Running amok.

This study examines twenty-four cases of amok, believed the largest number of cases ever collected. They were observed in Sarawak, East Malaysia. They occurred in all indigenous groups in Sarawak, excluding the Chinese, such as Malay, Sea Dayak, Land Dayak, Kayan, Punan and Melanau at frequencies more or less following the proportion of these groups in the total population. No differences were found according to religion, the Malay being Muslim and the other groups either predominantly Christian like the Iban or animistic. Only slight diminution in the frequency was observed from 1954 to 1968.

Diagnosis and treatment of physical disease in the mentally ill.

The inability of patients to provide a history and to cooperate in physical or technological examinations makes the diagnostic and therapeutic management of physical disease in psychotic and especially in elderly psychotic patients very difficult. There needs to be a disproportionate dependence upon laboratory and radiologic information in diagnosis of physical disease in such patients. Observations by personnel on the psychiatric wards are essential to identification of probable physical illness. An experience with 1800 patients on a medical service now approved for Medicare and Medicaid in a

[The concept of madness in Talmudic literature and its commentaries].

In spite of their spatial and temporal dispersion, the talmudic literature and its commentaries proceed from a fundamental unity. On one hand, the definition of madness are, a priori, extremely rigid; originally, they were exemplaries, quoted to resolve practical problems. On the other hand, the handling of these definitions requires a very important suppleness in the jurisprudence of legal implications of madness. Obviously, the dialectics of the rabbis show the care to respect the personal life of the individual and the spiritual life of the community. And if these conceptions have still an

'The normal' as a culture-related concept: historical considerations.

Three definitions of 'normal' are recognized: the normative or ideal, the average or statistical, and the medical or freedom from illness. We still retain from our historical past the notion that mental or emotional illness bespeaks, if not possession by spirits, at least an irreversible condition. A person who has once sought medical treatment for mental sickness is forever after viewed as suspect, as one who may relapse at any time. The modern norm of reason, as against unreason, reinforces distaste for mental illness. The withdrawal of the Eagleton candidacy in 1972 attests that our society

[Significance of cranial hyperostoses for the neurologic-psychiatric practice].

The present paper attempts to comment on the relation between diffuse concentric hyperostoses of the cranial vault and neurologic and psychiatric symptoms in a male aged 42 and a woman aged 23 when death occurred. In the past, similar discussions mainly concerned the hyperostosis frontalis interna (Hfi), although the demonstration of a true relation is difficult in the Hfi because of its late occurrence at an advanced age (60-70 years), in which mental disturbances due to other pathological circumstances are frequent as well. The diffuse concentric hyperostosis in young individuals has compara

Grief-related facsimile illness.

One aspect of the grief process which is frequently mentioned in the literature on mourning but has not yet received adequate clinical attention is the tendency for the bereaved to assume symptoms and behavior of the deceased. This paper presents evidence that pathologic identificantion 1) is an important clinical syndrome that is frequently misdiagnosed and mistreated, 2) defines a population at high medical and surgical risk, 3) can be diagnosed by history alone, and 4) carries a good prognosis when identified and properly managed. Four of a series of ten clinical cases are presented and dis

Coping with the aggressive patient: an alternative to punishment.

Extremely aggressive patients in mental institutions constitute a threat to staff and other patients that usually necessitates the use of physical restraints. With chronically aggressive patients this cycle of behaviors is defeating for both the staff and the patient. In this case history the authors describe how positive and negative reinforcement and shaping were used to alter the previously unmanageable aggression of a 36-year-old male psychiatric patient.

Patient work under Fair Labor Standards: the issue in perspective.

The 1973 federal court ruling in Souder v. Brennan required that patient workers in institutions for the mentally ill and mentally retarded be paid in accordance with the minimum wage and other provisions of the Fair Labor Standards Act. The author believes that ruling, which ran contrary to widely accepted institutional practices, is more readily understandable when viewed in the broader context of wage-and-hour legislation. He presents a brief history of the legislation, discusses the provisions relating to the employment of handicapped workers included in the 1966 amendments, and emphasizes

Premenstrual affective syndrome and psychiatric disorder.

The relationship between premenstrual affective syndrome and psychiatric disorder was investigated, using 81 women presenting to a Neurology Clinic with functional headache. Premenstrual affective syndrome was significantly associated with a history of depressive syndrome in the population studied. Patients judged to have a non-affective psychiatric disorder reported no greater frequency of definite or probable premenstrual affective syndrome than patients considered psychiatrically normal. The premenstrual occurrence or exacerbation of affective symptoms has been noted. This symptom exacerbat

Psychiatric problems in systemic lupus erythematosus.

Four patients with systemic lupus erythematosus (SLE) are described in whom there were major psychiatric complications. Two of these patients had cerebral lupus with psychiatric manifestations of the disease together with other features of disease activity and responding to treatment with high dose steroids. The first of these had had a ten-year history of recurrent episodes of depression before other features of the disease became evident; in the second patient recurrent psychotic episodes occurred after the onset of typical multi-system disease. The third patient had had a minor cerebro-vasc

The management and mismanagement of a suicidal patient.

The author begins with the premise that the actions of therapists and hospital staff can contribute to the likelihood that a suicidal patient will eventually take his life. He recounts a case history of a depressed 51-year-old man who was hospitalized following a suicide attempt and delineates eight "mistakes" that contributed to the patient's eventual suicide and that are common occurrences in dealing with a suicidal patient. He believes that if more therapists and mental health professionals share mistakes and experiences of this kind with each other, such errors might be minimized or even p

A follow-up study of Sydenham's chorea.

Twenty-five patients convalescing from Sydenham's chorea were contrasted by clinical examination, electroencephalograms, and psychometric and psychologic tests to 15 siblings and 20 matched rheumatic fever controls. A group of 10 postchoreic patients who had two or more signs could be identified. Patients in this group had all the signs classified as moderate or severe, performed less well than other choreic subjects on the Bender gestalt test, and had a higher percentage of abnormal electroencephalograms but not a higher incidence of behavioral disorders. This subgroup could not be predicted

An analysis of homicide by young persons in England and Wales.

An analysis of 11 children (one girl, 10 boys) convicted of homicide indicates a maternal over-dominant relationship in eight of the males studied. The murderers were found to have more cooperative personalities than other children found guilty of non-capital offences and showed both normal intelligence and personality factors. Despite a history of "blackouts" in several cases, all were found on investigation to be free from both major and minor epilepsy. Difficulty was experienced in determining the presence or absence of parental alcoholism, promiscuity, and criminal convictions. All of the

Mental retardation in a national population of young men in the Netherlands. II. Prevalence of mild mental retardation.

The prevalence of mild mental retardation in 19-year-old survivors of male births during 1944-1947 is derived from military records. The data are singular in that they are national, virtually complete for a total population of more than 400,000 men and include attributes not previously examined. They allow for the simultaneous use of three criteria: education (history of special schooling), psychometric (Raven intelligence test score), and clinical diagnosis (ICD (1948) 325.2, 325.3), which yielded rates per 1000 of 30,58 and 61, respectively. Rates by all three criteria varied in similar fash

[Agreement between traditional and computer diagnosis in psychiatric outpatients (author's transl)].

Psychiatric diagnosis based on personal interviews with 100 clinic outpatients was made and the data thus obtained were documented in Spitzer's and Endicott's CAPPS (Current and Past Psychopathology Scales). Two clinicians and the computer program DIAL - a modification of the DIAGNO II program of the same authors - diagnosed the same patients independently of each other basing their diagnoses solely on the entries in the CAPPS. The diagnostic agreement between the different diagnoses was evaluated and results compared with earlier findings by Schmid et al. These authors had, however, used CAPP

[Clinical aspects of pathogenesis of alcohol embryopathy (author's transl)].

68 cases of alcohol embryopathy are reported. The main symptoms are intrauterine and postnatal growth retardation (91%), microcephaly (87 per cent), psychomotor and mental retardation (84 per cent) and a typical craniofacial dysmorphism. Other malformations are frequently found such as cardiac defects (31 per cent), anomalies of joints (23 per cent) and genitalia (50 per cent). There is a marked variation in the intensity of the malformations. Taking into account the extent of the craniofacial dysmorphism and the cerebral damage, a classification into three types (I-111) of alcohol embryopathy

Diagnostic evaluation of the juvenile offender: toward the clarification of often overlooked psychopathology.

On the basis of previously reported findings that a high percentage of court-referred children suffered from serious psychopathology other than sociopathy, attention is called to some essential, though often overlooked, elements of diagnostic evaluation of delinquents. Emphasis is on a meticulous history, including careful attention to the significances of family histories of serious psychiatric and/or neurological disorders. The importance of a detailed perinatal and medical history, with emphasis on determining a possible history of significant insult to the central nervous system, is stress

[Mental and neurological disorders associated with heart operations. Pre- and postoperative studies].

Long before the era of heart surgery began, "cardiac psychoses" were known and described. They do not differ at all from postoperative psychoses after heart-surgery. On the other hand heart-surgery carries a far higher incidence of psychiatric complications than general surgery (general surgery 0,1--1,0%; heart surgery 10--60%). There is no reason to believe that the intensive care unit plays a decisive role in the origin of postoperative cardiac psychoses. Among 150 patients there were 60=40%, who did not show any psychiatric symptoms after surgery. These were exposed to the same environment

Familial cerebellar degeneration with slow eye-movements, mental deterioration and incidental nevus of ota (oculo-dermal melanocytosis).

Four members of a family with an autosomal dominant form of cerebellar degeneration all had slow eye-movements, i.e. slow pursuit with absence of both nystagmus and rapid saccadic movements. Three showed progressive mental deterioration. One patient had nevus of Ota (oculodermal melanocytosis) and a history of grand mal epilepsy. In these four patients the symptoms first occurred between the ages of 10 and 31 years--the onset apparently appearing earlier with successive generations. Current studies implicate a brain-stem lesion of the paramedian pontine reticular formation in the pathogenesis

Finding medical reasons for psychiatric behavior.

Medical disease was the cause of the primary disability in the majority (61.2 percent) of 116 elderly psychiatric patients treated at Bellevue Hospital Center. A change in behavior signaled the onset of medical illness with acute brain syndrome. The history that documented a change in behavior was derived from sources other than the patient. It is important to obtain such history during admission evaluation of elderly persons who are confused or whose memory is impaired. Multidiciplinary services enabled many patients to improve so that they could return home rather than being institutionalize
