelled to or come from an area in which malaria is endemic. Case histories of 2 pregnant women with malaria are presented and the diagnosis, treatment and chemoprophylaxis thereof are discussed.

Epidemiology of infection in pregnancy.

In this article the immunologic, clinical, and epidemiologic evidence for altered host susceptibility to infection during pregnancy is reviewed in an attempt to determine general principles that can be applied to interpret the wide range of information available and that can be utilized for epidemiologic analysis and study design. Gestational changes in immunity are related to the maternal history of infection during pregnancy. Primary infections are distinguished from recurrent infections, and the different patterns of recurrent infection in pregnancy are defined. This classification system i

Parasitic rheumatism presenting as oligoarthritis. A case report.

A 50-year-old Vietnamese boat refugee presented with an oligoarthritis which had been present for the two years since her arrival in The Netherlands. Her medical history was unremarkable but a peripheral eosinophilia of 20% prompted investigations leading to the isolation of Strongyloides stercoralis from the faeces. The clinical picture resolved completely after therapy with thiabendazole.

Infectious involvements in males with Reiter's syndrome.

To obtain data on infectious involvements contributing to joint attacks in Reiter's syndrome (RS) 72 consecutive males with RS were investigated. Controls were 219 personally interviewed males. A history of some type of oro-naso-pharyngeal or pulmonary infectious involvements were reported as often in the patients as in the controls, but the frequency of persons with a history of joint manifestations followed by these infections was significantly higher in the patients (23/72) than in the controls (8/219) (p less than 0.0001). Gastrointestinal involvements occurred only in a minority of cases

Infectious diseases of travelers and immigrants.

Many tropical and other unusual infections can occur in travelers or among foreign-born persons who have emigrated to the United States. The approach to the differential diagnosis includes considering the patient's geographic history, dates of travel, and clinical presentation, along with the geographic distribution and possible incubation periods of suspected pathogens. Important considerations include malaria, typhoid fever, rickettsial disease, dengue, brucellosis, tuberculosis, and leptospirosis. Traveler's diarrhea and dysentery are also common.

Isospora belli diarrheal infection in homosexual men.

Unsporulated Isospora belli oocysts were detected in the stool specimens of three homosexual men. The oocysts were ellipsoidal measuring 23-33 X 12-15 micron. It is acid fast with modified cold kinyoun stain (MCK) and reveals orange fluorescence with the Truant's stain. Sheather's sucrose flotation method is effective in concentrating Isospora oocyst as with other coccidial oocysts. One specimen showed many Charcot-Leyden crystals. All three patients responded to treatment. Isosporiosis is rare in this country and is endemic in the tropics and subtropics. Although one case acquired the infecti

Prevalence of Toxoplasma antibodies in pregnant African women in Tanzania.

Three hundred and fifty-seven serum samples from pregnant African women were examined for Toxoplasma gondii antibodies with indirect haemagglutination test in Dar es Salaam. Out of these 222 women were normal, sixty-nine had anaemia and sixty-six were suffering from hypertension. Infection rate in normal pregnant women was 41.9%, in anaemic women 52.5%, and in those suffering from hypertension 66.7%. Highly significant relationship was observed between Toxoplasma infection and anaemia, and hypertension. Infection rate was significantly high in women who had histories of abortion. The results s

[Uveitis: a rational diagnostic program for practice in internal medicine].

Uveitis, as an inflammation of the uvea, the vitreous body or the choroid, should not be regarded as a disease but only as a sign of an underlying multisystem disorder. The cause of iritis cannot be established by examining the eye only. Differential diagnosis should take into consideration geographic, possible environmental and genetic background factors. Furthermore, it should be recognized that some causes are infectious, some allergic and some autoimmune in nature. Many, even sophisticated tests are available to detect and underlying disorder. To establish an expedient program for investig

Screening for toxoplasmosis in pregnancy.

The prevalence of antibody against Toxoplasma gondi in a population of 715 pregnant women has been evaluated by two methods: indirect haemagglutination antibody (IHA) and indirect fluorescent antibody (IFA) test and all positive sera were checked by the dye test. Five hundred of the study population were questioned on diet and on animal contact to elucidate a possible relation to the prevalence of antibody. Results are expressed in international units (IU) of antibody against T gondi. Of the 715 sera, 171 were positive by IHA and 173 by IFA. One hundred and sixty-seven sera were positive by bo

Immunoglobulin M-immunosorbent agglutination assay for diagnosis of infectious diseases: diagnosis of acute congenital and acquired Toxoplasma infections.

An immunoglobulin M (IgM)-immunosorbent agglutination assay (IgM-IS-AGA) was negative in all sera from individuals negative in the Sabin-Feldman dye test, in sera from individuals with chronic Toxoplasma infection, and in cord blood samples from uninfected infants. In contrast, all sera that were obtained from individuals with a recent history of acute Toxoplasma infection and from infants with congenital Toxoplasma infection and that were positive in both the dye test and the IgM-indirect fluorescent-antibody (IgM-IFA) test were positive in IgM-ISAGA. A total of 21 (67.7%) of 31 sera that wer

Causes of death in a subarctic population.

Some of the main causes of death prior to 1900 are mentioned, including disastrous epidemics of high mortality such as plague, smallpox and the so-called hunger epidemics. Also discussed are two chronic diseases remarkable for Iceland in old times i.e., leprosy and hydatid disease. In the first third of the 20th century, infectious diseases still were the main cause of death in Iceland. The importance of tuberculosis in this connection is stressed. The very high infant mortality up to the beginning of this century is stressed. The changes in the main causes of death in the last decades are des

Selective primary health care: an interim strategy for disease control in developing countries.

Priorities among the infectious diseases affecting the three billion people in the less developed world have been based on prevalence, morbidity, mortality and feasibility of control. With these priorities in mind a program of selective primary health care is compared with other approaches and suggested as the most cost-effective form of medical intervention in the least developed countries. A flexible program delivered by either fixed or mobile units might include measles and diphtheria-pertussis-tetanus vaccination, treatment for febrile malaria and oral rehydration for diarrhea in children,

[Seasonal and periodic rhythms of infectious diseases (author's transl)].

The causes of epidemics are plainly not the pathogens alone as was initially assumed by Koch's school, predisposition and constitution of the population proved to be equally important. Ever since ancient times problems linked with the "constitutio epidemica" have been topical; the "physis", the "natura hominis" and the invironment of man play an increasingly important role in the symptomatology of disease, as can be gathered from such early documents as the "Corpus hippocraticum". Fracastoro distinguished between contagious and non-contagious epidemics. The casual organisms were considered to

The ocular manifestations of congenital infections produced by toxoplasma and cytomegalovirus.

Chorioretinitis is the most common ocular manifestation of congenital toxoplasmosis occurring in both symptomatic and the more freqeunt subclinical form of the disease. Chorioretinitis also occurs in symptomatic congenital cytomegalovirus (CMV) but not in the subclinical form of this congenital congenital infection. Diagnosis cannot be made from ocular signs alone. Knowledge of the epidemiology of both congenital infections along with information about the postnatal history will assist in the differentiation.
