Light injury to the human eye has been correlated exclusively with wellknown thermal effects (e.g., solar maculopathy; photocoagulation). One may speculate whether clinical conditions exist in which the light impact from strong conventional light sources--otherwise innocuous--may become harmful to the retina. A case history is presented of a patient suffering from acute posterior multifocal placoid pigment epitheliopathy in which a possible relationship is discussed between the appearance of the fundus changes and the damage due to direct viewing of a halogen head light.

Tympanometric measurements are versatile and accurate when more than one type of ear disease is present. Three cases are presented where information from tympanometry beyond that derived from otological and routine audiological testing yielded accurate diagnoses. The pattern reversal evoked visual potential (VEP) was recorded in 37 normal subjects and in 186 patients in whom a diagnosis of multiple sclerosis (MS) was established or suspected. Taking the upper limit of normal as the mean +2.5 SD (111 ms), prolonged latency was found in 75% of definite cases of MS, 58% of probable cases, and 38% of possible cases. Chorioretinitis is the most common ocular manifestation of congenital toxoplasmosis occurring in both symptomatic and the more freqeunt subclinical form of the disease. Chorioretinitis also occurs in symptomatic congenital cytomegalovirus (CMV) but not in the subclinical form of this congenital congenital infection. Diagnosis cannot be made from ocular signs alone. Knowledge of the epidemiology of both congenital infections along with information about the postnatal history will assist in the differentiation.

Motion pictures of 15 patients with essential blepharospasm were studied. Previously unrecognized signs indicated multiple cranial nerve involvement. These signs include impersistence of gaze, lid retraction, tongue thrust, head tilts, head jerks, vertical gaze spasms, and asymmetry. This is a prospective in-depth study of patients with sudden idiopathic sensorineural hearing loss. We found that 65% recover completely to functional hearing levels spontaneously and independent of any type of medical treatment. The majority do so within 14 days and many within the first few days. Two cases of solar retinopathy following prolonged sun gazing are presented. Both patients were seen within an interval of 11 months, both with the diagnosis of schizophrenia paranoid type. These patients gave a history of sun gazing while praying to God, resulting in pigmentary disturbances of the macula, characterized by central and parafoveal depigmentation with perifoveal hyperpigmentation, as well as permanent impairment of vision in both eyes.

Extracranial carotid arterial obstructive disease has been the entity most commonly associated with transient cerebrovascular insufficiency. A nonobstructive, frequently overlooked cause of cerebral ischemia is cardiac dysrhythmia. We have explored this by observations of experimental animals and of man.
