Institutional Variation in Fentanyl Use for Children on Extracorporeal Membrane Oxygenation.

OBJECTIVE: Fentanyl is an opioid analgesic commonly used for pain management in children who are critically ill. However, fentanyl use is discouraged during extracorporeal membrane oxygenation (ECMO) because of its lipophilic and protein-binding properties. These properties have been shown to increase the risk of opioid withdrawal and other poor health outcomes. Our objectives were to evaluate institutional variation and the factors associated with fentanyl use for children on ECMO. PATIENTS AND METHODS: This retrospective study included children aged less than 18 years who received ECMO betw

Risk Factors for Plaque Vulnerability in a Community Population: Endothelial Function and Arterial Stiffness Indexes.

BACKGROUND: Rupture of atherosclerotic plaques and subsequent acute cardiovascular complications remain a major cause of morbidity and mortality throughout the world. Despite recent advances in treatment, we have not yet identified the means to prevent atherosclerotic vascular disease has not been met. It remains a challenge to determine at an early stage whether an atherosclerotic plaque will become unstable and vulnerable. Therefore, this study aim to identify key risk factors associated with the stability and vulnerability of carotid artery plaques, which may offer valuable insights for the

Acute myocardial infarction secondary to giant right coronary artery aneurysm requiring surgical correction: a case report.

BACKGROUND: Giant coronary aneurysms are rarely described in current literature, with an incidence of only 0.02%. Treatment strategies vary and there is no definitive recommendation; however, surgical treatment should be considered to prevent future complications. CASE PRESENTATION: We report the case of a 58-year-old patient with a history of hypertension, diabetes, and obesity who presented with non-ST-segment elevation myocardial infarction (NSTEMI), ischemic cardiomyopathy with reduced ejection fraction, coronary artery disease, and a giant right coronary artery (RCA) aneurysm with compre

Autograft Enlargement After the Ross Procedure in Pediatric Patients: Somatic Growth or Pathologic Dilatation?

Purpose of ReviewPulmonary autograft autotransplantation represents a popular surgical approach for pediatric patients requiring aortic valve replacement due to the potential for autograft enlargement to accommodate somatic growth. Nevertheless, autograft dilatation and the subsequent need for reintervention are quite common. Published data suggest that autograft enlargement may result from pathological passive remodeling rather than active somatic growth and vice versa. The present review serves to comprehensively evaluate available evidence related to the fate of the pulmonary autograft afte

Rapid enlargement of a pre-existing intracranial aneurysm during infective endocarditis: a case report.

BACKGROUND: Infectious intracranial aneurysms (IIAs) are generally thought to develop de novo as a consequence of septic embolization during infective endocarditis (IE). Rapid infection-related enlargement of a pre-existing unruptured aneurysm during IE has not been previously documented. We report a case in which a small middle cerebral artery (MCA) aneurysm, previously identified on imaging, enlarged rapidly in the course of IE. CASE DESCRIPTION: A 67-year-old woman with a history of mitral valve repair presented with progressive general malaise and mild left hemiparesis. MRI demonstrated a

Optical coherence tomography and histological assessment of cutaneous vasculature and neural changes in long-term smokers: an exploratory study.

Smoking affects microcirculation by damaging endothelial cells and reducing blood flow. Vascular disease is also thought to contribute to the development of diabetic neuropathy, resulting in the decrease of nerve fiber density. The impact of nicotine on vasculature using optical coherence tomography angiography (OCT-A) has been extensively studied. We utilized an alternative method, reactive hyperemia OCT-A (RH-OCT-A), to provide a more complete visualization of microvasculature changes in the forearm of 5 old female smokers (mean age = 62.6yrs) and 7 young female smokers (mean age = 26.6yrs)

Relationship between epicardial adipose tissue and coronary function evaluated with [13 N]NH3 PET/CT.

Different reports have suggested that epicardial adipose tissue (EAT) is related to impaired coronary function, myocardial ischemia and major adverse cardiac events. The aim of this study was to evaluate whether a relationship between EAT volume (EATvol) and thickness (EATthick) with myocardial blood flow (MBF) and myocardial flow reserve (MFR) evaluated with [13N]ammonia ([13N]NH3) positron emission tomography/computed tomography (PET/CT) is present.MBF and MFR were extracted from stress and rest [13N]NH3 PET/CT images. EATvol was extracted from unenhanced CT images with a semiautomatic metho

Pulmonary artery aneurysm as a rare manifestation of giant cell arteritis.

A female patient in her 70s with a remote history of vocal cord paralysis was incidentally found to have a pulmonary artery aneurysm on a chest CT. Serial imaging performed over 3 years noted enlargement beyond 5.5 cm, prompting surgical repair. Histopathological examination revealed granulomatous arteritis with multinucleated giant cells and disruption of the elastic lamina, consistent with large-vessel giant cell arteritis (GCA). The patient was treated with corticosteroids and subsequently transitioned to subcutaneous tocilizumab, achieving postoperative stability on imaging and normalisati

Post-acute cardiovascular and stroke care and the potential of certification programs: A 'Call to Action'.

Over 40% of patients are discharged to post-acute care services which serve predominantly older adults with cardiovascular or cerebrovascular disease and substantial age-related complexity. Post-acute care settings are often siloed from the broader healthcare system, creating care gaps, suboptimal outcomes, and higher-than-average readmission rates. As value-based care models increasingly emphasize holistic, equitable care for an aging population, post-acute care represents a major opportunity for improvement, including the use of preventive strategies and standardized, evidence-based processe
