A Challenging Case of Miliary Tuberculosis in Pregnancy.

Miliary tuberculosis (TB) is an uncommon yet severe condition that can pose substantial risks to pregnant women and their unborn child. This case study describes a 22-year-old pregnant female at 23 weeks of gestation presented with breathlessness, fever accompanied by chills, dry cough, and loss of appetite. With no significant comorbidities, her initial symptoms were inconclusive, leading to her initial diagnosis as a case of pyrexia of unknown origin. Despite receiving iron supplements for anemia and broad-spectrum antibiotics, her health continued to decline, prompting her transfer to a spe

The prevalence of urogenital tuberculosis in people living with HIV.

The prevalence of urogenital tuberculosis (UGT) is not well known, especially in people living with HIV, therefore, our aim was to identify the prevalence of urogenital tuberculosis in HIV-infected individuals and characterize the clinical and laboratory risk factors associated with UGT. A cross sectional study was conducted to characterize the prevalence of UGT in people living with HIV. PCR tests and specific culture were performed on samples from 181 patients living with HIV, divided into two groups: A) patients with suggestive UGT symptoms and b) hospitalized patients without any suggestiv

Severe persistent mycobacteria antigen stimulation causes lymphopenia through impairing hematopoiesis.

Miliary tubersculosis (TB), an acute systemic blood disseminated tuberculosis mainly caused by Mycobacterium tuberculosis (M. tuberculosis), can cause signs of lymphopenia in clinical patients. To investigate whether/how persistent mycobacteria antigen stimulation impairs hematopoiesis and the therapeutic effect of interleukin-7 (IL-7), a mouse model of Mycobacterium Bovis Bacillus Calmette-Guérin (BCG) intravenous infection with/without an additional stimulation with M. tuberculosis multi-antigen cocktail containing ESAT6-CFP10 (EC) and Mtb10.4-HspX (MH) was established. Consistent with what

Miliary Tuberculosis with Acute Respiratory Distress Syndrome: A Deadly Combination.

A unique and deadly presentation of miliary tuberculosis is acute respiratory distress syndrome. In this case report, we present the case of a 22-year-old male patient who presented with a history of weight loss, appetite loss for eight months, and rapidly worsening dyspnea for 15 days, for which he was admitted to the intensive care unit. Chest X-ray and computed tomography (CT) of the thorax revealed bilateral miliary opacities. Routine blood tests revealed hyponatremia and leukocytosis. The patient was started on non-invasive ventilatory support, intravenous corticosteroids, and anti-tuberc

The role of early blood purification for miliary tuberculosis presenting as acute respiratory distress syndrome in pregnancy: A case report.

RATIONALE: Acute respiratory distress syndrome (ARDS) in miliary tuberculosis (TB) remains rare, especially in pregnant women. The role of blood purification is potential in managing ARDS due to miliary TB. PATIENT CONCERNS: A 36-year-old woman presenting with difficulty breathing 6 hours before admission. She never had any constitutional symptoms due to TB. DIAGNOSES: ARDS in TB was diagnosed based on the deterioration of PaO2/FiO2, increased acute phase reactants, positive gene-Xpert, and typical chest x-ray of miliary TB. INTERVENTIONS: A C-section was performed and followed by continuou

Cerebral venous thrombosis and miliary tuberculosis: a case report from Syria.

UNLABELLED: Cerebral venous thrombosis (CVT) is a relatively common disease. Risk factors for CVT include pregnancy, cancer, autoimmune diseases, and hypercoagulation. Acute and chronic meningitis are also considered predisposing conditions for CVT. Cases of CVT associated with tuberculous meningitis and miliary tuberculosis (TB) are rare in the medical literature, and here the authors report the first case report from Middle East. CASE PRESENTATION: The authors present a 33-year-old female patient diagnosed initially with CVT to discover the presence of tuberculous meningitis and miliary TB.

A Case Report and Literature Review of Intestinal Perforation Due to Tuberculosis.

Intestinal tuberculosis (ITB) is challenging due to its nonspecific clinical presentation, sometimes manifesting with acute complications such as intestinal perforation or obstruction. We present the case of a 39-year-old male presented with continuous fever, abdominal pain, and peritoneal irritation. A contrast-enhanced thoracoabdominopelvic computed tomography revealed free air and fluid, suggestive of intestinal perforation. Urgent surgical treatment was performed via exploratory laparotomy, including right hemicolectomy and construction of a Brooke ileostomy. Histopathological analysis con

A Very Unusual Presentation of Miliary Tuberculosis and Osteomyelitis as an Incidental Finding of Musculoskeletal Pain.

Tuberculosis (TB) is a highly infectious disease that takes the primary or latent route. In primary TB, the patient often presents with constitutional symptoms such as cough, fever, weight loss, and hemoptysis. This 83-year-old patient was sent to the emergency department (ED) by her primary care physician after abnormal imaging for acute back and neck pain. Imaging revealed pulmonary TB with possible tuberculosis osteomyelitis versus metastatic carcinoma to the cervical vertebrae. The case's unique presentation gives light to the need for more research on miliary TB and tuberculosis spondylit

Case report: Miliary tuberculosis complicated by pediatric acute respiratory distress syndrome in a 12-year-old girl.

Acute respiratory distress syndrome (ARDS) is a rare complication of miliary tuberculosis, particularly in pediatric patients. Comorbidities and delayed diagnosis can worsen the prognosis of patients with miliary tuberculosis. A 12-year-old girl presented with fever for 20 days, and cough and tachypnea for 4 days. She was diagnosed with miliary tuberculosis complicated by pediatric ARDS. She had atypical clinical manifestations and imaging findings, a negative contact history, and negative results of a tuberculin skin test (TST) and T-SPOT.TB. Diagnostic bronchoscopy and bronchoalveolar lavage

RETRACTED ARTICLE: Impact TB co-infections on immune tolerance among people living with HIV: a systematic review.

Источник пометил публикацию как отозванную

BACKGROUND: The high-burden regions of Sub-Saharan Africa, which accounted for greater than 70% of the HIV epidemic, are disproportionately affected by the high rates of TB coinfection. This might be explained by, the low immune tolerance of the population due to malnutrition and chronic infections aggravating immune suppression. In this review, we discuss the immunopathogenesis of this common co-infection that causes significant morbidity and mortality in people living with HIV globally. METHODS: We used published studies using a two-step search strategy. Initial search of Pub Med Central an

Acute lupus pneumonitis resembling miliary tuberculosis: A case-based review.

Systemic lupus erythematosus (SLE) is an autoimmune disease involving multiple systems and organs, with pulmonary involvement known to be associated with disease prognosis and mortality. Acute lupus pneumonitis (ALP) resembling miliary tuberculosis (TB) is rare. Here, we present a case of ALP mimicking miliary TB and review the literature. A 19-year-old male student was referred to our hospital with fever. Although the patient met the diagnostic criteria for SLE, diffuse miliary nodules in both lungs were observed on a chest computed tomography scan. A series of tests, including pathological t

Spectacular Improvement of Paradoxical Reaction in Tuberculosis after Tumor Necrosis Factor-Alpha Antagonist Therapy.

We report the case of a 42-year-old immunocompetent Indian patient presenting with miliary tuberculosis complicated by respiratory failure requiring intubation. Conventional quadritherapy was initiated for wild-type Mycobacterium tuberculosis. On day 29 of antibiotic treatment, persistent fever and neurological deterioration prompted the diagnosis of multiple brain and medullary tuberculomas, some surrounded by edema. Laboratory investigations ruled out meningitis and subtherapeutic drug concentrations. To enhance cerebrospinal fluid penetration, ethambutol was replaced with levofloxacin on da

Haemophagocytic lymphohistiocytosis in a patient with familial Mediterranean fever and miliary tuberculosis: a case report.

Haemophagocytic lymphohistiocytosis (HLH) is a lethal complication of several infections, especially viral origin. Mycobacterium tuberculosis infection can also lead to HLH, yet it is an uncommon trigger. Considering the role of increased cytokines in HLH, autoinflammatory conditions, such as familial Mediterranean fever (FMF), might contribute to its development. Nevertheless, the possible relationship between FMF and HLH has been suggested only in some case reports. We present a case of FMF who admitted to the hospital with consitutional symptoms and chest pain regarding to recurrent pericar

A Case of E-cigarette or Vaping Product Use-Associated Lung Injury Mimicking Miliary Tuberculosis.

E-cigarette usage or vaping is becoming more popular as an alternative option to cigarette smoking. Vaping is associated with a wide degree of pulmonary injuries such as asthma, chronic obstructive pulmonary disease or E-cigarette or vaping product use-associated lung injury (EVALI). E-cigarette or vaping product use-associated lung injury is an acute or subacute respiratory illness that can be severe and life-threatening. Miliary tuberculosis, on the other hand, is a rare form of tuberculosis that results from hematogenous dissemination of Mycobacterium tuberculosis, affecting multiple organs

Clinical Presentation of a Patient With Tuberculous Myocarditis: Case Report and Review of Literature.

Tuberculous myocarditis has a high mortality rate and is often associated with a delay in the diagnosis because of the low index of suspicion and insidious course. Most of the reported cases predominantly occur in young, immunocompetent patients. Delays in diagnosis may result in fatal complications. Through this case report, we aim to shed light on some of the clinical features of tuberculous myocarditis and promote a high index of suspicion for early diagnosis and timely management.
