Miliary Tuberculosis With Immune Thrombocytopenia in 50-Year-Old Ethiopian Woman: A Case Report and Brief Review of Literature.

Immune thrombocytopenia (ITP) is a rare but recognized hematologic complication of tuberculosis (TB), particularly miliary TB. We present a case of a 50-year-old female with severe thrombocytopenia and hemorrhagic manifestations secondary to miliary TB. The patient presented with mucocutaneous bleeding, anemia, and constitutional symptoms. Investigations revealed severe thrombocytopenia (11,000/μL), erythroid hyperplasia on bone marrow aspiration, and radiographic findings consistent with miliary TB, later confirmed by sputum GeneXpert. Management included short-course high-dose dexamethasone

Metagenomic next-generation sequencing unveils invasive aspergillosis masquerading as miliary tuberculosis in a neutropenic leukemia patient: a case report.

BACKGROUND: Empirical anti-tuberculosis therapy is a common strategy when patients with acute leukemia chemotherapy-induced neutropenia develop diffuse pulmonary small nodular and subsolid lesions. However, the absence of pathogenetic verification may lead to catastrophic consequences. METHODS: Following negative conventional microbiological cultures (bronchoalveolar lavage bacterial culture, Gram/Gram-negative test) and ineffective anti-infective therapy, a second bronchoscopy revealed caseous obstructive lesions in the right upper lobe bronchus. Metagenomic Next-Generation Sequencing (mNGS)

Landouzy sepsis in a young adult with multiorgan failure: successful ECMO-assisted management using an individualized antituberculous regimen.

BACKGROUND: Fulminant disseminated tuberculosis (TB) presenting as Landouzy sepsis is rare but carries high mortality. When complicated by acute respiratory distress syndrome (ARDS) and multiorgan failure, evidence for optimal management, including extracorporeal membrane oxygenation (ECMO) and modified antituberculous regimens, remains limited. CASE PRESENTATION: We describe a 23-year-old man with miliary tuberculosis who developed Landouzy sepsis, ARDS, hepatic dysfunction, and refractory hypoxemia requiring veno-venous (VV) and subsequent veno-veno-venous (VVV) ECMO for six weeks. Anorectal Tuberculosis Presenting With Multiple Abscesses and Fistulas in a Young Adult Male Patient With Alcohol Use Disorder: A Case Report.

Gastrointestinal tuberculosis (TB) is an uncommon form of extrapulmonary TB, with anorectal involvement representing an exceedingly rare subset. We report a fatal case of disseminated anorectal TB in a 28-year-old male immigrant from Kenya with a history of alcohol use disorder, a recognized risk factor for TB reactivation and spread. The patient initially presented with rectal pain and perirectal purulent discharge. On evaluation, he was found to have a left perirectal abscess, a right ischioanal abscess, and multiple fistulas. Acid-fast bacillus staining of debrided anorectal abscess fluid r

Successful treatment by high dose glucocorticoid of miliary tuberculosis complicated with acute respiratory distress syndrome: a case report.

A 25-year-old previously healthy man presented with a productive cough for 2 months and progressive dyspnea for 1 week. He did not respond to initial anti-infective treatment at a local hospital. Chest computed tomography (CT) demonstrated diffuse miliary nodules, ground-glass opacities throughout both lungs, and bilateral pleural effusion (more prominent on the right). Laboratory tests revealed type I respiratory failure, elevated inflammatory markers, hyponatremia, hypoproteinemia, and raised tumor markers (NSE, CYFRA21-1, SCC, CA125). Sputum acid-fast bacilli smears were negative, but pleur

Postoperative Diagnosis of Miliary Tuberculosis Masquerading as Acute Surgical Abdomen in an Immunocompromised Child: A Case Report.

Miliary tuberculosis (TB) is a severe, life-threatening form of disseminated disease. In the postoperative setting, particularly among immunocompromised patients, its diagnosis is frequently delayed because its non-specific clinical presentation can easily masquerade as routine surgical complications. We report the case of an eight-year-old female with juvenile idiopathic arthritis, managed with chronic immunosuppressive therapy (methotrexate and adalimumab), who presented with acute abdominal symptoms. Over a one-month period, she underwent three major surgical interventions for a suspected a

Fatal Tuberculous Meningitis With Hydrocephalus and SIADH (Syndrome of Inappropriate Antidiuretic Hormone Secretion) in an Immunocompetent Adult With Miliary Tuberculosis: Diagnostic Challenges Associated With Language Barriers.

Tuberculous meningitis is a severe manifestation of disseminated tuberculosis associated with high morbidity and mortality. I report the case of a 30-year-old previously healthy immunocompetent immigrant who initially presented with fever, malaise, and rhinorrhoea. A significant language barrier limited history-taking, contributing to diagnostic delay. His condition rapidly progressed to acute hydrocephalus and syndrome of inappropriate antidiuretic hormone secretion (SIADH) in the context of concomitant miliary pulmonary tuberculosis. Despite prompt initiation of anti-tubercular therapy, high

Lung ultrasound and the interstitial syndrome in tuberculosis endemic regions: illustrated insights from diverse clinical settings.

Lung ultrasound (LUS) is increasingly used as a diagnostic tool in respiratory medicine, particularly in resource-limited, tuberculosis (TB)-endemic settings. Sonographic interstitial syndrome (IS), characterized by B-line artefacts, has proven valuable in diagnosing cardiogenic pulmonary edema, pneumonia, and chronic interstitial lung disease. However, in TB-endemic areas, its interpretation remains challenging due to overlapping pathologies. This illustrated review explores the diagnostic and prognostic relevance of IS across three clinical contexts: acute respiratory distress, chronic inter

Reactivation of Cytomegalovirus in Colonic Tuberculosis in an Immunocompetent Patient.

Cytomegalovirus (CMV) reactivation in Mycobacterium tuberculosis (TB) infection is rare and has been only reported in immunocompromised patients. In this case report, we describe a patient with Type 2 diabetes with acute on chronic abdominal pain found to have miliary TB with terminal ileitis and ascending colon ulcers with biopsy-positive TB and CMV. The decision was made not to treat the local CMV reactivation, and he improved solely on standard drug-susceptible antituberculous therapy. This is the first reported case of gastrointestinal TB and CMV coinfection in a patient without known clas

Miliary Tuberculosis: A Comprehensive Review of Epidemiology, Clinical Manifestations, Diagnosis, Treatment, and Complications.

Miliary tuberculosis (TB) represents a potentially fatal form of disseminated TB resulting from the lymphohematogenous dissemination of Mycobacterium tuberculosis. Although accounting for approximately 1-2% of all TB cases in immunocompetent adults, its mortality rate exceeds 30%. This review screened peer-reviewed, original research studies via four databases (PubMed, EMBASE, MEDLINE, and CENTRAL) to synthesize up-to-date evidence. Key risk factors include human immunodeficiency virus (HIV) co-infection, anti-tumor necrosis factor-α (TNF-α) agents and chronic malnutrition. The clinical presen

Hepatosplenic involvement of tuberculosis - An uncommon presentation that demands attention in specific scenarios.

Abdominal tuberculosis (TB), a potential differential diagnosis for acute and chronic abdomen, should be considered in specific situations, such as immunosuppressant or biological drug use, HIV, or in patients residing in endemic areas. Although the presence of thoracic tuberculosis may indicate abdominal TB, only 15% of patients with abdominal TB have evidence of pulmonary disease. Involvement of the liver and spleen is a common autopsy finding in patients with disseminated TB. However, the most common pattern is of fine miliary lesions. Hepatic involvement with a macronodular pattern, a rare

Gallbladder and liver miliary nodules: a surprising diagnosis of disseminated tuberculosis during emergency laparoscopic cholecystectomy.

BACKGROUND: Disseminated tuberculosis (TB) presenting as miliary nodules on the gallbladder and liver is extremely rare and poses significant diagnostic challenges. This report describes a case of disseminated TB discovered during emergency laparoscopic cholecystectomy for acute cholecystitis. CASE PRESENTATION: A 77-year-old male presented with decreased appetite, weight loss, and fever. Preoperative imaging suggested gallstones and cholecystitis. During surgery, multiple miliary nodules were found on the surfaces of the gallbladder and liver, raising suspicion of metastatic cancer. Intraope

Beyond the Lungs: A Case Report of Disseminated Tuberculosis With Multisystem Involvement.

Tuberculosis (TB) is an infectious disease caused by acid-fast bacillus pertaining to the Mycobacterium tuberculosis complex. Pulmonary TB is the most common presentation, resulting either from primary infection or reactivation of latent disease. In rare cases, wide dissemination of M. tuberculosis can occur, usually by hematogenous or lymphatic route, leading to multiorgan involvement and potentially life-threatening conditions known as disseminated TB. We present the case of a 55-year-old man who presented to the emergency department (ED) with complaints of inflammatory polyarthralgia and my

A Perplexing Case Highlighting the Diagnostic Conundrum of Miliary Tuberculosis Mimicking Sarcoidosis and Progressing Into Hemophagocytic Lymphohistiocytosis.

Establishing the diagnosis of miliary tuberculosis (TB) can be challenging due to the heterogeneous clinical presentations and low sensitivity of diagnostic tests. Miliary TB shares overlapping clinical, radiological, and histopathological features with other chronic granulomatous diseases, such as sarcoidosis, often posing a significant diagnostic challenge for clinicians. A 36-year-old male from Haiti presented with a four-month history of recurrent fever, dry cough, night sweats, and weight loss. Chest imaging revealed innumerable widespread miliary nodules throughout the lungs bilaterally,

Disseminated Tuberculosis Driving Secondary Haemophagocytic Lymphohistiocytosis Following Adalimumab and Treatment for Latent Tuberculosis.

We describe the case of a 63-year-old man presenting with fevers, hyperferrintinaemia and pancytopaenia. He was known to have psoriatic arthritis, managed with adalimumab and methotrexate. Haemophagocytic lymphohistiocytosis (HLH) was diagnosed, and he was treated with intravenous anakinra whilst searching for an aetiology. Despite previous treatment for latent tuberculosis, he developed changes typical for miliary tuberculosis and was started on antituberculosis therapy; whole genome sequencing later demonstrated isoniazid monoresistance. This case demonstrates both the importance of recognis

Prevalence and short-term outcomes of acute complications of tuberculosis: a cross-sectional, monocentre, Italian study.

BACKGROUND: Tuberculosis (TB) is a major cause of morbidity and mortality worldwide. Acute complications may impact on disease progression but their prevalence is unclear and prognostic outcomes of complicated patients have been poorly studied. The aim of the study was to estimate the prevalence of acute complications of TB at the time of diagnosis. Short-term outcomes were also evaluated. METHODS: A cross-sectional study was carried out in Milan (Italy), from January 2018 to December 2023. RESULTS: 201 patients with TB were recruited. 88 complications were recorded in 65 (32.3%) patients. D

A Pathogen's Whisper: Reactivation of Quiescent Membranous Nephropathy by Disseminated Tuberculosis.

Primary membranous nephropathy (MN) is an autoimmune glomerular disease commonly associated with anti-PLA2R antibodies, with relapses typically attributed to spontaneous immune reactivation. We report the first documented case of a relapse of primary, PLA2R-positive MN that was temporally and immunologically linked to disseminated tuberculosis (TB) infection. A 42-year-old man, previously in complete remission, developed severe nephrotic syndrome and acute kidney injury unresponsive to standard immunosuppressive regimens. Concomitant diagnosis of miliary TB was confirmed by culture and imaging

Rare early hematogenous disseminated tuberculosis inducing hemophagocytic syndrome in conflict treatment.

This article presents a case of an acute onset in a middle-aged male who exhibited persistent high fever (temperature > 40 °C), delirium, and respiratory distress. Initial chest CT only suggested "bilateral pneumonia," and empirical anti-infection treatment proved ineffective. The patient subsequently developed pancytopenia, splenomegaly, and markedly elevated ferritin levels (848.90 μg/L). Bone marrow aspiration demonstrated hemophagocytic activity and granulomatous lesions. A positive TB-PCR, confirmed the diagnosis of early hematogenous disseminated tuberculosis complicated by the hemophago
