Allergy burden score and lung function impairment in children with asthma: a dose-response cross-sectional study.

UNLABELLED: Children with asthma commonly present with multiple allergic comorbidities; however, the quantitative dose-response relationship between cumulative allergic burden and lung function has not been established in pediatric populations. This retrospective cross-sectional study included 264 treatment-naive children with asthma aged 4-16 years. An allergy burden score (ABS) was constructed using a cumulative disease count strategy incorporating allergic rhinitis, atopic dermatitis, food allergy, drug allergy, and family history of allergic disease. Multivariable linear regression and res

Community-Based Case-Finding for Chronic Obstructive Pulmonary Disease and Self-Management Education in Partnership with Faith-Based Organizations.

PURPOSE: Chronic obstructive pulmonary disease (COPD) is poorly diagnosed with millions unaware they have the condition, preventing or delaying treatment and behavioral changes. We partnered with faith-based organizations (FBOs) to raise awareness of COPD among Black Americans by facilitating diagnosis and offering self-management education in the community. METHODS: Cross-sectional and pre-post study designs were applied. Three churches identified representatives to serve as COPD liaisons (CLs). CLs received training on screening procedures and an overview of COPD. Case-finding was conducted

Interactions between host genetics and gut microbiome influence susceptibility to childhood asthma and lung function.

BACKGROUND: The gut microbiome is thought to influence risk of childhood allergic diseases; however, the data on species-level links to childhood asthma and lung function are limited, and the role of host genetics in the gut-lung axis remains unclear. METHODS: In a multicenter cross-sectional study of children with a history of bronchiolitis from the 35th Multicenter Airway Research Collaboration, we performed shotgun metagenomic profiling of stool samples obtained at age 6 years and examined associations of the gut microbiome with asthma prevalence and lung function. We also calculated polyg

Preserved muscle oxidative capacity in COPD patients with hypoxemia treated by long term O2 therapy.

PURPOSE: Long-term oxygen therapy (LTOT) is an established treatment for patients with chronic obstructive pulmonary disease (COPD) diagnosed with hypoxemia at rest or during activity. Chronic hypoxemia is a potential mediator of loss of muscle mass and oxidative capacity in COPD. In this retrospective analysis, we sought to determine whether patients with COPD receiving LTOT have impaired muscle oxidative capacity compared to those without LTOT. METHODS: Personal characteristics, medical and smoking history, spirometry, diffusing capacity, use of LTOT, 6-min walk distance (6MWD), dyspnea sym

Meteorological, ozone, maternal and individual-level risk factors for childhood diseases in Singapore: A prospective birth cohort study from 2009 to 2019.

BACKGROUND: The children of today are growing up in an environment characterised by accelerated climate change and air pollution. Few studies have examined the effects of these phenomena on childhood respiratory and allergic diseases in tropical countries, hence we aim to bridge this research gap. METHODS: We ascertained the first occurrence of eczema, wheeze and bronchiolitis in 1124 mother-child pairs in GUSTO, a prospective birth cohort from tropical Singapore. We assigned daily measures of meteorological conditions and outdoor air quality exposures averaged over each month from 2009 to 20

The Impact of Physical Training on Circulating Retinol-Binding Protein 4: A Systematic Review.

Obesity and type 2 diabetes mellitus (T2DM) are strongly associated with insulin resistance and chronic inflammation. Retinol-Binding Protein 4 (RBP4), an adipokine secreted by the liver and adipose tissue, has been implicated in metabolic dysfunction, with elevated circulating levels linked to impaired glucose homeostasis. Exercise is known to improve insulin sensitivity; however, its effects on RBP4 remain unclear. This systematic review aimed to synthesize the available evidence on the impact of exercise interventions on circulating RBP4 concentrations and to evaluate differences by exercis

The Natural History of Chronic Lung Allograft Dysfunction in Single and Bilateral Orthotopic Lung Transplant Recipients.

BACKGROUND: A better understanding of chronic lung allograft dysfunction (CLAD) natural history could be useful in targeting interventions to those most likely to progress. We hypothesized that pre-CLAD lung function trajectory would predict post-CLAD lung function and survival. METHODS: We identified first lung transplant recipients between January 1, 2000, and January 1, 2020, diagnosed with probable or definite CLAD. We examined the trajectory of forced expiratory volume in 1 s (FEV1) in the 3 mo before CLAD onset and divided patients into 3 groups based on slow, intermediate, or rapid dec

Development and validation of an epidemiological risk score for early detection of pancreatic cancer in high-risk individuals within the IMAGene project.

BACKGROUND: Early detection is critical in pancreatic ductal adenocarcinoma (PDAC)-one of the most lethal malignancies due to its typically late diagnosis. METHODS: In this study, we aimed to validate an epidemiological risk score (ERS) designed to identify individuals at increased risk of developing PDAC prior to the use of imaging or other diagnostic procedures. The ERS was constructed through a meta-analysis of 24 well-established epidemiological risk factors. We applied this score to a prospective cohort of 178 high-risk individuals with a family history of PDAC within the IMAGene project

Risk factors, and outcomes of patients with carbapenem-resistant Enterobacterales bloodstream infection: an eight-year case-case-control study.

Carbapenem-resistant Enterobacterales bloodstream infection (CRE-BSI) represents a major and urgent challenge to global public health. Some patients with CRE-BSI have a greater risk for poor clinical outcomes, thus identifying risk factors for CRE-BSI is required to determine the most at-risk populations. Here, we investigated risk factors for CRE-BSI by conducting a retrospective case-case-control study at Tianjin Medical University General Hospital, between 2017 and 2024. A total of 144 patients with CRE-BSI were enrolled in this case-case-control study. Each case was matched simultaneously

Relationships between bronchodilator responsiveness, a COPD polygenic risk score, and COPD progression.

INTRODUCTION: Bronchodilator responsiveness (BDR) is associated with progression to COPD. Genetic risk for COPD, summarized by polygenic risk scores (PRS), predicts low lung function and COPD. However, it remains unclear whether genetic predisposition to COPD is related to BDR and whether PRS and BDR together influence lung function decline in individuals at risk for the disease. METHODS: We analyzed data from COPDGene participants with a smoking history and normal spirometry at study enrollment. We cross-sectionally examined the association of a PRS with 2005-BDR-FEV1 % (change relative to p
