Perilymphatic fistula and its interest to audiologists.

Forty-one patients who had sudden hearing loss associated with spontaneous perilymphatic fistllas were reviewed in this investigation. A history of physical exertion or an upper respiratory infection prior to the onset of hearing loss strongly suggests the presence of spontaneous perilymphatic fistula. However, absence of such a history does not necessarily preclude the possibility of membrane rupture. A great variation in audiologic and vestibular findings was observed. Tinnitus was present for most patients and vertigo or disequilibrium was reported by some. Findings support the need for sur

Degeneration patterns in human ears exposed to noise.

Various forms of sensorineural degeneration patterns related to noise exposure are illustrated in six pairs of temporal bones selected from a group of 33 male patients with histories of noise exposure. For the entire group the commonest form of lesion, associated with a 4-kHz dip in the audiogram, was a relatively diffuse degeneration in the second quadrant of the basal turn, in the 9-13 mm area. An advanced form of this lesion had a wide gap of more or less complete sensorineural degeneration affecting the entire second quadrant and displaying various degrees of extension toward the apex and

Horner'sy syndrome and its significance in the management of head and neck trauma.

The history, mechanism and aetiology of Horner'sy Syndrome is presented and the pharmacology of the pupil is discussed. The case reported is a rare combination of Horner's Syndrome in a patient who sustained bilateral fractures of the mandible and a chest injury. It is emphasised that the miotic changes in Horner's Syndrome, in combination with head injuries can lead to confusion in diagnosis and the potential anaesthetic hazards and their influence on the management of the facial injury are outlined.

[Round window membrane pathology in sudden deafness (author's transl)].

Exploration of the middle ear was performed on 15 patients with sudden deafness that had failed to improve after intravenous infusion of Eupaverin and Rheomacrodex. Three patients had abnormalities of the round window membrane, in 2 there was a perilymphatic fistula and in 1 an abnormal bulging of the membrane with no transmission of perilymphatic fluid movement. These 3 patients presented with a history of exertion or stress prior to the onset of symptoms. Surgical closure ofthe 2 fistulae led to improvement of hearing, and perilymphatic decompression in the third patient improved the tinnitu

Retinal detachment in a battered infant.

A 12-week-old battered infant boy had bilateral retinal detachments. The retina in the left eye was considered inoperable, but retinal detachment surgery was performed in the right eye. This retina was reattached and limited vision was restored. The history, the location of the retinal pathology, and the absence of a lens coloboma helped differentiate this result of physical abuse from a congenital retinal detachment.

A prospective study of cerebrovascular disease in Japanese rural communities, Akabane and Asahi. Part 1: evaluation of risk factors in the occurrence of cerebral hemorrhage and thrombosis.

An epidemiological study of cerebrovascular disease in Akabane and Asahi, Japan, was made. (These cities are located near Nagoy, Japan.) The study population included 4,737 men and women aged 40 to 79 at the time of entry into the study. There were 4,186 persons who were examined and, of these, 264 cases of cerebrovascular attacks were observed between 1964 and 1970. The incidence rate of stroke in those persons not responding to the survey was 15.9 times higher than in those persons examined according to person-year observation in Akabane. The risk factors for cerebral hemorrhage and thrombos

Puncture wound of the eyelid causing brain abscess.

A case of brain abscess following an apparently trival puncture wound to the eyelid is presented to emphasize the importance of meticulous examination of eye wounds in children to look for penetration into the cranial vault. Any suggestion of such penetrating injury either by history, examination, or X-ray should dictate neurosurgical consultation, and immediate local culture and debridement of the wound. If nonoperative management is elected, a careful watch must be kept for signs of infection, which dictate early exploration, and broad-spectrum antibiotic coverage of Gram-positive and Gram-n

Constitutional chromosomal breakage.

There were 18 individuals found to have a constitutional chromosome fragility causing an increase in break frequency. For each chromosome the breakpoint is always the same, whether it involves chromosomes from the same person, the same family, or different families. The fragile points are bands 10q24, 12q13, 16q21, 17p12, and Xq27. Autosomal constitutional fragility does not seem to have a phenotypic correspondence. They were found mostly in parents of children with chromosomal abnormalities or in couples with a history of repeated spontaneous abortions which permits one to raise the possibili

Pinna anomalies associated with congenital conductive hearing loss.

This paper compares the normal pinna with the abnormal congenitally malformed pinna. Then the abnormal pinna is related to case histories where a hearing loss was found due to a congenital malformation of the middle ear ossicles. Careful examination of the pinna, therefore, may be a simple diagnostic sign of an underlying potentially surgically correctable hearing loss. An abnormal pinna is more often associated with a middle ear anomaly than with any other congenital malformation including a urinary tract anomaly.

Vitreous opacities in primary amyloid disease. A clinical, histochemical, and ultrastructural report.

Three patients who initially presented to the ophthalmologist with a history of gradual deterioration of visual acuity showed, on biomicroscopic examination, a linear meshwork of opacities in the vitreous attached to the posterior surface of the lens in the form of pseudopodia lentis. Amyloidosis of the vitreous associated with systemic amyloid disease was diagnosed because of the characteristic nature of the opacities. This was confirmed initially in two of the cases by light and electron microscopical examination of a biopsy of the clinically normal conjunctiva, subsequently by systemic inve

General review of the literature makes it apparent that there exist various combinations of associated movements between cranial nerves. Most of them involve four of nerves, the oculomotor, the abducens, the facial, and the motor potion of the trigeminal nerve. We report two patients in whom left upper eyelids produced peculior up and down movements in accompany with mastication. The one was a 9-year-old female who had shown left eye-lid retraction since 3 months; the other, a 4-year-old male, had produced the synkinesis one year since. [Traumatic injury of ciliary ganglion].

The authors report a case in which slight cranial trauma was followed by manifestations of damage to the right ciliary ganglion. After 3 weeks the signs of damage began to regress and disappeared completely. Laboratory investigations failed to demonstrate any abnormalities. Apart from this trauma there were no other detectable causes, and especially there were no history data suggesting past infection, botulism or syphilis. The mechanism of ciliary ganglion injury is not known (contusion, haematoma, oedema etc.).

Long-standing anterior dislocation of the crystalline lens.

We believe this is the first reported case of an anteriorly dislocated lens that produced no pain or discomfort for at least 4 months, ie, March to July 1972. Even after July, the symptoms were not severe enough for this man to seek medical attention until September 1972. The lens may have been dislocated as early as age 5 since, according to his history, surgery was suggested at age 7 for the right eye. A pupillary block may have developed in March 1972, causing an intraocular pressure rise which eventually led to enucleation in September 1972. The microscopic evidence of fibrovascular adhesi

Positional nystagmus in association with macroglobulinemia.

Summary--Waldenstrom's macroglobulinemia is a malignant lymphoproliferative disease associated with a monoclonal macroglobulinemia. Most explanations of the cochlear and vestibular symptoms in this disease have dealt with vascular thrombotic or hemorrhagic phenomena. Five patients currently being treated for macroglobulinemia have been reviewed. Three patients gave a history of periodic dizziness and underwent extensive neuro-otologic examination. The results suggestsed an alternate mechanism for the pathophysiology of the vestibular disturbance in macroglobulinemia. Vestibular examination by

Otology in the Bible and Talmud.

The present paper is an English translation of the chapter of Otology in Preuss's classic book Biblisch-Talmudische Medizin. Described in the Talmud are a variety of anatomical ear abnormalities such as double ear, pierced ears, small ears, cut off ears, and pendulous ears. A slave whose ear was wounded had to be freed. Compensation for injury to the ear and to hearing is clearly noted. Remedies for earache are mentioned. The legal status of a deaf-mute, and the relationship of deafness to muteness are also discussed in the Talmud.

Solar retinopathy and foveomacular retinitis.

The visual disturbances caused by looking into the sun or a solar eclipse have been known for many years. Recently, there have been discussions about whether or not solar retinopathy is a distinct clinical entity or identical to the disease termed foveomacular retinitis. A series of 36 patients has been evaluated at Letterman Army Medical Center with the ophthalmoscopic appearance compatible with both conditions in various stages of the disease. By use of clinical history, funduscopy, visual fields, Amsler Grid, color acuity testing, color fundus photography, and fluorescein dye studies, it is
