The obstetric histories and early lives of 28 subsequently abused children are reviewed. The youngest child, more often male, was shown to be most 'at risk' especially of the mother was pregnant. Reduced antenatal care, separation of mother and child during the neonatal peroid, illness, poor features. It is felt that these and other difficulties may be regarded as 'environmental pressures' leading to child abuse by susceptible indivisuals, early recognition of such situations could lead to preventive and supportive measures.

One hundred and thirty-four deaths recorded as open verdicts in the Inner West London Coroner's District during 1969 and 1970 have been surveyed for evidence of mental illness. For 110 (82 per cent) of these deaths the probable verdicts were suicide or accident and they were reclassified as undetermined deaths. Forty-three patients, 38 women and 5 men, with self-inflicted skin lesions were studied. Thirty-three were followed up for up to 22 years. In most cases dermatitis artefacta was only one incident in a long history of psychogenic illness. Of the 43 patients, 13 (30%), 12 women and one man, continued to produce lesions or were disabled with other psychiatric disorders more than 12 years after the onset of symptoms. Prognosis was difficult but recovery seemed to occur when the patient's life circumstances changed rather than as a result of treatment.

One hundred heart surgery patients were followed throughout their postoperative periods to assess the incidence and etiology of postcardiotomy delirium. Electroencephalographic (EEG) studies of 60 patients with bipolar manic-depressive disease disclosed an incidence of small sharp spikes plus a few other variations in 47% of the sample. 698 children below age sixteen years with severe mental handicap (below IQ50) who were admitted to hospital between 1956 and 1959 were surveyed for genetic prognosis. A follow-up study of girls in a London remand home during the years 1966-8 showed that 20.6% of those taking non-narcotic drugs on admission, but only 1% of non-drug-taking control admissions, had used narcotics by June 1970. Narcotic use on admission and progression to narcotic use were associated with frequent drug taking, marked involvement in a drug milieu, and a high incidence of personal morbidity. Adolescents who use illicit drugs and have a history of court appearances for any reason are particularly vulnerable to subsequent narcotic usage and other forms of serious drug abuse.

The 425 cases of spina bifida cystica and cranium bifidum cysticum born in a population of 850,000 between 1956 and 1962 (with an incidence of 4·12 per 1000 births) were followed. Most did not receive the modern treatment for the condition.
